    Background: An effective doctor-patient communication has increasingly being recognized as an important factor in patient care. All means should be oriented towards narrowing communication gap. The essential methods must be searched and doctor must be able to conduct communication session in more pleasing manner.
    Methodology: This is a systematic review on observations made on doctor-patient relationship on various setting and supported by feedbacks from many scholars who are involved in research, teaching and also papers and studies on the said subject.
    Results: It is been realized that effective communication is not easily done if its process not well complemented and the gap is left widening. From all possible communication gaps recognized and listed, at least ten have identified to be the most essential methods to be prioritized while counseling or consulting a patient.
    Conclusion: Effective communication between patient and doctor is the essential prerequisite of good medical practice and especially important for accurate diagnosis and effective treatment. Its mutual benefit can only be observed if all efforts are centered towards managing the communication gap.

    Stres di tempat kerja terjadi apabila keperluan kerja tidak sepadan dengan kebolehan, sumber, dan kehendak pekerja. Ia boleh memberi kesan ke atas psikologi dan fisiologi manusia. Seseorang individu yang berasa tertekan dengan kerja yang dilakukan akan menunjukkan kemurungan yang berpanjangan. Apabila rasa tertekan, psikologi akan terganggu dan pekerja gagal membuat keputusan dengan baik. Situasi ini boleh menjejaskan prestasi kerja mereka dan menggugat produktiviti organisasi. Masalah paling dibimbangi adalah stres di tempat kerja boleh menyebabkan penyakit berbahaya seperti tekanan darah tinggi, sakit jantung dan melemahkan sistem pertahanan badan terhadap penyakit. Stres di tempat kerja juga boleh menyebabkan kemalangan di tempat kerja, peningkatan kos keselamatan dan kesihatan pekerjaan, gangguan trauma kumulatif, menjejaskan prestasi kerja serta mengganggu kehidupan sosial individu. Masalah keselamatan dan kesihatan pekerjaan akibat stres di tempat kerja seharusnya tidak diabaikan oleh organisasi kerana ia boleh menyebabkan sumber manusia sesebuah negara kehilangan daya saingnya. Sumber manusia yang berasa terancam akibat masalah keselamatan dan kesihatan akan gagal menggunakan sepenuhnya kreativiti mereka dan melakukan tugas pada tahap minimum. Fenomena ini seharusnya dibimbangi dalam persekitaran perniagaan masa kini yang sangat kompetitif. Di Malaysia, kajian yang berkaitan dengan stres seharusnya dipertingkatkan kerana kajian lepas terhadap stres di tempat kerja adalah tidak menyeluruh. Perkembangan ini sejajar dengan perubahan yang berlaku di persekitaran kerja seperti teknologi, sosial, ekonomi, undang-undang buruh dan seumpamanya. Kajian stres di tempat kerja perlu dijalankan kerana sumbangannya amat bernilai dalam menghasilkan sumber manusia yang lebih produktif dan berdaya saing.

    Introduction : Selangor’s private clinic registry system had been introduced in 2006 following gazettment of The Private Healthcare Facilities and Services Act. Through the act, data’s from the private clinics can be obtained and its characteristics can be compared between the urban and rural private clinic. An overview of the services can be known and an appropriate action can be planned.
    Methodology : A cross-sectional study was done on private clinics registered in state of Selangor. Using databased known as MedPCs (Medical Practice Control System), a purposive sampling was used to select four districts – two urban and two rural. All private clinics in the selected districts were studied and all details shall be collected online.
    Result : District of Gombak and Klang were selected as urban and Sabak Bernam and Kuala Selangor represented rural area. Of a total 625 clinics selected, 90.0% (562) from urban and 10.0% (63) rural. Distributions of clinic were in line with the act. The most prominent services were general treatment (89.4%) and ability to serve more than eight hours daily (89.4%). Medical clinic still dominated at 84.0% compared to dentistry at 16.0%. About 70.6% were operated by male doctors. Though clinic services in rural were relatively as good as urban, the different were significantly observed to the ethnicity of the doctor. Indian doctors more in rural (46.03%), whilst in urban, Malay doctors were more dominant (39.50%). Followed closely by seniority, where rural doctors were much senior (51.02 years old), however, most of the doctors in rural private clinics experienced less exposure in government sectors, compared to those in urban areas.
    Conclusion : There were obvious difference in terms of distribution, where more clinics were located in urban compared to rural, but all were still in line to the act and their services were comparable. Most noticeable differences were ethnicity, seniority and past experience working in the public sector. Indian doctors were dominant in rural and Malay in urban. Although rural doctors were more senior, relatively they were less experienced working in the government sector.

