    One hundred and seventy five patients treated for burns during 1983 and 1984 were reviewed. The majority of these patients were below eight years of age. These injuries were mainly sustained at home (83.4%) and were usually caused by hot liquids (41.7%). The infection rate was 57.1% and many developed septicaemia (21.7%). Mortality in patients sustaining burns involving greater than 30% of the body surface area was high at 52%.

    A prospective study of 94 burned children was carried out from January 1993 to October 1994. Scalding was the predominant cause of injury affecting mainly toddlers between 1 and 3 years old. Ninety-six per cent of the injuries occurred at home. There was 1 death following an 81% flame burns. An intense campaign to make parents aware of the hazards is required as almost all the injuries were preventable.

    Red tide of dinoflagellate was observed in brackish water fish ponds of Terengganu along the coast of the South China Sea during the study period between January 1992 to December 1992. The nearby coastal moat water facing the South China Sea is the source of water for fish pond culture activities of sea bass during the study period. An examination of water quality in fish ponds during the study period indicated that both the organic nutrients were high during the pre-wet monsoon period. The source of the nutrients in coastal water was believed to be derived from the agro-based industrial effluents, fertilizers from paddy fields and untreated animal wastes. This coincided with the peak production of dinoflagellate in the water column in October 1992. The cell count ranges from 8.3 to 60.4×10.4×10(4)/l during the bloom peak period and the bloom species were compared entirely of non-toxic dinoflagellates with Protoperidinium quinquecorne occurring >90% of the total cell count. However, both cultured and indigenous fish species were seen to suffer from oxygen asphyxiation (suffocation due to lack of oxygen). The bloom lasted for a short period (4-5 days) with a massive cell collapse from subsurface to bottom water on the sixth day. The productivity values ranged from 5-25 C g/ l / h with a subsurface maximum value in October 1992. Two species of Ciliophora, Tintinnopsis and Favella, were observed to graze on these dinoflagellates at the end of the bloom period.

    Percutaneous epiphysiodesis for arresting growth is a useful method for equalising moderate lower limb-length discrepancy. This paper reviews the result of this procedure performed at the Royal Hospital for Sick Children in Glasgowfrom 1989 to 1993. There were 8 children (5 girls and 3 boys) ranging in age (chronological) from 10 years 3 months to 13 years 3 months. Limb-length discrepancies (LLD) at the time of surgery were between 2.5 cm to 5.5 cm. The predicted LLD at maturity was obtained using the Moseleys graph. This ranged from 3.9 cm to 8.3 cm. The cause of discrepancy were hemihypertrophy (5), neurofibromatosis (1), AV malformation (1) and neonatal osteomyelitis (1). Bone age in 3 patients corresponded to their chronological age. In 2 patients their bone age were less than their chronological age by 1 year. In the remaining 3 patients their bone were less than their chronological age by 1 to 3 years. All patients had percutaneous epiphysiodesis of both the distal femur and proximal tibia. All patients except one had afinal LLD of 1 cm or less. There were 2 unsatisfactory results: one girl had a valgus knee due to asymmetric physeal closure. Another boy had a final LLD of 3.3 cm as the procedure was done near skeletal maturity. This technique is useful even with LLD greater than 5 cm in children whose predicted height is above average. The difference between chronological age and bone age ranged from I to 3 years.

    We report a case of a sixteen-month old boy with osteosarcoma of the left humerus. To the best of our knowledge this is the youngest case reported in Malaysia. This case illustrates that osteosarcoma although rare does occur in a very young child. The child died six weeks after presentation due to disseminated disease.

    We report a case of a lady presenting with a lesion in the distal radius with classical radiological features of a giant-cell tumour. These tumours are often resected without preliminary histological confirmation. A biopsy done in this patient showed it to be tuberculosis.

    Between May 1992 through October 1998, twenty-two patients who had adolescent idiopathic scoliosis were treated using Universiti Kebangsaan Malaysia (UKM) spinal instrumentation and fusion. The minimum length of follow-up was 2 years with an average of 2.9 years. Nineteen patients were female and three patients were male. The mean age at the time of the operation was 16.2 years (range, 13 to 24 years). The average blood loss was 1,878 ml and the average operating time was 291 minutes. The mean pre-operative curve was 61.2 degrees, with a range of 40 degrees to 90 degrees. The average post-operative correction of the Cobb's angle was 53.5 per cent, with a range of 33 per cent to 81 per cent. Post-operative immobilization consisted of a maximum of six months in a body cast. There were no neurological injuries, no deep wound infections, and no evidence of pseudoarthroses. Only three complications occurred and these did not disturb the progress of the fusion. This technique safely achieves the objectives of scoliosis correction, and is cost-effective in the treatment of adolescent idiopathic scoliosis.

