    BACKGROUND: Previous research in England showed that deprivation level of a person's place of residence affects the place of death and quality of care received at the end of life. People dying in their preferred place of death has also been shown to act as an indication for high quality of end of life care services and social equality. This study expands on current research to explore the effects of deprivation and place of residence on health related choices and place of death in Wales.

    METHODS: We used ten years combined mortality statistics from 2005 to 2014 and Welsh Index of Multiple Deprivation rankings for each lower super output area. After accounting for the population's age, the number of deaths in Hospital, Hospice, Home, Care Home, Psychiatric Units, and Elsewhere were compared across deprivation quintiles.

    RESULTS: Distribution of place of death was found to be concentrated in three places - hospital (60%), home (21%) and care home (13%). Results from this study shows a high number of hospital deaths, especially for more deprived areas, despite being the least preferred place of death.

    CONCLUSION: This is the first Welsh study investigating place of death in relation to deprivation, which could be of major importance to academics, end of life care providers and policy makers interested in to reduce health care inequality in Wales.

    The intercountry border areas of Thailand have high malaria receptivity and vulnerability that present numerous problems in the control of malaria transmission. This study focused on the 30 provinces of Thailand situated next to neighboring countries, which can be divided into 4 groups: the Thai-Myanmar border (10 provinces), the Thai-Cambodia border (6 provinces), the Thai-Lao border (10 provinces) and the Thai-Malaysia border (4 provinces). The purpose of the present study was to describe the pattern and trend of malaria incidence in the highly endemic provinces along the Thai borders for the 11 years from 1991 to 2001. Analysis of trends showed the distribution of malaria parasites to have shifted from a preponderance of Plasmodium falciparum to Plasmodium vivax along the western border with Myanmar, the northern border with Lao PDR and along the eastern border with Cambodia whereas the southern border with Malaysia the pattern changed from a preponderance of P. vivax to P. falciparum, since 1997. There was a significant difference in annual parasite incidence between borders and non-border districts, especially along the Thai-Myanmar and Thai-Cambodia borders. It is thus evident that all border districts should pay more attention to control of malaria transmission and the activities of the malaria surveillance system, and that monitoring and evaluation of the Thai Malaria Control Program needs to be performed consistently, including some areas where a few malaria cases were found as well as in malaria free areas.

    The Healthy City concept was taken by Malaysia in 1994 and Malacca State has initiated this project in late 1997 and launched in September 1998. The aim of the project is to had ways of achieving a better quality of urban life. The objective of this study was to assess the views and responses of Melaka Tengah community with regards to the existing facilities and services rendered in the district. The views will be incorporated into ideas for the policymakers and planners to develop Malacca into a healthy city. Three methods were used to collect the data. Questionnaires were given to the community of Melaka Tengah District. The respondents were selected by multistage sampling, Observation was carried out at selected public places to assess the community's practices and contribution, Ten focus group discussion were conducted consisting of health staff and public to discuss on environmental, social, physical and economic issues of Malacca. There were 3 sectors that had mean scores above 3.0 (the cut off level for being satisfed). They were health, housing and environment, ln terms of dissatisfaction, there were 4 sectors scoring below 3.0, These include domestic waste dnposal, road system, public transportation and recreational park. The community
    expected the services to be improved especially in terms of cleanliness, They agreed to contribute in their own ways in developing the sectors discussed except for public transportation, wet market and food premises which were beyond their control. Observation showed that some of the community members exhibit bad behaviours that can contribute to an unhealthy city. The Melaka
    Tengah community expected ejficient and quality services and they agreed to contribute in making Malacca into a Healthy City.

    Purpose – Evidences have shown that participation in a cardiac rehabilitation programme improves outcomes and quality of life for cardiac patients. Hospital Serdang has conducted this programme since 2007 and it is the first to be run by the Rehabilitation Medicine Department. The purpose of this survey is to find out whether the programme is suitable, informative and useful from the patient’s perspective as well as to determine whether the 6 weeks duration is sufficient.

    Design/ methodology/approach - A patient feedback survey using a self-administered questionnaire on the programme was conducted from January 2008 to December 2010 in Hospital Serdang.

