    Pencerapan dua dimensi (2D) yang diperoleh daripada keratan rentas sampel selalunya digunakan untuk mendapatkan
    maklumat ketebalan lapisan sebatian antara logam (IMC). Walau bagaimanapun, pencerapan 2D amatlah terhad
    berbanding maklumat yang sepatutnya diperoleh daripada struktur tiga dimensi (3D) sampel. Kajian ini bertujuan untuk
    menilai semula kaedah pengukuran lapisan IMC daripada pemerhatian 2D dengan mempertimbangkan faktor-faktor
    sebenar dalam bentuk 3D IMC. Nilai mod ketebalan didapati lebih mewakili nilai ketebalan IMC setelah mempertimbangkan
    pelbagai faktor stereometri berbanding dengan nilai purata. Ini memberikan suatu penanda aras dalam aktiviti
    menentukan ketebalan lapisan IMC berdasarkan pencerapan 2D daripada struktur 3D.

    BACKGROUND: Achieving malaria elimination requires the targeting of the human reservoir of infection, including those patients with asymptomatic infection. The objective was to synthesise evidence on the accuracy of the rapid-onsite diagnostic tests (RDTs) and microscopy for the detection of asymptomatic malaria as part of the surveillance activities in Asian countries.

    METHODS: This was a meta-analysis of diagnostic test accuracy. Relevant studies that evaluated the diagnostic performance of RDTs and microscopy for detection of asymptomatic malaria were searched in health-related electronic databases. The methodological quality of the studies included was assessed using the QUADAS-2 tool.

    RESULTS: Ten studies assessing RDT and/or microscopy were identified. The diagnostic accuracies in all these studies were verified by PCR. Overall, the pooled sensitivities of RDT, as well as microscopy for detection of any malaria parasites in asymptomatic participants, were low, while their pooled specificities were almost ideal. For the detection of Plasmodium falciparum, pooled sensitivity by RDT (59%, 95%CI:16-91%) or microscopy (55%, 95%CI: 25-82%) were almost comparable. For detection of Plasmodium vivax, pooled sensitivity of RDT (51%, 95% CI:7-94%) had also the comparable accuracy of microscopy (54%, 95%CI,11-92%). Of note are the wide range of sensitivity and specificity.

    CONCLUSION: The findings of this meta-analysis suggest that RDTs and microscopy have limited sensitivity and are inappropriate for the detection of asymptomatic Plasmodium infections. Other methods including a combination of PCR-based strategies, Loop-Mediated Isothermal Amplification (LAMP) technique must be considered to target these infections, in order to achieve malaria elimination. However, more data is needed for the wide acceptance and feasibility of these approaches. Studies to explore the role of asymptomatic and sub-patent infections in the transmission of malaria are of critical importance and are recommended.

    Terahertz technologies recently emerged as outstanding candidates for a variety of applications in such sectors as security, biomedical, pharmaceutical, aero spatial, etc. Imaging the terahertz field, however, still remains a challenge, particularly when sub-wavelength resolutions are involved. Here we demonstrate an all-optical technique for the terahertz near-field imaging directly at the source plane. A thin layer (<100 nm-thickness) of photo carriers is induced on the surface of the terahertz generation crystal, which acts as an all-optical, virtual blade for terahertz near-field imaging via a knife-edge technique. Remarkably, and in spite of the fact that the proposed approach does not require any mechanical probe, such as tips or apertures, we are able to demonstrate the imaging of a terahertz source with deeply sub-wavelength features (<30 μm) directly in its emission plane.

    A ZnO gas sensor was successfully prepared by RF sputtering. The maximum sensitivity of the sensor for vinegar test application was at 400oC. The ZnO based sensor showed good sensitivity for vinegar test in the concentration range of 4% to 9%. The work reveals the ability of using ZnO gas sensor to determine the acid concentrations of the vinegars for food requirements.

    Intervensi pemakanan berpotensi untuk melambatkan penurunan fungsi kognitif dalam kalangan warga tua yang mengalami kecelaan kognitif ringan (MCI). Walau bagaimanapun, kemampanan program sedemikian masih belum dapat dipastikan. Oleh itu, suatu kajian susulan dijalankan bagi menentukan kemampanan suplementasi minyak ikan dan pendidikan kesihatan bagi mengurangkan kemerosotan fungsi kognitif dalam kalangan warga tua yang mengalami MCI selepas setahun kajian intervensi dijalankan. Kajian ini melibatkan 18 orang subjek MCI iaitu 6 orang bagi setiap kumpulan intervensi suplementasi minyak ikan (A) dan pendidikan kesihatan (B) dan kumpulan kawalan (C). Analisis menunjukkan kesan intervensi yang signifikan bagi ujian Span Digit (p<0.05) dan aras serum folat (p<0.05) dengan kumpulan B menunjukkan min yang paling tinggi, diikuti dengan A dan C. Intervensi pendidikan kesihatan dan suplementasi minyak ikan mempunyai kesan kemampanan bagi meningkatkan aras serum folat dan memperbaiki status fungsi kognitif yang dinilai menggunakan Ujian Span Digit. Terdapat keperluan bagi promosi kesihatan dan suplementasi minyak ikan dalam mengekalkan fungsi kognitif individu warga tua yang mengalami MCI

