    Whereas scientific evidence is the basis for recommendations and guidance on radiological protection, professional ethics is critically important and should always guide professional behaviour. The International Commission on Radiological Protection (ICRP) established Task Group 109 to advise medical professionals, patients, families, carers, the public, and authorities about the ethical aspects of radiological protection of patients in the diagnostic and therapeutic use of radiation in medicine. Occupational exposures and research-related exposures are not within the scope of this task group. Task Group 109 will produce a report that will be available to the different interested parties for consultation before publication. Presently, the report is at the stage of a working document that has benefitted from an international workshop organised on the topic by the World Health Organization. It presents the history of ethics in medicine in ICRP, and explains why this subject is important, and the benefits it can bring to the standard biomedical ethics. As risk is an essential part in decision-making and communication, a summary is included on what is known about the dose-effect relationship, with emphasis on the associated uncertainties. Once this theoretical framework has been presented, the report becomes resolutely more practical. First, it proposes an evaluation method to analyse specific situations from an ethical point of view. This method allows stakeholders to review a set of six ethical values and provides hints on how they could be balanced. Next, various situations (e.g. pregnancy, elderly, paediatric, end of life) are considered in two steps: first within a realistic, ethically challenging scenario on which the evaluation method is applied; and second within a more general context. Scenarios are presented and discussed with attention to specific patient circumstances, and on how and which reflections on ethical values can be of help in the decision-making process. Finally, two important related aspects are considered: how should we communicate with patients, family, and other stakeholders; and how should we incorporate ethics into the education and training of medical professionals?

    Despite the benefits of standardization, the customization of Software as a Service (SaaS) application is also essential because of the many unique requirements of customers. This study, therefore, focuses on the development of a valid and reliable software customization model for SaaS quality that consists of (1) generic software customization types and a list of common practices for each customization type in the SaaS multi-tenant context, and (2) key quality attributes of SaaS applications associated with customization. The study was divided into three phases: the conceptualization of the model, analysis of its validity using SaaS academic-derived expertise, and evaluation of its reliability by submitting it to an internal consistency reliability test conducted by software-engineer researchers. The model was initially devised based on six customization approaches, 46 customization practices, and 13 quality attributes in the SaaS multi-tenant context. Subsequently, its content was validated over two rounds of testing after which one approach and 14 practices were removed and 20 practices were reformulated. The internal consistency reliability study was thereafter conducted by 34 software engineer researchers. All constructs of the content-validated model were found to be reliable in this study. The final version of the model consists of 6 constructs and 44 items. These six constructs and their associated items are as follows: (1) Configuration (eight items), (2) Composition (four items), (3) Extension (six items), 4) Integration (eight items), (5) Modification (five items), and (6) SaaS quality (13 items). The results of the study may contribute to enhancing the capability of empirically analyzing the impact of software customization on SaaS quality by benefiting from all resultant constructs and items.

    Introduction: Mega endoprosthesis replacement for resection of primary malignant bone tumour requires immediate and long-term stability, particularly in the young and active patient. Extracortical bone bridge interface (EBBI) is a technique whereby autograft is wrapped around the interface junction of bone and porous-coated implant to induce and enhance bone formation for biological incorporation. This procedure increases the mean torsional stiffness and the mean maximum torque, which eventually improves the implant's long-term survival.

    Material and methods: The extracortical bone bridge interface's radiological parameter was evaluated at the prosthesis bone junction two years after surgery utilising a picture archiving and communication system (PACS). The radiograph's anteroposterior and lateral view was analysed for both thickness and length in all four cortices. The analysis was done in SPSS Version 24 using One-Way ANOVA and independent T-Test. Results were presented as mean and standard deviation and considered significant when the p-value was < 0.05.

    Results: The mean average thickness was 2.2293mm (SD 1.829), and the mean average length was 31.95% (SD 24.55). We observed that the thickness and length of EBBI were superior in the young patient or patients with giant cell tumour that did not receive chemotherapy, compared to patients treated for osteosarcoma. The distal femur also had better EBBI compared to the proximal tibia. However, the final multivariable statistical analysis showed no significant difference in all variables. EBBI thickness was significantly and positively correlated with EBBI Length (p<0.001). We conclude that, for each 1mm increase in EBBI thickness, the length will increase by 0.06% on average. About 17.2% of patients out of the 29 showed no radiological evidence of EBBI.

