    To investigate the safety climate and knowledge, attitude and practice (KAP) on occupational safety and health (OSH)in the manufacturing industry, also to determine the association between safety climate factors and KAP of safety among manufacturing worker. A cross-sectional study was conducted among 59 respondents from two manufacturing plants located in Gebeng, Kuantan, Pahang. Most of the respondents were Malay (91.5%) and male (96.6%). Participants were administered a set of questionnaires (Cronbach alpha=0.674) that measured the safety climate as perceived by the workers towards their supervisor and KAP of the workers regarding safety-related matters at the workplace. Self-administered questionnaires consisted of 5 points Likert scale used to measure each of the items of safety climate and KAP. The scales for safety climate and KAP were probed using 16 items and 17 items in the questionnaires, respectively. The results were analysed using a non-parametric test, which is Spearman’s rho correlations and descriptive statistics. Bivariate analysis was performed.     Precast concrete panel is one of the Industrial Building System (IBS) components currently used in modern construction industry, especially, in a highly populated urban area in Malaysia, owing to its cost and time efficiency in a project completion. However, the constraints lie in Musculoskeletal Disorders (MSDs) when IBS workers manually lift the concrete panel repeatedly. The aim of this study is to analyze the recommended weight limit (RWL), lifting index (LI) and physiological experience among workers while lifting precast concrete panel with 3300mm x 600mm size and compressive strength of 40MPa. The methods involved include a survey through the Nordic questionnaires, interview sessions, NIOSH lifting index calculation, recommended work limit calculation, Rapid Upper Limb Assessment (RULA) and video recording. The results yielded the MSDs problem. The guidelines of recommended weight limit and lifting index are produced. They benefit workers and increase their awareness on the MSDs issues.

    The Department of Occupational Safety and Health Malaysia (DOSH) is the authority responsible to safeguard the occupational safety, health and welfare of workers in Malaysia. Occupational noise-related hearing disorders are the leading type of occupational diseases recorded by DOSH every year. Occupational Health Doctor (OHD) is a competency recognized by DOSH and their scope of duties in industries is currently confined to conduct medical surveillance for workers with hazardous chemical exposure and confined space medical fitness examination. The duties of OHD are good to be expanded by empowering more of their roles in industrial activities involving other legislations under DOSH. The noise exposure regulation under the Factories and Machinery Act (FMA) 1967 has been governing the worker’s exposure to hazardous industrial noise and preventing noise-induced hearing loss (NIHL) since 1989. However, the provisions of the regulation need to be strengthened in some essential medical elements of the Hearing Conservation Programme (HCP) for a comprehensive prevention of NIHL at the workplace. Recently enacted Occupational Safety and Health (Noise Exposure) Regulations 2019 offer a wider coverage of workers in ten sectors of industries applicable under the Occupational Safety and Health Act (OSHA) 1994. The current regulatory requirements for management of workplace noise have many improvements compared to the existing law. Enhancement in the medical requirements of industrial audiometry is made prominent by involving OHDs to interpret audiograms and conduct medical examination for workers. The reporting of occupational noise-related hearing disorders to DOSH is outlined better in the new regulation. The occurrence of NIHL and other related hearing disorders are expected to reduce eventually after the introduction of Occupational Safety and Health (Noise Exposure) Regulations 2019. The OHDs will play a pivotal role in industrial audiometry and prevention of hearing disorders among the working population.

    he practice of 5S has been adopted by many organizations as a method to organize the workplace towards reducing waste and improving productivity. Moreover, the practice also promotes a safe and efficient environment in organisations. As safety performance of the workplace is among the key issues in most industries including universities to become world class organizations, this study seeks to address empirically the impact of 5S practice on safety management performance. In this context, a survey questionnaire is employed and distributed to 60 employees of the selected university to examine the employees’ perception on the adoption of 5S practice as well as the impact of 5S practice towards safety management performance. The questionnaires were validated by a pilot test with an acceptable value of Cronbach's alpha. Overall findings highlight there was positive perceptions toward 5S practice among the respondents. The results also revealed that 5S Audit has a very strong positive correlation with safety management performance. Furthermore, 5S Training and Top Management Commitment were (2) factors that strongly correlated to safety management performance. These factors are highly important in contributing toward the success of safety management performance. Additionally, since most of the employees were not exposed with Occupational Safety and Health (OSH), this study reveals moderate negative correlation between compliance to OSH Requirements and safety management performance.

