    The main objective of a root end filling material is to provide an apical seal that prevents the movement of bacteria and the diffusion of bacterial products from the root canal system into periapical tissues. The aim of this study was to compare the microleakage of three root end filling materials Mineral trioxide aggregate (MTA), Glass ionomer cement (GIC) and Silver GIC (Miracle Mix) using dye penetration technique under stereomicroscope. Forty-five extracted human maxillary central incisors were instrumented and obturated with gutta percha using lateral compaction technique. Following this, the teeth were stored in saline. After one week, teeth were apically resected at an angle of 90ï° to the long axis of the root and root end cavities were prepared. The teeth were divided into three groups of fifteen specimens each and were filled with Group I -MTA, Group II - GIC and Group III - Miracle Mix. The samples were coated with varnish and after drying, they were immersed in 1% methylene blue dye for 72 hours. The teeth were then rinsed, sectioned longitudinally and observed under stereomicroscope. The depth of dye penetration was measured in millimeters. Microleakage was found to be significantly less in MTA (0.83 mm) when compared to GIC (1.32 mm) (p < 0.001) and with Miracle Mix (1.39 mm) (p < 0.001) No significant difference was found when microleakage in Miracle Mix was compared to that of GIC (p = 0.752). Thus we concluded that MTA is a better material as root end filling material to prevent microleakage, in comparison to GIC and Miracle Mix.

    Composite resins Amaris is claimed to have hydrophobic effect which minimizes the staining intake. This study is to investigate the colour stability of Amaris compared to Filtek Z250 in coffee solution. Sixty discs of composite resins Filtek Z250 (3M ESPE) and Amaris (Voco) with diameter of 5mm and depth of 2mm were fabricated by packing in a drinking straw and sectioned with hard tissue cutter (Exakt, Japan). The surfaces of the specimens were polished with Sof-Lex disc before each group of the samples is immersed in coffee solution. They were kept in the solution for 4 days at 370C and assessed at the period of 2 hours, 1 day, 2 days, 3 days, and 4 days.     Sialolithiasis is one of the commonest disease of salivary glands. Submandibular salivary gland
    or its duct is a major site of sialolithiasis and it is always leads to sialadenitis. However large sialolith
    formation and self-extrusion is a rare condition. A case of submandibular sialolithiasis is reported where
    patient presents with recurrent submandibular swelling and subsequently end up with a spontaneous
    extrusion. The mechanisms and management of the salivary gland calculi are also discussed.

    Intraclass correlation coefficient (ICC) measures the extent of agreement and consistency among
    raters for two or more numerical or quantitative variables. This review paper aimed to present several tables that
    could illustrate the minimum sample sizes required for estimating the desired effect size of ICC, which is a
    measurement of the magnitude of an agreement. Determination of the minimum sample size under such
    circumstances is based on the two fundamentally important parameters, namely the actual value of the ICC and
    the number of observations made by each subject. The sample size calculations are derived from Power Analysis
    and Sample Size (PASS) software where the alpha and minimum required power is fixed at 0.05 and higher than
    0.80 respectively. A discussion on how to use these tables for determining sample sizes required for each of the
    various scenarios and the limitations associated with their use in each of these scenarios is provided.

    Light intensity output is one of the determinants for adequate curing of visible light-cured materials. The aim of this survey was to evaluate the light intensity outputs (LIOs) of light curing units (LCUs) in dental clinics of Hospital Universiti Sains Malaysia (HUSM) and School of Dental Sciences, Universiti Sains Malaysia (USM). The respective LIOs of all functioning Quartz Tungsten Halogen (QTH) and Light Emitting Diode (LED) LCUs were tested using two light radiometers.     Prosthodontic rehabilitation and management of microstomia patient presents challenges at all stages
    during prosthesis fabrication; from making primary impressions to the prostheses insertion. These patients have
    reduced mouth opening, and hence it can be extremely challenging to make impressions and to fabricate
    dentures using the conventional methods. The present case report describes prosthodontic management of a
    partially dentate patient with microstomia that developed secondary to surgical resection and radiation therapy of
    head and neck cancer. A simplified novel approach has been advocated using the patient’s existing metal
    removable partial denture as a key in master cast preparation.

    Oral cancer is one of the common cancer cases identified in the developing countries. Genetic mutation and overexpression of certain genes and proteins have been associated in the development of this cancer. Notch signalling pathway is normally involved in controlling the development process of vertebrates and invertebrates; however, deregulation of this pathway was found to be responsible in the formation of certain cancers including oral cancers. Activation of this pathway requires binding of the ligands to its receptors. Four NOTCH receptors (NOTCH 1, 2, 3 and 4) have been identified in mammals. Disruptions within these molecules might interfere with the normal functions of Notch signalling pathway. Hence, this study was conducted to detect mutations of NOTCH1 and NOTCH2 receptor genes which might be occurring in the oral cancer cases obtained from the local population. DNA extracted from fresh-frozen tissue biopsy of the tongue and buccal mucosa from 10 confirmed cases of oral cancer were subjected for polymerase chain reaction (PCR) amplification using the specific sets of primers. The PCR products were sent for sequencing before final results were analysed.
    Due to time and cost limitation, only two out of four NOTCH receptor genes; NOTCH1 and NOTCH2, were used in this analysis. The results revealed absence of nucleotide changes for both NOTCH receptor genes amplified from these oral cancer samples. More samples and further analysis looking into other regions in these genes are required to conclude the involvement of NOTCH receptor genes mutation in causing oral cancer.

