Introduction: The use of traditional storage facilities by most of the grain farmers and traders in Nigeria promotes fungal contamination of stored grains and subsequently mycotoxins which are potent carcinogens, neurotoxic, hepatotoxic and immunotoxic when consumed. This study was conducted to determine the incidence and phenotypes of mycotoxigenic fungi associated with commonly consumed food grains in Katsina state, Nigeria. Method: Fungal population in 21 composite samples each of maize, wheat, rice and peanuts from three open markets were determined using standard mycological techniques. Aspergillus spp obtained from the sam- ples were screened for aflatoxigenicity and subsequently characterised by Attenuated Total Reflectance Fourier Transformed Infrared (ATR-FTIR) spectroscopy. Results: A total of 136 filamentous fungi belonging to 19 species were isolated, of which Aspergillus flavus (18.4%), Mucor racemosus (13.2%) and Aspergillus niger (10.3%), were predominant. The highest level of contamination was found in the peanuts (1.8 x 105 ± 2.5 x
105 CFU/g). All the 12 Aspergillus parasiticus and 18 (72%) of Aspergillus flavus isolates obtained from the samples produced aflatoxin B1 on solid media as observed under ultraviolet light and confirmed by Thin Layer Chromatography. The ATR-FTIR spectra of both toxigenic and atoxigenic Aspergillus spp showed similar pattern. Conclusion: The levels of the mycotoxigenic fungi in the food grains, except for rice, were above the permissible limit of 100 to 10,000 CFU/g set by ICMSF, this signifies that they are unsafe for use as food or feed ingredients and hence, the need for more stringent control measures.

Maternal depression has been linked to the development of adolescents’ emotional and behavioural problems. The main objective of this study was to determine the association between maternal depressive disorders and externalizing and internalizing problems among their adolescent children. This was a cross-sectional, comparison study of 35 mothers with depression and their adolescents, matched with 35 healthy mothers and their adolescents as controls. The mothers completed Quick Inventory Depressive Symptomatology (QIDS) for assessment of current depression. The emotional and behavioural problems in the adolescents were assessed independently by the mothers and their adolescents off-springs using Child Behavioral Checklist (CBCL) and Youth Self-Report (YSR), respectively. SPSS version 12.0 was used for statistical analysis. The findings showed that adolescents who have mothers with depressive disorders had significantly higher scores of externalizing (mean difference = 4.686 + 10.887, p = 0.016) and total emotional and behavioural problems (mean difference = 10.171 + 23.007, p = 0.013) than controls. The cases also scored higher than the controls in the following CBCL syndrome scales: aggressive behaviour (mean difference = 3.200 + 6.773, p = 0.008), social problem (mean difference = 1.286 + 2.865, p = 0.012), and attention problem (mean difference = 1.543 + 4.435, p = 0.047). Mothers with depressive disorders reported that their adolescents have greater emotional and behavioural problems than the controls. The findings suggested a need for preventive strategies to curb problematic behaviour focusing on this vulnerable group.

Making a medical diagnosis in an elderly person presenting with first time psychiatric symptoms can be challenging.
A 61 year old lady presented with 4 years history of tactile hallucinations in her legs and delusions of persecution,
and 2 weeks history of depression and mild cognitive impairment. There were no other significant physical findings,
except for dysarthria and fine bilateral hand tremors. As patient had no prior psychiatric history, she was investigated
for differential diagnoses of late-onset schizophrenia, psychotic depression and early dementia. A thorough
investigation for concomitant medical illnesses was done which revealed low serum calcium, high serum phosphate
and relatively low serum parathyroid hormone levels. A diagnosis of Fahr’s syndrome (FS) was made based on
history, hematological findings of idiopathic hypoparathyroidism and bilateral basal ganglia calcifications (BGC)
on neuroimaging. Treatment of FS is non-specific and mainly symptomatic. This patient improved with treatment
comprising antidepressant, antipsychotic and calcium supplement.