    Medical professionals work in a constantly stressful environment. The nature of the profession requires routine extended working hours, but little is known about the effect of long working hours on selective physiological measures. A cross-sectional feasibility study was conducted from 1 October 2017 until 30 April 2018 on medical doctors in a tertiary health centre in northwest Malaysia.     Transport of fuel is essential to ensure supplies are delivered as per requested by the industrial sites or other demands. Numerous accidents have been reported and recorded in which loss of containment of hazardous chemicals occurred and led to disastrous outcomes. This paper presents the analysis of Boiling Liquid Expanding Vapour Explosion (BLEVE) due to loss of containment for Liquefied Petroleum Gas (LPG) road tankers. The main objective of this paper is to evaluate the potential consequences resulting from overpressure blast and thermal radiation of tankers carrying LPG to the people and the surrounding. The aim is also to compare the outcomes obtained from PHAST software simulator 8.11 with that of established mathematical model. Malaysia North-south Expressway (NSE) was selected as the location of the incident. The volume, weather parameters and properties of LPG were identified. It was found that the effect of BLEVE on people and structures was catastrophic. The results obtained from the mathematical model were similar with that modelled using PHAST software simulator.

    Radon exists naturally in the air. It can accumulate inside the buildings which may affect the indoor air quality. Radon is a radioactive gas that produces alpha particles during decay time. The alpha particles might cause harm to the human lungs and stomach. Inhalation of radon is one of the causes of lung cancer disease. Samples of inhaled radon in different rooms at the workplace were taken hourly through a passive diffusion chamber. The detection method was done using Alpha Spectrometry. The short term measurement was applied in the study to monitor the average weekly radon reading in different rooms in the Medical Imaging Laboratory of the University of Selangor (UNISEL). All tested rooms showed the existence of radon gas with different concentrations. Some of them showed the maximum reading of radon concentration which was higher than the radon action level of 148Bq/m3 or 4pCi/l. Their weekly average of radon concentration is contributing almost 50% of the accumulated radon concentration in the laboratory. It is highly recommended that monitoring the concentration of radon in indoor air is performed to ensure it is at a safe and healthy level.

    Industries in Malaysia are entering a period of major disruption caused by new technologies such as Artificial Intelligent, Robotics, Blockchain, Nanotechnology as well as Building Information Modelling (BIM) and the Internet of Things (IoT). In this fourth industrial revolution where information is generated and exchanged at a rapid and huge scale, its reliability is of paramount importance. The success of Occupational Safety & Health Management System (OSHMS) is highly dependent on the reliability of the information gathered and used, where a large number of intermediaries authenticate the information to establish trust between the stakeholders. Blockchain technology is able to do verification by virtue of secured distributed storage brings about a paradigm shift in the way we establish trust. This paper gives an overview of the potential use of Blockchain technology for Occupational Safety & Health Management System. The discussions focused on the benefits and challenges of implementing the Blockchain technology in OSHMS. The conclusion is drawn based on the strength in the characteristics provided by the Blockchain technology itself.

