    50 cases of anterior chamber intraocular lens implantation at the University Hospital, Kuala Lumpur were studied. 80% of the cases achieved visual acuity of 6/9 or better. The percentage would be higher if cases with pre-existing pathology are excluded and the period of follow-up is longer. Complications, mainly minor and non sight-threatening, are discussed.

    Polypoidal choroidal vasculopathy (PCV), a subtype of neovascular age-related macular degeneration, requires repeated treatment. The objective of this pilot study was to evaluate and compare vision-targeted quality of life (QOL) at baseline and after 6 months of treatment in patients with PCV. Naive PCV patients were recruited. Visual functions assessed were distance visual acuity (DVA), near visual acuity (NVA), contrast sensitivity (CS), reading speed (RS), and QOL at baseline and after 6 months of treatment. Thirty patients (average age of 67.62 ± 8.05 years) revealed mean DVA and NVA improvements of 0.24 logMAR and 0.30 logMAR, respectively. Mean CS and RS improved by 0.39 log contrast and 25.58 words per minute, respectively. The National Eye Institute Visual Function Questionnaire 25 (NEI-VFQ-25) composite score significantly increased from a baseline of 66.73 ± 13.74 to 73.54 ± 14.26. Twenty-eight of the patients showed overall improvement in QOL score by 5 units or more or remained stable. Subscales of NEI-VFQ-25 significantly improved, with general vision, mental health, and role difficulties improving by 10 or more units. The present pilot study reports a significant improvement of QOL in PCV patients after 6 months of treatment, with mental health, role difficulties, social functioning, and distance vision activities being the most improved subscales.

    Introduction: Refractive error is the most common cause of visual impairment among the school children. The visual acuity screening has long been administered through the school health service in Malaysia. The objective of this study was to examine the status of refractive error in the school health screening from year 1990 to 2017. Methods: Every year an average of 1.2 million students were screened for refractive errors through school health service. Stu-dents with visual acuity reading of 6/9 or worst based on 6-meter Snellen chart ware referred for confirmation of refractive error. Secondary data on school visual acuity screening from the Health Informatics for the period of 1990 to 2017 were used to describe the changing pattern of refractive errors. Results: It was observed that the prevalent of suspected refractive errors amongst the Standard 1 student increased from 7 per 1,000 students examined in 1990 to 44.4 per 1,000 students in the 2000 and further increased to 74 per 1,000 students before slow decline to 55 per 1,000 students from year 2015 to 2017. A higher level of detection was observed for students in standard 6 and form 3 between the period of 1990 to 2009 (78/1,000 and 65/1,000 respectively) and further decline thereafter to lowest level in 2017 (33/1,000 to 21.5/1,000 respectively). Conclusion: The Standard 1 students in the 21st century have higher rate of suspected refractive errors compared to their counterpart that lived in the 20th century. Community awareness program need to be introduce for early detection of visual acuity problem.

    Despite the recent developments in modern cataract surgery and the application of a vast array of new devices and machines, late in-the-bag intraocular lens dislocation remains a devastating, albeit rare, complication. Various nonsurgical and surgical techniques have been used to manage this complication. We report a case of spontaneous repositioning in the left eye of an anteriorly subluxated in-the-bag intraocular lens. The spontaneous repositioning may have been caused by antagonistic effects related to the topical administration of brimonidine and prednisolone. The dislocation was treated without aggressive manipulation or surgical intervention.

    AIMS: To investigate the prevalence of sickle cell maculopathy (SCM), and associations with age, sex, genotype, proliferative sickle cell retinopathy (PSR) stage, and the impact on visual acuity.

    METHODS: Age, sex, and visual acuity were recorded and spectral domain OCT and ultra-wide-field images of the macula and retina were reviewed in a consecutive series of 74 adults with sickle cell disease.

    RESULTS: The median age was 37 years (range 19-73 years) and 36 cases (48.6%) were male. SCM was present in at least 1 eye of 40 cases (54.1%) or in 67 of all eyes (42.3%). SCM prevalence was 54.8%, 62.5%, and 25% for the HbSS, HbSC, and HbS/BThal or other genotypes, respectively. SCM was observed in 41 (39.4%) of the eyes with PSR stages 0, 1, and 2, and in 21 (51.2%) of the eyes with PSR stages 3, 4, and 5, respectively. Mild visual impairment or worse was present in 3 eyes (4.8%) with SCM but this was secondary to other pathology.

