    The present paper reports on the influence of sintering temperature on the porosity and strength of porous hydroxyapatite (HA). HA powder was first prepared by the sol-gel precipitation method using calcium hydroxide and ortho-phosporic acid. The fine HA powder, measuring <50 microm was then mixed into a slurry with the addition of binder agent, being a mixture of sago and PVA. A small amount of sodium dodecyl sulphate was also used as a foaming agent. Porous HA samples were then prepared via slip casting technique. The surface morphology of the sintered samples was observed under scanning electron microscopy at 20 kV and the compositions were determined via SEM-EDX. A universal testing machine was used to determine the compaction strength of the sintered samples.

    A mixture with different compositions of HA and TCP were synthesize in this work by precipitation method using Ca(NO3)2 4H2 and (NH4)2HPO4 as the starting materials. A mixture with HA and TCP phases in different ratios were produced. The powders were sintered from 1000 degrees C to 1250 degrees C. The phase compositions of the mixtures were then studied via XRD. This work shows that the pH value determines the different phase compositions of the HA-TCP mixture. Chemical analyses were carried out by FTIR. The microstructure was observed under SEM.

    The paper presents the effect of sintering temperature on the physical properties of porous hydroxyapatite (HAp In this study, the HAp was prepared using polymeric sponge techniques with different binder concentrations. The sintering process was carried out in air for temperature ranging from 1200 degrees C to 1600 degrees C. Different physical properties namely density and porosity were observed at different sintering temperatures. The HAp prepared with higher PVP binder showed a slightly decreased in apparent density with increasing sintering temperature, while those HAp prepared with lower PVP showed a slightly increase in apparent density with increasing sintering temperature. The total porosity was found to be approximately constant in the whole sintering temperature range. However, closed porosity decreases with increasing sintering temperature for HAp prepared by lower binder concentration. On the other hand, the HAp prepared by higher binder concentrations showed increasing closed porosity with increasing sintering temperature. Other features such as the influence of sintering temperatures on grain and strut would also be presented in this paper.

    The paper discusses the influence of sintering temperature on the microstructure and strength of hydroxyapatite ceramics prepared using the extrusion process. The average pore diameters observed were in the range of approximately 150mm to 300mm whereas the compaction strength was found to be around 120-160 MPa.

    Deep brain stimulation (DBS) was first introduced in 1987 to the developed world. As a developing country Malaysia begun its movement disorder program by doing ablation therapy using the Radionics system. Hospital Universiti Sains Malaysia a rural based teaching hospital had to take into consideration both health economics and outcomes in the area that it was providing neurosurgical care for when it initiated its Deep Brain Stimulation program. Most of the patients were from the low to medium social economic groups and could not afford payment for a DBS implant. We concentrated our DBS services to Parkinson's disease, Tourette's Syndrome and dystonia patients who had exhausted medical therapy. The case series of these patients and their follow-up are presented in this brief communication.

    Epilepsy surgery has been performed by a few centres in Malaysia, including Hospital Universiti Sains Malaysia (HUSM). To date, a total of 15 patients have undergone epilepsy surgery in HUSM. The epilepsy surgery included anterior temporal lobectomy (ATL) with amygdalohippocampectomy (AH) and Vagal nerve stimulation (VNS). The surgical outcomes of the patients were assessed using the International League Against Epilepsy (ILAE) outcome scale. The ILAE scores for patients who underwent ATL with AH were comparatively better than those who underwent VNS. One of the patient who underwent ATL with AH and frontal lesionectomy was found to have psychosis during follow up. Epilepsy surgery has proven to be an important treatment for medically resistant epilepsy. Thus it is important to raise public awareness regarding epilepsy and its treatment.

    BACKGROUND: Craniosynostosis is a congenital defect that causes ≥1 suture to fuse prematurely. Cranial expansion surgery which consists of cranial vault reshaping with or without fronto-orbital advancement (FOA) is done to correct the skull to a more normal shape of the head as well as to increase the intracranial volume (ICV). Therefore, it is important to evaluate the changes of ICV after the surgery and the effect of surgery both clinically and radiologically.

    OBJECTIVE: The aim of this study is to evaluate the ICV in primary craniosynostosis patients after the cranial vault reshaping with or without FOA and to compare between syndromic and nonsyndromic synostosis group, to determine factors that associated with significant changes in the ICV postoperative, and to evaluate the resolution of copper beaten sign and improvement in neurodevelopmental delay after the surgery.

    METHODS: This is a prospective observational study of all primary craniosynostosis patients who underwent operation cranial vault reshaping with or without FOA in Hospital Kuala Lumpur from January 2017 until Jun 2018. The ICV preoperative and postoperative was measured using the 3D computed tomography (CT) imaging and analyzed. The demographic data, clinical and radiological findings were identified and analyzed.

