    The severity of pulmonary aspiration depends mainly on the acidity of the aspirate. Mist magnesium trisilicate (MMT) has been used for many years at the maternity unit in General Hospital, Kuala Lumpur, to neutralise the acidic gastric contents in all obstetric patients requiring caesarian section. This preliminary study shows that a single dose of 15 mls of MMT before general anaesthesia raises the intragastric pH to above the critical level of 2.5 in 80% of the patients. Recently there have been doubts over the protective role of MMT. Sodium citrate which is the other antacid available may be a better alternative.

    The use of organochlorine pesticides has caused concern due to their effects on human health and the Malaysian aquatic ecosystem, particularly so in view of their persistent and bioaccumulative properties. Since the extent of organochlorine pesticide pollution in Malaysian waterways is unknown except for isolated instances, a systematic survey has now been carried out. Water samples from various rivers were extracted, cleaned up with Florisil and analysed for the individual organochlorine pesticides by gas chromatography (GC) with an electron capture detector (ECD). DDE, DDT and heptachlor were present in all the river water samples of the west coast of Peninsular Malaysia. Other organochlorine pesticides were also identified from the water samples. However, the levels of all these are still below criteria values for Malaysian aquatic life, indicating that organochlorine pesticide pollution is less of a problem than other organic or inorganic pollutants.

    The effect of temperature change on the working and setting time of a glass ionomer luting cement and a resin luting cement was measured using the oscillating rheometer. The time taken for each cement to set was calculated from the chart recordings. It was observed that as the temperature increased, the working and setting time of both materials decreased. However, the reduction was much more marked for the dual curing resin cement.

    Determinations of total calcium, total magnesium, calcium ion, parathyroid hormone and 6-keto-prostaglandin-F1 alpha levels were carried out on 84 blood samples from 4 groups of women categorised as non-pregnant normotensive (NNP), pregnant normotensive (NP), pregnancy-induced hypertension (PIH) and pre-eclampsia (PE). PIH was clinically diagnosed when the diastolic pressure was more than 90 mmHg and was only hypertensive during pregnancy while PE was with additional proteinuria after 20 weeks of gestation. Compared to NNP women, total calcium and parathyroid hormone levels were of lower levels (p < 0.05) in NP women while in PIH women, total calcium and 6-keto-prostaglandin-F1 alpha levels were also lowered (p < 0.05). Compared to NNP women, PE women's levels of total calcium, calcium ion and 6-keto-prostaglandin-F1 alpha decreased (p < 0.05) while parathyroid hormone level increased (p < 0.05). When compared to the NP women, PE women had decreased levels (p < 0.05) of total calcium as well as calcium ion and increased level (p < 0.05) of parathyroid hormone. Calcium ion was found to be negatively correlated (NNP : r = -0.883, p = 0.008/NP : r = -0.931, p = 0.000) while parathyroid hormone was positively correlated (NNP : r = 0.904, p = 0.013/NP : r = 0.913, p = 0.000) with mean arterial pressure.

    Standard renal clearance techniques were used to compare the effects of intravenous infusions of L-arginine, D-lysine and glycine on urinary calcium excretion in the rat. A significant calciuric response was evident following the infusion of all three amino acids in all the animals. The maximal effect was evident in rats receiving L-arginine. The mechanism for the increased urinary calcium excretion in rats infused with L-arginine and D-lysine appeared more due to a decreased fractional reabsorption of this cation as no significant changes in the glomerular filtration rate (GFR) were evident in these two groups. The calciuria in rats receiving glycine appears due to increased filtered load secondary to the increased GFR, suggesting that the mechanism for calciuria evident following protein ingestion or amino acid infusion may vary and may be dependent upon the amino acid ingested or infused.

    1. Thirty male rabbits of local strain (weighing 1.5-2 kg) were divided into five groups. Four groups were treated with an oral dose of paraquat, which was followed by either Fuller's Earth or activated charcoal 0.5 or 2.0 h later. The remaining group acted as the control group and was treated only with an oral dose of paraquat. The dose of paraquat was 20.0 mg/kg given in a concentration of 20.0 mg/mL. 2. Both adsorbents were administered in 15 mL normal saline as a 30% slurry. Blood was sampled from the ear vein 0.5, 1, 2, 4, 8 and 24h after the administration of paraquat. 3. Paraquat concentration was determined spectophotometrically at 600 nm by comparing against a standard curve of paraquat obtained by the addition of standard paraquat into normal rabbit serum and extracting interfering substances with ether. 4. The results of the present study show that either adsorbent can bring down the serum paraquat level. There was no significant difference found in the effectiveness of either adsorbent. 5. It is concluded that the administration of an adsorbent as early as possible will help in the reduction of paraquat absorption from the gastrointestinal tract. 6. Activated charcoal is still effective in lowering serum paraquat concentration when given more than 1 h after ingestion of paraquat.

