    We describe the results of serology for parasitic infection of 698 foreign workers. The 698 foreign workers participated included 115 Indonesians, 387 Bangladeshis, 101 Burmese, 81 Pakistanis, 6 Indians, 3 Thais, 3 Filipinoes and 2 others. Blood samples were taken from these workers and eight tests (Amoebiasis, Echinococcosis, Filariasis (bm and wb), Leishmaniasis, Malaria, Schistosomiasis and Trypanosomiasis) were performed on serum separated from the blood. Among the 698 sera tested, 38.1% were found to be positive for at least one parasitic infection. The most common antibody detected in the positive sera was antibody for amoebiasis (28.1%), followed by malaria (26.9%), echinococcosis (18.1%) and schistosomiasis (11.6%). Other tests showed a low percentage of infection with leshmaniasis (6.5%), filariasis (Brugia malayi (3%) and Wuchereria bancrofti (1%)) and trypanosomiasis (1%).

    We describe the results of serology for parasitic infection of 250 foreign workers who were seen at the University of Malaya Medical Centre, UMMC during 7-months period. The 250 foreign workers participated included 114 from Indonesia, 142 from Bangladesh, two from Myanmar and two from Pakistan. Blood samples were taken from these workers and eight tests (amoebiasis, echinococcosis, filariasis, leishmaniasis, malaria, schistosomiasis, toxoplasmosis, and trypanosomiasis) were performed on serum. Among the 250 sera tested, 92 (36.8%) were found to be positive for at least one parasitic infection. There was one case where the serum was found positive for 5 tests. The most common antibody detected in those positive sera was antibody for toxoplasma (80.%), followed by filaria (32.8%) and amoeba (30%). Other tests showed low percentage of infection with schistosomiasias, 10%; echinococcosis, 6% and malaria, 3.6%. None of the foreign workers were found positive for leishmaniasis or trypanosomiasias.

    During the period 1996-1998, 134 patients suspected of having ocular toxoplasmosis were seen in the Ophthalmology Clinic of the University Hospital, Kuala Lumpur. Clinical presentations in these patients ranged from poor vision to severe retinal detachment. Of these patients, 72% were confirmed positive for Toxoplasma gondii infection by serological methods. Chorioretinjtis and vitritis were found to be the most apparent symptoms, both having 100%correlation with serological positivity, This was followed by uveitis, floaters, and retinal detachment with correlation at 78%, 75%and 75%, respectively.     A survey of malnutrition and helminth infections among 268 pre-school children living in the Kuala Betis Orang Asli resttlement villages in Kelantan. The prevalence of helminth infections was 47.4% with Ascaris lumbricoides being the most common helminth (43.9%), followed by Trichuris trichiura (29.7%) and hookworm (6.3%). The prevalence of Ascaris lumbricoides and Trichuris trichiura infections increased with age, with the highest prevalence found in the 6-7 years age group. The overall prevalence of stunting, underweight and wasting were 61.7%, 60.4% and 17.5% respectively. Both stunting and underweight were significantly higher among the infected children. Factors associated with helminth infections in the pre-school children were older age group, poor water supply and households with more than 5 members. Rountine regular deworming is recommended based on the World Health Organisation recommendations for school children.

    Six patients underwent transcatheter closure of patent ductus arteriosus (PDA) using either conventional Gianturco coils or the Amplatzer Ductal Occluder (ADO) device. All patients were females with a median age of 23.3 (range 4 to 26 years). The mean PDA size measured on the lateral aortogram was 3.81mm (range 2.3-5.831nm).     Objective: The study was designed to determine the risk perception of the public population in Kota Kinabalu towards childhood immunisation.
    Methodology: This was a cross-sectional study where self-administered questionnaires were distributed to the public in Kota Kinabalu. The respondents who consented were 18 years old and older. The illiterate persons and the foreigners were excluded. The calculated sample size was 400. Only 313 samples collected were suitable for analysis using SPSS v21.0.
    Results: Fever, pain swelling and allergic reaction were correctly identified as risks of immunisation. Autism, mental retardation and even death could also result from vaccination. Of the total respondents, 76.7% agreed with the practice of childhood vaccination, 70.0% thought that childhood vaccinations were safe and effective and 58.1% felt that its benefit outweighed the risk. Other than that, 32.0% refused childhood vaccination from fear of its risks, and this fear was the main reason for the refusal of childhood vaccination. Age, marital status, race and income were the factors which influenced parental willingness to vaccinate their children (p<0.05).
    Conclusions: Most participants understood the risks of childhood vaccination, but few agreed to the practice of childhood immunisation for herd immunity. Public health campaigns are needed, to increase the understanding and acceptance of childhood vaccination especially in the rural community.