    Conclusion: From our study, there were no factors that significantly contributed to the formation and incorporation of EBBI.

    A simple high-performance liquid chromatography (HPLC) method to determine the content of betulinic acid (BA) in rat plasma collected at different times (0-8 h) after oral administration of Orthosiphon stamineus leaf extract was developed. The features of the assay include protein precipitation using acetonitrile and isocratic elution using reverse phase C-18 column with ultraviolet (UV) detection. The recovery of BA from plasma varied from 98.4 to 102.5%. The R.S.D of intra- and inter-day precision from rat plasma ranged from 4.2 to 9.8%. The maximum concentration of BA in the plasma was 1.2 +/- 0.3 microg/ml at 1 h after oral administration of the extract.

    Pengetahuan mengenai pendedahan plumbum (Pb) di kalangan kanak-kanak masih kurang baik di kalangan ibu bapa di Malaysia. Tujuan utama kajian ini adalah untuk menterjemahkan versi Bahasa Inggeris Chicago Lead Knowledge Test (CLKT) yang asal ke dalam Bahasa Melayu (bahasa tempatan) dan disesuaikan dengan latar belakang budaya negara ini. Langkah-langkah terjemahan adalah sistematik dan berdasarkan ‘International Society for Pharmacoeconomics and Outcome Research’ (ISPOR) iaitu persediaan, terjemahan ke depan, perangkuman, terjemahan mundur, penilaian terjemahan mundur, penyelarasan, pembahasan kognitif, penilaian pembahasan kognitif dan semakan akhir, pembacaan pruf, dan laporan akhir. Jumlah item adalah kekal 24, di mana satu item dihapus, satu item disemak semula, dan satu item ditambah ke dalam borang soalan kaji selidik. Keempat-empat tema iaitu pengetahuan umum, pendedahan, pencegahan dan pemakanan dalam soal selidik ini dikekalkan. Skala respon, skema jawapan, dan sistem pemarkahan adalah tetap sama. Sebilangan besar item mempunyai terjemahan langsung ke bahasa Melayu kecuali item 5, 7, 12, 20, dan 22. Purata (+SD) dan julat interkuartil (IQR) untuk skor keseluruhan pengetahuan adalah 9.50 + 2.45 markah dan 5 markah masing-masing. Skor terendah adalah 4 markah dan skor tertinggi ialah 13 markah. Ini merupakan satu-satunya kajian yang menggunakan pendekatan sistematik dan standard dalam terjemahan dan penyesuaian budaya di Malaysia. Terjemahan CLKT dan penyesuaian budaya di Malaysia akan menyumbang kepada penyelidikan semasa yang lain terutamanya mengenai pendedahan Pb di kalangan kanak-kanak.

    An activity supportive of the MOH QA Programme, the National EQAS for clinical chemistry monitors for analytical performance in core routine biochemical testing by the pathology laboratories, with unsatisfactory performance scores serving to alert against deficiencies or problems and the scores in subsequent challenges providing the feedback of effectiveness of remedial actions taken. While unacceptable individual analyte performance score (variance index score, VIS) indicated problems in instruments, reagent and calibrators, or the use of inherently poorer methods, repeated occurrence of unsatisfactory OMRVIS was traceable to generally poor laboratory management of usually inadequately-equipment small laboratories. The outcome has been one of slow but gradual improvement in the overall performance of participating laboratories, with a move towards methods upgrading and standardization to achieve greater concordance of results. Presently, the programme is limited to 61 government and 4 private hospital laboratories in the country for 12 commonly assayed clinical biochemistry analytes. It is hoped that the NEQAS could be extended to the other private laboratories and that of academic institutions. However, this is dependent to a large extent on the manpower and financial support obtainable by the organizing body of the programme in the future. Belk and Sunderman, 1947 demonstrated that laboratories participating in an quality assessment scheme could rapidly and dramatically improve their analytical performance. In some countries, participation has become mandatory, and acceptable performance is a requirement in laboratory accreditation. The need and value of the NEQAP is, therefore, evident. While there may be limitations in the national programme. efforts are being made at improving the programme within the means and resources of the organising body. The goals of the NEQAP are not just to monitor performance but also to educate. On this, matters related to and supportive of these goals have also been pursued. The annual workshop/forum on quality controls had allowed exchange of information between representatives of participating laboratories and the organising body. Recently in the 1997 MOH Quality Improvement evaluation, Quality Control has been evaluated together with the other 17 such activities. The study on knowledge, attitude and practice has provided the necessary feedback and will be used for future planning in making efforts at increasing the effectiveness and benefits of the all QC activities including this NEQAP for clinical chemistry. In addition, there is a need to look into areas such as selection of methods and test systems, and improvement of continuing education, training as well as research in quality improvement as suggested by the Quality Improvement evaluation.

