    Background: Clinical clerkship in a busy hospital environment forms an important part of undergraduate medical training. Regular objective assessment of this activity with feedback would be expected to improve outcome.

    Methods: We implemented fortnightly clinical assessments using modified OSLER (Objective Structured Long Examination Record), and over a 6-week clinical rotation. Modifications included provision of individualized feedback. The assessment process was evaluated by both students and teachers via a questionnaire measuring their perceived educational impact, feasibility and acceptability.

    Results: Students agreed that the patient spectrum was appropriate and fair, resulting in improved history taking and presentation skills (96.6%), clinical examination skills (89%) and clinical reasoning skills (90.7%). It was graded to have helped learning “tremendously” and “moderately” by 64.7% and 32.8% of students respectively. Perceived improvement was attributable mainly to the repetitive nature of the assessments since only 63% of students were provided with feedback. 96.6% of students and 94.1% of assessors perceived the format created a stressful but positive learning environment. 52.9% of assessors agreed that the exercise consumed significant time and resources but 88.2% rated it as manageable and supported its continuation.

    Conclusion: Frequent and regular in-course clinical assessments with emphasis on individual feedback is feasible, acceptable and has significant positive educational impact.

    Introduction: Standard precautions in health care is the essence of medical practice encompassing the safety of patients and health care workers including medical students. Barriers to the proper use of personal protective equipment (PPE) exist across the world but identification of areas of weaknesses and appropriate remedies will reduce them. This study assesses knowledge and use of PPE among fourth year students after a period of educational interventions.
    Objective: To evaluate appropriate use, awareness and knowledge about PPE among fourth year students after interventions.
    Method: A cross- sectional study where forty year 4 students (Group B) were randomly observed and later asked to answer a questionnaire. Students had undergone interventions to improve PPE use, which included lectures and video sessions during each posting. Results were compared with a previous group (Group A). Chi-square test or Fisher’s exact test was used to analyse the data.
    Results: There was statistically significant improvement in the use of PPE like wearing and removing mask during invasive procedures (p < 0.001) and hand-washing before and after a non- invasive task (p < 0.001) . Comparison of Groups A and B on the results of the questionnaire for ‘Questions in which more than 10% students answered incorrectly’, showed that there was improvement in Group B in all the questions, some being statistically significant with p value=0.01.
    Conclusion: An overall improvement in the use of the PPE and knowledge was noted. Sometimes, students’ attitude and personality may be a challenge and these students may defy changes, but this can be overcome if the strategies are embedded in the curriculum and taught from as early as the first semester.

    Background: House-officers and medical officers are at
    the forefront during medical emergencies in the ward
    and casualty which impose cognitive, communication,
    social and system challenges and yet, training in this
    area is commonly lacking. A workshop was conducted
    using simulation to provide training on some acute
    medical emergencies like cord prolapse, post- partum
    haemorrhage with collapse, poly-trauma and acute
    exacerbation of asthma.

    Objective: To determine the effectiveness of simulation
    in developing competency in managing selected clinical
    emergencies.

    Methodology: There were 22 participants consisting
    of house-officers, junior medical officers and nursing
    clinical instructors. Only doctors were included in
    the study. Four medical emergencies were chosen viz.:
    Cord prolapse; post- partum haemorrhage with collapse;
    poly-trauma and acute exacerbation of asthma. The
    simulated sessions were conducted using high fidelity
    manikins and simulated patients. Simulated patients
    were trained and moulage was applied accordingly. The
    skills stations were on airway equipment and techniques
    of application, latest cardiac life support algorithm and
    hands on chest compression using manikins.

    Results: A 5 point Likert scale used to rate the
    sessions. The skills station had 65% (n=13) rating as
    excellent and 35% (n=7) good. The skills simulation
    was rated excellent by 75% (n=15) and good by 25%
    (n=5) of participants. Verbal feedback was that it was
    very refreshing, informative, and helpful in terms of
    improving their skills.

