    Postprandial hyperglycaemia is associated with an increased risk of type-2 diabetes. This study aims to determine the glycaemic index (GI) of three varieties of rice-based mixed meals and their effects on glycaemic variability (GV), 24 h mean glucose levels and target ranges, and rice variety preferences among overweight and obese young adults using real-time continuous glucose monitoring (rtCGM). In a randomised controlled crossover design, 14 participants (22.8 ± 4.6 years, 32.9 ± 5.8 kg/m2) were randomly assigned to receive 3 rice-based mixed meals containing 50 g of available carbohydrates (white rice meal = WRM; brown rice meal = BRM; and parboiled basmati rice meal = PBRM) and 50 g of a glucose reference drink on alternate days. GI, GV, 24 h mean glucose levels and target ranges were measured. Rice variety preferences were compared with those of baseline data and determined at the end of the study period. Results: The analysis found that PBRM was low in GI (45.35 ± 2.06), BRM medium in GI (56.44 ± 2.34), and WRM high in GI (83.03 ± 2.19). PBRM had a significantly (p < 0.05) lower 24 h mean glucose level, higher in-target 24 h glucose level percentage and non-significantly (p > 0.05) lower GV compared to WRM. Prior to observing their postprandial glucose levels generated by rtCGM, the participants preferred WRM (64.3%) over other meals, whereas this preference changed significantly (p < 0.05) at the endpoint (PBRM, 71.4%). PBRM reduced 24 h glucose level and GV of overweight and obese young adults. The rtCGM is proven to be reliable in measuring GI, while providing robust continuous glycaemic information. This may serve as an educational tool that motivates eating behaviour changes among overweight and obese young adults.

    BACKGROUND: Compassionate care has been increasingly highlighted in the past few decades worldwide, including in Malaysia. Despite acknowledging its importance, Malaysia still lacks a validated tool that can be used to assess the level of compassionate care from the patient's perspective. Therefore, this study aims to validate and translate the Relational Aspect of Care Questionnaire (RAC-Q) into the Malay language.

    METHODS: Permission to use and translate the original RAC-Q into the Malay language was obtained. The RAC-Q was then translated into the Malay language following the 10 steps proposed for the translation of a patient-reported outcome questionnaire. A pretest was conducted based on 30 inpatients to assess the appropriateness and clarity of the finalized translated questionnaire. A cross-sectional study was performed based on 138 inpatients from six adult wards of a teaching hospital so as to validate the translated questionnaire. The data were analyzed using R software version 4.1.3 (R Core Team, Vienna, Austria, 2020). The results were presented descriptively as numbers and percentages or means and standard deviations. A confirmatory factor analysis was performed using robust estimators.

    RESULTS: The analysis showed that the measurement model of the RAC-Q Malay version (RAC-QM) fits well based on several fit indices: a standardized factor loading range from 0.40 to 0.73, comparative fit index (CFI) of 0.917, Tucker-Lewis fit index (TLI) of 0.904, root mean square error of approximation (RMSEA) of 0.06, and a standardized root mean square residual (SRMR) of 0.073. It has good reliability, with a Cronbach's alpha of 0.857 and a composite ratio of 0.857.

    CONCLUSION: The RAC-QM demonstrated good psychometric properties and is valid and reliable based on the confirmatory analysis, and it can thus be used as a tool for evaluating the level of compassionate care in Malaysia.

