    Dental caries is a major health problem affecting an estimated 90% of school children worldwide. This cross-sectional study aimed to evaluate oral health knowledge, attitude, and practices among secondary school students in Kuching, Sarawak. Data was collected using a pretested questionnaire on 209 randomly selected students from four schools. Results showed no significant differences between the gender and age groups in terms of knowledge level, but significant differences were observed between the schools. The students had positive attitude towards the dental services, but their dental visits were still low due to fear of dental needle and handpieces. Toothbrush and toothpaste were still the most commonly used oral hygiene aids. As compared to parents and friends, dentist was perceived to have more influence on oral hygiene practices among the students. Girls consumed more sweets, snacks and soft drinks than boys. However, girls spent longer time to brush their teeth and brush more frequently. Oral health education should be a life-long practice and incorporated into the school environment with the support of teachers and parents.

    The purpose of this study was to evaluate hardness (indicator for polymerization) and thickness of two types of resin cement at coronal, middle and apical level of tooth root canal. Ten extracted maxillary incisors were instrumented and post space was prepared for cementation of titanium post. Samples were divided into two groups and each group was cemented either of the two types of resin cements; Panavia F [dual-cured (PF)] and Rely X Luting 2 [self-cured (RL)]. The teeth were longitudinally sectioned; hardness and thickness was measured using Vickers hardness tester and a microscope (Leica DMLM). SEM observations along the cement line at the 3 different root levels were performed. Statistical analysis was performed to test significance of differences in hardness and thickness of the two types of cement (t-test; p= 0.05) and at different levels of the same type (one-way ANOVA followed by multiple comparison; p= 0.05). Significant difference of hardness was found at the apical level between the two groups and between the coronal and apical level of PF (p0.05). Moreover, voids were more obvious within the dual-cured group of cement. Dual-cured resin cement was found to be less polymerized than self-cured type at apical level. Increased thicknesses of resin cements in comparison to post space size were observed in both groups. Use of metallic post with resin cements needs further evaluation.

    Tracheocutaneous fistula (TCF) is a known complication of tracheostomy. It can cause problems such as saliva leak, predispose to infection from external skin into respiratory tract and cosmetically not acceptable. Treatment of the underlying infection is paramount important. Persistence of tract after sufficient duration of observation period should be surgically treated. Cases reported in the literature are mainly regarding paediatric TCF and the procedures are usually done under general anesthesia. We describe a case of surgical treatment of an adult TCF which was done under local anaesthesia.

    In the present case, granuloma formation and loss of tooth vitality following the use of bone wax in third molar surgery in a 16 - year- old male patient were reported. Endodontic and surgical treatments were carried out on the patient. As foreign body granulomatous reaction to bone wax is not uncommon in the literature review, alternative haemostatic agents should be taken into consideration to avoid any potential complications and untoward effects of using bone wax for haemorrhage control in surgical procedures.

    Pyogenic granuloma is a benign lesion which is commonly found in the oral cavity. It is a reactive inflammatory process of the injured mucosa to trauma. It can appear as a sessile or pedunculated mass with smooth or lobulated surface, which sometimes can mimic malignant lesion. Excision biopsy of the lesion is the confirmatory investigation which also a treatment tool. We report a case of middle-aged lady who had pyogenic granuloma of the tongue, which was safely excised using ultrasonic scissors. This case highlights the new technique of using ultrasonic instruments for excision of benign tongue lesion, with marked reduction of blood loss and operation time.

    Surgical ciliated cyst of the maxilla is a rare complication following surgical procedures or trauma
    involving the maxillary sinus. It can occur at any time after any procedures or trauma involving the maxillary
    sinus even though many years have lapsed. Clinically it may mimic other cysts of the maxillary sinus
    therefore a thorough history taking is necessary for diagnosis. Treatment is usually by enucleation or
    marsupialisation. This report highlighted a case of surgical ciliated cyst involving the right maxillary sinus
    probably secondary to maxillary sinus procedures, which were performed 30 years prior to presentation.

