    BACKGROUND: Renal cell carcinoma is a potentially lethal cancer with aggressive behavior and it tends to metastasize. Renal cell carcinoma involves the inferior vena cava in approximately 15% of cases and it rarely extends into the right atrium. A majority of renal cell carcinoma are detected as incidental findings on imaging studies obtained for unrelated reasons. At presentation, nearly 25% of patients either have distant metastases or significant local-regional disease with no symptoms that can be attributed to renal cell carcinoma.

    CASE PRESENTATION: A 64-year-old Indian male with a past history of coronary artery bypass graft surgery, congestive heart failure, and diabetes mellitus complained of worsening shortness of breath for 2 weeks. Incidentally, a transthoracic echocardiography showed a "thumb-like" mass in his right atrium extending into his right ventricle through the tricuspid valve with each systole. Abdomen magnetic resonance imaging revealed a heterogenous lobulated mass in the upper and mid-pole of his right kidney with a tumor extending into his inferior vena cava and right atrium, consistent with our diagnosis of advanced renal cell carcinoma which was later confirmed by surgical excision and histology. Radical right nephrectomy, lymph nodes clearance, inferior vena cava cavatomy, and complete tumor thrombectomy were performed successfully. Perioperatively, he did not require cardiopulmonary bypass or deep hypothermic circulatory arrest. He had no recurrence during the follow-up period for more than 2 years after surgery.

    CONCLUSIONS: Advanced extension of renal cell carcinoma can occur with no apparent symptoms and be detected incidentally. In rare circumstances, atypical presentation of renal cell carcinoma should be considered in a patient presenting with right atrial mass detected by echocardiography. Renal cell carcinoma with inferior vena cava and right atrium extension is a complex surgical challenge, but excellent results can be obtained with proper patient selection, meticulous surgical techniques, and close perioperative patient care.

    Colorectal cancer is one of the most common malignancies in the West, but in Asia the incidence is low. However in Malaysia, colorectal cancer is increasing with a reported figure of 15% of all cancer cases. Adjuvant chemo and radiotherapy are now more frequently used in such patients. The present retrospective analysis was performed to document the effect of such therapy among patients with colorectal cancer in Malaysia.

    Nonchromaffin paragangliomas are unusual tumours arising from widely distributed paraganglionic tissues probably of neural crest origin. In the head and neck region they are usually seen as carotid body or jugulotympanic tumours. Other rarely reported sites in the head and neck region are the orbit, nose and larynx. This report deals with a case of sinonasal paraganglioma which was initially treated with surgery and radiotherapy. Twenty two years later the tumour recurred and showed a rapid growth due to malignant transformation which we believe is late effect of radiotherapy. The clinical features, histopathology and role of radiotherapy in sinonasal paragangliomas together with a review of the medical literature have been discussed.

    Sinonasal teratocarcinosarcoma is very unusual malignant neoplasm histologically consisting of an epithelial element and one or more mesenchymal components. This is a report of teratocarcinosarcoma, in a 74-year-old male, involving the right nasal cavity and ethmoids with intracranial extension. The tumour was totally resected via the craniofacial approach and the patient was given post-operative chemotherapy. Extensive tumour necrosis, rapid growth and local destruction are the prominent features of this tumour. The clinical presentation, pathological features and clinical course of this rare malignancy are discussed with a review of the literature.

    A 30-year old female who initially had typical endometriosis treated according to a standard regimen later developed numerous highly vascular endometrial polyps on the vagina, cervix, ureter, serosal surfaces of the uterus, pouch of Douglas (POD) and other areas of pelvic peritoneum as well as the endometrium 8 months after withdrawal of treatment with Zoladex gonadotrophin releasing hormone (GnRH) agonist used for treatment of this disease. We postulate that these polyps developed as a rebound phenomenon upon withdrawal of Zoladex. We believe this is the first report of this complication following use of GnRH analogue.