Introduction: Musculoskeletal Disorder (MSD) is an injury that affects the movement of human body systems such as muscles, tendons, ligaments, nerves, blood vessels, and others. The prevalence of MSDs in Indonesia is 11.9% based on diagnosis and 24.7% based on symptoms. MSDs prevalence is 31.2% based on employment among farm- ers, fishermen, and workers. Methods: The design of this study has been included in observational research using a quantitative approach with Cross-Sectional study. Results: MSDs can appear based on gender, age, educational status, and non-medical supporting professions. Older workers tend to be at risk of developing MSDs complaints. Conclusion: To reduce the risk of complaints, work facilities such as ergonomic chairs and tables can be provided along with extension of work intervals. Workers must take advantage of good rest periods. There is no relationship between work tenure and MSDs complaints. If the working period is long, then it can cause boredom and the endur- ance of muscles as well as bones physically.

Introduction: Colon cancer is one of the leading cause of cancer death and current treatments often bring about undesired toxicities and resistance. Hence targeted therapeutic regimens for cancer are developed. Anticancer agent incorporated with copper has been synthesized to selectively target cancer cells that are reported to take up more copper compared to normal cells. Cu(SBCM)2 synthesized from the condensation of s-benzyldithiocarbazate and 3-aetylcoumarin was demonstrated to exhibit anti-proliferative effect towards MCF-7 cells and MDA-MB-231 breast cancer cells via cell cycle arrest and apoptosis. However, the mode of cell death of Cu(SBCM)2 on colon cancer cells has not been explored. This study investigated the anti-cancer properties of Cu(SBCM)2 on HT-29, colorectal cancer cell line. Methods: The growth inhibition of the copper complex was determined using MTT assay and xCELLigence real time cell monitoring analysis. Results: Cu(SBCM)2 was shown to inhibit the growth of HT-29 cells significantly in time- and dose- dependent manner with IC50 of 25.23 ± 8.22 uM at 48 hours. Morphological studies using inverted light microscope indicating Cu(SBCM)2-treated HT-29 cells displayed characteristics of apoptosis such as cellular shrinkage and membrane blebbing. Cell cycle analysis was carried out using flowcytometer and Cu(SBCM)2 was found to induce G2M cell cycle arrest at 24 and 48 hours. ROS assay was carried out to determine the involvement of oxidative stress on Cu(SBCM)2 treated HT-29 cells. Nevertheless, results indicated Cu(SBCM)2 significantly suppressed the formation of ROS compared to control. Conclusion: In summary, Cu(SBCM)2 shows promising potential in cancer therapy against colon cancer cells.

Introduction: Low socio-economic status (SES) has been associated with poorer access and worse outcome in kid-ney transplantation (KT) for high-income countries. In contrast, relatively little is known about the KT access and outcome between the socio-economic gradient in Malaysia, which is a low- and middle-income country. The aim is to determine the association of SES with access and outcome of kidney transplantation in Malaysia. Methods: We examined the Malaysian National Renal Registry for kidney transplant candidates listed between 2002 to 2011. Cox’s regression was used to assess associations of SES with transplant access and outcome (survival). Results: 1234 patients were transplanted in 2002-2011 after excluding pediatric patients and multiple transplantations. The mean age of patients was 42.3 ± 12.3 years, 789 (63.9%) were male and 797 (65.0%) were of Chinese ethnicity. After multivariate adjustment, the highest income group were 20% less likely to access KT compared the lowest income group (adjusted HR=0.80, CI: [0.62, 1.04] producing trend for P-Value = 0.001. While patient with highest education were 50% more likely to survive compared to patient with lowest education (adjusted HR=0.50, CI: [0.34, 0.73] producing trend for P-Value < 0.001. Conclusion: Even in a healthcare system that is almost achieving universal health coverage, disparities exist in both extremes of the socio-economic gradient for the access and outcomes for KT in Malaysia. Reducing the SES disparities in KT will require policy initiatives addressing the components of SES (income, education, and employment) as well as the pathways by which this affect the health of KT patient.