    The practice of quack dentistry worldwide is a challenge to the profession of dentistry as it undermines the trust of the general public and places the patients at unnecessary risks through their illegal practices. Although the ratio of patients to dentists is rapidly decreasing in Malaysia due to the increased output of qualified dentists from both local and foreign institutions, quack dentistry is still practiced in many areas. This report intends to highlight a popular treatment prescribed by these quack dentists which is removable partial dentures that is fixed to the adjacent teeth by cold cure acrylic. We also aim to discuss the design of the denture and the means employed to remove this ’fixed partial denture’. It is hoped that this case report will highlight the prevalence of the practice of quack dentistry especially in geographical area of this case report and allow the authorities to devise means to stop this illegal practice.

    Gingival overgrowth is frequently observed in patients taking certain drugs such as calcium channel blockers, anticonvulsants and immunosuppressant. This can have a significant effect on the quality of life as well as increasing the oral bacterial load by generating plaque retention sites. Amlodipine, a third generation calcium channel blockers has been shown to promote gingival overgrowth although in very limited cases reported. The management of gingival overgrowth seems to be directed at controlling gingival inflammation through a good oral hygiene regimen. However in severe cases, surgical excision is the most preferred method of treatment, followed by rigorous oral hygiene procedures. This case report describes the management of gingival overgrowth in a hypertensive patient taking amlodipine. Combination of surgical gingivectomy and CO2 laser treatment was used to remove the gingival overgrowth. CO2 laser surgery produced good hemostasis and less pain during the procedure and post operatively. This case report has also shown that periodontal treatment alone without a change in associated drug can yield satisfactory clinical response.

    A study was done to assess and compare the levels of infection control practices amongst Year
    3, 4 and 5 undergraduate dental students at the Faculty of Dentistry, University of Malaya, Malaysia. This
    study also compared the levels of infection control practices between the operators and the assistants
    performing a similar task. A checklist was designed based on the Infection Control Guidelines published by
    the Faculty of Dentistry. The audit checklist criteria were specific to the operators and assistants or common
    to both. 10 pairs of students (operators and assistants) were randomly selected from Polyclinics A, B and C,
    representing Year 3, 4 and 5 students respectively (n=60). The subjects were audited as soon as they
    entered the clinic, during treatment and up until they exited the polyclinic. The data collected were analysed
    using SPSS and Rasch model. Year 3 students performed better infection control practices (1.43 logit)
    followed by Year 5 (0.96 logit) and Year 4 (0.94 logit) students. The operators in Year 5 and the assistants
    in Year 3 and 4 were more compliant to infection control practices. 100% of students complied with removal
    of gown before leaving the clinic. However, only 1.7% of dental students wore protective goggles or face
    shield during treatment and disinfected prostheses before and after inserting into patient’s mouth.
    Introduction and reinforcement of infection control practices should be incorporated at the beginning of each
    academic year for all students to allow for better practice and delivery of treatment to patients.

    Osteomyelitis of the mandible, a serious complication of untreated odontogenic infection has been reported. This case report describes an interesting presentation of chronic suppurative osteomyelitis (CSO) of the mandible in a 13 years old anaemic male patient. Investigations revealed inversion of his permanent teeth leading to trans-cutaneous extra-oral eruption along with marked destruction of mandible on the affected side. The treatment included a pre surgical course of antibiotics followed by the removal of the retained second premolar tooth, surgical debridement of the affected bone, and resection of the cutaneous sinus tract. The post-operative healing was uneventful. A combination of antibiotic therapy and surgical debridement were effective in the treatment of chronic suppurative osteomyelitis.

    Assault injury can occur to any parts of the body. Usually the target area is the head, neck, chest and abdomen as they contain many major structures that are vital for life. However, movement of the victim few seconds before the instrument reach the body can redirect the impact to the less fatal part of the body.     Use of synthetic hydroxyapatite (HA) in biomedical applications is well warranted. It has shown to have an excellent biocompatibility in human tooth and bones. Additionally it has been documented to possess antibacterial potentials. The present study was conducted to assess the presence of any such potential in locally produced (HA) using Streptococcus mutans, a common pathogen in the oral cavity. The study was carried out using 50, 100, 150, 200, 300, 400 and 800 mg/ml concentration of HA. The antibacterial property of HA was assessed using Miles and Misra method. Our studies showed that bacterial growth inhibitions of S. mutans occurred from 50 mg/ml, and complete inhibition was perceived at concentrations at 200mg/ml of HA. The antibacterial property HA should be used to good advantage as a bioactive biomaterial in dental and maxillofacial applications.

