    Nodaviruses are small bipartite RNA viruses which belong to the family of Nodaviridae. They are categorized into alpha-nodavirus, which infects insects, and beta-nodavirus, which infects fishes. Another distinct group of nodavirus infects shrimps and prawns, which has been proposed to be categorized as gamma-nodavirus. Our current review focuses mainly on recent studies performed on nodaviruses. Nodavirus can be transmitted vertically and horizontally. Recent outbreaks have been reported in China, Indonesia, Singapore and India, affecting the aquaculture industry. It also decreased mullet stock in the Caspian Sea. Histopathology and transmission electron microscopy (TEM) are used to examine the presence of nodaviruses in infected fishes and prawns. For classification, virus isolation followed by nucleotide sequencing are required. In contrast to partial sequence identification, profiling the whole transcriptome using next generation sequencing (NGS) offers a more comprehensive comparison and characterization of the virus. For rapid diagnosis of nodavirus, assays targeting the viral RNA based on reverse-transcription PCR (RT-PCR) such as microfluidic chips, reverse-transcription loop-mediated isothermal amplification (RT-LAMP) and RT-LAMP coupled with lateral flow dipstick (RT-LAMP-LFD) have been developed. Besides viral RNA detections, diagnosis based on immunological assays such as enzyme-linked immunosorbent assay (ELISA), immunodot and Western blotting have also been reported. In addition, immune responses of fish and prawn are also discussed. Overall, in fish, innate immunity, cellular type I interferon immunity and humoral immunity cooperatively prevent nodavirus infections, whereas prawns and shrimps adopt different immune mechanisms against nodavirus infections, through upregulation of superoxide anion, prophenoloxidase, superoxide dismutase (SOD), crustin, peroxinectin, anti-lipopolysaccharides and heat shock proteins (HSP). Potential vaccines for fishes and prawns based on inactivated viruses, recombinant proteins or DNA, either delivered through injection, oral feeding or immersion, are also discussed in detail. Lastly, a comprehensive review on nodavirus virus-like particles (VLPs) is presented. In recent years, studies on prawn nodavirus are mainly focused on Macrobrachium rosenbergii nodavirus (MrNV). Recombinant MrNV VLPs have been produced in prokaryotic and eukaryotic expression systems. Their roles as a nucleic acid delivery vehicle, a platform for vaccine development, a molecular tool for mechanism study and in solving the structures of MrNV are intensively discussed.

    The zoogeographic distribution of lophogastrid species (Crustacea: Peracarida: Lophogastrida) occurring in the Indonesian waters is presented. For each species, data on general distribution, bathymetric ranges, habitat and localities reported on published data are provided. A total of 20 lophogastrid species belonging to three families and seven genera occur in Indonesian waters (about 38% of all known lophogastrids species worldwide), a number greater than other areas of Southeast Asia. Also, based on current information, the number of species or species richness is greater than other regions, such as Madagascar, North Pacific off Japan, Mediterranean, Canary Island, northern mid-Atlantic ridge, Iberian Peninsula, Mexico, and Angola Basin (SE Atlantic). Most of the Indonesian species are distributed worldwide, but one species, Lophogaster inermis appears to be endemic to Indonesia. Previous listings of Paralophogaster intermedius occurring in Southeast Asian waters is not verified in any collections, and has therefore been removed from our updated list.

    INTRODUCTION: The Richter Scale measures the magnitude of the seismic activity for an earthquake; however, it does not quantify the humanitarian need at the point of impact. This poses a challenge for humanitarian stakeholders in decision and policy making, especially in risk reduction, response, recovery, and reconstruction. The new disaster metrics tool titled "The YEW Disaster Severity Index" (DSI) was developed and presented at the 2017 World Congress of Disaster and Emergency Medicine, May 2017, Toronto, Canada. It uses a median score of three for vulnerability and exposure indicators, a median score percentage of 100%, and medium YEW DSI scoring of four to five as baseline, indicating the ability to cope within local capacity. Therefore, scoring more than baseline coping capacity indicates that external assistance is needed. This special real-time report was presented at the 2nd National Pre-Hospital Care Conference and Championship, October 2018, Malaysia.