    Solar thermal system or solar water heater system is one of the applications used to produce hot water in the residential sector. This paper describes HAZOP analysis and reliability assessment to evaluates the potential hazard and system probability for the closed loop solar thermal system applied for the residential area. Hazard identification for the main system components is analyzed while Fault Tree Analysis (FTA), Reliability Block Diagram (RBD) and Weibull distributions performed to determine the reliability for the overall system. The result shows that there are 49 potential hazards for the system with failure probability at 0.23822 and the reliability is 0.9693. Subsequently, this study determined the potential hazards for the system which can be anticipated by the residential consumer for the safety aspect. Furthermore, the evaluated reliability result shows that the application of closed loop type solar water heater system at residential premises is highly recommended due to its long lasting operational condition.

    Digital Millennials (i.e., people born from 1980 to 2004) have often been portrayed as quick to embrace emerging technologies. The envisioned future of Industry 4.0 entails the blending of hardware, software, and people to complete work; and given their affinity for digital resources, technology, and social networking applications, the Millennials could be regarded as a much-needed asset in this future workforce. However, systematic research on how this cohort understands and reacts to safety issues surrounding the use of technologies has been lacking. In particular, behaviours such as impulsivity, online disinhibition, and risk-taking among them remain unclear. The present study investigated this gap with a sample of 203 Millennials of various nationality groups. Using the short version of the Urgency, Premeditation, Perseverance, Sensation Seeking, and Positive Urgency (Short UPPS-P) scale, the Revised Online Disinhibition Scale, and the Online Risky Behaviour scale, the results indicated that the most significant predictors of online risk-taking behaviours are lack of premeditation, positive urgency, and toxic disinhibition. Interestingly, though not hypothesised, was gender as a significant predictor of the outcome. The findings implied that these variables might be the most relevant target areas for interventions that could modify risky behaviours in cyberspace. The study’s findings are discussed in relation to the key challenges associated with e-safety where the future trends would be heading towards the gig economy and crowd-working.

    Quantitative Risk Assessment (QRA) nowadays is an established risk assessment method used worldwide for the evaluation of risks on onshore plant and offshore facilities which associated with the major hazard installations. However, there are still many issues on QRA used. These include lack of consistency, complexity of the overall model structure, incorporation of new data, methodology and model analysis. Common problem observed for the onshore QRA methodology is conservatism of fire and explosion consequence results using DNV PhastRisk 6.7 software which is mainly contributed from the high release rate due to loss of containment. This paper presents an alternative way to predict the actual release rate for fire and explosion modelling which called limiting flowrate technique. This method has been applied for calculating risk in Onshore Gas Terminal (OGT) Plant. Adopting the limiting flowrate technique has provide more precise model towards real scenarios. Challenges facing during this research such as using the unmodified United Kingdom (UK) HSE hydrocarbon release database without integrate with the actual failure frequencies from the plant, the risk results tend to be much higher than actual experience. It should be noted that the development of improved onshore risk model has been used as an example for this research but many of the issues are equally applicable to offshore studies as well.

    The incident involving hydrogen release in industry has become a major concern since numerous incidents were observed to have occured over the years. This paper is designed to do the consequence modelling and analysis using PHAST Simulator for the release rate, potential fire and vulnerability to human by lethality versus probit simulated at 5 mm, 35 mm and 70 mm leak scenarios and three types of atmospheric stability at hydrogenation plant in Telok Panglima Garang. The simulation was carried out by inputting data of leak scenario, meteorological data, material data and process data related to the hydrogenation plant. The simulation results were analyzed and discussed on the discharge rate, dispersion concentration and effect of jet fire such as flame length, downwind distance and lethality for radiation intensity level of 4 kW/m2 , 12.5 kW/m2 and 37.5 kW/m2 . Based on the results, the discharge rate and radiation intensity are dependent on the leak sizes regardless of the different atmospheric conditions. However, the dispersion is dependent on both atmospheric stability and leak sizes. Lastly, the lethality and area of impact are simulated from the radiation intensity produced by the jet fire for each leak size. To conclude, adoption of PHAST software is vital for consequence modelling as this software is able to illustrate the outcomes of hazards due to loss of containment and with this will enable related personnel to respond effectively to any hazardous incidents. As a recommendation, hydrogen fixed gas detectors are proposed for installations at specific location after taking into account the smallest leak that may happen which is at 5 mm leak size.