    The aim of the study was to evaluate post-polymerization of resin composite by measuring NanoHardness (H), Young’s Modulus (E) and Degree of Conversion (DC) using nanoindentation and Micro-Raman spectroscopy. For this purpose a computer-controlled NanoIndenter™ and a Renishaw 1000 Raman Spectrometer fitted with an Olympus microscope attachment, operated at 638 nm, were used. A light-activated resin composite was used in this study, (Z250, 3MESPE). Sub-groups (n=3) of specimens were irradiated for 20, 30, 40 s. All samples for nanoindentation were polished metallographically and typically 50 nanoindentations were performed per specimen. After curing and polishing, half of the samples were tested immediately (Group 1); the others after being stored dry at 37 °C for 7 days (Group 2) to allow scope for postpolymerization. H values ranged from 1.08 to 1.40 GPa for Group 1, and from 1.64 to1.71 GPa for Group 2. E values in Group 1 ranged from 19.60 to 19.94 GPa and for Group 2, from 21.42 to 22.05 GPa.     Polymorphism in PAX9 (rs8004560), a gene responsible for craniofacial and tooth development, is often associated with Class II/Div2 malocclusion. This study aimed to detect the presence of PAX9 SNP (rs8004560) and to determine its genotype and allele distribution in Class II skeletal base malocclusion, contributed by retrognathic mandible, in the local Malaysian population. The association of PAX9 SNP (rs8004560) with Class II skeletal base malocclusion was also determined. A case control study was performed on 30 samples; 15 from Class II skeletal base malocclusion, and 15 from Class I skeletal base subject as control. Cephalometric measurements were performed prior to saliva samples collection. Genomic DNA was extracted from unstimulated saliva of all subjects, and the DNA was amplified using specific primers for marker rs8004560, followed by genotyping by sequencing. SHEsis online software was used to analyse Hardy-Weinberg Equilibrium (HWE) for cases and controls. Allelic and genotypic frequencies were compared between cases and controls. Significant difference in allele frequency was observed within the group whereby G allele was over-represented in the analysed population (p0.05). Although no genetic association between PAX9 SNP (rs8004560) with Class II skeletal base malocclusion was observed, significant difference in allele frequency observed might provide some indication in the involvement of PAX9 polymorphism in Class II skeletal base malocclusion contributed by retrognathic mandible. Further research utilising larger sample size will be required in order to determine the role of PAX9 gene in the aetiology of Class II skeletal base malocclusion observed in the local Malaysian population.

    Root canal treatment (RCT) requires high level of technical skills of the dentist. Its outcome is an important part of evidence-based practice and become the basis of treatment planning and prognostic considerations. Adequate removal of micro-organisms and prevention of recolonization of residual micro-organisms through the placement of root filling with satisfactory coronal seal ensures success. This retrospective record review study aimed to investigate the practices of RCT in Hospital Universiti Sains Malaysia (HUSM) Dental Clinic, Kota Bharu, Kelantan. It involved 333 randomly selected patient records at the HUSM Record Unit. Data was obtained by careful analyses of daily treatment progress sheets and analyzed using SPSS version 12.0. A total of 2996 RCT cases were seen and 59.8% of patients were females. The age range of patients varied from 14 to 64 years. The maxillary anterior teeth were most commonly treated (52.6%). Most operators (99.1%) used step-back technique and 97.6% used files to prepare root canals. The most commonly used material for obturation and sealing was gutta-percha and epoxy resin-based sealer (AH26). About 82.9% used calcium hydroxide as intra-canal medication. About 25.5% of cases had no periapical pathology, 65.8% with pre-existing periapical radiolucencies healed in 1-3 months whereas 2.1% of cases with periapical pathology eventually healed after a year. About 6.9% cases failed after retreatment. The number of radiographs taken was two to four pieces. RCT is a useful intervention to maintain longevity of teeth. Decision making and current updates of methods and materials are essential among practitioners as well as administrators to ensure success.

    Human salivary exosomes have been identified as a highly informative nanovesicle with clinical-relevant information for variation of diagnostic purposes. As a continued effort from previous studies on human salivary exosomes effect at gene expression level, this study is carried out to observe the morphology of human periodontal fibroblast (HPdLF) treated with exosomes cells under the same period of changes in genotypic level occurred. In vitro, HPdLF cells were cultured for 24 hours with 10 μg/ml of human salivary exosomes. The morphology of HPdLF cells was examined under inverted light microscopy and scanning electron microscopy (SEM) for both control samples and samples treated with human salivary exosomes, while the cell count was performed via trypan blue staining. There was no significant difference in the morphology under the inverted light microscopy and the cell number of HPdLF cells for both treated and untreated cells with exosomes. However, for SEM, the treated HPdLF with salivary exosomes showed slight observable changes on the filopodia, lamellipodia, cytoplasmic vesicles and the cytoskeleton of the cells. Even within a short period (24 hours) of culturing time for cells with human salivary exosomes, the samples showed minimal changes which positively suggested a simultaneous event of exchanging materials from human salivary exosomes to cells had occurred, hence, potentially proving that human salivary exosomes can enhance cell proliferation.