    Conclusion: The simulated skills training for the junior
    doctors was very well received and maybe beneficial for
    work preparedness and in the long run address patient
    safety.

    Background: Sensitisation to house dust mite (HDM) has been regarded as a major risk factor for development of asthma. This study was carried out to investigate the profiles of HDM sensitisation among Malaysian children with asthma.
    Material and Methods: The association between HDM sensitisation and control and severity of asthma was investigated. The salivary HDM specific IgE levels were quantified in different grades of control and severity of asthma in 125 unselected asthmatic children aged 5-12 years old attending the asthma follow-up clinic in Hospital Tuanku Ja’afar Seremban. An additional 29 non-asthmatic patients were selected as control. The skin prick test to assess sensitisation to Dermatophagoides pteronyssinus (DP) and Dermatophagoides farinae (DF) was performed on all the participants. A questionnaire regarding the control and severity of asthmatic symptoms of the subject was administered. Saliva was collected by voluntary spitting and ELISA was used to quantify the IgE specific to HDM antigen.
    Results: There was a significant association between sensitisation to DP and DF and the control of asthma. The association between DP sensitisation and severity of asthma just failed to reach a significant level although there is a clear trend for this. Significant association was found between DF sensitisation and severity. The HDM specific IgE in the saliva was significantly higher in asthmatic patients compared to non-asthmatic patients. There was no significant difference between the specific IgE levels in patients with different severity status of asthma.
    Conclusion: Salivary IgE levels may not be an appropriate indicator of the patients’ asthmatic condition in this study. However, it can be concluded that there is significant association between the sensitisation of HDM and the control and severity of asthma.

    In the developing world, clinical knowledge management in primary care has a long way to go. Clinical decision support systems, despite its promise to revolutionise healthcare, is slow in its implementation due to the lack of financial investment in information technology. Point-of-care resources, such as comprehensive electronic textbooks delivered via the web or mobile devices, have yet to be fully utilised by the healthcare organisation or individual clinicians. Increasing amount of applicable knowledge of good quality (e.g. clinical practice guidelines and other pre-appraised resources) are now available via the internet. The policy makers and clinicians need to be more informed about the potential benefits and
    limitations of these new tools and resources and make the necessary budgetary provision and learn how best to harness them for patient care.

    This is a questionnaire survey of dietary supplement usage among students in the International Medical University. Just over two-fifths of these students reported using dietary supplements daily. This high usage of dietary supplements is in contrast their expressed ambivalence about these products.

    Background: Many nurses are leaving for various
    destinations and leaving intention was found to be the
    immediate precursor for actual turnover, but studies
    examining the different forms of leaving intention (unit,
    hospital, country, and profession) and exit destinations
    are scarce.

    Objectives: To determine the different forms of nurses’
    leaving intention (i.e., leaving the unit, hospital, country,
    and profession), exit destinations and associations of
    demographic variables on the different forms of leaving
    intention and exit destinations.

    Methods: The study design was a cross-sectional survey.
    Private hospital nurses in the Peninsular Malaysia were
    the study population and a total of 942 (73% response
    rate) nurses participated in the study. A self-reported
    questionnaire was used for data collection.

    Results: The results revealed that intention of leaving
    the organisation (M = 2.81, SD = 1.33) was the
    highest and followed by intention of leaving the unit
    (M = 2.54, SD = 1.31). In terms of exit destinations,
    advancing nursing qualification (M = 2.95, SD = 1.31)
    and practising nursing in another country (M = 2.55,
    SD = 1.31) were the most preferred exit destinations
    among the nurses.

    Conclusions: Nursing managers play a significant role in
    retaining nurses within the units and organisations. The
    findings on nurses’ exit destinations are crucial because
    they serve as the direction for nurses’ retention strategies
    which include professional development opportunities
    through training, education and staff mobility.

    Background: The escalating rate of private hospital
    nurses leaving their workplace raised serious concern
    among the stakeholders. Past studies had found that
    nursing practice environment was the key influence
    on nurses’ leaving intention, but studies examining
    the quality of nursing practice environment of private
    hospital settings was scarce and therefore warrant
    investigation to provide direction for interventions in
    addressing nursing turnover.