    REPORT: The aim of this analysis is to present the real-time humanitarian impact and response to the 2018 earthquake and tsunami at Donggala and Palu, Sulawesi in Indonesia using the new disaster metrics YEW DSI. Based on the earthquake (measuring 7.7 on the Richter Scale) and tsunami at Donggala, the humanitarian impact calculated on September 29, 2018 scored 7.4 High in the YEW DSI with 11 of the total 17 indicators scoring more than the baseline coping capacity. The same YEW DSI score of 7.4 was scored on the earthquake and tsunami at Palu, with 13 of the total 17 indicators scoring more than baseline ability to cope within local capacity. Impact analysis reports were sent to relevant authorities on September 30, 2018.

    DISCUSSION & CONCLUSION: A State of Emergency was declared for a national response, which indicated an inability to cope within the local capacity, shown by the YEW DSI. The strong correlation between the earthquake magnitude, intensities, and the humanitarian impact at Donggala and Palu reported could be added into the science of knowledge in prehospital care and disaster medicine research and practice. As a conclusion, the real-time disaster response was found to be almost an exact fit with the YEW DSI indicators, demonstrating the inability to cope within the local capacity.

    Edible bird's nest (EBN) is recognized as a nourishing food among Chinese people. The efficacy of EBN was stated in the records of traditional Chinese medicine and its activities have been reported in many researches. Malaysia is the second largest exporter of EBNs in the world, after Indonesia. For many years, EBN trade to China was not regulated until August 2011, when a safety alert was triggered for the consumption of EBNs. China banned the import of EBNs from Malaysia and Indonesia due to high level of nitrite. Since then, the Malaysia government has formulated Malaysia Standards for swiftlet farming (MS 2273:2012), edible bird's nest processing plant design and management (MS 2333:2010), and edible bird's nest product quality (MS 2334:2011) to enable the industry to meet the specified standards for the export to China. On the other hand, Indonesia's EBN industry formulated a standard operating procedure (SOP) for exportation to China. Both countries can export EBNs to China by complying with the standards and SOPs. EBN contaminants may include but not limited to nitrite, heavy metals, excessive minerals, fungi, bacteria, and mites. The possible source of contaminants may come from the swiftlet farms and the swiftlets or introduced during processing, storage, and transportation of EBNs, or adulterants. Swiftlet house design and management, and EBN processing affect the bird's nest color. Degradation of its optical quality has an impact on the selling price, and color changes are tied together with nitrite level. In this review, the current and future prospects of EBNs in Malaysia and Indonesia in terms of their quality, and the research on the contaminants and their effects on EBN color changes are discussed.

    AIM: The aim of the present study was to survey the prevalence of antipsychotic polypharmacy and combined medication use across 15 Asian countries and areas in 2016.

    METHODS: By using the results from the fourth survey of Research on Asian Prescription Patterns on antipsychotics, the rates of polypharmacy and combined medication use in each country were analyzed. Daily medications prescribed for the treatment of inpatients or outpatients with schizophrenia, including antipsychotics, mood stabilizers, anxiolytics, hypnotics, and antiparkinson agents, were collected. Fifteen countries from Asia participated in this study.

    RESULTS: A total of 3744 patients' prescription forms were examined. The prescription patterns differed across these Asian countries, with the highest rate of polypharmacy noted in Vietnam (59.1%) and the lowest in Myanmar (22.0%). Furthermore, the combined use of other medications, expressed as highest and lowest rate, respectively, was as follows: mood stabilizers, China (35.0%) and Bangladesh (1.0%); antidepressants, South Korea (36.6%) and Bangladesh (0%); anxiolytics, Pakistan (55.7%) and Myanmar (8.5%); hypnotics, Japan (61.1%) and, equally, Myanmar (0%) and Sri Lanka (0%); and antiparkinson agents, Bangladesh (87.9%) and Vietnam (10.9%). The average psychotropic drug loading of all patients was 2.01 ± 1.64, with the highest and lowest loadings noted in Japan (4.13 ± 3.13) and Indonesia (1.16 ± 0.68), respectively.