    Arsenic is a common contaminant in gold mine soil and tailings. Microbes present an opportunity for bio-treatment of arsenic, since it is a sustainable and cost-effective approach to remove arsenic from water. However, the development of existing bio-treatment approaches depends on isolation of arsenic-resistant microbes from arsenic contaminated samples. Microbial cultures are commonly used in bio-treatment; however, it is not established whether the structure of the cultured isolates resembles the native microbial community from arsenic-contaminated soil. In this milieu, a culture-independent approach using Illumina sequencing technology was used to profile the microbial community in situ. This was coupled with a culture-dependent technique, that is, isolation using two different growth media, to analyse the microbial population in arsenic laden tailing dam sludge based on the culture-independent sequencing approach, 4 phyla and 8 genera were identified in a sample from the arsenic-rich gold mine. Firmicutes (92.23%) was the dominant phylum, followed by Proteobacteria (3.21%), Actinobacteria (2.41%), and Bacteroidetes (1.49%). The identified genera included Staphylococcus (89.8%), Pseudomonas (1.25), Corynebacterium (0.82), Prevotella (0.54%), Megamonas (0.38%) and Sphingomonas (0.36%). The Shannon index value (3.05) and Simpson index value (0.1661) indicated low diversity in arsenic laden tailing. The culture dependent method exposed significant similarities with culture independent methods at the phylum level with Firmicutes, Proteobacteria and Actinobacteria, being common, and Firmicutes was the dominant phylum whereas, at the genus level, only Pseudomonas was presented by both methods. It showed high similarities between culture independent and dependent methods at the phylum level and large differences at the genus level, highlighting the complementarity between the two methods for identification of the native population bacteria in arsenic-rich mine. As a result, the present study can be a resource on microbes for bio-treatment of arsenic in mining waste.

    H2S gas when exposed to metal can be responsible for both general and localized corrosion, which depend on several parameters such as H2S concentration and the corrosion product layer formed. Therefore, the formation of passive film on 316L steel when exposed to H2S environment was investigated using several analysis methods such as FESEM and STEM/EDS analyses, which identified a sulfur species underneath the porous structure of the passive film. X-ray photoelectron spectroscopy analysis demonstrated that the first layer of CrO3 and Cr2O3 was dissolved, accelerated by the presence of H2S-Cl-. An FeS2 layer was formed by incorporation of Fe and sulfide; then, passivation by Mo took place by forming a MoO2 layer. NiO, Ni(OH)2, and NiS barriers are formed as final protection for 316L steel. Therefore, Ni and Mo play an important role as a dual barrier to maintain the stability of 316L steel in high pH2S environments. For safety concern, this paper is aimed to point out a few challenges dealing with high partial pressure of H2S and limitation of 316L steel under highly sour condition for the oil and gas production system.

    Landfills are a potential threat to human health and the environment, especially from the detrimental and toxic heavy metals. This study focuses on the assessment of heavy metals contamination in leachate and surface soils from different landfills in Malaysia. Maximum quality rating scale (QRS) values of As (787) and Cr (552) denotes progressive deterioration of leachate contamination in landfill. The impacted soils showed high heavy metal concentrations especially at non-sanitary unlined landfills, as compared to background values, and natural soil nearby the landfills. In addition, to examine the environmental impacts of the landfill area (soil) in more detail, specific indexes; geo-accumulation index (Igeo), pollution index (PI) and integrated pollution index (IPI) were determined. Maximum As (3.122) and Cd (2.633) for Igeo and As (34.037) and Cd (20.881) for PI revealed that the soil samples in non-sanitary landfills were moderate to strongly polluted. The difference in range of IPI values for sanitary (0.294-0.322) and non-sanitary landfill soils (1.263-1.956) confirmed advanced decline of the soil quality in non-sanitary landfills. Arsenic concentrations were found to be statistically significant (ANOVA) for leachate and impacted soil in landfills investigated. It is also important to realize that rise in metal contents in landfill environments were not only caused by anthropogenic sources such as from the waste disposed, but also some other factors such as redox conditions, anoxic environments, pH, oxidation state of metals and microbial activities. Those conditions will actively promotes leaching of metals from waste and also natural soils in the landfill.