    The Kodiang Formation which outcrops in Kodiang and Alor Setar areas has been well studied and documented. Various geological aspects of the limestone formation have been reported in detail including its stratigraphy, sequence startigraphy, sedimentology and paleontology. Diagenetic aspect of this rock formation, however, had not been specifically discussed before. The Kodiang limestone has undergone a complicated diagenetic history which started immediately after the sediments were laid down on the sea bed. Various diagenetic products have been identified including, early marine cement, meteoric cement, and deeper subsurface cement. The diagenetic features identified are as follows; Fibrous calcite cement, sparry and poikilotopic calcites, calcite overgrowth, dolomite, silica cements and evaporites. Close examination at outcrops shows that dolomite occurs at several levels in the sequence. Their systematic occurrences can be correlated across the area. Petrographic studies show that dolomite occurs in several modes as stratified dolomite and as non-stratified dolomite. The two dolomite groups are attributed to two phases of dolomitizations. The stratified dolomites are interpreted to have been formed early in the limestone diagenetic history. The non-stratified dolomite type is, however, attributed to dolomitization during deep burial under the control of deeper subsurface physico-chemical conditions.
    Batu Kapur Kodiang yang tersingkap di Kodiang dan Alor Setar telah banyak dikaji dan didokumentasikan. Berbagai aspek geologi formasi batu kapur ini telah dilaporkan termasuk stratigrafi, startigrafi jujukan, sedimentologi dan paleontologi. Walau bagaimanapun aspek diagenesis formasi batuan ini tidak pernah dibincangkan secara khusus. Batu kapur Kodiang ini telah melalui sejarah diagenesis yang rumit yang bermula sebaik saja sedimen dimendapkan di atas permukaan dasar laut. Berbagai hasil diagenesis telah dikenal pasti termasuk penyimenan samudera awal, penyimenan meterorik dan penyimenan di bawah permukaan yang lebih dalam. Secara asas fitur-fitur yang dikenalpasti adalah seperti berikut; simen kalsit berserat, kalsit spar dan poikilotopik, tumbuhlampau kalsit, dolomit, simen silika dan evaporit. Pemeriksaan rapi pada singkapan menunjukkan dolomit hadir pada beberapa aras di dalam jujukan. Kehadirannya secara bersistematik boleh dikorelasikan pada setiap singkapan. Kajian petrografi menunjukkan dolomit wujud dalam berbagai mod yang boleh diklaskan sebagai dolomit berstrata dan dolomit tidak berstrata. Dua klas ini boleh dikaitkan dengan dua fasa pendolomitan. Dolomit berstrata ditafsirkan telah terbentuk awal di dalam sejarah diagenesis batu kapur. Dolomit tidak berstrata pula dikaitkan dengan pendolomitan semasa timbusan dalam di bawah keadaan fiziko-kimia bawah permukaan.

    The proximate composition including mineral and vitamin contents of 16 fruits and 46 vegetables (leaves, fruits, palm hearts and shoots) of indigenous origin in Sarawak are provided. Fruits like dabai (Canarium odontophyllum), kembayau (Dacryodes rostrata f. cuspidata), durian nyekak (Durio kutejensis) and durian kuning (Durio graveolens) are very nutritious with high values for energy, protein and potassium. Among the vegetables, the protein content of letup (Passiflora foetida), kepayang (Pangium edule) and tubu (Pycnarrhena tumetacta) is high, ranging from 6 to 7%. The range of nutrients among foods of indigenous origin are generally comparable with those of many cultivated species except for vitamin C, which is lower. Teh Kampung (Leucosyke capitellata) leaves are particularly high in magnesium (626 mg/100 g). Some of the indigenous vegetables contain antinutritional factors. Kepayang has very high levels of hydrogen cyanide (1834 µg/g on dry basis) but this poison can be completely evaporated by boiling. Indigenous fruits and vegetables which are pesticide residue free are important food sources for rural populations. Nutritious indigenous fruits and vegetables have the potential to be promoted for wider use, domestication and commercialization.