    CONCLUSION: Differences in psychiatrist training as well as the civil culture and health insurance system of each country may have contributed to the differences in these rates. The concept of drug loading can be applied to other medical fields.

    The spider suborder Mesothelae, containing a single extant family Liphistiidae, represents a species-poor and ancient lineage. These are conspicuous spiders that primitively retain a segmented abdomen and appendage-like spinnerets. While their classification history is nearly devoid of phylogenetic hypotheses, we here revise liphistiid genus level taxonomy based on original sampling throughout their Asian range, and on the evidence from a novel molecular phylogeny. By combining morphological and natural history evidence with phylogenetic relationships in the companion paper, we provide strong support for the monophyly of Liphistiidae, and the two subfamilies Liphistiinae and Heptathelinae. While the former only contains Liphistius Schiödte, 1849, a genus distributed in Indonesia (Sumatra), Laos, Malaysia, Myanmar, Thailand, we recognize and diagnose seven heptatheline genera, all but three removed from the synonymy of Heptathela: i) Ganthela Xu & Kuntner, gen. n. with the type species Ganthelayundingensis Xu, sp. n. is known from Fujian and Jiangxi, China; ii) a rediagnosed Heptathela Kishida, 1923 is confined to the Japanese islands (Kyushu and Okinawa); iii) Qiongthela Xu & Kuntner, gen. n. with the type species Qiongthelabaishensis Xu, sp. n. is distributed disjunctly in Hainan, China and Vietnam; iv) Ryuthela Haupt, 1983 is confined to the Ryukyu archipelago (Japan); v) Sinothela Haupt, 2003 inhabits Chinese areas north of Yangtze; vi) Songthela Ono, 2000 inhabits southwest China and northern Vietnam; and vii) Vinathela Ono, 2000 (Abcathela Ono, 2000, syn. n.; Nanthela Haupt, 2003, syn. n.) is known from southeast China and Vietnam.

    Edible bird's nest (EBN) is a high-value health food with various nutrients and bioactive components. With increasing demand for EBN, they are often adulterated with cheaper ingredients or falsely labeled by the origin information, thus harming consumer interests. In this study, high- and low-field nuclear magnetic resonance (HF/LF-NMR) technology combined with multivariate statistical analysis was used to identify the geographical marker of EBN from different origins and authenticate the adulterated EBN with various adulterants at different adulteration rates. Authentic EBN samples from Malaysia were used to simulate adulteration using gelatin (GL), agar (AG) and starch (ST) at 10 %, 20 %, 40 %, 60 %, 80 %, and 100 % w/w, respectively. The results showed significant differences in composition among EBN from different origins, with isocaproate and citric acid serving as geographical markers for Malaysia and Vietnam, respectively. Leucine, glutamic acid, and N-acetylglycoprotein serving as geographical markers for Indonesia. In addition, PLS model further verified the accuracy of origin identification of EBN. The LF-NMR results of adulteration EBN showed a linear correlation between the transverse relaxation (T2, S2) and the adulterated ratio. The OPLS-DA based on T2 spectra could accurately identify authentic EBN from adulterated with GL, AG and ST at 40 %, 20 %, and 20 %, respectively. Fisher discrimination model was able to differentiate at 20 %, 20 %, and 40 %, respectively. These results show that the 1H NMR combined with multivariate statistical analysis method could be a potential tool for the detection of origin and adulteration of EBN.