    Discovery of drugs from medicinal plants continues to provide major leads against various
    pharmacological targets, particularly in cancer diseases. Hence, there are increasing demands to discover
    more therapeutic agents from various species of medicinal plants. Chemical compounds in plants are
    important for human beings due to their therapeutic properties. Goniothalamus umbrosus, Typhonium
    flagelliforme, Myrmecodia pendens, Strobilanthes crispus and Clinacanthus nutans, are among the herbal
    species, which are consumed by cancer patients in order to combat against the growth of cancer cells. The
    present review aims to highlight on the anti-cancer properties of the listed Malaysian herbs.

    Fungal ball is an extramucosal mycosis. The patient may present with facial pain, nasal blockage, purulent nasal discharge and cacosmia, the fungal ball being present unnoticed for years. Some patients do present as having other nasal problems and later on are found out to have a fungal ball incidentally. We present a case of 38 yearold man who was clinically diagnosed as having left antrochoanal polyp. Intraoperatively, a fungal ball was discovered in the left maxillary antrum.

    Lasers were introduced into the field of clinical dentistry with the hope of overcoming some of the drawbacks posed by the conventional methods of dental procedures. Since its first experiment for dental application in the 1960s, the use of laser has increased rapidly in the last couple of decades. At present, wide varieties of procedures are carried out using lasers. The aim of this review is to describe the application of lasers in dental hard tissue procedures. Lasers are found to be effective in cavity preparation, caries removal, restoration removal, etching, and treatment of dentinal sensitivity, caries prevention and bleaching. Based on development in adhesive dentistry and the propagation of minimum intervention principles, lasers may revolutionize cavity design and preparation.

    Some restorative materials are susceptible to erosion but whether it also causes microleakage is still questionable. The aim of this study was to assess the microleakage of few restorative materials after immersion in acidic solution. Standardized ‘U’ shaped cavity of 4mm diameter and 2mm depth were prepared on buccal or lingual surface of 52 human premolar and molar teeth. The teeth were divided into 4 groups which contains 13 samples and 3 controls for each and were restored either with Filtek Z250 (Group 1), Fuji IX (Group 2), Fuji II LC (Group 3), or Silverfill amalgam (Group 4).
    All surfaces were painted with nail varnish leaving only 2mm of tooth structure surrounding the restoration before the study samples were immersed in acidic solution, lemon juice (pH 2.74) and control samples in deionised distilled water for 24 hours. Surface photos for erosion were taken before immersion in methylene blue for 7 days. After sectioning, the assessment of dye penetration was done using Leica Imaging System DMLM (Germany). Photos showed that Fuji IX demonstrated severe erosion but no obvious changes were seen on other materials.     This study was to investigate the association between functional dentition with inadequate calorie intake and underweight in elderly people living in “Pondok” in Kelantan. A cross-sectional study participated by 369 elderly people was carried out. A simple random sampling method was utilized for the selection of the subjects. Subjects were interviewed using a structured form to collect information about subjects’ demographic background and self-reported health status. The 24-hour recall method was used to provide information on the subject’s exact food intake during the previous 24 hours. Body mass index was calculated from subjects’ weight and height, followed by oral assessment. The “Nutrical Software” was used for nutrient calculation, followed by SPSS version 11.5 for data analysis. The response rate was 95.6%. Majority of the respondents were female (92.4%). The percentage of subjects who had inadequate calorie intake and underweight was 71.0% and 25.7% respectively. The percentage of edentulism was 81.0%. The proportion of elderly with compromised functional dentition was 48.2% and majority was edentate elderly without wearing any complete dentures or wearing defective dentures, 23.6% and 56.2% respectively, while 20.2% was dentate elderly. The odds of having inadequate calorie intake and getting underweight among elderly with a compromised functional dentition was 3.7 times and 42.0 times respectively, compared to elderly with a non-compromised functional dentition. There was a significant association between inadequate calorie intake and underweight with functional dentition among elderly living in “Pondok” in Kelantan.

    This is a cross sectional study to determine the attitudes and practices in smoking cessation counselling among dentists in Kelantan and to identify the barriers that prevent them from doing so. Self-administered questionnaires were distributed to 152 dentists practising in Kelantan. A total of 84 (55.2%) questionnaires were completed and returned. Although majority of the dentists (98.8%) agreed that they have a role in smoking cessation counselling, only few of them (17.9%) were actually involved in a considerable extent. The main barriers cited were lack of training and time in their practice. In conclusion, dentists in Kelantan believed that they have a role in smoking cessation counselling but their involvement was rather limited.