    The need for the sensing of environmental pollutants cannot be overemphasized in the twenty-first century. Herein, a sensor has been developed for the sensitive and selective detection of copper (Cu2+), lead (Pb2+) and cadmium (Cd2+) as major heavy metals polluting water environment. A screen-printed carbon electrode (SPCE) modified by fluorescent carbon dots (CDs) and gold nanoparticles (AuNPs) was successfully fabricated for sensing Cu2+, Pb2+ and Cd2+. Differential pulse voltammetry (DPV) and cyclic voltammetry (CV) were deployed for the analysis of ternary analytes. CV was set at a potential range of - 0.8 to + 0.2 V at a scan rate of 100 mV/s, and DPV at a potential range of - 0.8 to + 0.1 V, scan rate of 50 mV/s, pulse rate of 0.2 V and pulse width of 50 ms. DPV technique was applied through the modified electrode for sensitive and selective determination of Cu2+, Pb2+ and Cd2+ at a concentration range of 0.01 to 0.27 ppm for Cu2+, Pb2+ and Cd2+. Tolerance for the highest possible concentration of foreign substances such as Mg2+, K+, Na+, NO3-, and SO42- was observed with a relative error less than ± 3%. The sensitivity of the modified electrode was at 0.17, 0.42 and 0.18 ppm for Cd2+, Pb2+ and Cu2+, respectively, while the limits of detection (LOD) achieved for cadmium, lead and copper were 0.0028, 0.0042 and 0.014 ppm, respectively. The quality of the modified electrode for sensing Cu2+, Pb2+ and Cd2+ at trace levels is in accordance with the World Health Organization (WHO) and Environmental Protection Agency (EPA) water regulation standard. The modified SPCE provides a cost-effective, dependable and stable means of detecting heavy metal ions (Cu2+, Pb2+ and Cd2+) in an aqueous solution. Graphical abstract .

    Extensive use of recalcitrant azo dyes in textile and paper industries poses a direct threat to the environment due to the carcinogenicity of their degradation products. The aim of this study was to investigate the efficiency of Curvularia clavata NZ2 in decolorization of azo dyes. The ability of the fungus to decolorize azo dyes can be evaluated as an important outcome as existing effluent treatment is unable to remove the dyes effectively. C. clavata has the ability to decolorize Reactive Black 5 (RB5), Acid Orange 7 (AO7), and Congo Red azo dyes, utilizing these as sole sources of carbon and nitrogen. Ultraviolet-visible (UV-vis) spectroscopy and Fourier infrared spectroscopy (FTIR) analysis of the extracted RB5's metabolites along with desorption tests confirmed that the decolorization process occurred due to degradation and not merely by adsorption. Enzyme activities of extracellular enzymes such as carboxymethylcellulase (CMCase), xylanase, laccase, and manganese peroxidase (MnP) were also detected during the decolorization process. Toxicity expressed as inhibition of germination was reduced significantly in fungal-treated azo dye solution when compared with the control. The cultivation of C. clavata under sequential batch system also recorded a decolorization efficiency of above 90%. The crude enzyme secreted by C. clavata also showed excellent ability to decolorize RB5 solutions with concentrations of 100 ppm (88-92%) and 1000 ppm (70-77%) without redox mediator. This proved that extracellular enzymes produced by C. clavata played a major role in decolorization of RB5.

    Spray drying techniques are one of the methods to preserve and extend the shelf-life of coconut milk. The objective of this research was to create a particle swarm optimization-enhanced artificial neural network (PSO-ANN) that could predict the coconut milk spray drying process. The parameters for PSO tuning were selected as the number of particles and acceleration constant, respectively, for both global and personal best using a 2k factorial design. The optimal PSO settings were recorded as global best, C1 = 4.0; personal best, C2 = 0; and number of particles = 100. When comparing different types of spray drying models, PSO-ANN had an MSE value of 0.077, GA-ANN had an MSE of 0.033, while ANN had an MSE of 0.082. Sensitivity analysis was conducted on all three models to evaluate the significance level of each parameter on the model, and it was discovered that inlet temperature had the most significant influence on the model performance. In conclusion, the PSO-ANN was found to be more effective than ANN but less effective than GA-ANN in predicting the quality of coconut milk powder.

    BACKGROUND: Recent pandemics have had far-reaching effects on the world's largest economies and amplified the need to estimate the full extent and range of socioeconomic impacts of infectious diseases outbreaks on multi-sectoral industries. This systematic review aims to evaluate the socioeconomic impacts of airborne and droplet-borne infectious diseases outbreaks on industries.