    Background: Social misconduct amongst adolescents is a worrying and ever growing problem in Malaysia, despite efforts taken to overcome it.
    Methods : A cross sectional study was conducted among secondary school students in Melaka Tengah District to measure the prevalence and determine the factors associated with social misconduct amongst adolescents. A total of 414 form four students were recruited by systematic random sampling fulfilling the study criteria between Jun to September 2008, from eight randomly selected secondary schools in the district. Data were collected using a structured self-administered questionnaire and analyzed using SPSS software version 12.0. Socio-demographic, housing and family factors in relation with social misconduct were studied.
    Results : The prevalence of the social misconduct in this study was 70.5%. The most prevalent misconduct includes loitering (37.4%), truancy (30.0%), watching pornographic films (28.7%), and involved in pre-sexual activities (23.7%). The factors that showed significant association with social misconduct were sex and race.
    Conclusion : The prevalence of social misconduct is high among adolescent and males as well as Malays are at higher risk. Activities planned for adolescents should be stressed on the danger and effects of social misconduct and be more focused towards male and Malay adolescents.

    Background: Cardiovascular disease is the number one cause of death globally and is projected to remain the leading cause of death. If the trend is allowed to continue, by 2015 an estimated 20 million people will die from cardiovascular disease (mainly because of myocardial infarction and strokes). The number of cardiovascular disease cases in Malaysia has increased to 14% in five years from 96,000 cases in 1995 to 110,000 cases in 2000.

    Methods: The cost to treat patients admitted to Universiti Kebangsaan Malaysia Medical Center (UKMMC) Malaysia, diagnosed with Acute Uncomplicated ST Elevated Myocardial Infarction (STEMI) was calculated by using two different methodologies, namely step down costing methodology and activity based costing using clinical pathway.

    Results: Cost for each stay per day at the cardiology ward using the step down methodology is RM596.42. The treatment cost is estimated from RM1, 789.26 to RM 4,771.36. The average cost per episode for STEMI care with the average length of stay for 5.6 days is RM3, 340(SD ±596.42. The cost of coronary PCI procedure in step costing is RM 13,950.00. Hence, the total cost incurs for STEMI with PCI is RM 17,290.00(SD ±596.42) by using step down method. (an average cost per episode is RM3,340, plus the cost of coronary procedure of RM 13,950.00 . However by using the ABC the cost of STEMI (PCI) with an average length of stay for 5.6 days is RM 20,431.39. The study showed the ABC method was higher by 15.3% than the step down costing.

    Conclusion: The cost in managing STEMI (PCI) with the average length of stay of 5.6 days was calculated by using two different methodologies, namely step down costing methodology and activity based costing. Cost of treatment calculated by using activity based costing are higher because all resources used are incorporated in detail. The ABC method was higher by 15.3% than the step down costing. The difference is within 80-20 rules and the biggest percentage of cost in both methodsis procedure or PCI cost.

    Low back pain (LBP) is a major medical and social problem associated with disability and work absenteeism. Since the effect on unawareness on back care among hospital staff may affect the smooth running of the services to the public and patients, it is the objective of this study to identified the awareness of back care among nurses so that preventive measures can be taken. About 80 nurses working in the outpatient clinic was survey using self addressed questionnaires which were adapted from Zutphen Physical Activity Questionnaire at Queen Elizabeth Hospital, Kota Kinabalu. Demographic analysis demonstrated that among the eighty nurses that responded, 37.5% (n=30) are Malays, 25% (n=25) Chinese and 37.5% (n=30) ( to include other Sabahan tribes). Their age group varies between 23 to 55 years of age. A total of 72 nurses, both agreed (45%,n=36) and strongly agreed (45%,n=36) that understanding good postures is important to prevent LBP. They felt strongly (55%, n=44) about the importance of correct lifting techniques, and having a well-designed workplace (50%,n=40). Regarding factors that cause LBP, 55% (n=44) of them strongly agreed that good lifting technique can prevent LBP while 56 (70%) agreed that prolong sitting doing computer work can cause LBP. About 60% (n=48) also agreed that LBP can cause stress and that 45% (n=36) of them strongly agreed that being overweight than average can worsen LBP. However, 40% (n=32) provide a neutral answer to whether height have any influence on LBP while 50% (n=40) agreed that weak back muscles can worsen the backache further. However, 45% (n=36) agreed that games that involved back movement have high risks and 55% (n=44) agreed that swimming helps to strengthen back muscle. The environmental factors addresses issues of footwear and soft mattress where 60%(n=48) agreed while 15%(n=12) strongly agreed that good footware and appropriate use of soft mattresses 60% (n=48) can prevent LBP. This study have demonstrated that the nurses that participated had a clear understanding and knowledge on back care even though a wider study need to be carried out to ensure validity of study finding.