    PURPOSE: Guaiac fecal occult blood test (gFOBT) has been the standard for colorectal screening but it has low sensitivity and specificity. This study evaluated the use of fecal tumor M2-pyruvate kinase (M2-PK) for detection of colorectal cancer and to compare with the current surveillance tool; gFOBT in symptomatic adult subjects underwent colonoscopy.

    METHODS: Stool samples were collected prospectively from symptomatic adults who had elective colonoscopy from September 2014 to January 2016 and were analyzed with the ScheBo M2-PK Quick test and laboratory detection of fecal hemoglobin.

    RESULTS: The results were correlated to the colonoscopy findings and/or histopathology report. Eighty-five subjects (age of 56.8 ± 15.3 years [mean ± standard deviation]) were recruited with a total of 17 colorectal cancer (20.0%) and 10 colorectal adenoma patients (11.8%). The sensitivity of M2-PK test in colorectal cancer detection was higher than gFOBT (100% vs. 64.7%). M2-PK test had a lower specificity when compared to gFOBT (72.5% vs. 88.2%) in colorectal cancer detection. The positive and negative predictive values were 47.2% and 100% for M2-PK test and 57.9% and 90.9% for gFOBT.

    CONCLUSION: Fecal M2-PK Quick test has a high sensitivity for detection of colorectal cancer when compared to gFOBT, making it the potential choice for colorectal tumor screening biomarker in the future.

    Various aspects of task execution load balancing of Internet of Things (IoTs) networks can be optimised using intelligent algorithms provided by software-defined networking (SDN). These load balancing aspects include makespan, energy consumption, and execution cost. While past studies have evaluated load balancing from one or two aspects, none has explored the possibility of simultaneously optimising all aspects, namely, reliability, energy, cost, and execution time. For the purposes of load balancing, implementing multi-objective optimisation (MOO) based on meta-heuristic searching algorithms requires assurances that the solution space will be thoroughly explored. Optimising load balancing provides not only decision makers with optimised solutions but a rich set of candidate solutions to choose from. Therefore, the purposes of this study were (1) to propose a joint mathematical formulation to solve load balancing challenges in cloud computing and (2) to propose two multi-objective particle swarm optimisation (MP) models; distance angle multi-objective particle swarm optimization (DAMP) and angle multi-objective particle swarm optimization (AMP). Unlike existing models that only use crowding distance as a criterion for solution selection, our MP models probabilistically combine both crowding distance and crowding angle. More specifically, we only selected solutions that had more than a 0.5 probability of higher crowding distance and higher angular distribution. In addition, binary variants of the approaches were generated based on transfer function, and they were denoted by binary DAMP (BDAMP) and binary AMP (BAMP). After using MOO mathematical functions to compare our models, BDAMP and BAMP, with state of the standard models, BMP, BDMP and BPSO, they were tested using the proposed load balancing model. Both tests proved that our DAMP and AMP models were far superior to the state of the art standard models, MP, crowding distance multi-objective particle swarm optimisation (DMP), and PSO. Therefore, this study enables the incorporation of meta-heuristic in the management layer of cloud networks.