    CONCLUSION: SCM is a frequent finding in the eyes of adults with sickle cell disease. The prevalence is similar for the HbSS and HbSC genotypes and is not related to the PSR stage. High-contrast distance visual acuity is typically preserved.

    The purpose of this study was to identify the most common forms of age-related cataracts and to estimate possible risk factors for age-related cataracts in a sample of cataract patients at the National University of Malaysia Hospital. Thirty five patients (17 males and 18 female) with cataracts were recruited from the ophthalmology ward and clinic at the hospital. The age range of the patients was between 39 to 93 years (mean 64.9 11.5 years). The Lens Opacities Classification System (Locs) III was used to grade nuclear, cortical and posterior subcapsular lens opacities. Grading of the cataract was carried out and the following information was collected: initial visual acuity and best-corrected visual acuity, demographic details, health history, dietary intake of antioxidants and lifetime ocular ultraviolet B exposure. Analyses were conducted using a standard case-control design. T-tests were used to assess the significance of continuous variables and chi-squared tests were used for categorical variables. The overall prevalence of cortical cataract was 34.4% (12 patients), nuclear cataract was 60.0% (21 patients), and posterior subcapsular cataract was 57.1% (20 patients). No significant potential risk factor was found for nuclear cataracts. In the case of posterior subcapsular cataract, hypertension was the only potential significant risk factor (x2 = 4.38, p = 0.036), and in the case of cortical cataract, cigarette smoking was the only significant risk factor. Although lifetime effective ocular UV-B exposure was found to be not a significant potential risk factor, but it was seen that for cortical cataracts, the mean difference of lifetime effective ocular UV-B exposure between those with cortical and those without cortical cataracts was larger compared to others with nuclear and posterior subcapsular cataracts. Hypertension was found to be a potential significant risk factor for posterior subcapsular cataract whereas cigarette smoking was a potentially significant risk factor for cortical cataract. Although not statistically significant, lifetime effective ocular UV-B exposure maybe a potential risk factor for cortical cataract.

    Prevalence of visual impairment among students is increasing in trends especially in rural area. A cross sectional study was conducted among secondary school students in Taman PKNS Beranang, Selangor in March 2008 with the objective of determining the prevalence of visual acuity impairment and its influencing factors. A total of 77 secondary school students were interviewed using guided questionnaire. The prevalence of visual acuity impairment among the respondents was 25.0% and study found that being female and have been watching television at a distance of less than two meters were significantly associated with visual acuity impairment.     The purpose of this study is to assess the performance of allied health personnel, after attending a training programme, in conducting vision screening for preschool children. A total of 43 allied health personnel (20 assistant medical officers (AMOs) and 23 nurses) attended a two-day training program prior to conducting vision screening for preschool children. Vision screening was conducted among 136 preschool children using four similar HOTV visual acuity chart at 6 m. The cut-off referral criterion for visual acuity (VA) testing was 6/9 or worse in either eye. All children were referred to two qualified optometrists for a comprehensive eye examination. The accuracy was based on the sensitivity and specificity of screening by each group of personnel. The overall prevalence of reduced VA in the studied population was about 10%. The sensitivity and specificity of vision screening performed by AMOs were 100% and 98% respectively while that performed by nurses were 56% and 94% respectively. Thus, there were discrepancies in the sensitivity of visual acuity testing despite them being conducted by healthcare providers with similar background and training. This suggests that their performance might be influenced by factors other than their professional training.

    Yoga has recently been touted as a means to improve physical and mental well-being. However, no form of exercise is without its risks. A 32-year-old Chinese female with moderate myopia complained of right eye sudden onset of floaters and mild blurring of vision after the head-down posture. The visual acuity was 6/12 in the right eye and 6/9 in the left eye. A right eye fundus examination showed posterior vitreous detachment, with a small blood clot located at the inferior margin of the optic disc. The patient was diagnosed with right eye vitreous hemorrhage secondary to acute posterior vitreous detachment and was managed conservatively. Acute changes in posture, especially between an upright and a head-down position, may cause acute posterior vitreous detachment. As yoga practitioners may be required to assume this head-down position, myopic patients should be warned of the possible ocular complications of this exercise.