    Introduction: An economical and alternative diabetic wound care product is needed to address the escalating cost of diabetic wound ulcer treatment in Indonesia. Morindac itrifolia or commonly called noni, is a medicinal plant, dietary supplement, and traditional wound care product. This leaves extract contains antioxidant, anti-inflammatory, immune-stimulant, and antimicrobial properties. Trigona honey which is currently used to treat the diabetic wound at Kitamura Wound Specialist Clinic, Pontianak, Indonesia is known for its anti-inflammatory, anti-microbial and anti-proliferative properties. This study aims to compare the effectiveness of Morinda citrifolia leaves extract and Trigona honey in diabetic wound healing. Methods: A quasi-experimental study (a randomized control trial design) was conductedin the clinicon two subjects with diabetic foot ulcers (DFU). Trigona honey and Morinda citrifolia leaves extract packing was done to the ulcer of controlled subject and treatment subject respectively as a primary dressing. DFU assessment was done every 2 days using the MUNGS assessment tool and a wound closure rate for- mula. Wound C&S was analysed using a bacterial count machine. Results: Morinda citrifolia leaves extract was able to promote wound contraction, improved MUNGS score (from 9 to 8), reduced bacteria count significantly (from
    8.51 x 105 to 1.00 x 105), reduced the percentage of slough by 70% and increased granulating tissue by 60% after the first application. Whereas, Trigona honey extract showed no wound contraction but improved MUNGS score (from 11 to 7). However, bacteria count was maintained at the lowest level (1.00 x 105), reduce the percentage of slough by 10%, and only increased granulating tissues by 10% after the first application. Conclusion: Both products were able to promote diabetic wound healing but the Morinda citrifolia leaves extract was found to be more effective, thus concludes the potential use of Morinda citrifolia leaves extract in treating DFU.

    Today, insular Southeast Asia is important for both its remarkably rich biodiversity and globally significant roles in atmospheric and oceanic circulation. Despite the fundamental importance of environmental history for diversity and conservation, there is little primary evidence concerning the nature of vegetation in north equatorial Southeast Asia during the Last Glacial Period (LGP). As a result, even the general distribution of vegetation during the Last Glacial Maximum is debated. Here we show, using the stable carbon isotope composition of ancient cave guano profiles, that there was a substantial forest contraction during the LGP on both peninsular Malaysia and Palawan, while rainforest was maintained in northern Borneo. These results directly support rainforest "refugia" hypotheses and provide evidence that environmental barriers likely reduced genetic mixing between Borneo and Sumatra flora and fauna. Moreover, it sheds light on possible early human dispersal events.

    Edible bird nests (EBNs) are highly demanded globally. The industry was recently affected by an import ban to China due to high nitrite levels.Subsequently, many concerns have been raised. In this study, the microbial composition of both raw and commercial EBNs was investigated. The raw EBNs were purchased from swiftlet farms: Kuala Sanglang (Perlis), Pantai Remis (Perak), Kluang (Johor), Kajang (Selangor) and Kota Bharu (Kelantan). The commercial nests were purchased from five different Chinese traditional medicinal shops (Companies A-E) in Malaysia and one from Indonesia (Medan). A total of 123 and 34 isolates were successfully identified from unboiled raw and commercial EBNs respectively. The highest average CFU (1.77 x 104) was associated with raw EBNs obtained from Kluang, while for the commercial EBNs, those obtained from Company M1 had the highest CFU (5.50 x 104). Bacillus sp. accounted for the highest isolated species from both unboiled raw and commercial EBNs. Bacillus sp. and Brevibacillus sp. were mainly isolated from the boiled EBNs. Bacillus spp. were the dominant bacterial groups in all the raw EBNs except for those obtained from Kajang. The average number of bacteria isolated from the raw EBNs (average = 7) was higher compared with those isolated from the commercial EBNs (average = 4). The highest average number of bacterial isolates was reported in the raw EBNs obtained from Kota Bharu. Among the commercial EBNs, one EBN sample each from Companies A and M1 showed the highest number of isolates (n = 10). In general, there was a significant reduction in the number of bacteria isolated after boiling the EBNs. Raw EBNs obtained from Kajang had a distinct pool of bacterial species where the majority of the isolated species belonged to Staphylococcus species. The associated health impacts of these microorganisms to the consumers and public need to be addressed.

