OBJECTIVE: Continued use of secondary prevention medications after ischemic stroke or transient ischemic attack is an important component of long-term management. Reported estimates of adherence and persistence vary considerably across studies, limiting comparison and interpretability. Although some variation may reflect differences in patient behavior, inconsistencies in operational definitions and measurement approaches are also likely to contribute. We sought to examine sources of heterogeneity and to derive standardized estimates using a common reference measurement framework.

STUDY DESIGN AND SETTING: We searched bibliographic databases, dissertations, and trial registries from inception to September 10, 2025, for routine-care studies reporting adherence or persistence to secondary prevention medications after ischemic stroke or transient ischemic attack. Cutoff-based proportions were extracted. We used 3-level hierarchical Bayesian models to characterize heterogeneity and multivariable meta-regression to evaluate associations with measurement specifications, follow-up window, medication class, region, and study-level characteristics. From the fitted model, we derived estimates standardized to a prespecified reference specification: pharmacy or administrative data, moderate definition stringency, and a 9- to 15-month follow-up window.

RESULTS: The review included 141 studies; 101 (432 estimates; 976,020 participants) contributed to the quantitative synthesis. Unstandardized pooled estimates suggested that 74.1% of patients were adherent and 76.8% were persistent. Heterogeneity was substantial, with explained variation concentrated more in the measurement-related Assessment and Administration domains than in reported Patient characteristics. Under the common reference specification, the model-based estimates were lower, at 53.7% for adherence and 48.2% for persistence. Standardized persistence declined from 63.3% at 1 to 4 months to 28.1% beyond 30 months. Adjusted estimates were generally lower in Asia; persistence was numerically lowest for anticoagulants, although credible intervals were wide and overlapping.

CONCLUSION: A large proportion of the heterogeneity reported in medication adherence and persistence after ischemic stroke or transient ischemic attack appears to be attributable to methodological non-equivalence in how these outcomes are defined and measured. When evaluated within a standardized framework, suboptimal medication continuation appears more common than conventional pooled estimates indicate. Meaningful surveillance and benchmarking require longitudinal, standardized, and transparent measures of medication continuation rather than reliance on discharge prescribing alone.

Living in high expressed emotion (EE) environments tends to increase the relapse rate in schizophrenia (SZ). At present, the neural substrates responsible for high EE in SZ remain poorly understood. Functional near-infrared spectroscopy (fNIRS) may be of great use to quantitatively assess cortical hemodynamics and elucidate the pathophysiology of psychiatric disorders. In this study, we designed novel low- (positivity and warmth) and high-EE (criticism, negative emotion, and hostility) stimulations, in the form of audio, to investigate cortical hemodynamics. We used fNIRS to measure hemodynamic signals while participants listened to the recorded audio. Healthy controls (HCs, [Formula: see text]) showed increased hemodynamic activation in the major language centers across EE stimulations, with stronger activation in Wernicke's area during the processing of negative emotional language. Compared to HCs, people with SZ ([Formula: see text]) exhibited smaller hemodynamic activation in the major language centers across EE stimulations. In addition, people with SZ showed weaker or insignificant hemodynamic deactivation in the medial prefrontal cortex. Notably, hemodynamic activation in SZ was found to be negatively correlated with the negative syndrome scale score at high EE. Our findings suggest that the neural mechanisms in SZ are altered and disrupted, especially during negative emotional language processing. This supports the feasibility of using the designed EE stimulations to assess people who are vulnerable to high-EE environments, such as SZ. Furthermore, our findings provide preliminary evidence for future research on functional neuroimaging biomarkers for people with psychiatric disorders.

The first observation of the tt[over ¯]H process in a single Higgs boson decay channel with the full reconstruction of the final state (H→γγ) is presented, with a significance of 6.6 standard deviations (σ). The CP structure of Higgs boson couplings to fermions is measured, resulting in an exclusion of the pure CP-odd structure of the top Yukawa coupling at 3.2σ. The measurements are based on a sample of proton-proton collisions at a center-of-mass energy sqrt[s]=13  TeV collected by the CMS detector at the LHC, corresponding to an integrated luminosity of 137  fb^{-1}. The cross section times branching fraction of the tt[over ¯]H process is measured to be σ_{tt[over ¯]H}B_{γγ}=1.56_{-0.32}^{+0.34}  fb, which is compatible with the standard model prediction of 1.13_{-0.11}^{+0.08}  fb. The fractional contribution of the CP-odd component is measured to be f_{CP}^{Htt}=0.00±0.33.