    In 1985, when HIV testing first became available, the main goal of such testing was to ensure blood safety. Hemophiliacs and other patients who were transfusion-dependent were the initial patients that needed to be protected as they were exposed to iatrogenic risk. It dawned very quickly to health authorities even then that alternative testing sites had to be quickly established to deter persons from using blood bank facilities for HIV testing purposes. At that time, professional opinion was divided regarding the value of HIV testing and whether HIV testing should be encouraged because no consensus existed regarding whether a positive test predicted transmission to sex partners or from mother to infant. No effective treatment existed then and stigma and discrimination faced by those who were found positive was rampant in many parts of the world. Counseling was designed, in part, to ensure that persons tested were aware of the implications of a positive result and in part, to address the person’s risk behavior to reduce transmission to others.

    BACKGROUND: The global malaria decline has stalled and only a few countries are pushing towards pre-elimination. The aim of the malaria elimination phase is interruption of local transmission of a specified malaria parasite in a defined geographical area. New and improved screening tools and strategies are required for detection and management of very low-density parasitaemia in the field. The objective of this study was to synthesize evidence on the diagnostic accuracy of loop-mediated isothermal amplification (LAMP) test for the detection of malaria parasites among people living in endemic areas.

    METHODS: This study adhered to the Preferred Reporting Items for Systematic Reviews and Meta-Analysis for Diagnostic Test Accuracy (PRISMA-DTA) guideline. Relevant studies in the health-related electronic databases were searched. According to the criteria set for this study, eligible studies were identified. The quality of included studies was evaluated with the use of a quality assessment checklist. A summary performance estimates such as pooled sensitivity and specificity were stratified by type of LAMP. Bivariate model for data analyses was applied. Summary receiver operating characteristics plots were created to display the results of individual studies in a receiver operating characteristics space. Meta-regression analysis was performed to investigate the sources of heterogeneity among individual studies.

    RESULTS: Twenty-seven studies across 17 endemic countries were identified. The vast majority of studies were with unclear risk of bias in the selection of index test. Overall, the pooled test performances were high for Pan LAMP (sensitivity: 0.95, 95% CI 0.91 to 0.97; specificity: 0.98, 95% CI 0.95 to 0.99), Plasmodium falciparum (Pf) LAMP (sensitivity: 0.96, 95% CI 0.94 to 0.98; specificity: 0.99, 95% CI 0.96 to 1.00) or for Plasmodium vivax (Pv) LAMP from 6 studies (sensitivity: 0.98, 95% CI 0.92 to 0.99; specificity: 0.99, 95% CI 0.72 to 1.00). The area under the curve for Pan LAMP (0.99, 95% CI 0.98-1.00), Pf LAMP (0.99, 95% CI 0.97-0.99) and Pv LAMP was (1.00, 95% CI 0.98-1.00) indicated that the diagnostic performance of these tests were within the excellent accuracy range. Meta-regression analysis showed that sample size had the greatest impact on test performance, among other factors.

    CONCLUSIONS: The current findings suggest that LAMP-based assays are appropriate for detecting low-level malaria parasite infections in the field and would become valuable tools for malaria control and elimination programmes. Future well-designed larger sample studies on LAMP assessment in passive and active malaria surveillances that use PCR as the reference standard and provide sufficient data to construct 2 × 2 diagnostic table are needed.

    INTRODUCTION: Cystic fibrosis (CF) is a life-limiting autosomal recessive disorder as a result of CF transmembrane conductance regulator gene mutation. It has a wide range of disease severity in patients with the same genotype.

    CASE REPORT: A 5-year-old Malay boy with a history of recurrent pneumonia, presented with productive cough, fever and worsening tachypnoea. Physical examination revealed coarse crepitations, reduced breath sounds and clubbing. Biochemical investigations showed that he had respiratory type 2 failure as a result of bronchiectasis. Sweat conductivity done twice was raised supporting a diagnosis of CF. Other investigations such as bronchoscopy to look for congenital anomaly of the lung, infectious disease screening and tuberculosis, fungal and viral culture and sensitivity were negative. Further cascade screening revealed high sweat conductivity results in his siblings.

