    Problem-based learning (PBL) i s an educational reform that is now becoming a household word in higher education, particularly in medical schools. Many medical schools have implemented a full problem-based learning curriculum (PBLC) whiIe some have included PBL into selected units of the course in an otherwise conventional cumculum (embedded PBL) and others run their tutorials in a PBL manner within a modified conventional curriculum (hybrid curriculum). Yet there are others who claim that small components of PBL in a conventional curriculum are not PBL at all. Thus amateurs in the subject matter find difficulty in evaluating the logistics and outcome of these variations. This article focuses or, the general characteristics of PBL and how this learning method can help enhance independent learning and critical thinking, whether in a full, embedded or hybrid curriculum. The extent of PBL to be included and which of the three types is to be adopted depends on the objective of the undergraduate medical course as determined by the faculty, resources available, limitations, feedback on the existing curriculum and various other factors.     Epidermal cyst is the most common type of cyst to occur in subcutaneous tissue. It is usually small, asymptomatic, and solitary in lesion. When the size is greater than 5 cm, it is classified as a giant epidermal cyst. If the size is big, it may sometimes mimic a soft tissue sarcoma, hence requiring further evaluation with imaging or histopathological studies. We report a case series of two patients whom presented with a huge swelling at different body regions, involving gluteal and chest. Both patients underwent different modes of imaging to delineate the anatomical extent of the lesion. Excision biopsy was done and the histopathological report confirmed epidermal cyst with no malignancy. The purpose of this case series is to describe the rare occurrence of giant epidermal cyst and to elaborate the methods of approach to arrive at the diagnosis.

    Refractory dermal melasma is resistant to conventional treatment. Platelet rich plasma (PRP) may help to reduce the pigmentation of melasma. We present a case report on the clinical outcome of 2 patients with melasma, given PRP, as an adjunct therapy. PRP was administered at a monthly interval for 2 sessions in combination with a monthly Q-switched Nd Yag laser treatment and topical alpha arbutin application. A modified melasma area and severity index (MASI) was evaluated by two dermatologists who were blinded. At the follow up on the 3rd months, the MASI score was reduced by mean 33.5% for case 1 and 20% for case 2. There were no clinical complications for case 1. However recurrence of melasma was noted in case 2 by a worsening of the MASI score mean to 53% at the sixth months follow up. In conclusion, intradermal PRP injection as an adjunct to the conventional treatment of melasma presented with differing results in two cases.

    Graft-versus-host Disease (GVHD) is the main cause of morbidity and mortality after allogeneic hematopoietic
    stem cell transplantation (alloHSCT). In spite of immune-suppressive prophylaxis, most survivors suffer from
    acute and chronic GVHD (aGVHD and cGVHD). The outcome of alloHSCT may be affected by the presence of
    single nucleotide polymorphism (SNP) in non-HLA genes including those involved in innate immune responses.
    This study aimed to evaluate the impact of cytotoxic T-lymphocyte antigen-4 (CTLA-4) and caspase recruitment
    domain 15 (NOD2/CARD15) gene polymorphisms on the incidence and severity of aGVHD and cGVHD following
    alloHSCT. A structured literature review was carried out using various keywords and MESH terms such as
    stem cell transplantation, allogenic haematopoietic stem cell transplantation, GVHD, and non-HLA gene
    polymorphism, in PubMed, Google Scholar and Cochrane Database. A total of 8 studies that met inclusion
    criteria (English publications from 2006 to 2017) were included. Ten SNPs in CTLA-4 gene and three SNPs in
    NOD2/CARD15 gene were tested in patients with underlying haematological malignancies. Four studies tested
    the SNPs of CTLA-4 gene and two were found to have an association with CTLA-4 SNPs (rs3087243, rs231775)
    and increased incidence of aGVHD. The other four studies tested the SNPs of NOD2/CARD15 gene and one
    found an association between SNP13 and increased incidence of aGVHD. None of these eight studies found
    any effect on severity of GVHD. In conclusion, two SNPs in CTLA-4 and one SNP in NOD2/CARD15 increased
    the incidence of aGVHD but not its severity. The higher incidence of aGVHD in studies with larger sample size
    could support the impact of SNPs in the outcome of alloHSCT. However, due to the heterogeneity of studies in
    regard to the age of patients and donor, and conditioning regimen, it is difficult to draw a definite conclusion.