    Objectives: To determine nurses’ perceptions towards
    nursing practice environment and whether there is
    any significant associations with nurses’ demographic
    variables.

    Methods: Cross-sectional inferential survey study was
    conducted at four private hospitals in the Peninsular
    Malaysia using the Practice Environment Scale of
    the Nursing Work Index (PES-NWI) and 885 nurses
    participated in the study.

    Results: Nurses rated their practice environment as
    favourable. However, items stated in “staffing and
    resource adequacy” subscale warrant serious attention
    because nurses rated poorly on item related to “enough
    staff to get the work done” (M = 2.37, SD = .81) and
    “enough registered nurses to provide quality patient
    care” (M = 2.41, SD = .82). Furthermore, t-test analyses
    found that nurses with educational sponsorship bond
    (p < .001), higher educational qualifications (p < .05),
    and have been working in the hospital since graduation
    (p < .001) were more likely to rate their practice
    environment lower.

    Conclusion: Hospital administrators play significant
    role in sustaining and creating positive nursing practice
    environment in order to ensure steady supply of nurses
    to meet the challenging healthcare needs.

    Background: Positive physician-nurse collaborative
    relationship is the key determinant for patients’
    outcomes.

    Objective: The purpose of the study was to investigate
    registered nurses’ attitude towards physician-nurse
    collaboration and its association with demographic
    characteristics.

    Methodology: The study was descriptive and crosssectional.
    The data of the study was collected using
    the “Jefferson Scale of Attitudes toward Physician-
    Nurse Collaboration”. The sample size of the study was
    127 registered nurses recruited using convenience
    sampling. Descriptive statistics and inferential statistics
    t-test were used for data analysis.

    Results: The results showed that nurses’ attitude towards
    physician-nurse collaboration was positive (M=3.25,
    SD±0.29). The attitude of the participants was highly
    positive towards “shares education and collaboration”
    (M=3.42, SD±0.36), “nurse’s autonomy” (M=3.42,
    SD±0.44), and “caring vs. curing” subscales (M=3.41,
    SD±0.41). However, the participants rated lowest for
    “physician’s authority” subscale (M=2.12, SD±0.83) and
    particularly on the item “doctors should be dominant
    authority in all health care matters” (M=2.11, SD±0.99).
    Furthermore, t-test analysis revealed no significant
    association between nurses’ attitude towards physiciannurse
    collaboration and demographic characteristics
    such as age, gender, and educational level (p > 0.05).

    Conclusion: The results of the study provided some
    crucial evidences on nurses’ attitude towards physiciannurse
    collaboration. The evidences are useful for the
    relevant stakeholders to initiate relevant strategies to
    improve and strengthen the relationship gap between
    physicians and nurses.

    Introduction: The goal of this study was to assess the effectiveness of seated combined extension-compression and transverse load (ECTL) traction as a new method for increasing a reduced lordosis of less than 30 degrees in a Malaysian population between the ages of 18 and 60 years. Possible changes in disc height were measured in accordance with the underlying theoretical framework, that suggests the anterior cervical structures would elongate due to creep over the fulcrum of the traction device.
    Method: This was a single centre, randomised, blinded controlled clinical trial with parallel groups, used to test the superiority of the seated combined ECTL traction together with physiotherapy exercises when compared with the same physiotherapy exercises used as a control. Fifty randomly allocated subjects who completed the forty treatments over the fourteen weeks were analysed using non-parametric tests for changes in outcomes.
    Results: There were no significant changes in outcomes for disc height changes seen in this study. The findings of a greater overall increase in posterior disc height changes compared with anterior disc height changes were in contrast with the proposed underlying theoretical framework for this type of ECTL traction. The greater height changes occurring in the control group were also unexpected.
    Conclusion: The findings in this study of the contrasting changes in disc height of greater posterior than anterior height changes, question the underlying theoretical framework as postulated for this type of traction.

