    Researchers are exploring the utilisation of reclaimed asphalt pavement (RAP) as a recycled material to determine the performance of non-renewable natural aggregates and other road products such as asphalt binder, in the construction and rehabilitation stage of asphalt pavements. The addition of RAP in asphalt mixtures is a complex process and there is a need to understand the design of the asphalt mixture. Some of the problems associated with adding RAP to asphalt mixtures are moisture damage and cracking damage caused by poor adhesion between the aggregates and asphalt binder. There is a need to add rejuvenators to the recycled mixture containing RAP to enhance its performance, excepting the rutting resistance. This study sought to improve asphalt mixture performance and mechanism by adding waste frying oil (WFO) and crumb rubber (CR) to 25 and 40% of the RAP content. Moreover, the utilisation of CR and WFO improved pavement sustainability and rutting performance. In addition, this study prepared five asphalt mixture samples and compared their stiffness, moisture damage and rutting resistance with the virgin asphalt. The results showed enhanced stiffness and rutting resistance of the RAP but lower moisture resistance. The addition of WFO and CR restored the RAP properties and produced rutting resistance, moisture damage and stiffness, which were comparable to the virgin asphalt mixture. All waste and virgin materials produce homogeneous asphalt mixtures, which influence the asphalt mixture performance. The addition of a high amount of WFO and a small amount of CR enhanced pavement sustainability and rutting performance.

    The effect of various multi-walled carbon nanotubes (MWNTs) on the tensile properties of thermoplastic natural rubber (TPNR) nanocomposite was investigated. The nanocomposite was prepared using melt blending method. MWNTs were added to improve the mechanical properties of MWNTs/TPNR composites in different compositions of 1, 3, 5, and 7 wt.%. The results showed that the mechanical properties of nanocomposites were affected significantly by the composition and the properties of MWNTs. SEM micrographs confirmed the homogenous dispersion of MWNTs in the TPNR matrix and promoted strong interfacial adhesion between MWNTs and the matrix which was improved mechanical properties significantly.

    Adhesive small bowel obstruction (ASBO) is an annoying postoperative complication. Though the diagnosis can be made easily, the role of conservative management in children is controversial. Hence a study was conducted to determine the role of conservative management, and to identify the factors that can predict / influence the outcome of conservative treatment in children with ASBO. Children admitted with ASBO from 1980 to 2002 (22 year period) formed the material for this study. The data was analyzed with respect to the influence of age at the time of presentation, primary disease for which original laparotomy was done, time interval between the primary surgery and the development of ASBO and the number of laparotomies prior to the development of ASBO on the outcome of conservative management. There were 74 episodes of ASBO in 69 children (Five children had two episodes). Out of 74 episodes, 5 episodes (6.75%) needed immediate laparotomy for suspected gangrene. All others were managed conservatively. Of the 69 episodes managed conservatively, 36 responded to conservative treatment (2-5 days) while 33 required subsequent surgical intervention, with 11 of them requiring bowel resection (two for gangrene and 9 for bowel damage during adhesiolysis) and in the rest 22 cases adhesiolysis. A substantial number of children with ASBO respond well to conservative treatment. Majority of the children developed ASBO within three months after the primary laparotomy. Children below the age of one year (at the time of presentation with ASBO) responded poorly to the conservative management. Children who had primary surgery for Hirschsprung's disease and intussusception also appeared to have responded poorly to conservative management, but statistically not significant. Time interval between the primary surgery and the number of laparotomies before the child developed ASBO did not influence the outcome of conservative management.

    Abdominal sacrocolpopexy is a well-established procedure for the reconstruction of apical support in pelvic organ prolapse. Its long-term efficacy is well known; however, it is also associated with higher perioperative morbidity when compared with the less invasive transvaginal approach. Long-term risk of bowel-related complication from abdominal sacrocolpopexy is rare, but can be significant as it is often serious and requires major surgical intervention. Here we highlight an unusual case of strangulated small bowel (in this instance complicated with sepsis secondary to peritonitis), 14 years after an abdominal sacrocolpopexy procedure. This example amplifies the need for proper preoperative counseling; also, life-long follow-up is necessary for patients undergoing this procedure.