    Background: A cross sectional study was carried out to determine the prevalence of microbiological contamination in ready- to-eat food and factors affecting it in Melaka from February 2008 till May 2008 in collaboration with the Food Safety and Quality Unit and District Health Offices in Melaka Tengah, Alor Gajah and Jasin.

    Methods: A purposive sampling of 101 food premises was carried out and 202 ready- to-eat food samples were collected and analysed microbiologically using standard methods. A total of 202 food handlers were randomly selected for questionnaire interview while rectal and nasal swabs were taken from them to determine their healthy carrier status of pathogenic food bacteria.

    Results: Results showed that the overall prevalence of microbiological contamination in ready- to-eat food was 35.1%, while prevalence by type of food was 42.0% for main dishes and 9.8% for staple food. The majority of the contaminations were due to coliforms (14.9%), total plate counts (12.4%) and Staphylococcus aureus (3.5%). The mean score for food premise inspection was 77.21 ± 10.32 and the prevalence of healthy carrier status of pathogenic food bacteria among the food handlers was 15.8%. The level of food handling practices among food handlers was influenced by sex (p=0.012) and formal training on food handling courses (p=0.009). There was a significant negative correlation between age and level of food handling practices among the food handlers (r = -0.163; p = 0.02).     Introduction : Major flooding that occurred in Segamat District at the end of 2006 and early 2007 was a natural disaster that has left impact on physical and mental well-being of the victims. The aim of this study is to see the impact of the major flooding to the mental health of the health staff in Segamat District.
    Methodology : Cross sectional study was conducted among Segamat health staffs who were involved in the major floods. Structured questionnaire was used to assess the mental health status of the victims. Result : There were 119 health staffs from the district health office, Hospital Segamat, dental clinic and Community Nursing School were interviewed in this study. A total of 6.7% respondents claimed to have stress related symptoms with women were more effected than men. Nurses and married respondents were found to be more stressful in this study.
    Conclusion : Based on findings, therefore, top management has to pay more attention to the welfare of the health staff, directly or indirectly, in particular on the psychological aspect. By giving emphasis on continuous in-service training and counseling as well as other relevant support, stress-related symptoms could be minimized which in turn lead to higher individual productivity.

    Skim Sistem Saraan Malaysia (SSM) telah diperkenalkan pada tahun 2002 menggantikan skim Sistem Saraan Baru (SSB) kepada kakitangan sektor awam. Satu kajian keratan rentas telah dijalankan ke atas Pakar Perubatan Kesihatan Awam (PPKA) pada 2004 yang bertujuan untuk mendapatkan persepsi mereka mengenai skim SSM. Semua PPKA yang berdaftar dengan Persatuan Pakar Perubatan Kesihatan Awam (PPPKA) yang berkhidmat dengan Kementerian Kesihatan Malaysia (KKM) dipilih sebagai responden. Kajian ini menggunakan borang soalselidik yang diisi sendiri oleh responden. Kadar respon kajian ini ialah 70.0% iaitu 217 responden. Kajian ini mendapati sebanyak 80.6% PPKA tidak bersetuju dengan pelaksanaan SSM, hanya 7.4% bersetuju dan sebanyak 12.0% berkecuali. Kelemahan-kelemahan utama SSM yang dikenalpasti oleh responden yang tidak bersetuju dengan SSM ialah prosedur atau skim perkhidmatan yang kabur(83.9%), peperiksaan Tahap Kecekapan yang tidak releven (54.1%) dan kenaikan pangkat terjejas (40.5%). Hasil kajian ini dapat membantu pihak-pihak yang terlibat memperbaiki kelemahan-kelemahan SSM supaya pelaksanaannya menjadi lebih mantap dan dapat menangani kelemahan yang wujud di dalam skim tersebut.