    We report a case of occult primary spontaneous
    pneumothorax in a 30 years-old woman. She
    developed symptoms and signs that were suggestive of
    pneumothorax. However, chest radiograph failed to
    reveal pneumothorax. Therefore, we proceeded with
    computed tomography (CT) thorax which revealed
    significantly moderate right pneumothorax. The
    diagnostic approach and the management of this case
    are discussed.

    Background: Hypertension is estimated to cause4.5% of the global disease burden. The prevalence of hypertension in Malaysia is 32.2%.
    Objective: To determine the prevalence of hypertension and its associated risk factors in two rural communities in Penang, Malaysia.
    Methods: This cross sectional study was conducted among all consenting residents aged 18 years and above from two villages in Penang. Besides the baseline demographic information, blood pressure was measured using a manual sphygmomanometer according to the American Heart Association Guidelines.
    Results: 50 out of 168 people were hypertensive, giving a prevalence rate of 29.8%. 50.0% of those found with hypertension were undiagnosed and 48.0% of those who were diagnosed with hypertension had uncontrolled blood pressure. Logistic regression analysis showed that age, history of alcohol consumption and BMI were found to be independently associated with hypertension.
    Conclusions: Age, education level, alcohol consumption and BMI are important risk factors associated with the prevalence of hypertension among the villagers. These risk factors are comparable to those reported in National Health and Morbidity Survery 2006 in Malaysia.

    Introduction: Task-based learning (TBL) has been accepted as an effective tool in teaching and learning activities in most medical schools. Many studies have looked at competencies and learning outcomes essential for undergraduates. Among the essential competencies are interpersonal skills and the ability to engage in a group discussion which this study has focused on. The evidence supporting higher interpersonal skills is however limited because many relevant competencies are hard to measure and require long observational periods.
    Objective: To determine students’ self-perceived value of TBL in enhancing their interpersonal skills during the clinical phase.
    Material and Methods: All students’ (semesters 6-10) in the clinical school of International Medical University (IMU) were invited to participate in this cross-sectional study done in December 2007 utilising a self-administered questionnaire with a 5-point Likert scale. It assessed the students’ perception on TBL sessions conducted during their clinical attachments in the various disciplines. Mean-scores, standard deviations, and confidence interval were used.
    Results: Response rate was 62%. The results indicated that students were favorable in their opinion on TBL as a suitable forum for active communication and participation in group discussion. The results also show that both male and female students’ have similar perception. As for the comparison according to semesters, this showed that students’ maturity does not influence their perception as well.
    Conclusion: In conclusion, the study has shown positive students’ perception on the effect of TBL on acquired skills such as interpersonal communication. Our findings are consistent with many earlier studies which show students’ perception of the method of learning as important factor in the enhancement of their interpersonal skills which is fundamental to clinical practice. Further research is necessary; long-term and larger scale observational studies would undoubtedly be optimal to minimise response bias.

    Papillary thyroid microcarcinoma is not uncommon
    and constitutes almost one third of all differentiated
    thyroid carcinomas. It is generally regarded as low risk
    and usually an incidental finding from histopathology
    examination. Some areas of management of this entity
    remains uncertain and requires a multidisciplinary
    approach. We present a patient who initially came to
    us with symptoms of hyperthyroidism, later underwent
    thyroidectomy for a suspicious lesion but was found to
    have micropapillary thyroid carcinoma in another part
    of her thyroid gland.

    Primary sinonasal Non-Hodgkin’s Lymphoma’s
    (NHLs) are quite rare and emulate the presentation
    of benign inflammatory diseases. It is challenging to
    distinguish them morphologically and radiologically
    from other malignant neoplasms.
    We report a 37-year-old male patient who presented
    with obstruction of the nasal passages, rhinorrhoea,
    epistaxis, post nasal drip, facial swelling, orbital
    symptoms and pyrexia. The mass was a nasal diffuse large
    B-cell lymphoma confirmed by immunohistochemistry.
    After the first cycle of chemotherapy was started, the
    patient improved with resolution of the facial swelling,
    pain and visual defects.
    A high index of suspicion is required to differentiate
    sinonasal lymphomas from other lesions.