    Haemothorax is one of the less common presentations that requires hospital admission. Most cases are associated with underlying causes, but there is such an entity called spontaneous haemothorax. Spontaneous haemothorax is commonly associated with adhesions within the pleural cavity. Here, we reported two cases of young adults who presented with massive spontaneous haemothorax and required emergency thoracotomy for haemostasis purpose.

    The effectiveness of superheated steam (SHS) as an alternative, eco-friendly treatment method to modify the surface of pineapple leaf fiber (PALF) for biocomposite applications was investigated. The aim of this treatment was to improve the interfacial adhesion between the fiber and the polymer. The treatment was carried out in an SHS oven for different temperatures (190⁻230 °C) and times (30⁻120 min). Biocomposites fabricated from SHS-treated PALFs and polylactic acid (PLA) at a weight ratio of 30:70 were prepared via melt-blending techniques. The mechanical properties, dimensional stability, scanning electron microscopy (SEM), and X-ray diffraction (XRD) for the biocomposites were evaluated. Results showed that treatment at temperature of 220 °C for 60 min gave the optimum tensile properties compared to other treatment temperatures. The tensile, flexural, and impact properties as well as the dimensional stability of the biocomposites were enhanced by the presence of SHS-treated PALF. The SEM analysis showed improvement in the interfacial adhesion between PLA and SHS-treated PALF. XRD analysis showed an increase in the crystallinity with the addition of SHS-PALF. The results suggest that SHS can be used as an environmentally friendly treatment method for the modification of PALF in biocomposite production.

    This article presents a Wall Climbing Robot (WCR) that able to move on ferromagnetic vertical surface to carry out visual inspection process. Visual inspection process is important in the industry to check the condition of storage tank, surface of building, piping or equipment thus can prevents structures collapsing or explosion which would bring a huge loss to the company. Moreover, most of the structures nowadays is expose under the sun and rain, corrosion and cracks could easily occur on the surface after exposing under sunlight and rain a long period of time. Therefore the periodic visual inspection process need to be carry out to detect the damaged occur on the surface of the structure and take action at the fastest time to ensure the safety of the structures and extend the lifespan of the structures. With the well maintenance to the structures, the condition of the structures is monitored and the lifespan is longer. The risk of collapse of the building is decrease by a large margin. Normally, the periodic visual inspection process is performed by operator. Sometime the temporary scaffolding is needed to reach the higher place to carry out the inspection. However, this method create a hazardous environment to the operator and cause the safety of the operator threatened. Therefore, the proposed WCR could help operator to work at the hazardous environment. The permanent magnet is used to provide adhesion for WCR, thus WCR able to move on vertical ferromagnetic surface. The WCR is controlled by operator via wireless remote to reach the higher place or the hazardous environment. The operator then can stream the on the real time images via web browser which connected to the same network with the WCR. Hence, the condition of the surface can be observed.

    Mechanical strength, thermal conductivity and electrical breakdown of polypropylene/lignin/kenaf core fiber (PP/L/KCF) composite were studied. PP/L, PP/KCF and PP/L/KCF composites with different fiber and lignin loading was prepared using a compounding process. Pure PP was served as control. The results revealed that tensile and flexural properties of the PP/L/KCF was retained after addition of lignin and kenaf core fibers. Thermal stability of the PP composites improved compared to pure PP polymer. As for thermal conductivity, no significant difference was observed between PP composites and pure PP. However, PP/L/KCF composite has higher thermal diffusivity. All the PP composites produced are good insulating materials that are suitable for building. All PP composites passed withstand voltage test in air and oil state as stipulated in IEC 60641-3 except PP/L in oil state. SEM micrograph showed that better interaction and adhesion between polymer matrix, lignin and kenaf core fibers was observed and reflected on the better tensile strength recorded in PP/L/KCF composite. This study has successfully filled the gap of knowledge on using lignin and kenaf fibers as PP insulator composite materials. Therefore, it can be concluded that PP/Lignin/KCF has high potential as an insulating material.