    The majority of migrant workers studied in this pilot survey were male, from Bangladesh and Muslim. The mean age was 30 years and the majority were aged between 21-30 years. Although almost half of them had 7-13 years of schooling, or an equivalent to secondary education, the majority were work- ing in the Service industry, predominantly in cleaning services. It is noted that this enlployment trend varied from the national situation, whereby majority of legal migrant workers (Indonesian and Thai) are found in the agricultural sector. More than two thirds of the migrant workers were provided with various forms of housing by the employer. However, it is not known if such acconunodation was adequate or not, as there were no questions about housing structures aod extent of overcrowding. Majority of them stated that they had better amenities, such as piped drinking water and sanitaly toilets, here in Malaysia compared to those in their home countries. Yet, the real extent of better sani- tation is difficult to assess since verification of such amenities could not be done. From their self- reports, it appears that the majority did not engage in risk behaviours, such as smoking, alcohol and drug abuse. It is pertinent, however, to include other risk behaviours in the study, particularly the area of sexual behaviour. The pilot study yielded 28 female Indonesian migrant workers. More than two thirds of them were married. Although none of the married women reported that they were pregnant at the time of the survey, more than two thirds of then1 had betweenl-3 children in Malaysia. Since the age range of these children would be an important indicator of the need for preventive health care, it is proposed that age range of the accompanying children and their immunisation status be included in the questionnaire. Just below half of them were currently practising family planning, and more than two thirds were using modern methods, such as, pill, Norplant and IUD. It is recotnmended that in addition to pregnancy and family planning information, the study could also collect data on gynaeco- logical health and the health seeking behaviour for these problems.

    The assessment of surface water quality is often laborious, expensive and tedious, as well as impractical, especially for the developing and middle-income countries in the ASEAN region. The application of the water quality index (WQI), which depends on several independent key parameters, has great potential and is a useful tool in this region. Therefore, this study aims to find out the spatial variability of various water quality parameters in geographical information system (GIS) environment and perform a comparative study among the ASEAN WQI systems. At present, there are four ASEAN countries which have implemented the WQI system to evaluate their surface water quality, which are (i) Own WQI system-Malaysia, Thailand and Vietnam-and (ii) Adopted WQI system: Indonesia. A spatial distribution of 12 water quality parameters in the Selangor river basin, Malaysia, was plotted and then applied into the different ASEAN WQI systems. The WQI values obtained from the different WQI systems have an appreciable difference, even for the same water samples due to the disparity in the parameter selection and the standards among them. WQI systems which consider all biophysicochemical parameters provide a consistent evaluation (Very Poor), but the system which either considers physicochemical or biochemical parameters gives a relatively lenient evaluation (Fair-Poor). The Selangor river basin is stressed and impacted by all physical, biological and chemical parameters caused by both the aridity of the climate and anthropogenic activities. Therefore, it is crucial to include all these aspects into the evaluation and corresponding actions should be taken.