Introduction: Kidney transplantation (KT) is the preferred end-stage renal disease (ESRD) treatment because it pro-vides a better survival rate, quality of life as well as a cheaper alternative. However, Malaysia’s KT rates is consis-tently low considering that ESRD rates have been increasing exponentially. With only four hospitals performing KT, there’s a gap to indicate a lack of evaluation in KT system of Malaysia. Qualitative study was undertaken to explore and describe the barriers and solutions improve the rates and service of KT in Malaysia. Methods: Semi-structured interviews adopted as qualitative methodological approach to explore current KT policy and service in Malaysia be-tween March – May 2018 in Kuala Lumpur. Eight key-informants selected using stakeholder analysis and informed consent were obtained. Interviews were digitally audio-recorded, transcribed verbatim and analysed using thematic analysis. Results: Barriers and solutions of Malaysia’s KT are the results of complex interplay of personal, cultural, and environmental factors that can be categorized and described using the five levels of influence conceptualized by the socio-ecological model (SEM). Guidance for developing culturally appropriate and sensitive interventional strategies was elicited from the key informants’ experiences to improve KT rate and services in Malaysia. Conclusion:Malaysia is experiencing very low rate of KT compared to other countries. The use of SEM provided a framework to foster a better understanding of current practice, barriers and solutions to KT in Malaysia. Implications of these find-ings could prompt policy change for better KT service delivery model. Further stakeholder engagement and evalua-tion is required to align best practices to improve KT rates and service in Malaysia that is comparable to high-income countries.

Introduction: Cervical cancer remains the second commonest female malignancy worldwide and the seventh among Malaysians. Globally in 2012, an estimated 528 000 women developed cervical cancer and almost 266 000 died from this disease. Of all cervical cancers, 83% occur in the less developed world due to the absence of screening. To promote the community health awareness of cervical cancer which is preventable. Methods: By internet literature searching (Google Scholar) and textbooks. Results: The primary cause of cervical pre-cancer and cancer is persistent infection with one or more of the oncogenic types of HPV, the most common infection acquired during sexual inter-course, usually early in sexual life. Cervical cancer due to HPV can be prevented by HPV vaccination, participation in a screening program, avoidance of smoking, limitation of sex partners and use of a condom. Three HPV vaccines are available worldwide such as bivalent HPV 16/18 vaccine, quadrivalent HPV 16/18/6/11 L1 virus-like particles vaccine and 9-valent HPV 6/11/16/18/31/33/45/52/58 recombinant vaccine. The HPV test can be done on the same sample of cells collected from the Pap test and it can help to know HPV types. Conclusion: Cervical cancer due to HPV can be prevented by vaccination and the pre-cancerous phase of cervical cancer can be screened by the HPV testing with the Pap test. The community health awareness plays a major role in cervical cancer prevention.

Introduction: Tuberculosis (TB) is the ninth leading cause of mortality in the world while it is the most prevalent infection which is ranked abolve HIV/AIDS. In Malaysia, tuberculosis is still a public health problem. Sabah State on Malaysian Borneo had 20-30% of total TB cases of the country. In Sabah, case notification rate of almost 200 per 100,000 population was still present in the last 10 years. Hotspots are defined as TB notification rate more than 100/100.000 in a district or TB notification rate more than 100/100,000 in the squatters’ area. In this study, cycle threshold (ct) values in GeneXpertMTB/RIF (Xpert) were tried to be correlated with growth in Mycobacterim growth indicator tubes (MGIT) by measurement of time to detection (TTD). Methods: Sputum samples from six hotspots of Kota Kinabalu were studied by Xpert as well as MGIT culture after processing of sputum samples. The correlation between Mean ct value of Xpert and TTD in MGIT was analysed by using IBM SPSS Statistic 25 and the statistical test used was Pearson Correlation test. Results: The results of Xpert indicated 35 of 204 sputum samples were pos-itive whereas only one sample was rifampicin resistant. The mean ct values were very low, low and medium in all the hotspots with sputum from one hotspot showed medium ct values predominantly. The sputum from remaining hotspots showed mainly very low and low ct values. MGIT results showed no growth in five samples with two very low, two low and one medium mean ct values. Conclusion: The finding indicated that there were correlations be-tween mean ct values of Xpert and TTD in MGIT with a few exceptions.