    This is a prospective study to look at the outcome of unilateral proximal third femoral shaft fractures in children treated with a bilateral Thomas splint in the Department of Orthopaedic Surgery Universiti Kebangsaan Malaysia between the period of January 1996 and June 1998. Eighteen children aged between 2 years and 12 years old with unilateral proximal third fractures of the femoral shaft were treated using a bilateral Thomas splint. Angular deformities were measured using a goniometer metric scale before and after Thomas splints. The percentage of varus tilt corrected ranged from 17% to 72% with an average correction of 29% from the initial deformity and the difference was statistically significant (p < 0.05). The percentage of posterior tilt corrected ranged from 19% to 60% with the average correction of 20% from the initial deformity. The difference was statistically significant (p < 0.05). From this study, we conclude that bilateral Thomas splints can give a better correction of angular deformity for proximal third femoral shaft fractures in children below twelve years of age.

    This paper describes a system using lensed optical fiber sensors that are arranged in the form of two orthogonal projections. The sensors are placed around a process vessel for upstream and downstream measurements. The purpose of the system is for on-line monitoring of particles and droplets being conveyed by a fluid. The lenses were constructed using a custom heating fixture. The fixture enables the lenses to be constructed with similar radii resulting in identical characteristics with minimum differences in transmitted intensity and emission angle. By collimating radiation from two halogen bulbs, radiation can be obtained by the sensors with radiation intensity related to the nature of the media. Each sensor interrogates a finite section of the measurement section. Each sensor provides a view. Parallel sensors provide a projection. Signal processing is carried out on the measured data in the time and frequency domains to investigate the latent information present in the flow signals.

    Carbide sludge (10.4-11.5 tonnes day(-1)) is generated from the reaction of calcium carbide (900 kg) and water (6,000 L) in the production of acetylene (2,400 m3), in three selected acetylene manufacturing plants. The sludge (of pH 12.2 and containing Cu, Pb, Fe, Mn, Ni and Zn ions whose concentrations exceed the Department of Environment limits for industrial wastewater) was treated by vacuum filtration as a substitute for the ponding system, which is environmentally less acceptable. A similar system by flocculation was also developed. The filtration system represents an improvement over the ponding method, as shown by a pH of 7 for the clear filtrate; the solid cake, which contains 98% of the metals, can be conveniently disposed at an integrated scheduled waste treatment centre.

    We report a case of a 13-year-old girl with an osteosarcoma of the right humerus, which had been diagnosed as an aneurysmal bone cyst at our institution. She underwent curettage and bone grafting of the lesion, which resulted in implantation metastasis of the tumour to the ilium. She died 15 months after presentation owing to pulmonary metastases. This report highlights the possibility of metastasis occurring by direct implantation to a graft donor site. We strongly recommend that a biopsy be performed in cases of presumed benign lesions before proceeding with the definitive surgery.

    Hydroxyapatite (HA; Ca10(PO4)6(OH)2), is one of the significant implant materials used in Orthopaedics and Dental applications. However, synthetically produced HA may not be stable under ionic environment, which it will unavoidably encounter during its applications. In this paper, the in vitro effects of three HA materials derived from different resources, i.e. commercial HA (HAC), synthesised HA from pure chemicals (HAS) and synthesised HA from kapur sireh; derived traditionally from natural limestone (HAK), were studied. The HA disc samples were prepared and immersed in simulated body fluid (SBF) for 31-day period. The evaluation conducted focuses on the changes of the pH and the Calcium ion (Ca-ion) and Phosphate ion (P-ion) concentrations in the SBF solution, as well as the XRD and SEM data representing the reactions on the HA materials. From the XRD, it was found that HAK has the smallest crystallite sizes, which in turn affect the pH of the SBF during immersion. The Ca and P-ion concentrations generally decrease over time at different rates for different HA. Upon 1-day immersion in SBF, apatite growth was observed onto all three surfaces, which became more pronounced after 3-day immersion. However, the appetites formed were observed to be different in shapes and sizes. The reasons for the difference in the apatite-crystals and their subsequent effects on cells are still being investigated.