    Chronic periodontitis (CP) is an inflammatory disease of the supporting tissues of the teeth caused by specific microorganism. Hypertension is one of the major causes of cardiovascular disease whereas periodontitis has recently drawn increasing attention because of its potential relationship with cardiovascular disease. The objectives of this study were to determine the prevalence of the hypertension in CP patients as well as to evaluate the association between hypertension and severity of CP. One hundred and eighty five records of CP patients treated in Dental Clinic, Universiti Sains Malaysia Hospital (USM Hospital), Kelantan from 2010 until 2013 were retrieved and reviewed. The diagnosis of periodontal disease and the presence of hypertension were recorded. The severity of chronic periodontitis was classified into mild, moderate and severe according to the clinical attachment loss. The data was obtained and analyzed using SPSS version 20.0. Majority of the subjects were from Malay ethnic group (94.4%) at the age range between 41 and 61 years (67.8%). In conclusion, the prevalence of hypertension in chronic periodontitis patients was 12.2%. There was no significant association between hypertension and severity of CP (p=0.229).

    The aim of this study was to identify the position of impacted mandibular third molars based on the classifications of Pell & Gregory and Winter, the indications for extraction, and the relation of post-operative complications and position. Records of patients who attended Hospital Universiti Sains Malaysia between January and December 2007 for surgical removal of mandibular third molars were reviewed. The angulation type, width and depth of impaction were determined by reviewing the orthopantomograms. The indications of extraction and occurrence of any post- operative complications were recorded. A total of 238 impacted teeth were surgically extracted from 194 patients (97 males, 97 females). The reasons for extraction include recurrent pericoronitis (43.1%) followed by prophylactic purposes (33.5%). Mesioangular impactions accounted for 52.3% and Class IIA position of impaction accounted for 45.7% of extractions. The most common post-operative complication was persistent pain and swelling (14.7%) followed by trismus (4.1%) and dry socket (3.0%). There was no significant relationship between the angulation, width and depth of impaction and the occurrence of complication. Mesioangular type and Class IIA position of impaction were the most common impaction. Although the association was not significant, high frequency of post-operative complications was observed in mesioangular, horizontal, IIA and IIC positions.

    Clinacanthus nutans (C. nutans), a well-known ethnopharmacological plant consumed for its medicinal purposes by Southeast Asian communities. C. nutans is said to possess antipyretic, inflammatory, antiedemic as well as analgesic properties and used traditionally in treating various skin ailments, Herpes infection, cancer and diabetes. The young leaves of this C. nutans are consumed in Malaysia for maintaining health. In this study, the proliferative activity of human gingival fibroblast cells (HGF-1, ATCC®CRL-2014™, USA) treated with the ethanol extract obtained from C. nutans leaves at three different concentrations (250, 125 and 62.5 µg/ml) was compared with the untreated cells using alamarBlue assay. The proliferative activity of HGF-1 using alamarBlue assay showed that the cells treated with 62.5 μg/ml of ethanolic extract of C. nutans leaves exhibited increased proliferation compared to the other groups and hence does not exhibit any cytotoxicity on HGF-1.

    Infection of the oral cavity with high-risk human papillomavirus (HPV) has been implicated as one of the risk factors for the development of oral squamous cell carcinoma (OSCC). Among the high-risk HPV types, HPV 16 and 18 are the most common infective agents in oral cancers. This study aimed to compare the presence of high-risk HPV in genetic materials obtained from saliva, blood and tissues of OSCC patients in Malaysia. The genomic DNA was extracted from saliva (n=13), blood (n=59) and tissue (n=63) and subjected to polymerase chain reaction (PCR) amplification of human beta globin gene to confirm the presence and integrity of DNA. Positive amplification was then screened for high-risk HPV by nested PCR using MY11/09 and GP5+/6+ consensus primers, followed by a further confirmation by DNA sequencing of the positive samples. As a result, two saliva samples (2/13; 15.4%) were found to harbour HPV 16 and one tissue sample (1/63; 1.6%) was shown to be positive for HPV 18. However, none of the blood samples were positive for high-risk HPV. Thus, HPV is more likely to be found in the saliva of OSCC patients as compared to blood and tissue samples. The detection of high-risk HPV in OSCC patients is useful in deciding how to manage the patient as HPV-associated OSCC has better prognosis.

    Rubinstein-Taybi syndrome is a multiple anomalies congenital disorder characterised by broad thumb and halluces, facial dysmorphism with mental and growth retardation. Oral features include small mouth, retro and micronagthic jaws, highly arched and narrow palate. Dental anomalies such as teeth with talon cusps and screwdriver shaped permanent incisors together with crowded teeth are common features in these patients. Although hyperdontia is said to be one of the features of this condition, nevertheless, presence of multiple supernumerary teeth has never been documented. This report highlighted a case of an eleven-year-old boy with Rubinstein-Taybi syndrome referred for unerupted permanent incisors who exhibited multiple supernumerary teeth radiographically.