    Layered double hydroxide of Mg-Al-carbonate system (MACH) was prepared and its heat-treated product (MACHT) was obtained by calcination at 500 degrees C. The resulting materials were used as an adsorbent for removal of color from synthetic textile wastewater (STW) and textile wastewater (TWW). Batch kinetic study showed that these materials are an efficient adsorbent for textile dye. The maximum adsorption capacities between 16 to 32 mg of dyes per g of adsorbent was obtained by fitting the adsorption data to the Langmuir adsorption Isotherm. It was found that the adsorption capacity of MACHT is higher than MACH.

    Hydrotalcite-like inorganic layers of Zn-Al, a host containing an organic moiety, 2,4-dichlorophenoxy-acetate, as a guest, was prepared by the spontaneous self-assembly method from an aqueous solution for the formation of a new layered organic-inorganic hybrid nanocomposite material. In this synthesis, the host- and guest-forming species were simultaneously included in the mother liquor, aged, and separated. Various Zn/Al ratios (R = 2, 3, and 4), concentrations of 2,4-dichlorophenoxyacetic acid (0.03-0.1 M), and pH (7 and 10) were studied to optimize the formation of the layered nancomposite. It was found that the optimum conditions for the formation of the nanocomposite were R = 4, pH 7, and concentration of 2,4-dichlorophenoxyacetic acid = 0.08 M. X-ray diffraction shows that this sample affords a nanolayered structure with a basal spacing of 24.6 A.

    BACKGROUND: Cytochrome P4502C9 (CYP2C9), a principle drug-metabolizing enzyme is polymorphic in humans and is responsible for important pharmacokinetic and pharmacodynamic variations of CYP2C9 substrates. We developed an allele-specific multiplex polymerase chain reaction (PCR) method for the detection of common CYP2C9 alleles.
    METHOD: Genomic DNA was extracted from blood obtained from 40 unrelated healthy Malaysian Indian volunteers. The DNA was subjected to a first PCR that was used to amplify both exons 3 and 7 simultaneously in one reaction tube and a second PCR that was used to detect the polymorphic sites of CYP2C9 alleles using allele-specific primers. Sequencing was performed to validate the test results.
    RESULTS: We were successful in amplifying the fragments of interest from the DNA samples. The method was also reproducible and specific. The amplified sequences showed 100% homology to CYP2C9 sequence.
    CONCLUSION: This is the first nested allele-specific multiplex PCR method reported to allow for the simultaneously detection of five CYP2C9 alleles.

    Streptococcus zooepidemicus (SZ) is an aerotolerant bacteria and its ability to survive under reactive oxidant challenge raises the question of the existence of a defense system. Thus growth, hyaluronic acid (HA) and hydrogen peroxide (H2O2) production by SZ in the presence of increasing concentration of Mn2+ were studied. The results suggested that the tested strain supported growth and HA production in cultures treated with 1 and 10 mM of Mn2+ regardless of H2O2 presence in the medium. This showed that SZ have acquired elaborate defense mechanisms to scavenge oxygen toxicity and thus protect cells from direct and indirect effect of this radical. In contrast, cells treated with 25 mM Mn2+ were sensitive, in which, the HA production was reduced considerably. Thus showing that the oxygen scavenger systems of the cells may be fully saturated at this concentration.

    An organic solvent-tolerant S5 lipase was purified by affinity chromatography and anion exchange chromatography. The molecular mass of the lipase was estimated to be 60 kDa with 387 purification fold. The optimal temperature and pH were 45 degrees C and 9.0, respectively. The purified lipase was stable at 45 degrees C and pH 6-9. It exhibited the highest stability in the presence of various organic solvents such as n-dodecane, 1-pentanol, and toluene. Ca2+ and Mg2+ stimulated lipase activity, whereas EDTA had no effect on its activity. The S5 lipase exhibited the highest activity in the presence of palm oil as a natural oil and triolein as a synthetic triglyceride. It showed random positional specificity on the thin-layer chromatography.

    An organic solvent tolerant (OST) lipase gene from Bacillus sphaericus 205y was successfully expressed extracellularly. The expressed lipase was purified using two steps purification; ultrafiltration and hydrophobic interaction chromatography (HIC) to 8-fold purity and 32% recovery. The purified 205y lipase revealed homogeneity on denaturing gel electrophoresis and the molecular mass was at approximately 30 kDa. The optimum pH for the purified 205y lipase was 7.0-8.0 and its stability showed a broad range of pH value between pH 5.0 to 13.0 at 37 degrees C. The purified 205y lipase exhibited an optimum temperature of 55 degrees C. The activity of the purified lipase was stimulated in the presence of Ca2+ and Mg2+. Ethylenediaminetetraacetic acid (EDTA) has no effect on its activity; however inhibition was observed with phenylmethane sulfonoyl fluoride (PMSF) a serine hydrolase inhibitor. Organic solvents such as dimethylsulfoxide (DMSO), methanol, p-xylene and n-decane enhanced the activity. Studies on the effect of oil showed that the lipase was most active in the presence of tricaprin (C10). The lipase exhibited 1,3 positional specificity.

