    Objective: To investigate the prevalence of Female Orgasmic Dysfunction (FOD) focusing on the orgasm domain among female patients attending PPUKM Psychiatric clinic. To compare the prevalence of orgasmic dysfunction between female patients on Escitalopram and on Fluoxetine therapy.
    Methods: A validated questionnaire for sexual function was used to assess orgasmic function. A total of 112 women aged between 24 and 57 participated in this study. The orgasmic dysfunction was compared between patients on selective serotonin reuptake inhibitors (SSRIs) fluoxetine and escitalopram.
    Results: The prevalence of female orgasmic dysfunction was 58.9% (33/56) among patients treated with Fluoxetine and 41.1% (23/56) among patients treated with Escitalopram. However, there was no statistically significant difference between these two treatment groups (p=0.059). The odds to have FOD among patients on higher dose of antidepressants was found to be higher compared to those patients who were on lower dose of antidepressants (Odds ratio 5.32, p= 0.001).
    Conclusion: There was no significant difference of Female Orgasmic Dysfunction between patients on Fluoxetine and Escitalopram.

    Introduction: The transboundary smoke haze has become a major concern as it had a wide ranging impact in Southeast Asia from the aspect of public health to national economics. This review aims to synthesize available literature in epidemiology, economics and haze related studies to provide essential information for the valuation of health costs associated with haze in Malaysia. Methods: Evidence on health economic impact of haze was gathered by conducting a literature review and collecting information on the health effects of haze and the cost of it as well as the hospitalization cost from medical illnesses and hospital admissions due to haze. Articles were taken from those that were published from 1999 to 2016.Results: The 1997 Asian Haze episode has caused increasing trend of prematurity of mortality and respiratory problems and several series of haze later had caused increased number of hospitalization. The cost impact on hospital admission ranges from MYR1.8 million in 2005 to MYR118.9 million in 2013. During the 1997 haze, the incremental cost of illness (COI) was noted to be MYR 21million and it shot up to MYR 410 million during the 2013 haze. Conclusion: The haze gives a serious health effect to our country and our neighbours. These health effect has caused a significant health economy impact which include rise in hospital admission cost and medication, incremental cost of illness and extend to cost of medical-related leaves taken and lead to loss of income opportunities.

    Introduction Non-specific low back pain is one of the most common physical ailments
    affecting millions of people worldwide. This condition constitutes a
    significant public health problem and was listed as a prevalent health
    complaint in most societies. Even though there are many anecdotal claims
    for reflexology in the treatment of various conditions such as a migraine,
    arthritis and multiple sclerosis, but very little clinical evidence exists for
    reflexology on the management of low back pain per se. This study aims to
    evaluate the effects of foot reflexology therapy as an adjunctive treatment to
    the Malaysian low back pain standard care in relieving pain and promoting
    health-related quality of life among people with non-specific low back pain.
    Methods This is a parallel randomized controlled trial with pre and post-treatment
    study design. The study setting for the intervention located at Penawar
    Reflexology Center, Kuala Terengganu, Malaysia. A total of 100
    participants with non-specific low back pain will be allocated to one of two
    groups, using a randomization computer program of Research Randomizer.
    The control group will receive low back pain standard care, while the
    intervention group will receive standard care plus eight sessions of foot
    reflexology therapy. The pain intensity and health-related quality of life
    scores will be measured using Visual Analogue Scale and Euro-quality of
    life scale respectively in both groups. The study was approved by the
    Human Research Ethics Committee of University Sultan Zainal Abidin
    (UHREC/2016/2/011). The study protocol was registered at
    ClinicalTrials.gov, with the ID number of NCT02887430.
    Measurements Outcome measures will be undertaken at pre-intervention (week 1), postintervention
    (week 6) and follow-up (week 10).
    Conclusions This will be the first trial to compare the foot reflexology therapy with
    control group among people who medically diagnosed with non-specific low
    back pain in Malaysia. The result of this study will contribute to better
    management of this population, especially for Malaysia healthcare setting.