    Findings - A total of 323 patients were invited to join the programme but only 182 completed the programme and took part in this survey. 30% of the patients thought that the six weeks programme was not enough for them. 100% of the respondents agreed that the programme was beneficial, well suited to their problems and were willing to promote the programme to their friends. Therefore, it is recommended that this programme be extended to the community level.

    Conclusion - In conclusion, this programme is beneficial, suitable, informative and adequate from the patient’s point of view. Extension of this programme into the community as a continuation of the hospital-based programme is likely to improve the outcome further.

    Developmental disabilities in young children are common, and the prevalence is estimated as high as 0.1-1.75% in the children population for each disabilities. With increasing awareness and health expectation, more children with Developmental disabilities are presenting to health care professionals, and at earlier age. Literatures also suggested the importance of early intervention programme in determining the outcome of this group of children. Therefore, it is vital for health professionals who have direct contact with young children are competent in detecting children with possible Developmental disabilities, and have the basic knowledge about some of the common disorders in order to discuss the issues with the parents. It is also important to work with the various resources available in the local community, such as non-governmental organisations for children with special needs, schools offering special education programmes and inclusive classes, and the allied health who provide cares to this group of young children and their family. A lot of information are also available online, and may be offered to help parents gaining more understanding regarding these Developmental disabilities.

    The aim of the present study is to determine the “community indicator” for dental caries among secondary school children within the Federal Territory of Kuala Lumpur (FTKL). School factors were investigated for their ability to be used as community indicators. The components encompassing school factors include: type of school, school’s examination performances at the Peperiksaan Menengah Rendah (PMR) level, in the subjects of Mathematics, Bahasa Melayu (Malay Language), Science and English, school’s socioeconomic circumstances and school’s material deprivation. The study design is ecological in nature where schools were the unit of analysis. It used aggregated data for caries experiences acquired from the Oral Health Management Information System and the school factors which were extracted from the schools’ database. The subjects involved in this study were all the (75) day type secondary schools in FTKL. However, only 55 schools had complete information both for the dental caries factor and the school factors which were available for analysis. The use of bivariate analysis suggested that the school mean DMFT was significantly associated with the schools’ performance in English (p= 0.02) and the schools’ socioeconomic code (p= 0.005). The schools’ performance in English and socioeconomic code were able to explain about 10 percent and 14 percent of the variation in the school mean DMFT respectively. The final model that included both variables together explained about 17 percent of the variation in school mean DMFT. As a conclusion, this study suggests that the schools’ performance in English at the PMR level and the schools’ socioeconomic code could be used as community indicator to identify secondary schools with higher caries level in FTKL. Nevertheless, further
    improvement of the model is needed in order to create a more reliable indicator.

    Limited information is available about the oral health of elderly people living in “Pondok” in Kelantan. This study aimed to assess the dentition status of elderly population staying in Islamic Religious Community Setup, “Pondok” in Kelantan. A cross sectional study involving 369 elderly people was carried out. A simple random sampling method was utilized for the selection of the subjects. Oral examination was done to assess the dentition status and data was analyzed using SPSS 11.5 version. The response rate was 95.6%. Majority of the subjects were edentulous (81.0%). The mean number of natural teeth, functional natural teeth, decayed teeth (D), missing teeth (M) and teeth indicated for extraction (X) were 3.22 (SD 7.54), 2.75 (SD 6.70), 0.06 (SD 0.48), 28.80 (SD 7.51) and 0.41 (SD 1.60) respectively. Among 70 dentate subjects, the mean number of natural teeth, functional natural teeth, occluding pair of functional natural teeth, decayed teeth (D), missing teeth (M), teeth indicated for extraction (X) and DMFX(T) index were 17.1 (SD 8.00), 14.5 (SD 8.11), 6.3 (SD 4.22), 0.3 (SD 1.06), 15.1 (SD 8.20), 2.2 (SD 3.14) and 17.6 (SD 8.08) respectively. There was no teeth had been filled (F). The proportion of subjects having minimum 20 functional natural teeth was 7.3%. The above results indicate that dental caries status of elderly people living in “Pondok” in Kelantan was unsatisfactory with high percentage of edentulism, contributing to the high value of the mean DMFX(T) index, 29.3 (SD 6.63).