    Objectives: The objectives of our study were to determine the mean age of onset of thelarche and menarche,
    to determine the ages of each stage of puberty and to investigate if there are any ethnic differences in these
    ages amongst female adolescents in Kuala Lumpur, Malaysia.
    Methods: This was a cross-sectional study involving three secondary schools in Cheras, Kuala Lumpur. A total of
    730 students were recruited via stratified random sampling. Self-administered survey forms on menstruation
    and puberty were used.
    Results: Out of 730 recruited respondents, 385 (52.74%) completed the survey and informed consents were
    obtained from their parents/legal guardians. Our respondents’ ages ranged between 13 to 17 years old with
    the median (IQR) age of 14.00(2.00) years. With regards to ethnicity, 329 (85.50%) were Malay, 46 (11.90%)
    were Chinese, 9 (2.30%) were Indian and 1(0.30%) was a Bajau. The mean age for breast development
    (thelarche) and menarche were 11.72 ±1.10 and 11.97±1.11 years respectively. The majority, 363 (94.30%),
    out of 385 respondents had attained menarche. There appeared to be a later onset of puberty amongst
    Malays comparatively.
    Conclusion: The mean ages of pubertal onset and menarche amongst female adolescents in the Cheras area
    are similar to those reported worldwide.

    This is a retrospective case control study conducted between 1 January 2005 and 30 June 2006 at the Hospital Tuanku Jaafar, Seremban, Malaysia. The objective was to study the maternal sociodemographics and foetal characteristics associated with premature delivery. There were 387 cases selected universally and 387 controls selected by using systematic random sampling involving every 13 cases. The patients were women who had delivered their babies before 37 completed weeks, while the control were women who had term deliveries during the same period. Data were collected using structured questionnaire through secondary data. Results showed that having multiple pregnancies (OR=8.33, 95% CI: 2.91, 23.84), congenital abnormality (OR=4.6, 95% CI: 0.98, 21.84) and intrauterine growth retardation (OR=15.59, 95% CI: 3.69, 65.82) were the risk factors of premature delivery. Being an ethnic Indian also raised the odds (OR=1.67, 95% CI: 1.14, 2.43) but this was not significant in the multivariate analysis. Other sociodemographic characteristics did not contribute significantly to the risk factors for premature delivery in this sample. In conclusion, foetal characteristics were found to be significantly associated with premature delivery after adjustment for other confounding factors.

    A case of chronic fatigue syndrome (CFS) like illness was identified recently. Diagnosis CFS is commonly used in the western countries but not in Malaysia or other parts of Asia. It is probably because the diagnosis of neurasthenia has gradually disappeared especially in the United States and United Kingdom. Neurasthenia is dropped in the DSM-111 and DSM-1V but is still retained in the 10th International Classification of Disease. This paper is reporting the case and discussing the definition of chronic fatigue syndrome. (JUMMEC 2000; 2:103-104).

    Depression is a frequent companion of stroke. It is associated with a degree of functional impairment that might adversely affect rehabilitation. A study was carried out to determine the rate of self-reported depression at admission and after six months, and the associated factors, in patients with stroke who were referred for rehabilitation. A multiple binary logistic regression was used to determine the associated factors at six months. Depression was recorded in 45% of patients at admission and in 39% at six months after the stroke. Patients who were employed, those who were depressed at admission, those who did not return to work and those who did not return to driving six months after stroke, were significantly associated with depression.

