    BACKGROUND: Good coronary care begins from the patient's home, including early transportation. As such, it is recommended that the patients activate ambulances, rather than to use their own transportations to reach the hospitals. It is not known whether Malaysian patients prefer to use private transportations or ambulances when they develop chest pain.

    OBJECTIVES: This study is conducted to explore the question of the choice of transportation modes among patients with acute coronary syndrome and the reasons behind their choices.

    METHODS: This is a structured interview survey on patients diagnosed with acute coronary syndrome (ACS) in emergency department of Hospital Universiti Sains Malaysia from April 2012 to September 2012.

    RESULTS: Out of the 110 patients surveyed, 105 (95.5%) patients chose to use own transportation when they developed symptoms suggestive of ACS. Only 3 patients (2.7%) came to the emergency department within 1 hour of onset, and all these 3 patients chose to use ambulances as their modes of transportation. None of the patients who chose own transportation came within the first hour of symptoms onset. This is shown to be statistically significant (p<0.001). The level of education as well as past history of ischemic heart disease did not significantly influence the patients' choice of transportation.

    CONCLUSION: The admonishment by various international resuscitation councils that patients with chest pain should be transported via ambulances may not be as straightforward as it seems. Numerous local and regional socio-cultural and logistic factors may need to be addressed.

    Preventive tests and diagnosis of in-service power transformer are important for early fault prediction and increased reliability of electricity supply. However, some existing diagnostic techniques require transformer outage before the measurement can be performed and need expert knowledge and experiences to interpret the measurement results. Other measurement techniques such as chemical analyses of insulating oil may cause significant variance to measurement results due to different practices in oil sampling, storage, handling and transportation of oil. A cost-effective measuring technique, which is simple, providing fast and an accurate measurement results, is therefore highly required. The extended application of Polarisation and Depolarisation (PDC) measurement for characterisation of different faults conditions in-service power transformer has been presented in this paper. Earlier studies on polarisation and depolarisation current of oil samples from in-service power transformer shows that depolarisation has provided significant information about the change of material properties due to faults in power transformer. In this paper, a new approach based on Depolarisation Current Ratio Index (DRI) was developed for identifying and classifying different transformer fault conditions. The DRI at time interval of 4s to 100s was analysed and the results show that DRI of depolarisation current between 5/100s and 10/100s provides higher correlation on the incipient faults in power transformer.

    An experiment was conducted to determine the effects of 2 types of housing systems and early age feed restriction on heat shock protein (hsp) 70 expression and blood parameters in broiler chickens subjected to road transportation. On d 1, female chicks were housed either in windowless environmentally controlled chambers (temperature was set at 32 degrees C on d 1 and gradually reduced to 23 degrees C by d 21; CH) or in conventional open-sided houses (OH) with cyclic temperatures (minimum, 24 degrees C; maximum, 34 degrees C). Equal number of chicks from each housing system were subjected to either ad libitum feeding or 60% feed restriction on d 4, 5, and 6 (FR). On d 42, all of the birds were crated and transported for 6 h. Birds raised in OH had smaller increases in heterophil:lymphocyte ratios and plasma corticosterone concentrations than those of CH. Subjecting birds to FR dampened heterophil:lymphocyte ratios and corticosterone reactions to transportation. After 4 h of transportation, the OH birds had greater hsp 70 expression than their CH counterparts. Within the CH, the FR chicks showed higher hsp 70 density than those of the ad libitum-fed group. Except for glucose, housing system had a negligible effect on serum levels of cholesterol, potassium, and chloride. Collectively, the results suggest that the improved tolerance to transport stress in OH and FR chicks could be associated with better hsp 70 expression.