    Objective: The objectives of this study were to determine the effect of a one and a half year educational intervention on the job dissatisfaction of teachers in 30 Community Based Rehabilitation (CBR) centres in Kelantan, Malaysia, and to identify the factors influencing changes in job dissatisfaction following the intervention. Method: Ten educational modules were administered to the teachers. A validated Malay version of Job Content Questionnaire (JCQ) was used pre intervention, mid intervention and post intervention. Result: Repeated Measure ANOVA revealed there was a statistically significant reduction in the mean of job dissatisfaction (p = 0.048). Multiple Linear Regression revealed that co- worker support (β= 0.034 (95% CI = 0.009, 0.059)), having less decision authority (β: -0.023; 95% CI: -0.036, -0.01) and being single (β: -0.107; 95% CI: -0.176,-0.038) were significantly associated with decreases in job dissatisfaction. Conclusion: The intervention program elicited improvement in job satisfaction. Efforts should be made to sustain the effect of the intervention in reducing job dissatisfaction by continuous support visits to CBR centres.

    Injury Hncluding home injury} is an important contributor to the nations’ statistics on mortality and morbidity. However, statistics on injury is still lacking in Malaysia. In 1996, the National Health and Morbidity Survey was conducted nationwide that includes injury as one ofthe scope studied at the community level. It was found that the prevalence of sefreported home injury in Malaysia was 2.5%. The prevalence was higher among the 0-4 years and more than 80 years groups. There was no dwzrence in the prevalence of injury by urban/rural location, ethnicity, citizenship, religion, marital status, level of education, income group or type of occupation. Females however reported a slightht higher figure than males (2. 7% i· 95% CI and 2.3% i 95% CD. Recommendations made include planning and designing of preventive intervention strategies and sreas for future studies.

    Introduction : Despite the number of information generated by researchers, cardiovascular diseases problem has not clearly declined and perhaps in certain situation it is gradually increasing, affecting people who are previously at low risk. There is a tendency to believe that favourable outcomes can always be expected once intervention activities, like exercise promotion, are carried out, but practical experience gives rise to serious doubt.
    Methods : A greater understanding of the socialization mechanisms operating in the adoption of physical activity in CVD control and allow specific exercise prescriptions for communitybased prevention and control is important. This paper highlighted the author’s feeling about controlling chronic diseases by mean of community intervention.
    Results : This analysis has strongly believed that the important impetus of any community intervention approach should be oriented in the form of “from people to people”.
    Conclusion : More emphasis needs to be placed on effective management and parameters for assessment of its management success.

    Substance misuse, in particular heroin addiction contributes to health and social problems. Although effective medical treatment was available, earlier efforts confined the treatment of heroin addicts to in-house rehabilitation which required them to be estranged from the community and their families for 2 years. The in-house rehabilitative programme, implemented for at least three decades has produced low abstinence rates. On the other hand, being ‘away’ meant that many heroin addicts faced employment problems and family relationship difficulties upon completing the in-house rehabilitation. However, recently, the concerted efforts by various government and non-government organisations, and the acknowledgement that heroin addiction is a medical illness has resulted in a revamp to approaching treatment of heroin addiction. At present, methadone substitution programmes have been offered as part of treatment programme for heroin addicts in Malaysia. This new programme has been shown to be effective in treating heroin addiction and would need support and cooperation from all groups involved.

    A community-based study of childhood injuries in Kedah was undertaken in January-March 1996. The aims were to determine the types and frequencies of injuries reported; to assess the association between injury and selected variables (age, sex, place, number of children in the family, presence of care-givers and treatment); and to recommend further research and policy for childhood injury prevention. A total of 448 injuries were recorded from 1089 children in 451 randomly selected households. The incidence was 411.4/1000. There appeared to be a decrease in risk of injury as the number of children per household increased. The effect of lower age on injury was significant (p>0.05). Male children were 1.5 times more likely to injure themselves than female children when data were controlled for number of children in the household. Falls accounted for 53.3% of all injuries sustained, and occurred more frequently when the child was with non-family members. Different injury categories were associated with different caregiver categories. It is concluded that child injury prevention programmes require more data on injury situations and collaborative efforts between clinical, health and and behavioural professionals.