    Scleral buckle placement is a well-established technique for the treatment of primary rhegmatogenous retinal
    detachment. Complications associated with scleral buckle are uncommon and its presentations can be vary. We
    report a case of recurrent orbital cellulitis with anterior segment ischemia following a forgotten episode of previous
    scleral buckling surgery, presenting with blurring of vision, redness and swelling of the lids. The presence of scleral
    buckle was detected by detailed examination and confirmed by orbital imaging. Orbital infection and rubeosis iridis
    were successfully treated with scleral buckle removal, intravenous antibiotics and intracameral ranibizumab.
    However, the retinal detachment recurred and the visual acuity deteriorated to light perception. There was no further
    intervention as the family declined in view of her old age. In cases of recurrent orbital infection, detailed clinical
    examination is important to look for evidence of ocular prostheses as a source of infection. Orbital imaging is an
    adjunct for making the diagnosis especially in cases where history is unreliable. Anterior segment ischemia due to
    scleral buckle responds well to buckle removal with ranibizumab injection.

    Introduction: Microbial keratitis (MK) is an important cause for corneal blindness and understanding its risk factors enable us to improve management and minimise its complications. Methods: In this retrospective case review, medical records of all patients treated for MK from 2006 to 2013 was analysed to know the socio-demography, preceding risk factors, clinical characteristics, causative organisms and final visual outcome. Results: A total of 174 patients (180 eyes) were included in this study. Identifiable ocular risk factors included contact lens (CL) usage (85, 47.2%), ocular trauma (50, 27.8%), ocular surface disease (21, 11.6%), steroid use (6, 3.3%) and immuno-compromy (19, 10.5%).     Intracranial tuberculoma is a rare manifestation of tuberculosis involving the central nervous system. The involvement of the ventricular system is extremely uncommon. We describe a young woman with bilateral papilledema secondary to intraventricular tuberculoma with hydrocephalus. She was treated with anti-tuberculosis therapy and intravenous dexamethasone. Her visual acuity deteriorated after one month of treatment. We provide a literature review of this uncommon ocular sequelae.

    This study aimed to assess the visual function, reading performance, and compensatory head posture (CHP) in schoolchildren with infantile nystagmus. A total of 18 participants aged between 13 to 18 years old were divided into spectacle (n = 9) and null zone group (n = 9) based on their visual acuity. Visual acuity (LogMAR), contrast sensitivity (Pelli-Robson), reading time and rate (Tobii TX300), and CHP were measured pre and post null zone reading training. Participants in the null zone group received 10 sessions of training (5 weeks). Visual acuity and contrast sensitivity of participants in the spectacle and null zone groups were not significantly different pre and post training. Reading performance, i.e., reading time (z = -1.36; p = 0.173) and reading rate (z = -0.06; p = 0.953), of participants in the spectacle group was not significantly different after 5 weeks. Reading time (z = -2.55; p = 0.011) and reading rate (z = -2.07; p = 0.038 of participants in the null zone group showed significant improvement post training. After 5 weeks, CHP improved in six out of the nine participants (66.7%) of the null zone group and was unchanged in all participants in the spectacle group. Null zone reading training could benefit children with infantile nystagmus in improving reading performance and compensatory head posture.

    Introduction: The corneal endothelium is made up of an extremely thin layer of non -regenerating cells essential in keeping the cornea clear. The purpose of this study was to quantify the corneal endothelial cells morphology, visual acuity (VA) and refractive error in patients having femtosecond laser-assisted cataract surgery (FLACS) and conventional phacoemulsification surgery (CPS). Methods: Clinical data from 56 eyes were included in this study. 28 eyes had FLACS surgery and another 28 eyes had CPS. Distance VA, refractive error, corneal thickness and endothelial cells morphology were evaluated pre-operatively and at 1 month follow up. Refractive error was determined subjectively and VA was taken using Snellen chart. Specular microscope was used to measure endothelial cells morphology and central corneal thickness. Mann Whitney test and Spearman correlation analysis were used to analyse the results. Results: There were no statistically significant difference in the mean endothelial cell density (p=0.24), mean central corneal thickness (p=0.68), mean coefficient of variance (p=0.19), mean hexagonality (p=0.44), VA (p=0.72) and postoperative refractive outcome (p=0.66) between both groups.     Hyperreflective dots (HRD) are activated retinal microglial cells induced by retinal inflammation in diabetic patients. This study was conducted to compare the HRD count of normal and diabetic subjects; to determine the correlation between hemoglobin A1c (HbA1c) levels and HRD count; to determine HbA1c cut-off levels for the appearance of HRD in diabetic patients. A cross-sectional study was conducted among normal and diabetic patients. Fundus photos, SD-OCT images and HbA1c levels were taken. A total of 25 normal subjects, 32 diabetics without retinopathy and 26 mild-to-moderate nonproliferative diabetic retinopathy (NPDR) diabetics were recruited. There was a statistically significant difference between the mean count of HRD among the normal group, the diabetic without retinopathy group and the mild-to-moderate NPRD group. The mean HRD count in the inner retina layer was significantly higher compared to the outer retina layer. There was a significant linear relationship between the HbA1c levels and HRD count. Using the receiver operating curve, the HbA1c level of 5.4% was chosen as the cut-off point for the appearance of HRD. The positive linear correlation between the HbA1c levels and the appearance of HRD may indicate that hyperglycemia could activate retina microglial cells in diabetic patients.