    Periodontal bio-repositories, which allow banking of clinically validated human data and biological samples, provide an opportunity to derive biomarkers for periodontal diagnosis, prognosis and therapeutic activities which are expected to improve patient management. This article presents the establishing of the Malaysian Periodontal Database and Biobank System (MPDBS) which was initiated in 2011 with the aim to facilitate periodontal research. Partnerships were established with collaborating centres. Policies on specimen access, authorship and acknowledgement policies were agreed upon by all participating centres before the initiation of the periodontal biobank. Ethical approval for the collection of samples and data were obtained from institutional ethics review boards. A broad-based approach for informed consent was used, which covered areas related to quality of life impacts, genetics and molecular aspects of periodontal disease. Sample collection and processing was performed using a standardized protocol. Biobanking resources such as equipment and freezers were shared with the Malaysian Oral Cancer Database and Tissue Bank System (MOCDTBS). In the development of the MPDBS, challenges that were previously faced by the MOCDTBS were considered. Future challenges in terms of ethical and legal issues will be faced when international collaborations necessitate the transportation of specimens across borders.

    In 2010, Klang Valley has only 17% trips each day were completed using public transport, with the rest of the 83% trips were made through private transport. The inclination towards private car usage will only get worse if the transport policy continues to be inefficient and ineffective. Under the National Key Economic Area, the priority aimed to stimulate the increase of modal share of public transport in the Klang Valley to 50% by 2020. In the 10th Malaysia Plan, the Klang Valley Mass Rapid Transit was proposed, equipped with 141 km of MRT system, and will integrate with the existing rail networks. Nevertheless, adding kilometers into the rail system will not help, if people do not make the shift from private into public transport. This research would like to assess the possible mode shift of travellers in the Klang Valley towards using public transport, based on the utility function of available transport modes. It intends to identify the criteria that will trigger their willingness to make changes in favour of public transport as targeted by the NKEA.

    Environmental noise remains a complex and fragmented interplay between industrialization, population growth, technological developments, and the living environment. Next to the circulatory diseases and cancer, noise pollution has been cited as the third epidemic cause of psychological and physiological disorders internationally. A reliable and firm relationship between the cumulative health implications with the traffic annoyance and occupational noise has been established. This agenda has called for an integrated, coordinated, and participatory approach to the reliable protection of noise interference. Despite several fragmented policies, legislation and global efforts have been addressed; the noise pollution complaints have been traditionally neglected in developing countries, especially in Malaysia. This paper was undertaken to postulate an initial platform to address the dynamic pressures, gigantic challenges, and tremendous impacts of noise pollution scenario in Malaysia. The emphasis is speculated on the traffic interference and assessment of industrial and occupational noise. The fundamental importance of noise monitoring and modeling is proposed. Additionally, the confronting conservation program and control measure for noise pollution control are laconically elucidated.

    Bus services naturally tend to be unstable and are not always capable of adhering to schedules without control strategies. Therefore, bus users and bus service providers face travel time variation and irregularity. After a comprehensive review of the literature, a significant gap was recognized in the field of public transportation reliability. According to literature, there is no consistency in reliability definition and indicators. Companies have their own definition of bus service reliability, and they mostly neglect the passengers' perspective of reliability. Therefore, four reliability indicators were selected in this study to fill the gap in the literature and cover both passengers' and operators' perceptions of reliability: waiting time and on-board crowding level from passengers' perspective, and headway regularity index at stops (HRIS) and bus bunching/big gap percentage from operators' perspective. The primary objective of this research is to improve the reliability of high frequency of bus service and simulation tools currently being used by the public transportation companies. Therefore, a simulation model of bus service was developed to study the strategies to alleviate it. Four different types of strategies were selected and implemented according to Route U32 (Kuala Lumpur) specifications. Model out-put showed that control strategies such as headway-based dispatching could significantly improve headway regularity by almost 62% and the waiting time by 51% on average. Both holding strategies at key stops (previous and Prefol holding) have shown an almost similar impact on reliability indicators. Waiting time was reduced by 44% and 43% after the previous and Prefol Headway strategies were adopted, respectively. However, the implementation of the component of headway-based strategies at the terminal and key stops showed the best impact on reliability, in terms of passenger waiting time. Waiting time and excess waiting time were both significantly reduced by 52.86% and 81.44%, respectively. Nevertheless, the strategies did not show any significant positive effect on the level of crowding during morning peak hours.