    METHODS: A structured, systematic review was performed according to the PRISMA guidelines. Databases of PubMed, Scopus, Web of Science, IDEAS/REPEC, OSHLINE, HSELINE, and NIOSHTIC-2 were reviewed. Study quality appraisal was performed using the Table of Evidence Levels from Cincinnati Children's Hospital Medical Center, Joanna Briggs Institute tools, Mixed Methods Appraisal Tool, and Center of Evidence Based Management case study critical appraisal checklist. Quantitative analysis was not attempted due to the heterogeneity of included studies. A qualitative synthesis of primary studies examining socioeconomic impact of airborne and droplet-borne infectious diseases outbreaks in any industry was performed and a framework based on empirical findings was conceptualized.

    RESULTS: A total of 55 studies conducted from 1984 to 2021 were included, reporting on 46,813,038 participants working in multiple industries across the globe. The quality of articles were good. On the whole, direct socioeconomic impacts of Coronavirus Disease 2019, influenza, influenza A (H1N1), Severe Acute Respiratory Syndrome, tuberculosis and norovirus outbreaks include increased morbidity, mortality, and health costs. This had then led to indirect impacts including social impacts such as employment crises and reduced workforce size as well as economic impacts such as demand shock, supply chain disruptions, increased supply and production cost, service and business disruptions, and financial and Gross Domestic Product loss, attributable to productivity losses from illnesses as well as national policy responses to contain the diseases.

    CONCLUSIONS: Evidence suggests that airborne and droplet-borne infectious diseases have inflicted severe socioeconomic costs on regional and global industries. Further research is needed to better understand their long-term socioeconomic impacts to support improved industry preparedness and response capacity for outbreaks. Public and private stakeholders at local, national, and international levels must join forces to ensure informed systems and sector-specific cost-sharing strategies for optimal global health and economic security.

    BACKGROUND/OBJECTIVES: Dietitians frequently use nutrition education tools to facilitate dietary counselling sessions. Nevertheless, these tools may require adaptation to keep pace with technological advancements. This study had a 2-fold purpose: first, to identify the types of nutrition education tools currently in use, identify their limitations, and explore dietitians' perspectives on the importance of these tools; second, to investigate the features that dietitians prefer in digital nutrition education tools.

    SUBJECTS/METHODS: A semi-structured face-to-face interview was conducted among 15 dietitians from selected public hospitals, primary care clinics, and teaching hospitals in Malaysia. Inductive thematic analysis of the responses was conducted using NVivo version 12 software.

    RESULTS: Most dietitians used physical education tools including the healthy plate model, pamphlets, food models, and flip charts. These tools were perceived as important as they facilitate the nutrition assessment process, deliver nutrition intervention, and are time efficient. However, dietitians described the current educational tools as impersonal, outdated, limited in availability due to financial constraints, unhandy, and difficult to visualise. Alternatively, they strongly favoured digital education tools that provided instant feedback, utilised an automated system, included a local food database, were user-friendly, developed by experts in the field, and seamlessly integrated into the healthcare system.

    CONCLUSION: Presently, although dietitians have a preference for digital educational tools, they heavily rely on physical nutrition education tools due to their availability despite the perception that these tools are outdated, impersonal, and inconvenient. Transitioning to digital dietary education tools could potentially address these issues.

    In light of the increasing importance digital economy, the significance of computational thinking has grown exponentially, becoming imperative in both workplace and academic settings such as universities. This article addresses the critical need to comprehend the factors influencing the acceptance of computational thinking. The dataset introduces an extensive questionnaire comprising five constructs and 25 items, rooted in the extended Technology Acceptance Model. Notably, the model incorporates facilitating conditions and subjective norm, providing a comprehensive framework for understanding acceptance. Data collection involved 132 undergraduate university students sampled through purposive sampling, specifically targeting courses with a focus on computational thinking. The resulting dataset serves as a valuable resource for future research, offering detailed insights into the factors determining the acceptance of technology in educational contexts beyond mere thinking skills. Given the scarcity of research on technology acceptance in developing nations, this dataset holds particular significance, serving as a foundation for potential cross-cultural comparisons. The dataset contributes to the field by presenting a robust acceptance model, explaining 74.2 per cent of the variance in behavioural intention, 60.2 per cent in perceived usefulness, and 56.1 per cent in perceived ease of use. This high explanatory power positions the dataset as a superior resource for replication, benchmarking, and broader applicability in diverse contexts, thereby enhancing the understanding of computational thinking acceptance across different populations and settings. This dataset stands among the pioneering efforts to assess the novel covariance-based structural equation model algorithm within SmartPLS 4, presenting a valuable resource for future research employing the same mechanism.