    Introduction : An organization performs well with proper management methods. Participation is a process involving cooperation and collaboration within all levels of individuals, groups or community for the purpose of organization improvement. Empowerment brings the organization further by delegation of power and responsibilities to others for better result.
    Methodology : This is a systematic review on various publications, articles and studies based on the findings and researches conducted by experts and scholars involved with participation and empowerment of organizations.
    Results : Participation and empowerment are crucial in every organization including healthcare. Its success depends on factors such as proper planning, mutual understanding, regular training and continuous education. They contribute towards better outcome in goal setting, decision making, problem solving and organizational development. There are also various levels of empowerment and participation depending on the type of activities, organisation’s performance and level of authority.
    Conclusion : The prerequisite for an organisation’s success is dependent upon holistic participation and appropriate empowerment. The benefits gained from these measures are indeed invaluable. It creates sense of ownership, increases motivation and satisfaction, optimizing resources, promote innovations, enhancing quality and improving efficiency.

    Introduction : Cervical cancer can be prevented through pap smear screening and its proper management of high risk cases. High level of knowledge regarding pap smear screening will encourage women to do pap smear as recommended.
    Methadology : A cross sectional study was carried out in Jun 2007 to find out the level of knowledge regarding pap smear among married women in Kg. Sesapa Kelubi, Hulu Langat Selangor. Universal sampling was conducted using guided questionnaire and data was analysed using SPSS version 12.
    Result : A total of 154 women participated in this study. Only 3.9% of respondents practiced pap smear as recommended and only 51.3% of them have high level of knowledge with 27.9% has no knowledge regarding pap smear. Women between the ages of 20-35 years old have higher level of knowledge (77.9%). The only factor that showed significant association with level of knowledge in this study is level of education. Age, income, gravida and occupation did not show any significant association.
    Conclusion : The effectiveness of pap smear screening program should be regularly evaluated to determine the problem of its low coverage. Proper strategy should be planned to increase knowledge among women and their participation in pap smear program.

    Introduction : Despite the number of information generated by researchers, cardiovascular diseases problem has not clearly declined and perhaps in certain situation it is gradually increasing, affecting people who are previously at low risk. There is a tendency to believe that favourable outcomes can always be expected once intervention activities, like exercise promotion, are carried out, but practical experience gives rise to serious doubt.
    Methods : A greater understanding of the socialization mechanisms operating in the adoption of physical activity in CVD control and allow specific exercise prescriptions for communitybased prevention and control is important. This paper highlighted the author’s feeling about controlling chronic diseases by mean of community intervention.
    Results : This analysis has strongly believed that the important impetus of any community intervention approach should be oriented in the form of “from people to people”.
    Conclusion : More emphasis needs to be placed on effective management and parameters for assessment of its management success.

    Introduction : Health management is a tool to ensure an effective running of health program and at the same time avoiding flaws to its components including patients, workers and the entire organization. Risk management is a new concept in health management where the determinants of the problems or the risks are put to minima. It’s strategies are oriented towards prevention as well as controlling to all its administrative system.
    Methodology : This is a systematic review on various papers, studies and observations put forwards by risk management experts. It is also supported by feedbacks from many scholars who are involved in research and teaching.
    Results : It is been observed that risk management activities have been on practiced in health care delivery system. It is not a new form, but rather a situational action based on certain issues. In fact, it happens in any organization – public or private. Close observation on this area has triggered development of various risk management models including some administrative standard and guidelines.
    Conclusion: Risk management is a new approach that need to be practiced by all manager and leader. Its discipline involves interpretation of risk at all angles. It is not only confined to the explicit component, but should also be expended to others area as well. It needs a good sense and good attitude of the manager. The essence of risk management is to minimize the risk to patient, staff, public and the entire organization.