    DISCUSSION: Although CF prevalence is low in Malaysia, it is nevertheless an important diagnosis to be recognised as it is associated with increased morbidity.

    BACKGROUND: Malaysia has already achieved remarkable accomplishments in reaching zero indigenous human malaria cases in 2018. Prompt malaria diagnosis, surveillance and treatment played a key role in the country's elimination success. Looking at the dynamics of malaria distribution during the last decades might provide important information regarding the potential challenges of such an elimination strategy. This study was performed to gather all data available in term of prevalence or incidence on Plasmodium infections in Malaysia over the last four decades.

    METHODS: A systematic review of the published English literature was conducted to identify malaria distribution from 1980 to June 2019 in Malaysia. Two investigators independently extracted data from PubMed, Scopus, Web of Science and Elsevier databases for original papers.

    RESULTS: The review identified 46 epidemiological studies in Malaysia over the 39-year study period, on which sufficient information was available. The majority of studies were conducted in Malaysia Borneo (31/46; 67.4%), followed by Peninsular Malaysia (13/46; 28.3%) and in both areas (2/46; 4.3%). More than half of all studies (28/46; 60.9%) were assessed by both microscopy and PCR. Furthermore, there was a clear trend of decreases of all human malaria species with increasing Plasmodium knowlesi incidence rate throughout the year of sampling period. The summary estimates of sensitivity were higher for P. knowlesi than other Plasmodium species for both microscopy and PCR. Nevertheless, the specificities of summary estimates were similar for microscopy (40-43%), but varied for PCR (2-34%).

    CONCLUSIONS: This study outlined the epidemiological changes in Plasmodium species distribution in Malaysia. Malaria cases shifted from predominantly caused by human malaria parasites to simian malaria parasites, which accounted for the majority of indigenous cases particularly in Malaysia Borneo. Therefore, malaria case notification and prompt malaria diagnosis in regions where health services are limited in Malaysia should be strengthened and reinforced to achieving the final goal of malaria elimination in the country.

    In medical imaging practice, the act of removing any clothes from the region of interest is justified as to prevent the presence of artefacts on radiographs. However, by doing so, the ‘aurah’ of the patients, especially for the Muslims, are not observed and can be considered as violating their privacy if they are not well-informed beforehand. Previous studies have proved that radiographs with the presence of some fabric materials on the region of interest are radiographically acceptable. Therefore, the aims of this study are to tackle the issue of exposing one’s ‘aurah’ for a knee x-ray examination to take place and also to add insufficiency from the previous studies.

    Pengetahuan mengenai pendedahan plumbum (Pb) di kalangan kanak-kanak masih kurang baik di kalangan ibu bapa di Malaysia. Tujuan utama kajian ini adalah untuk menterjemahkan versi Bahasa Inggeris Chicago Lead Knowledge Test (CLKT) yang asal ke dalam Bahasa Melayu (bahasa tempatan) dan disesuaikan dengan latar belakang budaya negara ini. Langkah-langkah terjemahan adalah sistematik dan berdasarkan ‘International Society for Pharmacoeconomics and Outcome Research’ (ISPOR) iaitu persediaan, terjemahan ke depan, perangkuman, terjemahan mundur, penilaian terjemahan mundur, penyelarasan, pembahasan kognitif, penilaian pembahasan kognitif dan semakan akhir, pembacaan pruf, dan laporan akhir. Jumlah item adalah kekal 24, di mana satu item dihapus, satu item disemak semula, dan satu item ditambah ke dalam borang soalan kaji selidik. Keempat-empat tema iaitu pengetahuan umum, pendedahan, pencegahan dan pemakanan dalam soal selidik ini dikekalkan. Skala respon, skema jawapan, dan sistem pemarkahan adalah tetap sama. Sebilangan besar item mempunyai terjemahan langsung ke bahasa Melayu kecuali item 5, 7, 12, 20, dan 22. Purata (+SD) dan julat interkuartil (IQR) untuk skor keseluruhan pengetahuan adalah 9.50 + 2.45 markah dan 5 markah masing-masing. Skor terendah adalah 4 markah dan skor tertinggi ialah 13 markah. Ini merupakan satu-satunya kajian yang menggunakan pendekatan sistematik dan standard dalam terjemahan dan penyesuaian budaya di Malaysia. Terjemahan CLKT dan penyesuaian budaya di Malaysia akan menyumbang kepada penyelidikan semasa yang lain terutamanya mengenai pendedahan Pb di kalangan kanak-kanak.