    BACKGROUND: Ketamine is recognized as an alternative for pain management; however, concerns about emergent adverse reactions have limited its widespread adoption. This study aimed to assess the efficacy of a short infusion of low-dose ketamine (LDK) compared to intravenous morphine (MOR) as adjunctive analgesia for acute long bone fracture pain.

    METHODS: This single-blinded, randomized controlled trial was conducted in a single emergency department. Patients with acute long bone fractures and numerical rating scale (NRS) pain scores ≥ 6 following an initial dose of intravenous morphine were assigned to receive either a LDK (0.3 mg/kg) over 15 min or intravenous MOR at a dose of 0.1 mg/kg administered over 5 min. Throughout a 120-min observation period, patients were regularly evaluated for pain level (0-10), side effects, and the need for additional rescue analgesia.

    RESULTS: A total of 58 subjects participated, with 27 in the MOR group and 31 in the LDK group. Demographic variables and baseline NRS scores were comparable between the MOR (8.3 ± 1.3) and LDK (8.9 ± 1.2) groups. At 30 min, the LDK group showed a significantly greater mean reduction in NRS scores (3.1 ± 2.03) compared to the MOR group (1.8 ± 1.59) (p = 0.009). Similarly, at 60 min, there were significant differences in mean NRS score reductions (LDK 3.5 ± 2.17; MOR mean reduction = 2.4, ± 1.84) with a p-value of 0.04. No significant differences were observed at other time intervals. The incidence of dizziness was higher in the LDK group at 19.4% (p = 0.026).

    CONCLUSION: Short infusion low-dose ketamine, as an adjunct to morphine, is effective in reducing pain during the initial 30 to 60 min and demonstrated comparability to intravenous morphine alone in reducing pain over the subsequent 60 min for acute long bone fractures. However, it was associated with a higher incidence of dizziness.

    CONTEXT: Menhaden fish oil (FO) is widely recognized for inhibiting neuroinflammatory responses and preserving brain function. Nevertheless, the mechanisms of FO influencing brain cognitive function in diabetic states remain unclear.

    OBJECTIVE: This study examines the potential role of FO in suppressing LPS-induced neuroinflammation and cognitive impairment in diabetic animals (DA).

    MATERIALS AND METHODS: Thirty male Wistar rats were divided into 5 groups: i) DA received LPS induction (DA-LPS); ii) DA received LPS induction and 1 g/kg FO (DA-LPS-1FO); iii) DA received LPS induction and 3 g/kg FO (DA-LPS-3FO); iv) animals received normal saline and 3 g/kg FO (NS-3FO) and v) control animals received normal saline (CTRL). Y-maze test was used to measure cognitive performance, while brain samples were collected for inflammatory markers and morphological analysis.

    RESULTS: DA received LPS induction, and 1 or 3 g/kg FO significantly inhibited hyperglycaemia and brain inflammation, as evidenced by lowered levels of pro-inflammatory mediators. Additionally, both DA-LPS-1FO and DA-LPS-3FO groups exhibited a notable reduction in neuronal damage and glial cell migration compared to the other groups. These results were correlated with the increasing number of entries and time spent in the novel arm of the Y-maze test.