    Background: Finding a new target or a new drug to overcome chemoresistance is difficult due to the heterogenous nature of cancer. Meta-analysis was performed to combine the analysis of different microarray studies to get a robust discovery. Materials & methods: Herein, we analyzed three microarray datasets on combination of folinic acid, fluorouracil, and oxaliplatin drugs (FOLFOX) resistance that fit our inclusion/exclusion criteria and performed a meta-analysis using the OmiCC system. Results: We identified several deregulated genes and we discovered HNF4A as a hub gene. We performed functional validation and observed that by targeting HNF4A, HCT116 cells were more sensitive toward both oxaliplatin and 5-fluorouracil significantly. Conclusion: Our findings show that HNF4A could be a potential target in overcoming FOLFOX chemoresistance in colorectal cancer.

    The protein profile of serum samples from patients with amoebic liver abscess (ALA) was compared to those of normal individuals to determine their expression levels and to identify potential surrogate disease markers. Serum samples were resolved by two dimensional electrophoresis (2-DE) followed by image analysis. The up and down-regulated protein spots were excised from the gels and analysed by MS/MS. The concentration of three clusters of proteins i.e. haptoglobin (HP), α1-antitrypsin (AAT) and transferrin in serum samples of ALA patients and healthy controls were compared using competitive ELISA. In addition, serum concentrations of HP and transferrin in samples of patients with ALA and pyogenic liver abscess (PLA) were also compared. The results of the protein 2-DE expression analysis showed that HP cluster, AAT cluster, one spot each from unknown spots no. 1 and 2 were significantly up-regulated and transferrin cluster was significantly down-regulated in ALA patients' sera (p<0.05). The MS/MS analysis identified the unknown protein spot no.1 as human transcript and haptoglobin and spot no. 2 as albumin. Competitive ELISA which compared concentrations of selected proteins in sera of ALA and healthy controls verified the up-regulated expression (p<0.05) of HP and the down-regulated expression (p<0.01) of transferrin in the former, while there was no significant difference in AAT expression (p> 0.05). However, when ALA and PLA samples were compared, competitive ELISA showed significant increased concentration of HP (p<0.05) while transferrin levels were not different. In conclusion, this study showed that HP is a potential surrogate disease marker for ALA.

    Reactive arthritis can be an elusive diagnosis especially in the elderly. A 77-year-old lady, presented with recent history of hip pain. She had been treated for urinary tract infection caused by Chlamydia sp. and had associated weight loss. She was also investigated for possible tuberculosis and occult malignancy. CT scan abdomen/pelvis and MRI revealed peri-articular muscle inflammation. Biopsy of her hip joint failed to find the causative factor. Whole body 18F-FDG PET/CT scan revealed increased FDG uptake at bilateral hip and shoulder joints. She recovered after an intensive course of antibiotics. Thus, she was diagnosed with reactive arthritis. Reactive arthritis is usually a diagnosis of exclusion made by a high index of suspicion and positive serology test. Molecular imaging can be an alternative investigation for joint pains in the elderly, which enables excellent anatomical and functional information to exclude more sinister conditions such as malignancy.

    Objective: This study aims to determine the prevalence of irritable bowel syndrome (IBS) and its association with perceived stress levels among nursing students at the International Islamic University Malaysia.
    Methods: A cross-sectional design was used in this study. A three-part questionnaire was used to gather data on stress levels and IBS symptoms. Part A posed questions on socio-demographic aspects. In part B, a Perceived Stress Scale was used to measure individuals’ perceived stress. Part C used a questionnaire developed by the World Gastroenterology Organisation (WGO) with the support of Danone.
    Results: The response rate was 85.5%: out of 200 students, 171 completed the questionnaires. According to the results, there is a significant relationship between stress levels and IBS (p = 0.006), where students who experienced moderate stress (151, 88.3%) were more exposed to IBS (60, 39.7%). There is no association between the characteristics of subjects with IBS and their level of stress (P > 0.05).
    Conclusion: The prevalence of IBS among the students in this study was roughly moderate. The frequency of moderate stress levels was high and there was a significant association with IBS. On the other hand, subject characteristics, e.g. smoking habits, were insignificantly associated with IBS and stress level (SL).

    PURPOSE: Reporting and analysing the data on medication errors (MEs) is important and contributes to a better understanding of the error-prone environment. This study aims to examine the characteristics of errors submitted to the National Medication Error Reporting System (MERS) in Malaysia.

    METHODS: A retrospective review of reports received from 1 January 2009 to 31 December 2012 was undertaken. Descriptive statistics method was applied.