    Introduction: Due to rapid urbanization, Malaysia is currently undergoing extensive amount of construction activities and significant rise in the construction waste. Improper construction wastes management practices have led to rise in the waste open dumping sites which may significantly affect the environment and public health. This study was undertaken to assess air pollution in such a site and the impact to the respiratory health of children studying in a nearby school. Methods: A comparative cross-sectional study was conducted in Sungai Buloh (exposed) and Kuala Selangor (comparative). Concentration of PM10 was monitored at the open dumping site (n=15), a nearby primary school (n=45) and a comparative primary school (n=12). Parents who gave consent (n=229) answered a questionnaire related to child’s respiratory health whereas their children participated in lung function assessment. Results: Concentration of PM10 at the dumping site, exposed school and comparative school was 0.245±0.048mg/ m3 , 0.270±0.020mg/m3 and 0.051±0.016mg/m3 respectively, with the first two significantly exceeded the 24-hour Malaysian Air Quality Standard (MAQS) for PM10 (0.150mg/m3 ).     The ever-expanding human population puts tremendous pressure on global food security. With climate change threats lowering crop productivity and food nutritional quality, it is important to search for alternative and sustainable food sources. Microalgae are a promising carbon-neutral biomass with fast growth rate and do not compete with terrestrial crops for land use. More so, microalgae synthesize exclusive marine carotenoids shown to not only exert antioxidant activities but also anti-cancer properties. Unfortunately, the conventional method for fucoxanthin extraction is mainly based on solvent extraction, which is cheap but less environmentally friendly. With the emergence of greener extraction techniques, the extraction of fucoxanthin could adopt these strategies aligned to UN Sustainable Development Goals (SDGs). This is a timely review with a focus on existing fucoxanthin extraction processes, complemented with future outlook on the potential and limitations in alternative fucoxanthin extraction technologies. This review will serve as an important guide to the sustainable and environmentally friendly extraction of fucoxanthin and other carotenoids including but not limited to astaxanthin, lutein or zeaxanthin. This is aligned to the SDGs wherein it is envisaged that this review becomes an antecedent to further research work in extract standardization with the goal of meeting quality control and quality assurance benchmarks for future commercialization purposes.

    Accurate inspection of welded materials is important in relation to achieve acceptable standards. Radiography, a non-destructive test method, is commonly used to evaluate the internal condition of a material with respect to defect detection. The presence of noise in low resolution of radiographic images significantly complicates analysis; therefore attaining higher quality radiographic images makes defect detection more readily achievable. This paper presents a study pertaining to the quality enhancement of radiographic images with respect to different types of defects. A series of digital radiographic weld flaw images were smoothed using multiple smoothing techniques to remove inherent noise followed by top and bottom hat morphological transformations. Image quality was evaluated quantitatively with respect to SNR, PSNR and MAE. The results indicate that smoothing enhances the quality of radiographic images, thereby promoting defect detection with the respect to original radiographic images.

    Characterization and quantification of carotenoid compound is complicated, costly and timeconsuming. The accuracy and reliability of the data depend solely on the standard and to High Performance Liquid Chromatography (HPLC) analysis but the major constraint is to acquire and to maintain the pure standards. Carotenoid standards are commercially available but they are expensive and are prone to isomerization and oxidation. Thus, the purpose of this study is to establish an analytical method for isolating β-carotene by using open column chromatography (OCC) from pumpkin (Cucurbita moschata) to be used as one of the carotenoid standards for determination of total and individual carotenoid. Pumpkin with orange flesh has been chosen due to the non-seasonal nature and its availability all year-round. This study demonstrated that the purity of β-carotene standard; determined by HPLC was ranged from 92.21 to 97.95%. The standard curves with five different concentrations of β-carotene extract from pumpkins in triplicate were constructed by plotting the peak area against the concentration. The coefficient of correlation was 0.9936. Therefore, this study established that pumpkin can be a reliable source of beta-carotene standard as it is cheap and commonly available throughout the year.

    A sensitive and selective high-performance liquid chromatographic method was developed for the determination of itraconazole and its active metabolite, hydroxyitraconazole, in human plasma. Prior to analysis, both compounds together with the internal standard were extracted from alkalinized plasma samples using a 3:2 (v/v) mixture of 2,2,4-trimethylpentane and dichloromethane. The mobile phase comprised 0.02 M potassium dihydrogen phosphate-acetonitrile (1:1, v/v) adjusted to pH 3.0. Analysis was run at flow-rate of 0.9 ml/min with excitation and emission wavelengths set at 260 and 365 nm, respectively. Itraconazole was found to adsorb on glass or plastic tubes, but could be circumvented by prior treating the tubes using 10% dichlorodimethylsilane in toluene. Moreover, rinsing the injector port with acetonitrile helped to overcome any carry-over effect. This problem was not encountered with hydroxyitraconazole. The method was sensitive with limit of quantification of 3 ng/ml for itraconazole and 6 ng/ml for hydroxyitraconazole. The calibration curve was linear over a concentration range of 2.8-720 ng/ml for itraconazole and 5.6-720 ng/ml for the hydroxy metabolite. Mean recovery value of the extraction procedure for both compounds was about 85%, while the within-day and between-day coefficient of variation and percent error values of the assay method were all less than 15%. Hence, the method is suitable for use in pharmacokinetic and bioavailability studies of itraconazole.