    The present study compared and evaluated the shear bond strength (SBS) of two types of glass
    ionomer cement (GIC), Riva Self Cure HVTM (SDI Ltd., Victoria, Australia) and GC Fuji IX GP EXTRATM (GC
    America Inc., Alsip, USA) with and without the use of Riva Bond LCTM (SDI Ltd., Victoria, Australia), a lighted
    cured resin-modified glass ionomer cement (RMGIC) universal adhesive. Sixty extracted sound premolars with
    prepared exposure of the dentine on the occlusal surface were randomly assigned into four groups according to
    the tested restorative materials. Shear bond strength (SBS) tests were performed by using the Shimadzu
    Universal Testing Machine at a crosshead speed of 0.5 mm/minute, and the values obtained were statistically
    analysed using one-way ANOVA and Tukey tests. The inter-group comparison showed statistically significant
    differences in the SBS values between all the test groups (p < 0.001). A stereomicroscope was used to assess
    the modes of failure. Adhesive failures were predominant in adhesive groups (>80%) compared to higher
    cohesive failures found in the non-adhesive groups (>86%). A Spearman's rho correlation test performed to
    determine the association between SBS values and mode of failures had indicated positive correlations between

    This is a cross-sectional study in a representative sample of preschool children from 12 preschools (TADIKA KEMAS) Pasir Mas, Kelantan, Malaysia. Data on socioeconomic status and sources of water supply at home were collected through interview with mothers. Children’s anthropometric data (height and weight) and body-mass-index-for-age (BMI-for-age) was calculated. Oral examinations of ECC status was based on the dmft index (WHO, 1997).     carious teeth were very high (dmft 11.1±4.8) and almost every preschool child was affected with ECC (prevalence 98.1%).     The aim of this study was to determine the efficiency of different human amniotic membrane (HAM) processing methods on the concentration, purity and integrity of RNA. Two different techniques (Technique1 andTechnique2) were employed for the processing of HAM, which differed in terms of washing solution, sample storage conditions and processing time. Based on preservation of HAM, three groups were formed under each technique. In Technique 1, the groups were fresh frozen 1 (F1), glycerol preserved (GP) and gamma irradiated glycerol preserved (IGP); where else in Technique 2, the groups were fresh frozen 2 (F2), 50%glycerol/Dulbecco’s modified Eagle medium (DMEM) cryopre served HAM diluted with phosphate buffered saline(GB) and 50% glycerol/DMEM cryop reserved HAM diluted with diethyl procarbonate water (GD). Total RNA was extracted from the samples and their concentration, purity and integrity were examined. The F2 sample of which there was no pre-washing step and involved direct sample storage at-80oC, shorter processing time and chilled processing conditions had yielded better quality of RNA compared to the others.

    Dental caries and gingivitis are common oral health problems affecting schoolchildren worldwide. Effective tooth brushing in children is fundamental in preventing dental caries and gingivitis and maintaining good oral health. Children routinely present with unsatisfactory oral hygiene due to poor compliance and poor dexterity in tooth brushing. The purpose of the present study was to evaluate the effectiveness of a novel T-shaped toothbrush in plaque removal and gingival health among children and to obtain feedback regarding its use. Nineteen participants aged 8-10 years who fulfilled the criteria enrolled in this study. The children were taught the proper technique of using the new toothbrush and instructed to use it for a period of 2 weeks. The plaque scores and gingival scores were measured at baseline and after 2 weeks. They were asked to complete a questionnaire regarding the use of the toothbrush after 2 weeks.     Use of synthetic hydroxyapatite (HA) in biomedical applications is well warranted. It has shown to have an excellent biocompatibility in human tooth and bones. Additionally it has been documented to possess antibacterial potentials. The present study was conducted to assess the presence of any such potential in locally produced (HA) using Streptococcus mutans, a common pathogen in the oral cavity. The study was carried out using 50, 100, 150, 200, 300, 400 and 800 mg/ml concentration of HA. The antibacterial property of HA was assessed using Miles and Misra method. Our studies showed that bacterial growth inhibitions of S. mutans occurred from 50 mg/ml, and complete inhibition was perceived at concentrations at 200mg/ml of HA. The antibacterial property HA should be used to good advantage as a bioactive biomaterial in dental and maxillofacial applications.
