    1. Revised ARA criteria (1987) the diagnosis of RA requires presence of at least 4 of the following 7 features: 1) morning stiffness> 1 hour 2) swelling of 2 or more joints (of 6 weeks duration) 3) symmetrical joint involvement 4) arthritis of more than 1 hand joints (of at least 6 weeks duration) 5) positive rheumatoid factor, 6) presence of rheumatoid nodule 7) X-ray changes. 2. Use of DMARD should start early, along with pain control by non steroidal anti inflammatory drugs (NSAID) 3. Early referral to rheumatologists is the current recommendation 4. Indications for early referral include: i. Early morning stiffness (EMS) of 30 mins ;- ii. Metatarsophalangeal (MTP) / metacarpophalageal (Mep) involvement. iii. 2 or more swollen joints.

    A serosurvey of rubella antibody among nursing personnel was carried out. 452 blood samples were received from a total staff of 829. 65% was found to be immune by the radial haemolysis method. The highest immune rate was in the intensive care unit followed by maternity, paediatric and gynaecology wards. The lowest rate was in the ophthalmology ward. This study shows that there is lack of compliance among medical personnel and the high immune rate is compatible with the occupational risk of contact with rubella.

    A 13-year old Malay girl presented with multiple parasuicide attempts. She made multiple attempts at suicide and at the last attempt she developed liver damage. This article discusses the etiology and different methods used to prevent future suicidal attempts in this patient.

    The validity of the 2-hour post-prandial (2HPP) urine glucose was determined among a Malay community in Kuala Selangor. Fifty glycosuric and 80 non-glycosuric samples had their capillary blood glucose and fasting blood sugar tested to determine their diabetic status. It was found that the test was very sensitive but less specific (sensitivity 100% and specificity 72.1%). Samples with 2HPP urine glucose of 3 + had a higher probability of being diabetic than the others.

    A prospective case controlled study was conducted to determine the incidence of asymptomatic bacteriuria in pregnancy (ASB), causative organisms and fetal outcome. Out of 510 antenatal mothers screened, 46 (9%) had ASB. The main organisms isolated were E. coli (16%), Staphylococcus aureus (15%) and Beta-Haemolytic Streptococcus (11%). Oral Nitrofurantoin and Cefuroxime axetil achieved sensitivity levels of 96%. There was no significant difference in period of gestation at birth and birthweight between the control and study groups. The reasons for routine antimicrobial therapy for ASB are discussed.