    A simple and sensitive high-performance liquid chromatographic (HPLC) method using ultraviolet detection was developed for the determination of testosterone in human plasma. Testosterone and the internal standard, griseofulvin, were extracted from 0.50 ml plasma sample using a mixture of dichloromethane-2,2,4-trimethylpentane (3:2, v/v). The mobile phase, consisted of 0.02 M sodium dihydrogenphosphate-acetonitrile-methanol (51:47:2, v/v) adjusted to pH 3.1 and delivered to a C(18) analytical column (150 x 4.6 mm I.D., 4 microm particles) at a flow-rate of 1 ml/min while the detection wavelength was set at 240 nm with a sensitivity range of 0.005 a.u.f.s. The method has a quantification limit of 1.6 ng/ml. Recoveries of testosterone were all greater than 92% over the linear concentration range of 1.6-400 ng/ml while that of griseofulvin was approximately 95%. The within- and between-day RSD values were all less than 8% while the accuracy values ranged from 96.0 to 106.0% over the concentration range studied. The method was applied to the analysis of early morning plasma testosterone levels of 12 healthy human male volunteers. The levels were found to range from 3.1 to 8.4 ng/ml, within the normal range reported in the literature.

    The Belt and Road Initiative (BRI) represents the largest infrastructure and development project in human history, and presents risks and opportunities for ecosystems, economies, and communities. Some risks (habitat fragmentation, roadkill) are obvious, however, many of the BRI's largest challenges for development and conservation are not obvious and require extensive consideration to identify. In this first BRI Horizon Scan, we identify 11 frontier issues that may have large environmental and social impacts but are not yet recognised. More generally, the BRI will increase China's participation in international environmental governance. Thus, new cooperative modes of governance are needed to balance geopolitical, societal, and environmental interests. Upgrading and standardising global environmental standards is essential to safeguard ecological systems and human societies.

    Due to the limitations in the range of antibodies recognising avian viruses, quantitative real-time PCR (RT-qPCR) is still the most widely used method to evaluate the expression of immunologically related genes in avian viruses. The objective of this study was to identify suitable reference genes for mRNA expression analysis in chicken intraepithelial lymphocyte natural killer (IEL-NK) cells after infection with very-virulent infectious bursal disease virus (vvIBDV). Fifteen potential reference genes were selected based on the references available. The coefficient of variation percentage (CV%) and average count of these 15 genes were determined by NanoString technology for control and infected samples. The M and V values for shortlisted reference genes (ACTB, GAPDH, HMBS, HPRT1, SDHA, TUBB1 and YWHAZ) were calculated using geNorm and NormFinder. GAPDH, YWHAZ and HMBS were the most stably expressed genes. The expression levels of three innate immune response related target genes, CASP8, IL22 and TLR3, agreed in the NanoString and RNA sequencing (RNA-Seq) results using one or two reference genes for normalisation (not HMBS). In conclusion, GAPDH and YWHAZ could be used as reference genes for the normalisation of chicken IEL-NK cell gene responses to infection with vvIBDV.