    Introduction: Zika infection was declared as Public Health Emergency of International Concern since year 2015. Despite of no new reported case via National Surveillance System for flavivirus, an underestimated seroprevalence might occur as the country contributes to the Asian lineage of the virus. Methods: Systematic literature search using PICO framework and PRISMA checklist across four databases for articles published from year 2013-2018 yielded 189 results, 37 articles accepted by titles following criteria were subjected to abstract screening, leaving 8 articles with clear risk proceed to full text analysis using Cochrane checklist and GRADE assessment. Results: There were four high quality articles and four low quality articles based on biases in studies. Blood product management and vac-cination are strategies strongly recommended to be implemented as Zika response while vector control and family planning are public health measures to be proposed as policy if feasible. Successful factors to improve Zika surveil-lance and management includes developing algorithm for blood product management, anti-Zika vaccine research, algorithm for new-born screening, participation of policy makers, healthcare capacity building, raising healthcare and public awareness on the infection, international funding, utilization of technology in data management and bio-logical control of vector. Conclusion: Implementation of Zika response as policy is timely, should be evidence-based and follow guidelines from WHO / CDC / FDA US after cost-effectiveness evaluation for Malaysia setting.

    Introduction The extensive and intensive use of pesticides in agricultural practices has exposed farmers to various hazards resulting in varying degrees of health
    outcomes.
    Methods We conducted a cross-sectional study among paddy farmers in Sabak Bernam district, Malaysia. The objective of this study was to gather baseline information on chlorpyrifos blood level and its relationship with pesticides exposure symptoms.
    Results We detected chlorpyrifos in farmers’ blood in 7 percent of the respondents, with mean 7.29 nanogram per millilitre blood (sd 5.84 nanogram per millilitre). The percentage of farmers who experienced at least one pesticide exposure symptoms was 75 percent. However, we found no significant association between chlorpyrifos blood level and its exposure symptoms. The farmers had low scores on safe practice of pesticide use even though they have high marks on knowledge and attitude. We found no significant association between the scores on knowledge, attitude and practice on pesticide use and the chlorpyrifos blood level.
    Conclusions The presence of pesticide exposure symptoms proved that most of the farmers were exposed to hazardous effects of pesticides. Specific trainings on safe use and handling of pesticides should be given on regular basis to these farmers to ensure they are protected from hazardous effects of pesticides exposure.

    Men’s health remain unclear term for majority of general population as well as physician worlwide. Nowadays there is an increase interest in addressing men’s requirement in health care as a separate branch. When discussing about men’s health, it is fair to say that even a man himself does not know much about men’s health. Most of them think that men’s health is just a discussion on sex. This thought is not entirely right. The scope of men’s health is actually larger than the male sex organ itself. To define men’s health, we have to look at man holistically.

    Air pollution has been widely known to have an influence on health of the general population.
    Air pollution can result from natural causes, human activities and transboundary air pollution.
    Weather and climate play crucial role in determining the pattern of air quality. In recent years,
    air pollution and recurrent episodes of haze has become a major concern in Malaysia.
    Surveillance data on concentrations of main air pollutants such as carbon dioxide, (CO2),
    Nitrogen Dioxide (NO2), Ozone (O3), sulphur dioxide (SO2) and particulate matter (PM10)
    were found to be higher during the haze days and this may have an impact on health of the
    community as reflected by an increase in hospital admissions particularly the respiratory and
    cardiovascular diseases.