    DISCUSSION AND CONCLUSION: This study indicates that supplementation of menhaden FO inhibits the LPS signaling pathway and protects against neuroinflammation, consequently maintaining cognitive performance in diabetic animals. Thus, the current study suggested that fish oil may be effective as a supporting therapy option for diabetes to avoid diabetes-cognitive impairment.

    The incorporation of intermittent renewable energy sources into a consistently controlled power transmission system hinges on advancements in energy storage technologies. Sodium ion batteries (SIBs) are emerging as a primary and viable alternative material due to their electrochemical activity, presenting a potential replacement for the next generation of lithium-ion battery (LIB) energy storage materials. However, this transition may necessitate significant alterations in the anode material, given the incompatibility of the current anode with sodium ions and the electrolyte. This review provides a comprehensive summary of various anode materials employed in SIBs, categorized according to their storage mechanisms. Additionally, it explores the growing focus on utilizing hard carbon as an anode material, driven by factors such as its relatively high specific capacity compared to graphite, cost-effective production, and eco-friendly properties as it can be derived from biomass. The review further addresses recent progress in hard carbon, detailing production methods, modifications, challenges, limitations in integrating hard carbon into the anode of SIBs, and suggests potential directions for future research.

    BACKGROUND AND IMPORTANCE: Acute cardiogenic pulmonary oedema (ACPO) is a common indication for non-invasive ventilation (NIV) in the emergency department (ED). HACOR score of >5 is used to predict NIV failure. The predictive ability of HACOR may be affected by altered physiological parameters in ACPO patients due to medications or comorbidities.

    OBJECTIVES: To validate the HACOR scale in predicting NIV failure among acute cardiogenic pulmonary oedema (ACPO) patients.

    DESIGN, SETTINGS AND PARTICIPANTS: This is a prospective, observational study of consecutive ACPO patients requiring NIV admitted to the ED.

    OUTCOME MEASURE AND ANALYSIS: Primary outcome was the ability of the HACOR score to predict NIV failure. Clinical, physiological, and HACOR score at baseline and at 1 h, 12 h and 24 h were analysed. Other potential predictors were assessed as secondary outcomes.

    MAIN RESULTS: A total of 221 patients were included in the analysis. Fifty-four (24.4%) had NIV failure. Optimal HACOR score was >5 at 1 h after NIV initiation in predicting NIV failure (AUC 0.73, sensitivity 53.7%, specificity 83.2%). As part of the HACOR score, respiratory rate and heart rate were not found to be significant predictors. Other significant predictors of NIV failure in ACPO patients were acute coronary syndrome, acute kidney injury, presence of congestive heart failure as a comorbid, and the ROX index.

    CONCLUSIONS: The HACOR scale measured at 1 h after NIV initiation predicts NIV failure among ACPO patients with acceptable accuracy. The cut-off level > 5 could be a useful clinical decision support tool in ACPO patient. However, clinicians should consider other factors such as the acute coronary and acute kidney diagnosis at presentation, presence of underlying congestive heart failure and the ROX index when clinically deciding on timely invasive mechanical ventilation.

    The essential oil in leaves of Polygonum minus Huds., a local aromatic plant, were identified by a pipeline of gas chromatography (GC) techniques coupled with mass-spectrometry (MS), flame ionization detector (FID) and two dimensional gas chromatography time of flight mass spectrometry (GC x GC-TOF MS). A total of 48 compounds with a good match and high probability values were identified using this technique. Meanwhile, 42 compounds were successfully identified in this study using GC-MS, a significantly larger number than in previous studies. GC-FID was used in determining the retention indices of chemical components in P. minus essential oil. The result also showed the efficiency and reliability were greatly improved when chemometric methods and retention indices were used in identification and quantification of chemical components in plant essential oil.

    OBJECTIVE: To compare the contributions of UVB exposure and diet to total vitamin D among Asians living in Kuala Lumpur (KL) and Aberdeen (AB).

    DESIGN: Longitudinal study.

    SETTING: UVB exposure (using polysulfone film badges) and skin colour and dietary vitamin D intake (by web-based questionnaire) were measured at each season in AB and during south-west (SWM) and north-east monsoons (NEM) in KL.