    Layered organic-inorganic hybrid nanocomposite material was synthesised using 4-chlorophenoxyacetate (4CPA) as guest anion intercalated into the Zn-Al layered double hydroxide inorganic host by direct co-precipitation method at pH = 7.5 and Zn to Al molar ratio of 4. Both PXRD and FTIR results confirmed that the 4CPA was successfully intercalated into the Zn-AI-LDH interlayer. As a result, a well-ordered nanolayered organic-inorganic hybrid nanocomposite, with the expansion of the basal spacing from 8.9 angstroms in the layered double hydroxide to 20.1 angstroms in the resulting nanocomposite was observed. The FTIR spectrum of the nanocomposite (ZAC) showed that it composed spectral features of Zn-AI-LDH (ZAL) and 4CPA. The nanocomposites synthesized in this work are of mesoporous-type containing 39.8% (w/w) of 4CPA with mole fraction of Al3+ in the inorganic brucite-like layers (xAI) of 0.224. The release studies showed a rapid release of the 4CPA for the first 600 min, and more sustained thereafter. The total amount of 4CPA released from the nanocomposite interlayer into the aqueous solution were 21%, 66%, and 72% in 0.0001, 0.00025, and 0.0005 M sodium carbonate, respectively. In distilled water, about 75, 35, and 57% of 4CPA could be released in 1000 min, when the pH of the release media was set at 3, 6.25, and 12, respectively. In comparison with a structurally similar organic moiety with one more chlorine atom at the 2-position of the aromatic ring, namely 2,4-dichlorophenoxyacetate (24D), the 4CPA showed a slower release rate. The slightly bulkier organic moiety of 24D together with the presence of chlorine atom at the 2-position presumably had contributed to its higher release rate, and it seems that these factors may be exploited for tuning the release rate of intercalated guest anions with similar properties. This study suggests that layered double hydroxide can be used as a carrier for an active agent and the chemical structure of the intercalated moiety can be used to tune the desired release kinetics of the beneficial agent.

    Persistent pulmonary hypertension of the newborn (PPHN) occurs in approximately 1.9 per 1000 newborns and may be more frequent in developing countries. There is strong evidence for the use of inhaled nitric oxide (iNO) and extra corporeal membrane oxygenation (ECMO) in the treatment of PPHN. However, many developing countries do not have access or the technical expertise required for these expensive therapies. Magnesium sulfate is a potent vasodilator and hence has the potential to reduce the high pulmonary arterial pressures associated with PPHN. If magnesium sulfate were found to be effective in the treatment of PPHN, this could be a cost effective and potentially life-saving therapy.

    Batu Reput’ is primary sediment mineral and abundantly found in Perlis. Perlis is one of the major producers of ‘Batu Reput’ in Malaysia that content large deposit of high-purity dolomite [CaMg (CO3)2]. Pure samples of ‘Batu Reput’ recently explored in the Koperasi Rimba Mas Padang Besar Quarry were investigated for their physical, chemical and mineralogical composition. SEM and XRD analysis methods were applied. The potential of ‘Batu Reput’ as a raw material in fertilizer production was investigated in this paper.

    Nitridation behaviour of Al-Mg-Si alloys was studied as a function of temperature by means of thermogravimetry method. A reactive gas, N2-4%H2 at a rate of 10 ml/min was purged into the thermogravimetry analyser chamber. The Al alloys were heated from 25oC to 625oC at the heating rate of 15oC/min and then reduced to 3oC/min until it reached 1500oC. It was found that by varying the amount of Mg and Si in Al-Mg-Si alloys significantly influenced the growth of the composites. A differential thermogravimetric curve shows the Mg containing alloys experienced many steps of chemical reactions. This indicates that besides AlN presence as a major phase, other compounds also exist in the final product. The X-ray diffraction results confirmed the existence of oxide phases such as a-Al2O3, MgAl2O4 and MgO in addition to residual Si and Al metal. The presence of oxide compounds is believed to be due to the reaction between the alloying elements and residual oxygen gas left in the reaction atmosphere. It was also found that Si could play a role in promoting the weight gain of the composite produced. The heating rate has also a profound effect on the weight gain, whereby higher heating rate resulted in low yielded of AlN during the nitridation reaction of the Al-Mg-Si alloys.