    This study explores the quality of life, social integration and the effects of perceived stigma of people with mental illness living in the community. Adopting a complimentary mixed method, this study was represented by 165 people with mental illness in Kuching, Sibu and Miri. Findings indicated that the quality of life scale was poor, 49.67% and 78.43% often experienced stigma. Anticipated stigma 43.79%; 16.99% all the time and 4.58% never experienced stigma. Findings also noted that people with mental illness living in the community are still largely depending on and needed continuous support from their family members/carers for financial aid and their living arrangement for a “better” quality of life. Where treatment is concerned, follow up care at home by health care providers continue to play a significant role. In order to “erase” the perceived or stigma experienced, establishing therapeutic relationship, communication and creating awareness on “stigma discrimination paradigm” poses a phenomenal challenge in the current misrepresentations of mental health messages.

    This survey was done to determine the knowledge, attitude and practice of preventive measures concerning Severe Acute Respiratory Syndrome (SARS) among Alor Gajah town community. The purpose of this study was to determine the knowledge level of SARS, to know their attitude towards SARS and health information provided, to know which sources that provide the best infomation about SARS and to identify the expectations and preferences of population on gaining health infomation. A cross-sectional study involving respondents age 18 and above staying in Alor Gajah town was done. The respondents were assessed using questionnaires comprising of 25 questions. A total 180 respondents had answered the questionnaire. All of them were aware of SARS with 93 .3% with high scores in the knowledge level. Furthermore, 9l .7% of respondents had high scores in their attitude towards SARS. About 98.9% of respondent got their information regarding SARS from television, and 83.9% of them believed the information given. Firty four percent choose television as the best method to convey information concerning SARS or other new emerging diseases. The knowledge and attitude of the respondents concerning SARS were good.

Despite these mountains of information, the trend of cancer problems has not significantly declined and perhaps in certain occasion, it gradually increased affecting those who are previously known to be less risky'. Cancer is still identified to be one of the top leading killer worldwide and is also the main source of psychosocial burden of the community particularly to the caregiver' Its problems are expected to be further increased as human beings are constantly modified and manipulated by unpredictable and dynamic human activities and globalization phenomena. Furthermore, the current advances in cancer therapy and others clinical interventions are not always guaranteed of a permanent cure, improve survival or prognosis, sustainable compliances, prolonging life and improving quality of life, thus the mortality and its associated biological and management complications are still prevalence.     Evidence on the associations between built environment and overweight in children outside developed countries is scarce. We examined associations between access to local food and physical activity environments and overweight in 5- to 17-year-old Malaysians in two states with differing overweight levels. Information on children was measured in the National Health and Morbidity Survey 2015 and combined with Geographic Information System-derived data on local food and physical activity environments. Access to the built environment was measured by presence and distance from child's residence. Complete data were available for 880 children. Access to local food outlets and parks was higher and associated with higher occurrence of overweight in children living in the state with higher overweight prevalence. When adjusted for sociodemographic factors, higher presence of and shorter distance to convenience stores and parks were associated with overweight. Both built environment and children's sociodemographic backgrounds should be considered when planning interventions to curb the overweight epidemic in Malaysia.

    We conducted mosquito surveillance at outdoor breeding habitat in 459 households at 7 urban locations in Putrajaya, Malaysia from January to December 2010 to determine the predominant species and breeding locations. The most common species found at all locations was Aedes albopictus. Gardening utensils were the most common breeding sites. Of the 1,885 mosquito larvae specimens found, 1,774 (94.1%) were Ae. albopictus larvae, 84 (4%) were Ae. aegypti larvae and 27 (1%) were Culex quinquefasciatus larvae. The Aedes index for each of the locations was higher than the goal set by the Ministry of Health for Malaysia. However, the container index at each of the locations was within the goal. The Breateau index was above the goal set by the Ministry of Health at Precinct 9B1 but the other locations were within the goal.

    The incidence of osteoporosis is increasing worldwide. It has great impact on the life of the elderly population. The most significant medical consequence of osteoporosis is fragility fracture which without proper treatment will cause severe medical and psychosocial complications. The overall cost in managing osteoporosis and its related fractures is escalating. Using bone densitometry to measure bone mineral density is useful in the diagnosis of osteoporosis but it is costly and not feasible in the community. Drugs such as estrogen replacement, raloxifene and calcitonin are effective in prevention and treatment of osteoporosis but they are also expensive. Identifying modifiable risk factors such as smoking, lack of exercise, low dietary calcium and vitamin D intake and healthy life style remain strategy in the primary prevention of osteoporosis in the community.