    Refractory dermal melasma is resistant to conventional treatment. Platelet rich plasma (PRP) may help to reduce the pigmentation of melasma. We present a case report on the clinical outcome of 2 patients with melasma, given PRP, as an adjunct therapy. PRP was administered at a monthly interval for 2 sessions in combination with a monthly Q-switched Nd Yag laser treatment and topical alpha arbutin application. A modified melasma area and severity index (MASI) was evaluated by two dermatologists who were blinded. At the follow up on the 3rd months, the MASI score was reduced by mean 33.5% for case 1 and 20% for case 2. There were no clinical complications for case 1. However recurrence of melasma was noted in case 2 by a worsening of the MASI score mean to 53% at the sixth months follow up. In conclusion, intradermal PRP injection as an adjunct to the conventional treatment of melasma presented with differing results in two cases.

    Porto-spleno-mesenteric vein thrombosis is a rare, life-threatening condition of extrahepatic portal venous
    system thrombosis. We report a rare case of a 49-year-old lady with late presentation of acute portal vein
    thrombosis in a non-cirrhotic liver with an incidental finding of left adnexal teratoma. She presented with a
    one-week history of severe abdominal pain associated with vomiting and diarrhea. She gave no history of prior
    risk for venous thromboembolism or liver diseases. Physical examination revealed a tender mass extending from
    suprapubic to left iliac fossa. Abdominal computed tomography scans showed a well-defined fat-containing left
    adnexal mass, likely a benign teratoma, with no involvement of surrounding structures or calcification. There
    was evidence of porto-splenic-mesenteric vein thrombosis with liver infarction, bowel and splenic ischemia.
    Management of the extensive thrombosis causing multi-organ failure includes resuscitation, supportive care
    and treatment of thrombosis. Treatment options include early anticoagulation and if feasible, thrombolysis.

    Reverse transcriptase polymerase chain reaction (RT-PCR) assays for coronavirus disease 2019 (COVID-19) should be interpreted with clinical, epidemiological history and exposure risk to avoid misdiagnosis. We report a cruise-ship worker with significant travelling history, presented with acute respiratory symptoms and radiographic evidence of viral pneumonia. Initial RNA-dependent RNA polymerase (RdRp) gene confirmatory assay was negative. Use of a more robust RT-PCR assay detected ORF1ab, N and S genes for COVID-19, and the diagnosis was supported by an IgM and IgG positive COVID-19 serology. Subsequent follow up samples which reported inconsistencies in detecting RdRp gene also raise the concern of reliability of RdRp gene as the confirmatory assay for diagnosis of COVID-19. Patient later had prolonged viral shedding beyond serological recovery, with a negative viral culture reflecting non-infectivity.

    Assisted Reproductive Technology (ART) is a suite of laboratory techniques designed to rescue infertile phenotypes. While ART has led to the birth of 5 million ART babies worldwide, success rates rarely exceed 40%. One potential factor for this could be iatrogenic (‘clinician-induced’) damage to critical sperm proteins, such as phospholipase C zeta (PLCζ) and protamine, which are fundamental for oocyte activation and sperm DNA integrity, respectively. This report describes how we have begun to investigate the adverse effects of ART techniques upon these key sperm proteins. We also describe the pathway taken by Miss Suseela Yelumalai to acquire a scholarship from the Malaysian Government and her postgraduate experience at the University of Oxford. We introduce the facilities and learning opportunities available at the Institute of Reproductive Sciences (IRS) which houses Dr Kevin Coward’s research laboratory, and finally, highlight the potential for collaborative development between the Universities of Oxford and Malaya.

    Bisphosphonates are synthetic analogues of pyrophosphate. Their main pharmacological effect is to iuhibit bone resorption by a variety of mechanisms, not all of which are clearly understood. The activity of the bisphosphonates varies depending on the compound. In clinical trials, they have been shown to stop postmenopausal bone loss and increase bone density, with a concomitant reduction in fracture rate with some agents. This article reviews the currently known mechanisms of action of the bisphosphonates and the evidence that they are useful in the treatment of osteoporosis.

    Breast feeding has been actively encouraged in Malaysia in the last few years in all public hospitals. This study proposes to find out the prevalence of breast feeding in three villages in a rural community in Kedah, Malaysia. This was a cross sectional study on breastfeeding practices in Kubang Pasu, a district of Kedah. Majority of the mothers initiated breast feeding but exclusive breastfeeding was only 21% for four months and predominant breastfeeding was about 12.6%.