    RESULTS: A total of 14 cases (6 males and 8 females) with 28 3D CT scans were identified. The mean age of patients was 23 months. Seven patients were having syndromic synostosis (4 Crouzon syndromes and 3 Apert syndromes) and 7 nonsyndromic synostosis. The mean preoperative ICV was 880 mL (range, 641-1234 mL), whereas the mean postoperative ICV was 1081 mL (range, 811-1385 mL). The difference was 201 mL which was statistically significant (P  1.0). However, there was 100% (n = 13) improvement of this copper beaten sign. However, the neurodevelopmental delay showed no improvement which was statistically not significant (P > 1.0).

    CONCLUSION: Surgery in craniosynostosis patient increases the ICV besides it improves the shape of the head. From this study, the syndromic synostosis had better increment of ICV compared to nonsyndromic synostosis.

    BACKGROUND: Arachnoid cysts are intraarachnoid benign cystic lesions filled with cerebrospinal fluid and should be treated without incurring further morbidity to the patients.

    CASE DESCRIPTION: The authors present a case of a 68-year-old elderly female with a large right fronto-parieto-temporal arachnoid cyst who has been suffering from mild left hemiparesis for the past 4 years and presented with sudden onset of seizures. The 3 Tesla MR system with diffusion tensor imaging (DTI) and MR tractography of the brain showed a large right fronto-parieto-temporal cystic lesion measuring 7 × 5 × 5 cm with a midline shift of 1 cm, suggestive of an arachnoid cyst with surrounding ipsilateral white matter projection pathways and inferior occipito-frontal fasciculus or inferior longitudinal white matter tracts. The cyst was successfully treated with neuronavigation-guided endoscopic and hodotopical approach to fenestrate the arachnoid cyst into the sylvian cistern, avoiding inadvertent injury to major white matter tracts portrayed by DTI. Postoperatively, a repeated computed tomography (CT) scan of the brain revealed a smaller arachnoid cyst with correction of the midline shift. The patient was weaned off from the ventilator and her hemiplegia improved gradually.

    CONCLUSION: This case report emphasizes the value of neuronavigation-guided endoscopic and hodotopic approach to fenestrate the intra-axial arachnoid cyst.

    Shunt surgery is frequently chosen to manage periventricular metastasis of pineal region tumours which obscured the floor of the third ventricle. However, this procedure falls short due to distant metastasis. Neuronavigation-guided endoscopic surgery offers a viable alternative.

    Phase composition of calcium phosphate ceramic is a characteristic directly related to the biological response of implants due to the differences in mechanical and biochemical properties of these compounds. The biodegradation rate of biphasic calcium phosphate (BCP) can be controlled by altering the HA to β-TCP ratios. In this study the crystalline phase evolution of BCP synthesized via precipitation from aqueous solution of (NH4)2PO4 titrated into heated solution of Ca (NO3)2 was evaluated. The resulting powder was fabricated into porous scaffold using polyurethane foam method. Bulk powders were sintered from 700 - 1400°C to determine the most significant sintering temperature to obtain a stable and well crystallize BCP phases. The porous scaffolds were then sintered at selected temperature and the effects of various sintering times from 5,7,9,11,13 and 15 h were investigated. Bulk powders were characterized by dilatometer, IR analysis and XRD and porous scaffolds were analyzed by XRD and SEMEdx. RIR method was performed to show that the HA to β-TCP ratios were increased with increasing of sintering time and reached the maximum HA value at 11h. It is found that, the possibilities to manipulate the HA to β-TCP ratios in BCP porous scaffold by just controlling the sintering time of the scaffold without controlling the starting powder characteristics.

    Plant virus infections are known to alter host plant attractiveness and suitability for insect herbivores. This study was conducted to determine how cucumber mosaic virus (CMV)-infected chilli plants affect the fitness and settling preferences of nonvector whitefly, Bemisia tabaci adults under dual-choice conditions with volatile organic compounds analyzed using solid phase microextraction coupled with gas chromatography-mass spectrometry (GC-MS). Results showed that the presence of CMV in chilli plants substantially affects the settling preferences of the B. tabaci, which preferred to settle on noninfected plants. Duration of the egg stage and the longevity and fecundity of adult B. tabaci on CMV-infected chilli plants were not markedly different from those on noninfected chilli plants. In contrast, the developmental time from egg to adult was significantly reduced in CMV-infected chilli plants compared to the noninfected plants. The results also showed that CMV-infected chilli plants released significantly more linalool and phenylacetaldehyde than noninfected plants. Overall, it was suggested that the behavioral response of B. tabaci might be modified by CMV-infected plants, which alter the release of specific headspace volatiles. Based on these results, the modification of plant volatile profiles may help in enhancing the effectiveness of biological control and the protection of crop plants against B. tabaci.

    Autonomy is widely accepted to be the third pillar of medical ethics. However, if it comes to refusal of life saving treatments, some extra considerations are necessary, especially if decisions are made by surrogate decision makers. Four cases of problematic decision making are presented here, followed by a discussion about the cultural and religious misconceptions about the rights of surrogate decision makers.