    BACKGROUND/PURPOSE: The effect of orthokeratology (OK) on low myopia is well known, but there are a few reports on its effect on high myopia. In this study, the parametric changes in high and low myopia as results of wearing OK lenses for a period of 6 months have been analyzed.
    METHODS: Records of schoolchildren (age 7-17 years) undergoing OK treatment from an optometry clinic were retrospectively reviewed. Data involving refractive errors, uncorrected visual acuity, and corneal curvatures at baseline and after 1 day, 1 week, 1 month, 3 months, and 6 months of OK treatment from 25 patients who fulfilled the inclusion criteria were examined. For the analysis, the participants were arbitrarily divided into two groups comprising high myopia (< -6.00 D) and low to moderate myopia (from -1.00 D to -6.00 D).
    RESULTS: Significant reductions of refractive error, improvement in visual acuity, and corneal-curvature flattening were found in all participants after 6 months of OK lens wear compared to the baseline. No significant changes were found in corneal toricity in both high and low to moderate myopic groups. Almost all of these occurred after one night of lens wear in both the high- and low-myopia groups.
    CONCLUSION: The OK lens wear significantly reduced the refractive error and corneal curvature that results in the improvement in visual acuity in both high- and low-myopia groups, and the reduction seemed to occur nearly at the same time despite the difference in initial myopic power. High myopes with refractive power up to -8.25 D would benefit significantly from OK lenses.

    PURPOSE: To describe the excellent outcome of surgery for bilateral giant retinal tears (GRTs) with better options of endotamponade.

    METHODS: This is a case report of a 62-year-old man who presented with bilateral GRTs and associated retinal detachment. The tear in the right eye was supero-temporal and silicone oil was used as an endotamponade. The tear in the left eye was infero-temporal and perfluorocarbon liquid was used as an endotamponade.

    RESULTS: The outcome at 6 months after surgery was excellent with visual acuities of 6/6 in both eyes.

    CONCLUSION: Improved availability of endotamponade agents allows repair of bilateral GRTs to be done at the same time, with good surgical outcomes.

    Primary pterygium in children is uncommon but is associated with severe visual problems. Astigmatism is the main visual problem caused by pterygium. Significant amounts of astigmatism occur long before a pterygium encroaches the visual axis. Early surgical intervention is safe and effective. It is associated with significant visual improvement in outcome. This is a case report on seven-year-old Malay boy who presented with a growth over nasal aspect of the right eye of 1 year duration. His right eye visual acuity is affected up to 6/12. The dilemma pased to early surgical interview is the high rate of recurrancean the young age group. This problem is highlighted in this case report.

    BACKGROUND: Visual acuity is an essential estimate to assess ability of the visual system and is used as an indicator of ocular health status.
    AIM: The aim of this study is to investigate the consistency of acuity estimates from three different clinical visual acuity charts under two levels of ambient room illumination.
    MATERIALS AND METHODS: This study involved thirty Malay university students aged between 19 and 23 years old (7 males, 23 females), with their spherical refractive error ranging between plano and -7.75D, astigmatism ranging from plano to -1.75D, anisometropia less than 1.00D and with no history of ocular injury or pathology. Right eye visual acuity (recorded in logMAR unit) was measured with Snellen letter chart (Snellen), wall mounted letter chart (WM) and projected letter chart (PC) under two ambient room illuminations, room light on and room light off.
    RESULTS: Visual acuity estimates showed no statistically significant difference when measured with the room light on and with the room light off (F1,372 = 0.26, P = 0.61). Post-hoc analysis with Tukey showed that visual acuity estimates were significantly different between the Snellen and PC (P = 0.009) and between Snellen and WM (P = 0.002).
    CONCLUSIONS: Different levels of ambient room illumination had no significant effect on visual acuity estimates. However, the discrepancies in estimates of visual acuity noted in this study were purely due to the type of letter chart used.