    RESULTS: A total of 17,357 MEs reported were reviewed. The majority of errors were from public-funded hospitals. Near misses were classified in 86.3 % of the errors. The majority of errors (98.1 %) had no harmful effects on the patients. Prescribing contributed to more than three-quarters of the overall errors (76.1 %). Pharmacists detected and reported the majority of errors (92.1 %). Cases of erroneous dosage or strength of medicine (30.75 %) were the leading type of error, whilst cardiovascular (25.4 %) was the most common category of drug found.

    CONCLUSIONS: MERS provides rich information on the characteristics of reported MEs. Low contribution to reporting from healthcare facilities other than government hospitals and non-pharmacists requires further investigation. Thus, a feasible approach to promote MERS among healthcare providers in both public and private sectors needs to be formulated and strengthened. Preventive measures to minimise MEs should be directed to improve prescribing competency among the fallible prescribers identified.

    Giardia duodenalis remains a neglected tropical disease. A key feature of the sustained transmission of Giardia is the ability to form environmentally resistant cysts. For the last 38 years, proteomics has been utilised to study various aspects of the parasite across different life cycle stages. Thirty-one articles have been published in PubMed from 2012 to 2022 related to the proteomics of G. duodenalis. Currently, mass spectrometry with LC-MS/MS and MALDI-TOF/TOF has been commonly utilised in proteomic analyses of Giardia, which enables researchers to determine potential candidates for diagnostic biomarkers as well as vaccine and drug targets, in addition to allowing them to investigate the virulence of giardiasis, the pathogenicity mechanisms of G. duodenalis, and the post-translational modifications of Giardia proteins throughout encystation. Over the last decade, valuable information from proteomics analyses of G. duodenalis has been discovered in terms of the pathogenesis and virulence of Giardia, which may provide guidance for the development of better means with which to prevent and reduce the impacts of giardiasis. Nonetheless, there is room for improving proteomics analyses of G. duodenalis, since genomic sequences for additional assemblages of Giardia have uncovered previously unknown proteins associated with the Giardia proteome. Therefore, this paper aims to review the applications of proteomics for the characterisation of G. duodenalis pathogenicity and the discovery of novel vaccine as well as drug targets, in addition to proposing some general directions for future Giardia proteomic research.

    Mitragyna speciosa, also known as kratom, has been used for mitigating the severity of opioid withdrawal in humans. Its main indole alkaloid, mitragynine, has been considered as a pharmacotherapy for pain conditions and opioid replacement therapy. However, at high doses, chronic mitragynine may also have an addiction potential. The effects of chronic action of mitragynine in the brain are still unknown. The present study developed a mitragynine withdrawal model in rats and used it for a proteomic analysis of mitragynine withdrawal effects. Mitragynine (30 mg/kg, i.p.) was administered daily over a period of 14 days and then withdrawn. A proteomic analysis revealed that from a total of 1524 proteins identified, 31 proteins were upregulated, and 3 proteins were downregulated in the mitragynine withdrawal model. The Rab35 protein expression increased most profoundly in the mitragynine withdrawal group as compared to vehicle group. Therefore, it is proposed that Rab35 in the brain might be considered as a potential biomarker during mitragynine withdrawal and might be valuable target protein in developing new pharmacotherapies in the future.

    The current study aimed to investigate the efficiencies and mechanisms of slag filter media for removing phosphorus from synthetic wastewater. The steel slag with high ferric oxides (Fe2O3) was subjected for the electric arc furnace (EAF) and selected as the filter media (HFe). The chemical characteristics of HFe were determined using pH, point of zero charge (PZC) and XRF. The phosphorus removal efficiency was studied in a designed vertical steel slag column rock filters in unaerated HFe (UEF) and aerated HFe (AEF) system. The microstructure of HFe was analyzed by FTIR, XRD and SEM-EDX analysis. The results of XRF revealed that ferric oxide (Fe2O3) ranged from 26.1 to 38.2%. PZC for Filter HFe was recorded at pH 10.55 ± 0.27. The highest efficiencies were recorded by UEF and AEF systems at pH 3 and pH 5 (89.97 ± 4.02% and 79.95 ± 6.25% at pH 3 and 72.97 ± 8.38% and 66.00 ± 12.85% at pH 5 for UEF and AEF, respectively). These findings indicated that AEF exhibiting higher removal than UEF systems might be due to presence high Fe concentration in AEF which play important role in the phosphorus removal. The main elements available on the surface of HFe included carbon, oxygen, iron, calcium, magnesium, silicon, platinum, sulphur, manganese, titanium and aluminium. The XRD analysis indicated that the precipitation of orthophosphate as calcium and iron-phosphates was the removal mechanism as confirmed using FT-IR analysis. These findings demonstrated the efficiency of HFe in removing of phosphorus from wastewater.