Introduction: Obesity is a growing public health concern. Poor diet and lifestyle choices are the main contributors to its development. Lifestyle modifications should be aggressively promoted in the community. Recent studies found that worksite obesity prevention and control programs are effective in increasing physical activity and weight reduction among employees. In Malaysia, there is insufficient data on prevalence studies of workplaces. This study investigated the proportion of overweight and obesity among workers in a private university and hope to serve as a baseline for future healthy workplace programs. Methods: This cross-sectional study investigated overweight and obesity and examined its association with occupational stress, sleeping quality, dietary behaviour and physical inactivity among employees in an academic institution. Weight and height were measured, and BMI was calculated and coded as underweight, normal, overweight and obese according to the cut-off points for the Asian population. Results: Out of ninety-five employees, 55 participated in the survey. Among the 55 employees, 4 (7.3%) were underweight, 10 (18.2%) were normal, 25 (45.5%) were overweight, and 16 (29.1%) were obese. The obese and overweight proportion was similar to the national prevalence. Poor sleep quality and high Job stress scores were reported by 40% of the employees, while physical inactivity of more than 6 hours a day was reported by 50%, and the majority (90%) do not consume enough fibre. Among the risk factors investigated, none was associated with obesity. Conclusion: The proportion of overweight and obesity in this community was like the general population. While not significant in this study, the association between obesity and the four risk factors should be further investigated using a larger sample size. Based on the results, community-based health promotion intervention is suggested to reduce obesity among the employees.

Introduction: Lipoprotein L21 (LipL21) has been used as a molecular marker for leptospirosis as it is highly expressed in pathogenic Leptospira species during infection. However, it lacks specificity due to the newly emerging pathogenic serovars. Therefore, interrogation of LipL21 in all serovars needed to understand the pathogenesis of leptospirosis to enable early diagnosis. This study was carried out to determine the suitability of LipL21 as a molecular marker for leptospirosis by identifying the conserved sequences of LipL21 mRNA and amino acids in different Leptospira strains. Methods: Location of LipL21 conserved regions in 15 pathogenic and 2 non-pathogenic strains of five Leptospira species, were identified using bioinformatics database and tools such as National Center of Biotechnology, Rapid Annotation Subsystem Technology blast search, Muscle program and Jalview software. Results: Multiple sequence alignment analysis revealed that two conserved regions were observed in 10 pathogenic Leptospira strains from nucleotide position 29 to 53 and 100 to 137, however conserved amino acid sequences (111-149 and 155-192) were found in all the pathogenic strains. The distinction between gene and amino acid results is due to the degenerate genetic code feature. Conclusion: In conclusion, this study suggests that LipL21 protein has a potential to be used as a diagnostic marker for detection of Leptospira pathogens compared to LipL21 mRNA.

Introduction: CRISPR/Cas9 nuclease has gained popularity as a genome editing tool due to its straight-forward mechanism. However, there are concerns that CRISPR nuclease would cause off-target and toxicity. The CRISPR/ Cas9 D10A nickase was designed to enhance genome editing. Nevertheless, this raised the question of whether the efficiency of nickase is compromised compared to CRISPR/Cas9 nuclease. Targeting HIV genes, we investigated if CRISPR nuclease performed better than the nickase in efficacy and safety. Methods: CRISPR nucleases and nickases were designed to target Gag, Pol, Rev, Vif, Tat and LTR. HIV latently infected cell line, ACH-2, was transfected with the nucleases and nickases. Changes to viral load after CRISPR treatment was measured using p24 ELISA. Safety of nuclease and nickase was monitored using GFP expression with fluorescence microscopy and flow cytometry. Targeting two sites within the same gene, and targeting multiple genes concurrently were also studied to determine efficacy of CRISPR in reducing viral load. Results: A 44.9 to 68.1% and a 34.4 to 49.7% decrease in viral load was seen in CRISPR nuclease and nickase respectively. Microscopy and flow cytometry results showed that the nickase system was slightly toxic with a 0.31 to 0.7-fold cell death. There was a 34% decrease in viral load when two sites were targeted within a gene, and the largest decrease was seen when all the nucleases were combined, giving a 75.4% decrease in viral load at day 5. Conclusion: The knowledge gained from this study will be employed to im- prove genome editing in other disease models.