    Company A is a brownfield refinery that had been in service for over 25 years and has its own system to generate GOX for its needed utility usage. Noting of the hazards of GOX and in consideration of an aged refinery, this research is of the intent to evaluate the risk of GOX in the aspect of personnel and process safety; and to provide recommendation or mitigations planning with regards to Company A’s existing hardware through Bow Tie review. The analysis was done taking into consideration the data compiled as well as the inherited Process Safety Assessment (PSA) findings of Company A that served as secondary data to this research. It was observed that Company A personnel are well versed with the risk and hazards of GOX system and through the plant rejuvenation and material upgrade works, the hazards were mitigated to a lower risk within the risk matrix. The implementation and upgrade works had served to add more barriers to the left side of the bow tie as well as ensuring that the aged complex is well equipped with needed safeguarding strategies (from inherent safer design, passive & active safeguarding and procedural controls) to avoid the occurrence of potential oxygen fire or explosion incident.

    The effects of propylene stored in pressurized spherical vessel were investigated using radiation & explosion modeling using PHAST 6.7 software in one of the refinery in Malaysia. The simulations were performed for various weather conditions with different leak scenarios in deterministic approach. Modeling approach was standard with current industry practice. Resulting events such as jet fire, vapor cloud explosion, boiling liquid evaporating vapor explosion effects shown in thermal radiation and overpressure towards targeted technical buildings. The effects of resulting jet fire flame length increase with release rate and explosion overpressure effects increase with degree of confinement and volume fraction respectively. The results were reviewed, interpreted against industry standard. The sensitivity cases show that, using lower inventory with moderate operating conditions will keep the consequence in acceptable region. This consequence analysis will form a basis for layout development, safety distance and fire zone segregation during conceptual design stage. Propylene storage conditions, layout arrangements and blast protections were recommended as part of preventive and mitigative measures.

    Fire emergencies are threat to the occupants of a residential college. Some of the Malaysian residential colleges were built in the 1970s. Back then, the compliancy to Uniform Building By-law 1984 was not entirely practiced. This study aims to evaluate fire safety measures in selected residential colleges of a Malaysian University, which were built before 1984. This includes occupants’ level of awareness and knowledge of the occupants regarding fire safety measures. This study was conducted in selected residential colleges, built before 1984, which were named as A, B, C, and D Colleges. One new college building was selected to be the control variable, the E College. Survey questionnaires were given to 401 respondents to obtain information regarding the fire safety awareness and knowledge. Fire safety inspections were conducted to determine the level of fire safety protection systems in colleges and the documentation of emergency response plan were reviewed. From the study, the level of fire safety awareness among the occupants were higher compared to their fire safety knowledge. Fire safety inspection result indicated that overall buildings inspected complies with the local regulation while safety documentation reviews were satisfactorily adequate. Overall, the score for fire safety measures in all selected colleges were sufficient and in good condition. This study is significant for those in the field of safety and health practice pertaining to fire safety engineering and regulations, to plan for better and more efficient fire hazard and risk assessment.