    BACKGROUND: The involvement of community pharmacists (CPs) in the provision of pretravel health services is increasing due to the increase in international travelers, the increased risk of travel-related diseases, and the expansion of pharmacists' scope of practice in some countries. In order to improve the quality and effectiveness of pretravel health care provided by CPs, a greater understanding of the practices, barriers, and facilitators is required.

    OBJECTIVE: This scoping review aimed to identify and describe existing studies on pretravel health services provided by CPs, and the barriers, and facilitators.

    METHODS: The PubMed, Science Direct, Scopus, and Web of Science databases were searched for pertinent studies from their inception to February 2023. A manual search was also conducted of prominent travel medicine journals, Google Scholar, and the reference lists of the included studies. Potential barriers and facilitators were mapped to the 14 domains of the Theoretical Domains Framework (TDF).

    RESULTS: There were twelve studies included in the review. Pretravel health advice was the most prevalent form of pretravel health services. Within ten domains of the TDF, various factors that either facilitate or impede the provision of pretravel health services by CPs were identified.

    CONCLUSION: The provision of pretravel health services by CPs may be affected by a number of practitioner and organizational factors. The provision of pretravel health services can be facilitated by informational resources, training and education in travel medicine, and collaboration amongst healthcare providers.

    Waste electrical and electronic equipment or e-waste has recently emerged as a significant global concern. This waste contains various valuable metals, and via recycling, it could become a sustainable resource of metals (viz. copper, silver, gold, and others) while reducing reliance on virgin mining. Copper and silver with their superior electrical and thermal conductivity have been reviewed due to their high demand. Recovering these metals will be beneficial to attain the current needs. Liquid membrane technology has appeared as a viable option for treating e-waste from various industries as a simultaneous extraction and stripping process. It also includes extensive research on biotechnology, chemical and pharmaceutical, environmental engineering, pulp and paper, textile, food processing, and wastewater treatment. The success of this process depends more on the selection of organic and stripping phases. In this review, the use of liquid membrane technology in treating/recovering copper and silver from industrial e-waste leached solutions was highlighted. It also assembles critical information on the organic phase (carrier and diluent) and stripping phase in liquid membrane formulation for selective copper and silver. In addition, the utilization of green diluent, ionic liquids, and synergist carrier was also included since it gained prominence attention latterly. The future prospects and challenges of this technology were also discussed to ensure the industrialization of technology. Herein, a potential process flowchart for the valorization of e-waste is also proposed.

    This study aimed at determining the effects of technology adoption on farmers' well-being. Specifically, we analysed and extended the current understanding of the topic by focusing on the concepts of technology adoption and well-being. The data were sourced from 30 papers that were selected based on specific criteria from the WoS and Scopus databases. They were acquired through a systematic search and quality appraisal process. The adopted technology can be grouped into sustainable agricultural practices and innovations, digital agriculture and information technology, precision farming and resource management, financial inclusion and agricultural transactions, remote sensing and satellite technology, and agricultural programs and initiatives. Most papers indicated that technology adoption improved farmers' well-being which was basically measured using productivity and income. The measure however lacked farmers' value judgments, such as happiness. Agricultural technology could have a mixed effect on farmers' well-being, depending on the type of technology adopted and the compatibility of farmers with technology in their agricultural practices. This study will assist researchers in developing an appropriate framework and strategy to improve farmers' welfare, which can alleviate poverty. These may further help policymakers identify the best technological innovations that can improve farmers' economic and social welfare, thereby facilitating the development of plans to meet farmers' needs and interests. Finally, the study suggested future research directions for researchers and academics.