    SUBJECTS: One hundred and fifteen Asians in KL and eighty-five Asians in AB aged 20-50 years.

    RESULTS: Median summer UVB exposure of Asians in AB (0·25 SED/d) was higher than UVB exposure for the KL participants (SWM=0·20 SED/d, P=0·02; NEM= 0·14 SED/d, P<0·01). UVB exposure was the major source of vitamin D in KL year-round (60%) but only during summer in AB (59%). Median dietary vitamin D intake was higher in AB (3·50 µg/d (140 IU/d)), year-round, than in KL (SWM=2·05 µg/d (82 IU/d); NEM=1·83 µg/d (73 IU/d), P<0·01). Median total vitamin D (UVB plus diet) was higher in AB only during summer (8·45 µg/d (338 IU/d)) compared with KL (SWM=6·03 µg/d (241 IU/d), P=0·04; NEM=5·35 µg/d (214 IU/d), P<0·01), with a comparable intake across the full year (AB=5·75 µg/d (230 IU/d); KL=6·15 µg/d (246 IU/d), P=0·78).

    CONCLUSIONS: UVB exposure among Asians in their home country is low. For Asians residing at the northerly latitude of Scotland, acquiring vitamin D needs from UVB exposure alone (except in summer) may be challenging due to low ambient UVB in AB (available only from April to October).

    The advent of digital concrete fabrication calls for advancing our understanding of the interaction of 3D printing with material rheology and print parameters, in addition to developing new measurement and control techniques. Thixotropy is the main challenge associated with printable material, which offers high yield strength and low viscosity. The higher the thixotropy, the better the shape stability and the higher buildability. However, exceeding a minimum value of thixotropy can cause high extrusion pressure and poor interface bond strength if the printing parameters are not optimized to the part design. This paper aims to investigate the effects of both material and process parameters on the buildability and inter-layer adhesion properties of 3D printed cementitious materials, produced with different thixotropy and print head standoff distances. Nano particles are used to increase the thixotropy and, in this context, a lower standoff distance is found to be useful for improving the bond strength. The low viscosity "control" sample is unaffected by the variation in standoff distances, which is attributed to its flowability and low yield stress characteristics that lead to strong interfacial bonding. This is supported by our microscopic observations.

    The materials and substances required for sustainable water treatment by adsorption technique, are still being researched widely by distinguished classes of researchers. Thus, the need to synthesize substances that can effectively clean up pollutants from the environment cannot be overemphasized. So far, materials in bulk forms that are rich in carbon, such as biochar and varieties of activated carbon have been used for various adsorptive purposes. The use of bulk materials for such purposes are not efficient due to minimal surface areas available for adsorption. This study explores the adsorption task at nano dimension using carbon dots (CDs) from tapioca. The properties of carbon structure and its influence on the adsorptive efficacy of carbon nanoparticles were investigated by energy-dispersive spectroscopy (EDS), X-ray photoelectron spectroscopy (XPS), Fourier transform infrared spectroscopy (FTIR), high resolution transmission electron microscopy (HrTEM), and atomic force microscopy (AFM). The results implied carbon present in CDs are good adsorbents for effective adsorption of heavy metal ions (lead) with removal efficiency of 80.6% in aqueous environment. The adsorption process as explored by both Langmuir and Freundlich isotherms have proven favorability of the adsorption process. Langmuir form two and three have correlation coefficients R2 at 0.9922 and 0.9912, respectively. The Freundlich isotherm confirms CDs as having defined surface heterogeneity and the exponential distribution of active sites. The adsorption of lead unto CDs obeyed the second order kinetic model with coefficient of determination, R2 of 0.9668 and 0.9996 at an initial lead concentration of 20 mg/L and 100 mg/L, respectively. The findings validated the efficiency of CDs derived from tapioca as an excellent material for further utilization in the environmental fields of wastewater pollution detection and clean up, bio-imaging, and chemical sensing applications.