    A study on the effects of alkali treatment and compatibilising agent on the tensile properties of pineappleleaf fibre (PALF) reinforced high impact polystyrene (HIPS) composite is presented in this paper. Thetensile properties of natural fibre reinforced polymer composites are mainly influenced by the interfacialadhesion between the matrix and the fibres. In this study, several chemical modifications were employedto improve the interfacial matrix-fibre bonding and this resulted in the enhancement of tensile propertiesof the composites. In this study, the surface modification of pineapple fibre with alkali treatments andcompatibilizer were used to improve the adhesion between hydrophilic pineapple fibre and hydrophobicpolymer matrix. There are two concentrations of NaOH treatments and compatibilizer used in this study,namely, 2 and 4 wt. %. The results show that the alkali treated fibre and the addition of compatibilisingagent in PALF/HIPS composites have improved the tensile strength and tensile modulus of the composites.

    OBJECTIVE: To show that raised iron levels in the peritoneal fluid (PF) of patients with endometriosis catalyze free radical reactions that results in the tissue destruction and fibrosis seen in these patients.

    DESIGN: A case-controlled study of the iron levels (microgram/mL) in the pelvic PF of 12 patients with moderate-to-severe disease, 15 patients with minimal-to-mild disease and in 17 women with normal pelvises were compared. As an index of free radical reactions through lipid peroxidation, the levels of malondialdehyde levels (ng/mL) were assessed simultaneously in the same specimens.

    RESULTS: Controlling for the phase of the menstrual cycle, significantly higher levels of iron were seen in patients with endometriosis, the levels being correlated with the severity of the disease. However no such corresponding relationship was seen in the malondialdehyde levels in the PF.

    CONCLUSIONS: These results suggest that raised iron levels in the PF do not play a role in catalyzing free radical reactions as judged by the degree of lipid peroxidation.

    OBJECTIVE: Urinary tract endometriosis involves the bladder and/or the ureters and is present in approximately 1% of women with endometriosis [1]. Bladder endometriosis is the most frequent type of urinary tract endometriosis, occurring in about 70% to 85% of cases [2,3]. Bladder endometriosis is defined as the presence of endometrial glands and stroma in the detrusor muscle. Surgically, there are 2 ways of excising this disease. The first is by transurethral bladder resection of the tumor, and the second is laparoscopic/robotic/open partial cystectomy of the bladder endometriosis. Because the nodule develops from the outer layer of the bladder wall toward the inner layer, complete excision of the endometriotic lesion is virtually unachievable with transurethral resection surgery. There is also a high risk of bladder perforation [4-8]. Partial cystectomy of the bladder runs a risk of excising normal bladder tissues because it is difficult to ascertain the margins of the bladder nodule. However, we found the best method to deal with bladder endometriosis is a combined approach whereby the margins of the bladder nodule are cut via a cystoscopy and then excision of the bladder nodule is done laparoscopically. This particular technique is presented here with an accompanying video.

    DESIGN: Excision of bladder endometriosis by first delineating the tumor via cystoscopy and simultaneously excising the nodule laparoscopically SETTING: Mahkota Medical Centre, Melaka, Malaysia.