Leptospirosis has a wide range of presentation which ranges from mild flu-like symptoms, to severe form including renal failure, liver failure, and hemorrhage. Pulmonary involvement can progress from subtle clinical features to life threatening pulmonary hemorrhage and acute respiratory distress syndrome. Although benefits of corticosteroids in adult respiratory distress syndrome have been proven and accepted, evidence for use of corticosteroids in pulmonary leptospirosis is still limited. Given the vasculitic nature of severe leptospirosis, it has been proposed that addition of intravenous corticosteroid therapy, particularly in cases of pulmonary involvement is beneficial. We report a case of leptospirosis with suspected pulmonary hemorrhage which deteriorates after a few days of admission in our tertiary hospital. We have demonstrated that the prescription of a lower dose of corticosteroid than what was widely reported in the literature can equally led to a satisfactory recovery of the pulmonary hemorrhage.

Introduction: Dysmenorrhoea is the most common gynaecology problems female’s goes through in their life. In Malaysia, the overall prevalence of dysmenorrhoea is 78%. There were studies revealed that there is significant asso- ciation between dysmenorrhoea and quality of life (QoL). Female who is having dysmenorrhoea has a lower quality of life compared to others. However, most female ignore this issue as they feel it is a normal female phenomenon. Nevertheless, health education and interventions should be given to reduce dysmenorrhoea, thus improve QoL. The main aim of this research was to determine the menstrual characteristics, primary dysmenorrhoea, and their associa- tion with selected socio-demographic variables among Chinese undergraduate students. Methods: A cross-sectional study was carried out among 317 participants, aged between 19 to 25 years. Purposive sampling method was uti- lized to recruit the participants. Questionnaire on menstruationwas distributed to collect the information regarding socio-demographic background, menstrual characteristics, and primary dysmenorrhoea. Results: The incidences of primary dysmenorrhoea were reported by 63.4% (n=201)in this study. Only a few socio-demographic variableswere detected to have a significant association with menstrual characteristics and primary dysmenorrhoea. Associations between age and menstrual duration was discovered (p=0.005). Besides, menarche was associated with presence of blood clot during menstruation (P

Introduction: Filipino β°-deletion is predominant among the β-thalassaemia patients in the indigenous population of Sabah, Malaysia particularly among the Kadazandusun. Individuals who co-inherit with α- and β-thalassaemia will demonstrate milder clinical symptoms with modified complete blood count (CBC) and Hb subtype parameters. HBS1L-MYB variants act as one of the key regulator of haematopoiesis and erythropoiesis and display strong association
with variation of HbF levels. Therefore, this study aims to evaluate the association between genetic variants in HBS1L-MYB with Hb subtypes level among Filipino β°-deletion carriers co-inherited with -α3.7 deletion. Methods: Filipino β°-deletion and -α3.7 deletion were identified using gap-polymerase chain reaction (PCR). A total of 34 subjects found with coinheritance of Filipino β°-deletion and -α3.7 deletion were subjected for HBS1L-MYB intergenic polymorphisms (HMIP) analysis. Hb subtypes level were quantified using BioRad Variant II Hb analyser. Genotyping of HBS1L-MYB variants rs9399137 and rs11759553 was done using own designed tetra primer ARMS-PCR. Results: The minor allele frequencies (MAF) of the two HMIP is found more than 0.05 (rs11759553, MAF=0.18 and rs9399137, MAF=0.15), indicating the significance of these variants among the study subjects. Significant difference was found between HbF level and HBS1L-MYB variant rs11759553 with p-value less than 0.05 (p=0.001). Subjects with homozygous genotype for rs11759553 (T/T) was found with higher HbF, followed by heterozygous (A/T) and wild type (A/A). rs11759553 and rs9399137 was found did not influence the level of HbA and HbA2. HMIP of rs11759553 and rs9399137 are found significant among Filipino β°-deletion carriers co-inherited with -α3.7deletion with its high minor allelic frequency and high HbF level. Strong association with HbF level was demonstrated when
coinheritance of rs11759553. Conclusion: This study demonstrates that there are significant associations between certain genetic variants in HBS1L-MYB with Hb subtypes level among Filipino β°-deletion carriers co-inherited with -α3.7 deletion.