    Ganoderma boninense, the causal agent of basal stem rot (BSR) disease, has been recognized as a major economic threat to commercial plantings of oil palm (Elaeis guineensis Jacq.) in Southeast Asia, which supplies 86% of the world's palm oil. High genetic diversity and gene flow among regional populations of 417 G. boninense isolates collected from Sabah, Sarawak, and Peninsular Malaysia (Malaysia) and Sumatra (Indonesia) were demonstrated using 16 microsatellite loci. Three genetic clusters and different admixed populations of G. boninense across regions were detected, and they appeared to follow the spread of the fungus from the oldest (Peninsular Malaysia and Sumatra) to younger generations of oil palm plantings (Sabah and Sarawak). Low spatial genetic differentiation of G. boninense (FST = 0.05) among the sampling regions revealed geographically nonrestricted gene dispersal, but isolation by distance was still evident. Analysis of molecular variance (AMOVA) confirmed the little to no genetic differentiation among the pathogen populations and the three genetic clusters defined by STRUCTURE and minimum spanning network. Despite G. boninense being highly outcrossing and spread by sexual spores, linkage disequilibrium was detected in 7 of the 14 populations. Linkage disequilibrium indicated that the reproduction of the fungus was not entirely by random mating and genetic drift could be an important structuring factor. Furthermore, evidence of population bottleneck was indicated in the oldest oil palm plantations as detected in genetic clusters 2 and 3, which consisted mainly of Peninsular Malaysia and Sumatra isolates. The population bottleneck or founding event could have arisen from either new planting or replanting after the removal of large number of palm hosts. The present study also demonstrated that migration and nonrandom mating of G. boninense could be important for survival and adaptation to new palm hosts.

    In order to manage blue swimming crabs in Pangkajene Kepulauan, management measures are required. Since the environment which affects the abundance of the blue swimming crab varies seasonally, it is necessary to take into account the seasonal nature with the aim of developing a management strategy. The objectives of this study are to define the abundance of and fishing season of blue swimming crabs in the Pangkajene Kepulauan waters, South Sulawesi, Indonesia. The fishing season was analysed using seasonal index analysis, while fish abundance was analysed by means of Equilibrium-Schaefer. The result of this study demonstrated that fishermen allocate their fishing gear all year, although the fish catch is seasonal. Based on analysis of the result, the fishing season for the blue swimming crabs is short. The peak fishing season starts in May and finishes in June. However, in order to enable their families to earn a living, fishermen operated their fishing gear throughout the year. As a result, both catch landing and effort were close to maximum sustainable yield (MSY). In order to reduce fishing pressure, it is necessary to reduce fishing gear and have a seasonal arrangement regarding fishing gear allocation.

    OBJECTIVE: Neurosurgery departments worldwide have been forced to restructure their training programs because of the coronavirus disease 2019 (COVID-19) pandemic. In this study, we describe the impact of COVID-19 on neurosurgical training in Southeast Asia.

    METHODS: We conducted an online survey among neurosurgery residents in Indonesia, Malaysia, Philippines, Singapore, and Thailand from May 22 to 31, 2020 using Google Forms. The 33-item questionnaire collected data on elective and emergency neurosurgical operations, ongoing learning activities, and health worker safety.

    RESULTS: A total of 298 of 470 neurosurgery residents completed the survey, equivalent to a 63% response rate. The decrease in elective neurosurgical operations in Indonesia and in the Philippines (median, 100% for both) was significantly greater compared with other countries (P < 0.001). For emergency operations, trainees in Indonesia and Malaysia had a significantly greater reduction in their caseload (median, 80% and 70%, respectively) compared with trainees in Singapore and Thailand (median, 20% and 50%, respectively; P < 0.001). Neurosurgery residents were most concerned about the decrease in their hands-on surgical experience, uncertainty in their career advancement, and occupational safety in the workplace. Most of the residents (n = 221, 74%) believed that the COVID-19 crisis will have a negative impact on their neurosurgical training overall.

    CONCLUSIONS: An effective national strategy to control COVID-19 is crucial to sustain neurosurgical training and to provide essential neurosurgical services. Training programs in Southeast Asia should consider developing online learning modules and setting up simulation laboratories to allow trainees to systematically acquire knowledge and develop practical skills during these challenging times.