    INTERVENTION: Here we describe a simultaneous cystoscopic and laparoscopic excision of bladder endometriosis. The patient was first seen in 2005 at age 19 years with an endometrioma. She was single (virgo intacta) at that time. She underwent a laparoscopic cystectomy. Postoperatively, she received 3 doses of monthly gonadotropin-releasing hormone (GnRH) analogue injection. She was last seen in 2006 and was well. She conceived spontaneously after that and delivered 2 babies spontaneously in 2007 and 2010 in another city. She consulted me again in April 2016 complaining of dysuria, dysmenorrhea, and inability to hold her urine. She had consulted a urologist 6 months earlier. Cystoscopy performed by the urologist showed bladder endometriosis. No further surgery was performed, and she was given GnRH analogues for 6 months. However, her symptoms persisted after completion of the GnRH analogue. Examination and ultrasound showed a large bladder nodule measuring 4.17 × 2.80 cm. Intravenous urogram showed stricture in the upper right ureter. She underwent a combined urology and gynecology surgery to excise the bladder nodule. Informed consent was obtained from the patient, and the local institutional board provided the approval. The surgery was performed with the patient in the dorsosacral position. A Verres needle was inserted into the abdomen at the umbilicus, and carbon dioxide insufflation was performed. A 10-mm trocar was inserted in the umbilicus, and a 3-dimensional laparoscope (Aesculup-BBraun Einstein Vision; BBraun, Melsungen AG, Germany) was inserted to view the pelvis. Three 5-mm trocars were inserted, 1 on the right side and 2 on the left side of the abdomen. A RUMI (CooperSurgical, Trumbull, CT) uterine manipulator was placed into the uterine cavity. Laparoscopy showed no adhesions in the upper and mid-abdomen. The appendix and the intestines looked normal. Both the ovaries and fallopian tubes were normal. Uterine insufflation with methylene blue showed that both tubes were patent. There was dense endometriosis between the bladder and fundus of the uterus. The omentum was also adherent to the site of the endometriosis. There were endometriotic nodules on the left uterosacral ligaments and the peritoneum in the wall in the pouch of Douglas. The omentum was released, and laparoscopic adhesiolysis was performed. Both the paravesical spaces lateral to the nodule were dissected out. The bladder was released from the uterus with some difficulty. The peritoneal endometriosis in the Pouch of Douglas and the nodules in the left uterosacral ligament were excised. Cystoscopy was performed and stents were first placed in both ureters. The nodule was found to be in the central position, and the margins were about 2 cm from both the ureteral orifices. The nodule was seen protruding into the bladder containing bluish lesions. Demarcation of the bladder endometriosis was done using a resectoscope. Using a needle electrode, a deep circular incision was made around the bladder nodule and into the detrusor muscle. Cystoscopic perforation of the bladder was done and was seen laparoscopically. The bladder endometriotic nodule was completely excised laparoscopically after the demarcation line created via the cystoscopy. Stay sutures were first placed at the superior and inferior edges of the defect. The bladder was repaired continuously in 1 layer using polyglactin 3-0 sutures. The nodule was placed in a bag cut into smaller pieces and removed through the umbilical incision. At the end of the surgery a cystoscopy was perform to check the integrity of the suture. The pelvis was then washed. A bladder catheter was placed. The trocars were then removed under vision, and the rectus sheath was closed using polyglactin 1 suture. The skin incisions were closed. The operation time was 2 hours. The patient received antibiotics for 10 days. She was discharged with a catheter in place on day 3. She underwent a cystogram on day 10 of the surgery, and the bladder was found to be intact. The catheter was then removed. She was seen 6 weeks after the surgery and was well without any symptoms. The ureteric catheters were removed. Histopathology confirmed bladder endometriosis. Five months later she conceived spontaneously and delivered her third child naturally in June 2017. She was seen after her delivery and was advised to take oral contraceptive pills continuously or an intrauterine contraceptive device to prevent recurrence of the endometriosis. She took the oral contraceptive pills for 3 months and then refused any further treatment. She was last seen in February 2019 and was well without any symptoms.

    CONCLUSION: In bladder endometriosis a combined approach with the urologist can assist in safely excising deep bladder endometriosis without removal of normal bladder tissue. Stents placed in the ureter assist in avoiding injury to the ureters. Demarcating the endometriotic nodule by the urologist through the bladder and excising the bladder nodule laparoscopically is both safe and effective.