    Spray drying is used widely for converting liquid food products into powder form as the dried powder is known to have a longer shelf life at ambient temperature, convenience to use and low transportation expenditure. In this study, the Sarawak pineapple puree was spray-dried and the characterization of the resulting powder was performed. The process of enzyme liquefaction was optimized with Pectinex® Ultra SP-L and Celluclast® 1.5 L (single and combined treatment) at different concentrations (0–2.5 %) and incubation time (0-2.5 hours). The combined treatment with both enzymes (1.5% v/w Pectinex® Ultra SP-L + 0.5% v/w Celluclast® 1.5 L, 1.5 hour) was found to be the best parameter, which produced purees with the lowest viscosity of 67.98 ± 4.27 cp. Optimization of spray drying process was carried out using different inlet temperatures (150-180°C) and maltodextrin concentrations (15-30 % w/w). Results indicated that the spraydried powder produced at 160°C with 15% w/w of maltodextrin has the highest yield (31.63 %). The spray-dried powder was further characterized for the moisture content (6.00 ± 0.63%), water activity (0.36 ± 0.01 Aw), hygroscopicity (17.35 ± 0.64%), bulk density (0.46 ± 0.04 g/ cm3 ) and solubility (87.33 ± 2.08 seconds). The fruit powder of this study can be incorporated into different fruit added –value products, such as fruit juice, yogurt, jelly and other beverages.

    In light of a slow buildup in CO2 emissions since the recovery, this paper revisits the relationship between CO2 emissions and the US economy using a nonlinear autoregressive distributed lag model, in which the determinants are identified through an expanded real business cycle model. We find convincing evidence that CO2 emissions decline more rapidly during recessions than increase during expansions over the long run. Of all determinants considered, long-run asymmetry is fostered once vehicle miles traveled is controlled. This calls for a greater attention to public transportation development and vehicle miles traveled tax for slowing down stock buildup of CO2 emissions during good times.

    BACKGROUND: The novel coronavirus disease 2019 (COVID-19) pandemic has set a huge challenge to the delivery of neurosurgical services, including the transfer of patients. We aimed to share our strategy in handling neurosurgical emergencies at a remote center in Borneo island. Our objectives included discussing the logistic and geographic challenges faced during the COVID-19 pandemic.

    METHODS: Miri General Hospital is a remote center in Sarawak, Malaysia, serving a population with difficult access to neurosurgical services. Two neurosurgeons were stationed here on a rotational basis every fortnight during the pandemic to handle neurosurgical cases. Patients were triaged depending on their urgent needs for surgery or transfer to a neurosurgical center and managed accordingly. All patients were screened for potential risk of contracting COVID-19 prior to the surgery. Based on this, the level of personal protective equipment required for the health care workers involved was determined.

    RESULTS: During the initial 6 weeks of the Movement Control Order in Malaysia, there were 50 urgent neurosurgical consultations. Twenty patients (40%) required emergency surgery or intervention. There were 9 vascular (45%), 5 trauma (25%), 4 tumor (20%), and 2 hydrocephalus cases (10%). Eighteen patients were operated at Miri General Hospital, among whom 17 (94.4%) survived. Ninety percent of anticipated transfers were avoided. None of the medical staff acquired COVID-19.

    CONCLUSIONS: This framework allowed timely intervention for neurosurgical emergencies (within a safe limit), minimized transfer, and enabled uninterrupted neurosurgical services at a remote center with difficult access to neurosurgical care during a pandemic.