    Revision surgery following failed open reduction for developmental dysplasia of the hip (DDH) is technically demanding. We reviewed 12 patients in our institution that required a revision surgery between January 1994 and December 2003. The aims of this study are to identify the causes of redislocation after the primary open reduction and to determine the clinical and radiological outcome after the revision surgery. The mean age at presentation for DDH was 31 months (range 1-84) and the mean age at primary open reduction surgery was 38 months (range 15-84) and the mean age at revision surgery was 69 months (range 21-180). The mean follow-up period after revision surgery was 20 months (range 3-84). All the revision surgery was performed via an anterior Smith Peterson approach. The most common cause for redislocation was inadequate exposure and failure to release the obstructing soft tissues around the hip. The bony factors for redislocation included failure to perform a femoral shortening and excessive derotation of an anteverted femoral head combined with a Salter osteotomy. Three cases with posterior acetabular wall deficiency needed immobilization of the hip in extension as the hip tended to dislocate in flexion. Six patients had limb length discrepancy ranging from 1 cm to 4 cm and 50% developed avascular necrosis. Only 2 patients were asymptomatic and the majority had a limp and limitation of motion. All were pain free except one. In view of the technically demanding surgery and poor results after revision, the surgeon should recognize the pathology and ensure that the primary procedure achieves a stable and concentric reduction.

    Slipped capital femoral epiphysis (SCFE) is a relatively uncommon hip disorder in adolescents and its prevalence in Malaysia has not been studied. This retrospective study is undertaken to provide an overview of a 12-year review of SCFE treated in our institution. Fourteen patients (19 hips) with slipped capital femoral epiphysis (SCFE) admitted to Hospital UKM from 1990 to 2002 were reviewed with respect to demographic profile, functional outcome according to the Iowa Hip Score, and complications. There were ten boys (average age, 12.5 years) and four girls (average age, 12 years). Eight were Malays and six were Indians. The average body mass index was 26.1 verweight). The left hips (11 hips) were affected more than the right hips (eight hips). Five patients had bilateral slips. Thirteen hips were considered stable while the other six hips were unstable. The majority of cases were moderate slips (12 hips), four hips had severe slips while three hips had mild slips. Several methods of treatment were instituted. These include in situ cannulated screw fixation (11 hips), Knowles pin fixation (three hips) and gentle closed manipulative reduction with cannulated screw fixation (three hips). One patient with bilateral slips refused surgical treatment. Based on the Iowa Hip Score, most patients (nine) had satisfactory results (excellent or good), three had fair results while one patient had a poor result. Avascular necrosis developed in five hips while chondrolysis occurred in one hip. In situ cannulated screw fixation is the treatment of choice. SCFE is an uncommon condition in Malaysia.

    We report a case of a broken K-wire migrating to the cervical spine from the right clavicle in a 9-year-old child. The initial diagnosis, fracture of the clavicle with an acromioclavicular joint dislocation, was treated by open reduction and K-wiring. One K-wire broke and migrated to the neck, posterolateral to the C6 vertebra. The K-wire was removed percutaneously under image intensification. Acromioclavicular joint dislocation in children is rare since the distal clavicle does not ossify until the age of 18 or 19 years meaning that almost all closed fractures of the clavicle in children can be treated nonoperatively.

    The Klippel-Trenaunay syndrome is a combination of venous and capillary malformations associated with soft tissue and/or bony limb hypertrophy, with or without lymphatic malformations. Although persistent foetal veins are rare, the persistence of the lateral marginal vein is a common association in this syndrome. It results in venous hypertension, which gives rise to venous varicosities, which are commonly seen in this syndrome. This is a case report of a 28-year-old man with Klippel-Trenaunay syndrome, with persistence of the lateral marginal vein, affecting his right lower limb. He was treated with an above-knee amputation. The amputated limb was dissected to demonstrate the anatomy of the lateral marginal vein. To the best of the authors' knowledge, the gross anatomy of the lateral marginal vein has not been previously reported.

    Myiasis is a pathological condition in humans and animals caused by various species of dipterous larvae. Myiasis which occurs in a newborn baby is referred as neonatal myiasis. It is a rare condition and there are only a few reports to date. A case of neonatal aural myiasis in a two day old infant is reported in this paper.
