    BACKGROUND: Despite a better understanding of the pathophysiology of asthma, presence of reliable diagnostic tools, availability of a wide array of effective and affordable inhaled drugs and simplified national and international asthma management guidelines, asthma remains poorly managed in India.
    OBJECTIVE: The Asia-Pacific Asthma Insight and Management (AP-AIM) study was aimed at understanding the characteristics of asthma, current management, level of asthma control and its impact on quality of life across Australia, China, Hong Kong, India, Malaysia, Singapore, South Korea, Taiwan and Thailand. This paper describes the results of asthma management issues in India in detail and provides a unique insight into asthma in India.
    METHODOLOGY: The AP-AIM India study was conducted in eight urban cities in India, viz: Ajmer, Delhi, Kolkata, Rourkela, Chennai, Mangalore, Mumbai and Rajkot from February to July 2011. Face-to-face interviews were conducted in adult asthmatics and parents of asthmatic children between the ages of 12 and 17 years with a confirmed diagnosis or a treatment history of 1 year for asthma.
    RESULTS: Four hundred asthmatics (M:F::1:1.273), with a mean age of 50 ± 17.8 years, from across India were studied. 91% of the asthmatics in India perceived their asthma to be under control, however, none of the asthmatics had controlled asthma by objective measures. Asthmatics in India believed that their asthma was under control if they have up to 2 emergency doctor visits a year. The quality of life of these patients was significantly affected with 93% school/work absenteeism and a loss of 50% productivity. Seventy-five percent of the asthmatics have never had a lung function test. The common triggers for asthmatics in India were dust (49%) and air pollution (49%), while only 5% reported of pollen as triggers. Eighty-nine percent of Indian asthmatics reported an average use of oral steroids 10.5 times a year. Only 36% and 50% of Indian asthmatics used controller and rescue inhalers with a majority preferring the oral route of asthma medication.
    CONCLUSIONS: This study has clearly highlighted the fact that asthma management in India remains very poor, with a significant proportion of patients experiencing bothersome symptoms and worsened quality of life. There is a need for an urgent review of this situation and initiate active measures at local as well as national levels to improve asthma care in India.

    The proportion of older adults is on the rise. Management of dental problems in this group is different from the general population, and hence requires special training. Gerodontology is yet to find its place in the Indian dental curriculum. A lack of training would result in inadequate care delivery. In this article, we share our views on the need for inclusion of the subject, potential challenges, and a guide for incorporation of gerodontology in undergraduate and postgraduate curriculum in the Indian dental institutes. We propose a framework based on the salient features of Kern's 6-step approach for curriculum development and Kotter's 8-step change management model. Some features are common to both the models. A combination of these models includes the following salient features: Problem identification and general needs assessment, beginning with a sense of urgency and targeted needs assessment, communication of the vision for change, working in guided coalitions and defining clear goals and objectives, adopting the relevant educational strategies, implementation strategies to enable change and generating short-term wins, evaluating the effectiveness of the curricular reform and sustaining and anchoring the change. The proposed framework may also be useful for countries where gerodontology is yet to be implemented.

    Agriculture, meteorological, and hydrological drought is a natural hazard which affects ecosystems in the central India of Maharashtra state. Due to limited historical data for drought monitoring and forecasting available in the central India of Maharashtra state, implementing machine learning (ML) algorithms could allow for the prediction of future drought events. In this paper, we have focused on the prediction accuracy of meteorological drought in the semi-arid region based on the standardized precipitation index (SPI) using the random forest (RF), random tree (RT), and Gaussian process regression (GPR-PUK kernel) models. A different combination of machine learning models and variables has been performed for the forecasting of metrological drought based on the SPI-6 and 12 months. Models were developed using monthly rainfall data for the period of 2000-2019 at two meteorological stations, namely, Karanjali and Gangawdi, each representing a geographical region of Upper Godavari river basin area in the central India of Maharashtra state which frequently experiences droughts. Historical data from the SPI from 2000 to 2013 was processed to train the model into machine learning model, and the rest of the 2014 to 2019-year data were used for testing to forecast the SPI and metrological drought. The mean square error (MSE), root mean square error (RMSE), adjusted R2, Mallows' (Cp), Akaike's (AIC), Schwarz's (SBC), and Amemiya's PC were used to identify the best combination input model and best subregression analysis for both stations of SPI-6 and 12. The correlation coefficient ([Formula: see text]), mean absolute error (MAE), root mean square error (RMSE), relative absolute error (RAE), and root relative squared error (RRSE) were used to perform evaluation for SPI-6 and 12 months of both stations with RF, RT, and GPR-PUK kernel models during the training and testing scenarios. The results during testing phase revealed that the RF was found as the best model in forecasting droughts with values of [Formula: see text], MAE, RMSE, RAE (%), and RRSE (%) being 0.856, 0.551, 0.718, 74.778, and 54.019, respectively, for SPI-6 while 0.961, 0.361, 0.538, 34.926, and 28.262, respectively, for SPI-12 scales at Gangawdi station. Further, the respective values of evaluators at Karanjali station were 0.913 and 0.966, 0.541 and 0.386, 0.604 and 0.589, 52.592 and 36.959, and 42.315 and 31.394 for PUK kernel and RT models, respectively, during SPI-6 and SPI-12. Machine learning models are potential drought warning techniques because they take less time, have fewer inputs, and are less sophisticated than dynamic or scientific models.

    Since the approval of COVID-19 vaccines, international efforts have intensified on vaccination schemes perceived as the only light at the end of the tunnel. Governments are working tirelessly to scale up the number of vaccinated people, just as vaccine manufacturers are stretching their facilities to meet the increasing demand for doses. The international community is trying to help the poorest countries in the world by improving vaccine supplies and removing obstacles. In this regard, India and South Africa have applied to World Trade Organisation to waive vaccine-related intellectual property rights. The proposal has sparked off academic debates as to its merit. This article addresses the waiver controversy. Following a critical review of both dimensions of the controversy, the article concentrates on the extent to which the waiver application contradicts the theoretical justification of the patent system. It concludes that the concerns raised over the conflict between the waiver proposal and the patent right philosophy are indefensible.

    The microsporidian parasite Enterocytozoon hepatopenaei (EHP) is an emerging problem in the marine shrimp industry, primarily in Asian countries such as China, Thailand, India, Malaysia, Indonesia, and Vietnam. A screening was conducted to investigate the prevalence of EHP after a fixed period of culturing for 1 rearing cycle in 3 states of Malaysia. The screening stages covered Penaeus vannamei post larvae (PL) and after 14-30, 31-50, 51-70, and 71-90 d of culture in 1 production cycle. A total of 279 samples were amplified using a PCR assay targeting the gene encoding a spore wall protein (SWP) of EHP. The EHP infection was initially detected in the hatchery and increased to 96.6% after the shrimp were transferred to the pond. The positive EHP sequence showed 91 to 100% similarity to sequences from India, Thailand, Vietnam, Indonesia, and Latin America. EHP infection increased throughout 1 rearing cycle due to factors such as the cannibalistic feeding habits of shrimp and the presence of unknown vectors or carriers of EHP in the culture ponds. Hence, the finding from the current study will be fundamental for other studies concerning EHP.