    Introduction: Behavioural-based safety (BBS) programme is an accident prevention programme emphasized on the aspect of unsafe behaviour, which considerably new in Malaysia. The oil and gas industry is among the industries in Malaysia that has successfully implemented and sustaining the programme. The aim of this study is to measure and evaluate the effectiveness of BBS towards the safety performance in the oil and gas industry in Malaysia that certainly has owned an outstanding safety performance.
    Methodology: A cross-sectional study using secondary data involving five indicators in the safety performance in the department of health, safety and environment (HSE) was performed at one of the oil and gas refinery companies in Malaysia. These indicators were chosen based on the frequency of its occurrence from January 2004 to December 2008.
    Result: The results of the study show the presence of a significant difference in the incidence rates of near misses (p=0.008) and medical treatment cases (p=0.035) before and after the implementation of BBS and a very strong negative relationship (r=-0.885) between the incidence rates of near-misses and the rates of BBS observations done. However, analysis on unsafe behaviour does not show a significant reduction from 2006 to 2008. Instead, a significant reduction was seen on equipments failure from year 2006 to 2007 (p=0.021) and 2006 to 2008 (p=0.032).
    Conclusion: The study has proven that the BBS is effective towards the reduction of the indicators in the safety performance, involving only the incidence rates of the nearmisses and the medical treatment cases. However, a very strong negative relationship only involves the incidence rate of near misses. A significant reduction was also seen on the occurrence of instrument failure, which is one of the root causes of near-misses. Although there is no significant reduction on thye unsafe behaviour, the reduction of equipment failure is expected to be related to the increase in the level of awareness and safe behaviour as a result of BBS implementation.

    Introduction : Shift work is practised in manufacturing industry to increase production capacity up to three times compared to the normal daily eight hours working system and able to optimize the utilization of machine and equipment. However, shift work has negatif effects on human social interaction, health and safety.
    Methodology : The study was conducted to evaluate production workers’ perception on the effects of working at night shift. The respondents of the study were production workers in Company X in Kuala Lumpur. The Data was collected using self administered questionnaires. The study objectives was to study the work schedule design, to find out their perceptions on the effects of night shift and to study on personal factors, employees’ level of acceptance on the work schedule design and personal factors that cause safety and health disruption.
    Result : A total of 200 production workers participated in the study. The result of the study shows 61% of production workers took sick leave and 43.5% were absent between 1 and 2 times a month. In terms of health and safety disruption, 77% of respondents agreed that they faced lack of focus with family and friends, 75.5% of them did not have enough sleep and 76.5% felt sleepy during working time. Regarding the work schedule and workstation design, 81.5% said they felt uncomfortable because they were required to stand during working and 77% felt that the resting period provided by the factory was too short and inadequate. More than 70% of the respondents proposed the rest period should be extended from the current 40 minutes to one hour. More than 80% of respondents agreed they would feel comfortable if standing at work is changed to sitting work system.
    Conclusion : To minimize the unwanted effect of night shift among the production.

    Introduction: Non-verbal cues are untold words that carry a significant meaning. As a doctor, this remark needs to be perceived correctly. It is an inevitable communication knowledge and skills ought to be mastered in any doctorpatient relationship, particularly when making patient’s assessment.
    Methodology: This was a systematic communication assessment of a real observations on doctor-patient relationship in various setting. It was supported by feedbacks from many scholars who were involved in research and teaching and also reviews, papers and studies on the said subject.
    Results: It has been realized that non-verbal communication was equally important and necessary to complement the verbal one. From all non-verbal cues we have recognized and listed, we have identified at least ten to be the most vital areas to be observed while counseling or consulting a patient.
    Conclusion: Non-verbal cues were always present to support doctor in making a final conclusion about his/her patient’s state. If doctor was patience enough, he or she could has a complete profile of his/her patient and this would lead to the improvement in a person’s life. Having an observant attitude and being able to nose around appropriately were skills that have to be learnt, experienced and applied as these might make an effective doctor-patient communication.

