    A differential pulse adsorptive cathodic stripping voltammetric (DPAdCSV) method for the determination of lead levels in water sample by using pairs of ligands (complexing agents) / – nitrite and 1,10 phennanthroline - nitrite were investigated. Variables affecting the response such as pH, supporting electrolyte, deposition potential, deposition time, stirring speed, drop size, sweep rate, pulse amplitude and ligand concentration were studied. Under optimized conditions the relationship between peak current and lead concentration with 2,2ʹ-bipyridine - nitrite and 1,10 phennanthroline - nitrite were linear in the range of 10 - 500 ng mL-1 and 7–500 ng mL-1 with limit of detection of 0.48 ng mL-1 and 0.36 ng mL -1 respectively . The relative standard deviations (RSD) for 9 determinations of 25 ng mL−1 Pb (II) using 2,2ʹ-bipyridine - nitrite and 1,10 phennanthroline - nitrite were 1.74% and 1.84%, respectively. The values obtained for the proposed technique correlated well with those of atomic absorption spectrometry (AAS) as the standard method with correlation coefficient (R2 ) of 0.9991 and 0.9986. The method was applied for the determination of lead in sea, lake and tap waters. The interferences of other metals and major salts present were negligible except iron (III) and Aluminum (III)

    Chikungunya virus (CHIKV) is a mosquito-borne alphavirus which causes epidemic fever, rash and polyarthralgia in Africa and Asia. Two outbreaks have been reported in Malaysia, in Klang, Selangor (1998) and Bagan Panchor, Perak (2006). It is not known if the outbreaks were caused by the recent introduction of CHIKV, or if the virus was already circulating in Malaysia. Seroprevalence studies from the 1960s suggested previous disease activity in certain parts of the country. In Asia, CHIKV is thought to be transmitted by the same mosquitoes as dengue, Aedes aegypti and Ae. albopictus. Due to similarities in clinical presentation with dengue, limited awareness, and a lack of laboratory diagnostic capability, CHIKV is probably often underdiagnosed or misdiagnosed as dengue. Treatment is supportive. The prognosis is generally good, although some patients experience chronic arthritis. With no vaccine or antiviral available, prevention and control depends on surveillance, early identification of outbreaks, and vector control. CHIKV should be borne in mind in sporadic cases, and in patients epidemiologically linked to ongoing local or international outbreaks or endemic areas.

    Irreducible dislocation of the interphalangeal joint (IPJ) big toe is a rare injury Hitori et al. (2006) . We report a case of the right big toe IPJ dislocation following a trauma. The problem was diagnosed and managed at other medical centers with standard treatment of closed manual reduction and splint. The right big toe was splinted accordingly and the patient was referred to our orthopedic outpatient clinic. At the clinic, a repeat plain radiograph was ordered due to high suspicion of the irreducible IPJ.

    Functional renal reserve is a measure of the capacity of the kidney to increase the glomerular filtration in response to the stimulus of a protein meal or amino acid infusion. The aim of this study was to evaluate the usefulness of protein meal test to measure functional renal reserve in normal subjects and various groups of renal patients. One hundred and twenty five subjects from the Nephrology Clinic, Kuala Lumpur General Hospital were subjected to a protein loading test to measure their renal reserve. Each subject had to eat a 100 gram cooked chicken meat and timed 2 hours urine collections before and after the test meal were done to measure the creatinine clearances. The 62 healthy subjects showed a mean renal reserve (creatinine clearance after protein loading-baseline creatinine clearance) of 31.0 ml/min. The 31 subjects with various kidney diseases showed a mean renal reserve of 13.5 ml/min. The 19 renal transplant recipients showed renal reserve of 13.2 ml/min. The 12 nephrectomised donors showed renal reserve of 5.4ml/min. Renal reserve may be used to assess suitability of living related transplant donor for nephrectomy.