    BACKGROUND: Nipah virus (NiV) is a fatal zoonotic agent that was first identified amongst pig farmers in Malaysia in 1998, in an outbreak that resulted in 105 fatal human cases. That epidemic arose from a chain of infection, initiating from bats to pigs, and which then spilled over from pigs to humans. In Thailand, bat-pig-human communities can be observed across the country, particularly in the central plain. The present study therefore aimed to identify high-risk areas for potential NiV outbreaks and to model how the virus is likely to spread. Multi-criteria decision analysis (MCDA) and weighted linear combination (WLC) were employed to produce the NiV risk map. The map was then overlaid with the nationwide pig movement network to identify the index subdistricts in which NiV may emerge. Subsequently, susceptible-exposed-infectious-removed (SEIR) modeling was used to simulate NiV spread within each subdistrict, and network modeling was used to illustrate how the virus disperses across subdistricts.

    RESULTS: Based on the MCDA and pig movement data, 14 index subdistricts with a high-risk of NiV emergence were identified. We found in our infectious network modeling that the infected subdistricts clustered in, or close to the central plain, within a range of 171 km from the source subdistricts. However, the virus may travel as far as 528.5 km (R0 = 5).

    CONCLUSIONS: In conclusion, the risk of NiV dissemination through pig movement networks in Thailand is low but not negligible. The risk areas identified in our study can help the veterinary authority to allocate financial and human resources to where preventive strategies, such as pig farm regionalization, are required and to contain outbreaks in a timely fashion once they occur.

    Consumer interest in privately managed urban transport services in Indonesia are showing a declining trend. On the other hand, the presence of Transportation on Demand (ToD) based on Internet of Things (IoT) has attracted the majority of conventional fleet customers which contribute to the declining trend of occupancy. Therefore, this study aims to present a feasibility study of a small car RE60 Three Passenger and One Driver (3P+1D) four-wheeler as an alternative to replace conventional fleets. The Break-Even Point (BEP), Net Present Value (NPV), Internal Rate of Return (IRR), and Payback Period (PP) are analyzed based on vehicle operating data. The analysis shows that there is an additional monthly income flow of IDR 1,533,122 and IDR 2,536,946 from RE60 operations compared to the high and low consumption of fuel from existing fleets, respectively. In conclusion, it is feasible to invest in small car RE60 as a replacement for the existing fleets since the fundamental indicators (BEP, NPV, IRR, and PP) showed positive results before the specified instalment period. Sensitivity analysis also shows good results, NPV shows positive results (>0) although the input conditions are made pessimistic to -30% from NPV base case. The BEP of RE60 will cut distance travelled at 190,670 km and 115,225 km, respectively. NPV of IDR 33,088,000 and IDR 80,841,000 will be obtained at the 60th month after the operation and IRR also enabled good scores, at 2.24% and 4.17%.

    In Malaysia, the greenhouse gases (GHGs) emissions reduction via composting of source-separated organic waste (SOW) in municipal solid waste (MSW) has not been assessed. Assessment of GHG emissions reduction via composting of SOW is important as environmental impacts from waste management are waste-specific and local-specific. The study presents the case study for potential carbon reduction via composting of SOW in University of Malaya (UM). In this study, a series of calculations were used to evaluate the GHG emission of different SOW management scenarios. The calculations based on IPCC calculation methods (AM0025) include GHGs emissions from landfilling, fuel consumption in transportation and SOW composting activity. The methods were applied to assess the GHG emissions from five alternative SOW management scenarios in UM. From the baseline scenario (S0), a total of 1,636.18 tCO2e was generated. In conjunction with target of 22% recycling rate, as shown in S1, 14% reduction in potential GHG emission can be achieved. The carbon reduction can be further enhanced by increasing the SOW composting capacity. The net GHG emission for S1, S2, S3 and S4 were 1,399.52, 1,161.29, 857.70 and 1,060.48 tCO2e, respectively. In general, waste diversion for composting proved a significant net GHG emission reduction as shown in S3 (47%), S4 (35%) and S2 (29%). Despite the emission due to direct on-site activity, the significant reduction in methane generation at landfill has reduced the net GHG emission. The emission source of each scenario was studied and analysed.