    INTRODUCTION: The aim of this study was to investigate the efficacy of resveratrol and octreotide, agents that are used to prevent intra-abdominal adhesions in experimental models, in preventing intraperitoneal adhesions when used alone or in combination.

    MATERIALS AND METHODS: The study employed 28 young female Wistar albino rats weighing 250-300 grams. An experimental adhesion model was created in each rat using serosal abrasion and peritoneal excision. They were divided into four groups, each comprising seven rats: Group 1, adhesion induction only; Group 2, resveratrol administration only; Group 3, octreotide administration only; and Group 4, administration of resveratrol and octreotide combination. The rats were monitored under appropriate conditions for 14 days and then underwent laparotomy. Macroscopic intensity and extensiveness of adhesions and microscopic changes in the granulation tissue (cellular intensity, reticular and collagen fibers, capillaries, elastic and smooth muscle fibers, fibrosis) were evaluated and graded. Kruskal-Wallis and Mann-Whitney U-test were used in statistical analysis and the level of statistical significance was established as p <0.05.

    RESULTS: There was no significant difference between the groups in terms of the intensity and extensiveness of macroscopic adhesions (p=0.377 and p=0.319). There was a statistically significant difference between the microscopic scores of the groups according to Zühlke's classification (p=0.026). The Bonferroni correction used to test for the differences revealed that the rats in Group 1 achieved significantly higher scores than the rats in Group 3 (p=0.016).

    CONCLUSION: Octreotide showed higher efficiency compared to the control group in microscopic classification; however, the two agents were not superior to each other or their combination was not superior in preventing intra-abdominal adhesions.

    Foamed concrete (FC) is a high-quality building material with densities from 300 to 1850 kg/m3, which can have potential use in civil engineering, both as insulation from heat and sound, and for load-bearing structures. However, due to the nature of the cement material and its high porosity, FC is very weak in withstanding tensile loads; therefore, it often cracks in a plastic state, during shrinkage while drying, and also in a solid state. This paper is the first comprehensive review of the use of man-made and natural fibres to produce fibre-reinforced foamed concrete (FRFC). For this purpose, various foaming agents, fibres and other components that can serve as a basis for FRFC are reviewed and discussed in detail. Several factors have been found to affect the mechanical properties of FRFC, namely: fresh and hardened densities, particle size distribution, percentage of pozzolanic material used and volume of chemical foam agent. It was found that the rheological properties of the FRFC mix are influenced by the properties of both fibres and foam; therefore, it is necessary to apply an additional dosage of a foam agent to enhance the adhesion and cohesion between the foam agent and the cementitious filler in comparison with materials without fibres. Various types of fibres allow the reduction of by autogenous shrinkage a factor of 1.2-1.8 and drying shrinkage by a factor of 1.3-1.8. Incorporation of fibres leads to only a slight increase in the compressive strength of foamed concrete; however, it can significantly improve the flexural strength (up to 4 times), tensile strength (up to 3 times) and impact strength (up to 6 times). At the same time, the addition of fibres leads to practically no change in the heat and sound insulation characteristics of foamed concrete and this is basically depended on the type of fibres used such as Nylon and aramid fibres. Thus, FRFC having the presented set of properties has applications in various areas of construction, both in the construction of load-bearing and enclosing structures.