    In 2015, almost half of low birth weight babies in the world were born in Southern Asia. It is contributed by multiple factors including maternal exposure to the elements in the environment during the antenatal period. Particulate matter (PM10) pollution in Southeast Asian region have been extensively studied with known attributions and sources. It is also known that PM10 is able to restrict foetal growth at molecular level. This review intends to investigate if the unborn in Asia are affected by air pollutants indirectly through their mothers. Methods Publications from Scopus and Science Direct digital databases in Asian region from 2015 onwards were reviewed. Details collected were the year of publication and study location, the study design, investigated air pollutants, exposure estimation methods used, the timing of exposure in relation to pregnancy, pregnancy outcome measured and the relative risk or odds of effect. Results A total of eight full text articles were included. Most of the studies were of cohort and quasi experimental designs, involving local air monitoring measurements to assign exposure. Conclusions There were more studies considered multiple air pollutants as contributing risk rather than a single pollutant. The exposure was measured according to stages of pregnancy and the trimester stratification is the most often method used. Modalities used in representing birth outcomes were not confined to birth weight alone but also included the length of gestation. Exposure to PM10 have been found to be associated with reduction in birth weight and increased risk for preterm birth in Asia.

    Dengue is a disease propagated by vectors namely Aedes spp. mosquitoes. One of the effective approaches to control dengue is through integrated vector management and intervention programs. COMBI or Communication for Behavioral Impact is a strategic approach to control the Aedes population as well as dengue cases. This study was conducted at Taman Desa Kolej, Nilai, Negeri Sembilan, an suburban residential area to determine the effectiveness of COMBI, by using questionnaire and entomological survey as well as implementation of an intervention program.     Objective: Truancy is a disciplinary problem, which frequently occurs among school students and it has many contributory as well as inter-related factors. It is a growing problem in this country and it often becomes a prelude to other delinquent behaviours. The study objective is to determine the prevalence of truancy as well as factors related to it including psycho-behavioural factors. Methods: A cross-sectional study involving 556 Malay student’s selected using multi-stage sampling was conducted. Results: The overall prevalence of truancy is 30.2%. The predictors to truancy are age, students who frequenting entertainment centre, students who have not completed Quran recital, coping strategies using problems solving methods and time spent watching television/video. There is a significant association between truancy and psycho-behaviour such as watching video/internet pornography, frequenting entertainment centre, smoking, motorcycle racing and dating a special friend. Conclusion: Truancy is a social issue, which must be given serious attention by all concerned components of the society.

    Kajian ekologi nyamuk merupakan kajian yang penting dalam merangka sesuatu aktiviti kawalan penyakit bawaan nyamuk. Satu kajian ekologi nyamuk Aedes telah dijalankan di Senawang, Negeri Sembilan dari Julai 2009 hingga Febuari 2010. Senawang terletak di pinggir bandaraya Seremban yang merupakan kawasan perindustrian kecil dan sederhana dengan jumlah penduduk yang padat. Kajian ekologi vektor ini bertujuan menentukan komposisi nyamuk secara keseluruhan terutamanya kepadatan populasi Aedes, masa kemuncak gigitan nyamuk, mengenal pasti kehadiran tempat pembiakan nyamuk dan menentukan hubung kait antara nyamuk Aedes dengan faktor persekitarannya seperti suhu, kelembapan relatif, halaju angin dan curahan hujan. Pensampelan nyamuk Aedes menggunakan empat kaedah iaitu perangkap cahaya (CDC), tangkapan nyamuk berumpan kaki (BLC), ovitrap dan kajian Aedes. Terdapat dua spesies nyamuk di lokasi kajian yang didominasi oleh Ae. albopictus (93.29%) dan Ae. aegypti (0.11%). Terdapat dua masa kemuncak gigitan nyamuk Ae. albopictus iaitu awal pagi (0700-0900 pagi) dan lewat petang (jam 1700-1900). Suhu persekitaran, kelembapan relatif dan kelajuan angin didapati mempunyai kaitan dengan gigitan nyamuk Aedes pada perumah (p < 0.05). Taburan hujan didapati tidak mempunyai hubungan dengan kepadatan nyamuk Aedes (r = -0.137, n =6 , p > 0.05). Kepadatan nyamuk Ae. albopictus adalah tinggi di lokasi kajian dengan Indeks Ovitrap 100% bagi kesemua stesen. Kajian Aedes mendapati daripada 30 bekas yang positif larva, 93.33% daripadanya didominasi oleh Ae. albopictus. Kajian ini menunjukkan kepadatan Ae. albopictus adalah tinggi dan berpotensi membawa penyakit demam denggi dan chikungunya di lokasi kajian. Penjagaan persekitaran perlu ditekankan kepada masyarakat setempat untuk mengawal penyebaran penyakit-penyakit tersebut.