    Introduction : Flood disaster in Johor started at the end of 2006 until the early year of 2007 causing the distruction of property and human life and it was the worse flood disaster in the history of Malaysia. The Muar and Kluang Health Office had been taken all the measures in the District Plan of Action for flood disaster in the early phase of the flood. Management of the Health and Medical team was one of the measures taken for the deployment of staff systematically and optimumly use of man power during a disaster.
    Metodology : The objective for this article is to share the experience regarding human resource management during flood disaster. Data collected base on flood activities rosters used by health staff during morning briefing, analysis of record and daily flood report, interviewing the staff and flood victim involved regarding the experience and challengers they face and lastly by observing the services given to flood victim during the disasters.
    Finding : There were 41 Medical and Health Team formed and responsible at 108 flood relived centre homing 26,824 flood victims in Muar District while in Kluang, 21 Medical and health Team were providing services in 60 flood relieve centre with 36,126 flood victims. All of the activities conducted by the Medical and Health team were coordinated by the district flood operation centre. District of Muar had been receiving 16 additional staff from other state while 34 additional staff had been providing services in Kluang. Challengers that had been identified include shortage of human resource compared to the increasing need and task during the flood disaster, shortage of personal protective equipments, frequent changers and inconsistency in the format use during flood reporting and lack of psychosocial support and motivation among the staff involved in the flood disaster operation.
    Conclusion : Partnership among other department are very importance and the collaboration between them were very good.

    Outbreak management in disaster has to be planned and implemented prior to, during and after the disaster is over. The risk of outbreaks following disaster is related to the size, health status and living conditions of the displaced population. The risk is increased due standing water in floods for vector borne diseases, overcrowding, inadequate water and sanitation and poor access to health care. The 2006-2007 flood in Johore resulted in 2 episodes of food poisoning and an outbreak of coxsackie A24 acute haemorrhagic conjunctivitis. Only 19,667 (12.5%) of the 157,018 displaced persons suffered from communicable diseases which comprised of acute respiratory disease 7361(28%), skin infection 4241(19%), acute gastroenteritis 1872(8%) and conjunctivitis 589 (2%). The routine disease surveillance and environmental control were enhanced to cover the relief centers and flood areas. Risk assessment of communicable disease carried out resulted in prompt control measures and good coverage of preventive activities. In conclusion the Johore State Health Department has successfully manage the outbreaks during the major flood.

    Introduction: The aim of this study was to assess the overall diet quality of husbands and wives in a selected urban area in Selangor.
    Methodology: This cross-sectional study was carried out in Bandar Baru Bangi, Selangor among 150 married couples aged 20 and above, who voluntarily agreed to participate and were not practicing any special diet. Data were collected using a 2-day 24-Hour Dietary Recall and a Food Frequency Questionnaire (FFQ) to evaluate the quality of diet among husbands and wives using the Diet Quality Index Revised (DQI-R).
    Result: Majority of husbands and wives in this study were in the middle-age group, had received tertiary education, had household monthly income of more than RM3, 500 and encompass household size of 3 to 5 persons. The mean DQI-R score for husbands (mean age= 43.33 + 11.16 years) and wives (mean age= 41.28 + 10.93 years) were 67.8 + 9.1 and 64.4 + 9.3 respectively and there was a significant different in scores between the husbands and wives (p < 0.001). In general, the diet quality of this study population was not satisfactory and that the diet quality of husbands was better compared to their wives.
    Conclusion: This study gives an insight picture of husbands and wives’ quality of diet. It can be used as a guideline to improve health intervention programs in the future.

    Background: Level of comfort in working environment can contribute to increase level of health, emotion during working, level of safety, quality and productivity of work. A study of physical factors (heat, noise and lighting) is important to determine the level of comfort during working. This study was carried out to study those physical factors upon comfort level during working among Casting Shop workers in a car manufacturing factory.

    Methods: Instruments for the physical monitoring including Questemp°36 Thermal Environment Monitor, Sound Level Meter and Lux Meter were used at seven measured areas. The information about the level of comfort during working was collected using questionnaires among 65 respondents by random sampling method.

    Results: Measured data showed there were four measured areas which Wet Bulb Globe Temperature indoor (WBGTi) value are above the standard limit recommended by ACGIH, three measured areas recorded noise level above the standard limit recommended by Factories and Machineries (Noise Exposure) 1989, while there was no measured area recorded lighting reading below the standard limit recommended by MS ISO 8995:2005. Result from questionnaire found that the majority of the workers did not feel comfortable towards the heat and noise level in their workplace while most of the respondents felt comfortable towards lighting level in their workplace.