    Objective. To determine the diagnostic accuracy of placental alpha microglobulin-1 assay and standard diagnostic methods for detecting rupture of membrane. Study Design. Prospective diagnostic study, between June 2011 to November 2011 at a tertiary centre. Initial evaluation included both the standard diagnostic methods for rupture of membranes and placental alpha microglobulin-1 immunoassay. The actual rupture of membranes was diagnosed on review of the medical records after delivery (absence of membrane or a positive pad chart). Main Outcome Measures. Placental alpha microglobulin-1 immunoassay and standard diagnostic methods for diagnosis of rupture of membrane. Results. A total of 211 patients were recruited. At initial presentation, 187 patients (88.6%) had ruptured membranes, while 24 patients (11.4%) had intact membranes. Placental alpha microglobulin-1 immunoassay confirmed rupture of membranes at initial presentation with a sensitivity of 95.7% (179 of 187), specificity of 100% (24 of 24), positive predictive value of 100% (179 of 179), and negative predictive value of 75.0% (24 of 32). By comparison, the conventional standard diagnostic methods had a sensitivity of 78.1% (146 of 187), specificity of 100% (24 of 24), positive predictive value of 100% (146 of 146), and negative predictive value of 36.9% (24 of 65) in diagnosing rupture of membrane. Conclusion. Placental alpha-microglobulin-1 immunoassay is a rapid and accurate method for confirming the diagnosis of rupture of membrane. It was superior to conventional standard diagnostic methods (pooling, nitrazine, and ferning), the nitrazine test alone or fern test alone.

    A simple and sensitive preconcentration strategy using sequential electrokinetic and hydrodynamic injection modes in micellar electrokinetic chromatography with diode array detection was developed and applied for the separation and determination of anticancer agent, 5-fluorouracil and its metabolite, 5-fluoro-2'-deoxyuridine, in human plasma. Sequential injection modes with increased analyte loading capacity using the anionic pseudo-stationary phase facilitated collection of the dispersed neutral and charged analytes into narrow zones and improved sensitivity. Several important parameters affecting sample enrichment performance were evaluated and optimized in this study. Under the optimized experimental conditions, 614- and 643-fold and 782- and 803-fold sensitivity improvement were obtained for 5-fluorouracil and its metabolite when compared with normal hydrodynamic and electrokinetic injection, respectively. The method has good linearity (1-1,000 ng/ml) with acceptable coefficient of determination (r2  > 0.993), low limits of detection (0.11-0.14 ng/ml) and satisfactory analyte relative recovery (97.4-99.7%) with relative standard deviations of 4.6-9.3% (n = 6). Validation results as well as the application to analysis of human plasma samples from cancer patients demonstrate the applicability of the proposed method to clinical studies.

    A new resveratrol dimer, acuminatol (1), was isolated along with five known compounds from the acetone extract of the stem bark of Shorea acuminata. Their structures and stereochemistry were determined by spectroscopic methods, which included the extensive use of 2D NMR techniques. All isolated compounds were evaluated for their antioxidant activity using the 2,2-diphenyl-1-picrylhydrazyl (DPPH) radical scavenging activity (RSA) and the β-carotene-linoleic acid (BCLA) assays, and compared with those of the standards of ascorbic acid (AscA) and butylated hydroxytoluene (BHT). All compounds tested exhibited good to moderate antioxidant activity in the DPPH assay (IC₅₀s 0.84 to 10.06 mM) and displayed strong inhibition of β-carotene oxidation (IC₅₀s 0.10 to 0.22 mM). The isolated compounds were evaluated on the Vero cell line and were found to be non-cytotoxic with LC₅₀ values between 161 to 830 µM.

    Non-alcoholic fatty liver disease (NAFLD) is one of the main causes of fibrosis. Liver biopsy remains the gold standard for the confirmation of fibrosis in NAFLD patients. Effective and non-invasive diagnosis of advanced fibrosis is essential to disease surveillance and treatment decisions. Herein we used routine medical test markers and logistic regression to differentiate early and advanced fibrosis in NAFLD patients from China, Malaysia, and India (n1 = 540, n2 = 147, and n3 = 97) who were confirmed by liver biopsy. Nine parameters, including age, body mass index, fasting blood glucose, presence of diabetes or impaired fasting glycemia, alanine aminotransferase, γ-glutamyl transferase, triglyceride, and aspartate transaminase/platelet count ratio, were selected by stepwise logistic regression, receiver operating characteristic curve (ROC), and hypothesis testing and were used for model construction. The area under the ROC curve (auROC) of the model was 0.82 for differentiating early and advanced fibrosis (sensitivity = 0.69, when specificity = 0.80) in the discovery set. Its diagnostic ability remained good in the two independent validation sets (auROC = 0.89 and 0.71) and was consistently superior to existing panels such as the FIB-4 and NAFLD fibrosis score. A web-based tool, LiveFbr, was developed for fast access to our model. The new model may serve as an attractive tool for fibrosis classification in NAFLD patients.