    River outgassing has proven to be an integral part of the carbon cycle. In Southeast Asia, river outgassing quantities are uncertain due to lack of measured data. Here we investigate six rivers in Indonesia and Malaysia, during five expeditions. CO2 fluxes from Southeast Asian rivers amount to 66.9 ± 15.7 Tg C per year, of which Indonesia releases 53.9 ± 12.4 Tg C per year. Malaysian rivers emit 6.2 ± 1.6 Tg C per year. These moderate values show that Southeast Asia is not the river outgassing hotspot as would be expected from the carbon-enriched peat soils. This is due to the relatively short residence time of dissolved organic carbon (DOC) in the river, as the peatlands, being the primary source of DOC, are located near the coast. Limitation of bacterial production, due to low pH, oxygen depletion or the refractory nature of DOC, potentially also contributes to moderate CO2 fluxes as this decelerates decomposition.

    PURPOSE: The purpose of this paper is to improve the Siloam Hospitals' (SHs) patient satisfaction index (PSI) and overcome Indonesia's geographical barriers.

    DESIGN/METHODOLOGY/APPROACH: The topic was selected for reasons guided by the Institute of Healthcare Improvement virtual breakthrough series collaborative (VBSC). Subject matter experts came from existing global quality development in collaboration with sales and marketing, and talent management agencies/departments. Patient satisfaction (PS) was measured using the SH Customer Feedback Form. Data were analysed using Friedman's test.

    FINDINGS: The in-patient (IP) department PSI repeated measures comparison during VBSC, performed using Friedman's test, showed a statistically significant increase in the PSI, χ2 = 44.00, p<0.001. Post hoc analysis with Wilcoxon signed-rank test was conducted with a Bonferroni correction applied, which resulted in a significant increase between the baseline and action phases ( Z=3.317, p=0.003) between the baseline and continuous improvement phases ( Z=6.633, p<0.001), and between the action and continuous improvement phases ( Z=3.317, p=0.003), suggesting that IP PSI was continuously increasing during all VBSC phases. Like IP PSI, the out-patient department PSI was also continuously increasing during all VBSC phases.

    RESEARCH LIMITATIONS/IMPLICATIONS: The VBSC was not implemented using a control group. Factors other than the VBSC may have contributed to increased PS.

    PRACTICAL IMPLICATIONS: The VBSC was conducted using virtual telecommunication. Although conventional breakthrough series might result in better cohesiveness and commitment, Indonesian geographical barriers forced an alternative strategy, which is much more cost-effective.

    ORIGINALITY/VALUE: The VBSC, designed to improve PS, has never been implemented in any Indonesian private hospital group. Other hospital groups might also appreciate knowing about the VBSC to improve their PSI.

    PURPOSE: The purpose of this paper is to assess the association between shift schedule realignment and patient safety culture.

    DESIGN/METHODOLOGY/APPROACH: Using difference in differences model, BIMC Hospitals and Siloam Hospital Bali were compared before and after shift schedule realignment to test the association between shift schedule realignment and patient safety culture.

    FINDINGS: Shift schedule realignment was associated with a significant improvement in staffing (coefficient 1.272; 95% CI 0.842 - 1.702; p<0.001), teamwork within units (coefficient 1.689; 95% CI 1.206 - 2.171; p<0.001), teamwork across units (coefficient 1.862; 95% CI 1.415 - 2.308; p<0.001), handoffs and transitions (coefficient 0.999; 95% CI 0.616 - 1.382; p<0.001), frequency of error reported (coefficient 1.037; 95% CI 0.581 - 1.493; p<0.001), feedback and communication about error (coefficient 1.412; 95% CI 0.982 - 1.841; p<0.001) and communication openness (coefficient 1.393; 95% CI 0.968 - 1.818; p<0.001).

    PRACTICAL IMPLICATIONS: With positive impact on patient safety culture, shift schedule realignment should be considered as quality improvement initiative. It stretches the compressed workload suffered by staff while maintaining 40 h per week in accordance with applicable laws and regulations.

    ORIGINALITY/VALUE: Shift schedule realignment, designed to improve patient safety culture, has never been implemented in any Indonesian private hospital. Other hospital managers might also appreciate knowing about the shift schedule realignment to improve the patient safety culture.