    This study quantitatively investigated the scientific progress of walkability research landscape and its future prospects using bibliometric indicators to highlight the research hotspots. The results accentuated multifaceted nature of walkability research landscape with a strong association towards public health disciplines. Keyword co-occurrence analysis emphasized that majority of the walkability studies centred on the interactions between walking and other three main factors such as built environment attributes, transportation and obesity. Based on the identified research hotspots, a brief state-of-the-art review of walkability studies was presented.     Absorbed dose rates in vehicles during travelling by different modes of transport in Malaysia were measured. Radiation levels measured on roads in Peninsular Malaysia were within a broad range, i.e. between 36 and 1560 nGy h(-1). The highest reading, recorded while travelling near monazite and zircon mineral dumps, was 13 times the mean environmental radiation level of Malaysia. It is evident that radioactive material dumps on the roadsides can influence the radiation level on the road. The absorbed dose rates measured while travelling on an ordinary train were between 60 and 350 nGy h(-1). The highest reading was measured when the train passed a tunnel built through a granite rock hill. The measurement during sea travelling by ferries gave the lowest radiation level owing to merely cosmic radiation at the sea level.

    INTRODUCTION: Aging is associated with physiological changes in multiple systems in the body and may impact the transportation choices of older adults. In this study, we examine the associations between biopsychosocial factors and the transportation choices of Malaysian older adults.

    METHODS: One hundred and nineteen (119) older adults, aged 60 and above, living in Klang Valley, Malaysia were recruited for this cross-sectional study. Participants were interviewed face-to-face to obtain sociodemographic data, health status (whether there were and, if yes, the number of comorbidities), outdoor mobility and transportation patterns, Instrumental Activity Daily Living (IADL) status and cognitive function. Participants' physical performance (dominant handgrip strength, 10-m walk, and timed up and go tests), hearing threshold (pure tone audiometry), and vision function (visual acuity, contrast sensitivity) were measured. Transportation patterns of older adults were categorized into three groups, that is, flexible (using public transport and/or private vehicles), using only private vehicles and restricted (relying on others or walking).

    RESULTS: Further information is needed to enable such older adults as older women, those with comorbidities and poorer functional status to access transportation, especially to meet their health care needs.

    DISCUSSION: The majority (51%) of participants were in the 'using only private vehicles' group, followed by the 'flexibles' (25%) and the 'restricted' (24%). Factors significantly associated with the restricted transportation group were: (a) being female (AdjOR 15.39, 95% CI 0.86-23.39, p

    Twenty transport-stressed goats were divided into two groups. The first group was further stressed with steroid. Pasteurella haemolytica was found at various sites in the nasal cavity of goats in this group as early as 2 weeks post-transportation. The successful isolations continued consistently with more goats having pure growth of P. haemolytica at later stages. Mild catarrh rhinitis, loss of epithelial cilia and erosions were the main lesions observed in the nasal cavity. Goats in the second group that were not given steroid injections had inconsistent bacterial isolation and less severe pathological lesions.

    The scour phenomenon around bridge piers causes great quantities of damages annually all over the world. Collars are considered as one of the substantial methods for reducing the depth and volume of scour around bridge piers. In this study, the experimental and numerical methods are used to investigate two different shapes of collars, i.e, rectangular and circular, in terms of reducing scour around a single bridge pier. The experiments were conducted in hydraulic laboratory at university of Malaya. The scour around the bridge pier and collars was simulated numerically using a three-dimensional, CFD model namely SSIIM 2.0, to verify the application of the model. The results indicated that although, both types of collars provides a considerable decrease in the depth of the scour, the rectangular collar, decreases scour depth around the pier by 79 percent, and has better performance compared to the circular collar. Furthermore, it was observed that using collars under the stream's bed, resulted in the most reduction in the scour depth around the pier. The results also show the SSIIM 2.0 model could simulate the scour phenomenon around a single bridge pier and collars with sufficient accuracy. Using the experimental and numerical results, two new equations were developed to predict the scour depth around a bridge pier exposed to circular and rectangular collars.