    Dengue is one of the main vector-borne diseases affecting tropical countries and spreading to other countries at the global scenario without cease. The impact of climate variability on vector-borne diseases is well documented. The increasing morbidity, mortality and health costs of dengue and dengue haemorrhagic fever (DHF) are escalating at an alarming rate. Numerous efforts have been taken by the ministry of health and local authorities to prevent and control dengue. However dengue is still one of the main public health threats in Malaysia. This study was carried out from October 2009 by a research group on climate change and vector-borne diseases. The objective of this research project is to assess the community vulnerability to climate variability effect on dengue, and to promote COMBI as the community responses in controlling dengue. This project also aims to identify the community adaptive measures for the control of dengue. Various research methodologies were applied in this research project in different localities. Site visits, review on surveillance data and mapping on Aedes population, dengue cases and climate variability, community survey on the knowledge prevalence , opinions and practices (KOP) and mosquito ecology were carried out during pre- and post intervention phases. Community vulnerability towards Aedes and dengue were mapped out applying GIS. A series of workshops, group discussions and activities such as COMBI activities to promote Aedes control were conducted involving the Ledang communities, the health district officers and UKM researchers. These activities also included interventions and documentation of community responses and their adaptive capacities towards dengue. Trends on Aedes population, dengue cases and community surveys pre and post-interventions, the processes for dengue control activities were analysed. The research findings could provide understanding on the community vulnerability to dengue against climate variability, their responses and adaptive measures. The community advocacy on combats against Aedes is a possible effective solution in dengue control. This research could provide other dimensions in public health management to address the impacts of climate change on vector-borne diseases.

    Penyakit denggi merupakan penyakit bawaan vektor yang menjadi salah satu ancaman utama kesihatan awam di Malaysia. Pemetaan taburan kes denggi daripada aspek reruang-masa boleh menjadi kaedah yang berguna dalam menilai risiko denggi kepada masyarakat. Kajian ini bertujuan untuk memetakan taburan reruang dan reruang-masa kes-kes denggi di dalam daerah Seremban, dijalankan dengan Sistem Maklumat Geografi (GIS) khususnya analisis reruang dan reruang-masa. Analisis taburan reruang menggunakan Indeks Moran, purata kejiranan terdekat (ANN) dan anggaran kepadatan Kernel. Analisis reruang-masa ditentukan dengan indeks kekerapan, jangka masa dan intensiti untuk mengenal pasti kawasan berisiko denggi mengikut masa. Sejumlah 6076 kes denggi dicatatkan di Pejabat Kesihatan Daerah Seremban dari tahun 2003 hingga 2009. Kadar insiden denggi adalah tinggi pada tahun 2003, 2008 dan 2009 dengan nisbah denggi : denggi berdarah adalah 21.6:1. Indeks Moran menunjukkan kes denggi berlaku dalam pengelompokan dengan skor Z adalah 16.384 (p=0.000). Analisis ANN dengan 0.264 (p= 0.000) dengan purata jarak insiden antara kes denggi di dalam kawasan kejiranan adalah 55 m. Anggaran kepadatan Kernel menunjukkan lokasi kawasan panas kes denggi tertumpu di Nilai dan Ampangan. Analisis reruang masa dengan purata nilai tertinggi indeks kekerapan, jangka masa dan intensiti masing-masing melebihi 0.023, 0.614 dan 0.657 di kawasan berisiko tinggi denggi di Nilai, Seremban dan Ampangan. Pengawalan denggi perlu diberi tumpuan kepada kawasan berisiko tinggi ini.