    Air pollution in steel making operations effect to respiratory health. This study aimed to measure the dust exposure and evaluate the respiratory health among steel workers. A cross sectional study was conducted among 402 male workers. Respiratory symptoms using British Medical Research Council (BMRC) Questionnaire while lung function was measured The airborne dust [PM2.5, PM10, and Total Particulate Matter (TPM)] were monitored by Handheld 3016 Counter. All the parameters studied exceeded the limit of Malaysian guideline standard. Prevalence of chronic phlegm, chest of tightness, and shortness of breath were 35.8 %, 32.8 %, 23.4 %, and 22.4 %, respectively. significant differences between shortness of breath and work section (2=9.236, p=0.026) and %FEV1/FVC with work section [F (3, 3.98=3.194), p=0.025]. Smoking was associated with chronic cough (Adj OR =1.07, 95% CI: phlegm (Adj OR =1.05, 95% CI: 1.03 - 1.08), and shortness of breath (Adj OR = 1.05, 95% CI: 1.00 past respiratory illnesses was associated with chest tightness (Adj OR = 2.24, 95% CI: 1.04 - 4.84) and (Adj OR = 4.16, 95% CI: 1.92 - 9.92). Duration of employment was associated with FEV1 (β=-0.025, 0.020) while past respiratory illnesses was associated with %FEV1/FVC (β =-1.784, 95% CI: -3.017 workers are at risk of developing respiratory symptoms and lung function impairment.

    Today, more accidents reported in school, resulting not only minor injuries but also severe injuries such as fire in schools, mercury spills and falling from high places. The study was aimed at measuring the level of occupational safety and health practices in schools. The study was conducted at 205 schools in Kelantan, Malaysia. This study is a semi-quantitative study using observation method. Data collection is conducted based on Hazard Identification, Risk Assessment and Risk Control (HIRARC) Guidelines provided by the Department of Occupational Safety and Health, Malaysia. Hazard identification, risk assessment and risk control are determined using the HIRARC form. The findings were measured by determining the discrete data according to the number of low, medium and high-risk levels found. The study found that there were two dominant hazards namely physical hazard and ergonomic hazard. Therefore, the management should increase and improve classroom safety awareness to ensure safety and health of all occupants.

    communication, full compliance and good behavior) as among the crucial elements required to be possessed by the principal employers in three sectors, namely manufacturing, public services and construction. In relation to this, this paper will describe the development and validation of instruments prior to the measurement of principal employers’ roles and responsibilities in the implementation of OSH. Three assessment tools were developed, namely the Benchmarking Interview, Questionnaire and Workplace Inspection. Fifteen companies were selected for the benchmarking interview, 50 employers conveniently selected for the survey interview (covering three sectors) and 90 employers selected for the workplace inspection (30 respondents for each sector). The development of benchmarking interview and workplace inspection scores are briefly discussed while the main focus is on the validation of the survey constructs (or items). The reliability check on 53 items representing four elements (i.e., Commitment, Communication, Compliance, Behaviour) of employers’ roles and responsibilities in the implementation of OSH showed that the Cronbach’s Alpha coefficient is more than 0.90 which indicates that the internal consistency is extremely reliable. It also indicates that the set of items in each element are closely related and well understood by the respondents. Validity check on the items based on the Rasch measurement infit and outfit mean square statistics and standardized z-score found that nine items had misfitting values and finally corrected for further analysis. This study had shown that a valid and reliable instruments are important in ensuring that accurate and precise findings are obtained in measuring the roles and responsibilities of principal employer in the implementation of OSH.

    The study objective is to examine the effects of the identified attributes (i.e. employee characteristics and ergonomics) that influences the chronic back pain disabilities acquired and benefits awarded via the recognition of workplace relatedness. As these factors are considered in work- related chronic back pain, we hypothesized that the ascertainment of work relatedness would depend on employees’ workplace conditions (poor ergonomic practices) and innate personal conditions (excellent pre-existing medical health). Two set of data was collected from a registry owned by a social security organization in 2012. Factor analysis and structural equation modeling (SEM) was used to analyze the data. As predicted, workplace conditions and employees’ pre- existing medical health status mediated occupational back pain to develop disability and monetary benefit relationships. In conclusion, the results support the importance of obtaining workplace evidence and employees’ pre- existing medical health status that produces stronger relationship in terms of the amount of benefits to be given compared to the lesser effects on disability. Both models with different samples showed consistent SEM results. The proposed model would be applicable in the heterogenous Asian setting and that occupational back pain consensus is the key element in providing effective social security protection.