    The International Medical University, Kuala Lumpur (IMU) has just completed 20 years of success and had a series of events to celebrate its 20th anniversary as well as its achievements in 2012. As part of the 20th anniversary celebrations, IMU successfully co-hosted the Ottawa conference with the European Medical Education Association in Kuala Lumpur. This was the first time this conference was hosted in Asia and it was one of the biggest and most successful of the Ottawa Conference series ever held. This conference focuses on medical education with the major emphasis on assessment.

    Healthcare investment is critically important for the health and well-being of the population, and different health systems are developed to meet the needs and priorities of each country. What has become clear has been that despite major advances in medicine, science and technology, there are major issues related to access and equity as well as quality and patient safety in healthcare services. The issue of patient safety was highlighted by the reports of the Institute of Medicine, USA1,2 and this had received worldwide attention. It is also an irony that despite being in an age of major advances in medicine, science and technology, with the acceptance of evidence-based medicine, so much of medicine and healthcare delivered is of little or no proven value. This poses a major challenge on health policy, and on how this can be addressed in any health reform process that focuses on improving access, equity, efficiency and effectiveness in healthcare services.

    In ancient times, before Hippocrates, disease and illness were attributed as a sign of divine dismay, and the Gods needed to be appeased to achieve a cure. Hippocrates and his followers showed courage in breaking away from this paradigm. They moved away from the divine and supernatural, to focus on the biology of the body. In the process they put the patient at the centre of their focus. They collected detailed case histories, dismissed religious and supernatural explanations and developed remedies in the form of diets, exercise and mixed minerals and herbs based on their understanding of ill health.

    Background: At the International Medical University (IMU), a half day cardiac life support teaching session was provided to fourth year medical students which included training on the use of the defibrillator machine, how to handle cardiac or respiratory arrest and drugs used for resuscitation. A new CLS (cardiac life support) training session was introduced and increased to a oneday course where students were given practical training first, which included 5 stations (airway equipment, mega codes, drugs for resuscitation, defibrillator use and cardiac rhythm identification) , MCQ (multiple choice questions) test and a mega code (practical)assessment. Objective: To evaluate the students’ knowledge on cardiac resuscitation after a change in the delivery of the cardiac life support training (CLS).
    Methodology: Group I, consisted of 82 students taught using the traditional teaching and Group II consisted of 77 students taught using hands on simulation. The students in both groups had an online manual to read prior to the session, were given an identical written exam six months after the CLS training. Group II, however, had an online pre-test.
    Results: There was a statistical difference in the final mean marks between the two groups with group II scoring higher (67.3) than group 1 (62.1). No significant marks difference was noted between male and female students for both the cohorts.
    Conclusion: There is a significant difference in medical students’ knowledge when cardiac life support is taught using simulation. IMU has adopted the new teaching method with simulated training for the cardiac life support courses with plans to implement higher fidelity and technology to the existing simulated teaching in other areas of medicine.

    Background: This cross sectional study was done to identify the areas of lack of knowledge, practice and awareness of students about the effective use of personal protective equipment (PPE).
    Methods: A total of 40 students were selected when they were posted to the accident and emergency unit (A&E) in Seremban Hospital; all of them answered a questionnaire and were observed unaware on the effective use of PPE in the A&E.
    Results: We found that 17.5% of students were unaware of the right technique of removing the gloves after a procedure and 25% of students were unaware of safety of hand washing. During invasive procedures, 12.5 % of students did not wash their hands before invasive procedures, 65% did not wear aprons and 57.5% did not wear masks. During non- invasive procedures more than 25% of students did not wash hands before or after the procedures.
    Conclusion: There is still significant lack of knowledge in students about the effective use of PPE that needs to be addressed.