    Adhesives serve many functions in daily life, starting from sticking envelopes to rejoining broken
    materials. Adhesives are usually developed for a specific purpose and the performances can vary
    according to their specific end-use. Most of the commercially available adhesives comprised nonrenewable or petroleum derived raw materials. Thus, in order to mitigate negative impact of using nonrenewable material as the raw material for adhesives, a new type of adhesive containing epoxidised
    natural rubber (NR) latex is developed. Epoxidised NR latex adhesive was initially prepared and pigment
    was subsequently added to produce desired colours of the adhesive. Hence, the newly developed adhesive
    can serve as adhesive and also as paint for art. The adhesives were characterised and the results indicated
    that they were free from heavy metal contents and volatile organic compounds (VOCs). The adhesives
    exhibited comparable odour concentration to commercial synthetic-based adhesive. In terms of toxicity
    level, the adhesive developed exhibited low acute oral toxicity. Peel adhesion test of A4 and drawing
    papers on stainless steel and glass substrates, opacity and glossiness were also investigated in the present
    study. This coloured adhesive is deemed to support the STEM (science, technology, engineering and
    mathematics) learning by indirectly imparting polymer science and technology in art education and
    further promotes creative learning among school children. The adhesive is also derived from renewable
    material rendering it more environmentally friendly.

    Iatrogenic diaphragmatic entry following thoracic surgery is rare with only two cases reported till date. This case reports on a 46-year-old male with persistent pleural sepsis due to right empyema thoracis despite best medical ef- forts. Following a right thoracotomy and decortication, the right diaphragm was inadvertently incised due to dense adhesions between the diaphragm and thickened cortex. To our knowledge, this is the third case reported of iatro- genic diaphragmatic entry following thoracic surgery for empyema thoracis. The pearls from this case report is that any evidence of empyema thoracis involving the lower lobe on imaging should warn the surgeons to be aware of inadvertent entry into the peritoneal cavity, as the diaphragm can be adherent to the cortex trapping the lower lobe.

    The main focus of this study was to obtain the optimum alkaline treatment for banana fibre and the its effect on the mechanical and chemical properties of banana fibre, its surface topography, its heat resistivity, as well as its interfacial bonding with epoxy matrix. Banana fibre was treated with sodium hydroxide (NaOH) under various treatment conditions. The treated fibres were characterised using FTIR spectroscopy. The morphology of a single fibre observed under a Digital Image Analyser indicated slight reduction in fibre diameter with increasing NaOH concentration. The Scanning Electron Microscope (SEM) results showed the deteriorating effect of alkali, which can be seen from the removal of impurities and increment in surface roughness. The mechanical analysis indicates that 6% NaOH treatment with a two-hour immersion time gave the highest tensile strength. The adhesion between single fibre and epoxy resin was analysed through the micro-droplet test. It was found that 6% NaOH treatment with a two-hour immersion yielded the highest interfacial shear stress of 3.96 MPa. The TGA analysis implies that alkaline treatment improved the thermal and heat resistivity of the fibre.

    Epoxidized natural rubber (ENR-50) has been used as a compatibilizer for natural rubber-recycled acrylonitrile-butadiene
    rubber (NR/NBRr) blends, vulcanized by sulfur. NBR gloves have excellent resistance to punctures, tear and many types
    of chemicals, while NR has good physical and mechanical properties. Incorporation of ENR-50 into the rubber blends
    has improved processability, stiffness, resilience and excellent oil resistance. NR/NBRr blends were prepared by two-roll
    mill with five different compositions with the ENR-50 content fixed at 10 phr. Cure characteristics, mechanical properties
    and morphology (SEM) studies were performed to determine the compatibility of NR/NBRr blends in the presence of ENR-
    50. The cure characteristics showed that NR/NBRr blends with the presence of ENR-50 have lower scorch time (ts2) and
    cure time (t90) than NR/NBRr blends without ENR-50. However, the NR/NBRr blends with ENR-50 exhibited higher minimum
    torque (ML
    ) and maximum torque (MH) which indicated difficult processability of the blends after compatibilization.
    Incorporation of ENR-50 into NR/NBRr blends improved all the tensile properties (tensile strength, tensile modulus and
    elongation at break) compared with NR/NBRr blends without ENR-50. The improvement in hardness upon compatibilization
    is due to an increase in crosslink density. Scanning electron microscopy (SEM images) of the fracture surfaces indicates
    that, with the addition of ENR-50 in NR/NBRr blends, better adhesion between NR and NBRr was obtained, thus improving
    the compatibility of NR/NBRr blends.