    Introduction: The health effects of inhaling rice dust contaminated with endotoxin include respiratory problems, asthma, chronic bronchitis and chronic obstructive pulmonary disease.Thus, this comparative cross-sectional study aims to associate the concentration of endotoxin levels in inhalable rice dust and the lung function decline among rice millers.

    Materials and Methods: The endotoxin level in inhalable dust for both area and personal samplings were collected using 25 mm Glass Fiber (A) filter loaded in IOM samplers connected to a pump by tygon tubing. The pump was operated at 2.0 litres/min and clipped around the breathing zone of the rice millers for eight hours. The endotoxin concentrations were analysed using Limulus Amoebocyte Lysate Chromogenic Endpoint assay at 405 nm. Lung function tests were carried out using Spirometer (Pony FX), for both the rice millers and the non-exposed groups.

    Results: Findings for the mean concentration of endotoxin for areas was 0.26(standard deviation (SD) = 0.12) EU/m3 whereas the mean personal inhalable endotoxin level among the rice millers was 0.29(SD = 0.15) EU/m3. Post-shift lung function tests for FEV1/FVC measured appeared lower among rice millers (54%) compared to non-exposed workers (62%), but not statistically significant (p = 0.313). However, there were significant correlations between endotoxin concentration and post-shift LFT parameters of measured FVC, FEV1 and PEFR (p < 0.05).

    Conclusion: Despite the low level of endotoxin exposure, proper protective measures should be applied for rice millers for long term protection.

    To see the trend in managing singleton breech pregnancy after the term breech trial. Secondly to compare the safety of different modes of delivery for term, singleton breeches by looking at the immediate neonatal outcome, based on our own experience. Breech infants were identified by examining computer-stored maternal discharge records of hospitalization for the years 1990 and 2000 respectively. Parameters studied included planned mode of delivery, actual mode of delivery, parity, previous vaginal delivery, Apgar score at five minute, birth weight, referral to special care nursery and neonatal morbidity. Of 6,496 deliveries in 1990 and 5,081 in 2000, there were 220 (3.4%) and 148 (2.9%) term breech infants respectively, of which 115 (for 1990) and 102 (for 2000) case records were available. In 1990, 62.6% of the women had trial of vaginal breech delivery but only 24.5% of the women in 2000 were allowed to do so (p < 0.05). Caesarean section rate for singleton breeches increased from 51.3% in 1990 to 84.3% in 2000 (p < 0.05). Mean Apgar score at five minutes was significantly lower after vaginal breech delivery (9.40 ± 1.36) compared to after Caesarean section (9.72 ± 0.712) but there was no clinical significance. There was a noticeable trend towards Caesarean section and less trial of vaginal delivery. Neonatal outcomes of babies born abdominally were statistically better than those born vaginally but there was little clinical impact. Perhaps in properly selected cases, a planned vaginal breech delivery still has a role to play.

    We aimed to establish a method for quantitative analysis of mixed haematopoietic chimerism based on microchip electrophoresis of selected molecular markers following PCR amplification for accurate monitoring of graft status post-transplantation. A 12-year-old girl with relapsed acute lymphoblastic leukaemia who underwent allogeneic bone marrow transplantation had qualitative chimerism analysis using short tandem repeat markers at three time points following the procedure. Her archived DNA samples were then used to test the ability to correlate her clinical course with changes in the quantity of donor chimerism at the different time points. Quantitative chimerism analysis was performed on the Agilent 2100 bioanalyser and donor-recipient ratios were calculated from generated electropherograms. Complete donor chimerism (98%) was demonstrated three weeks post- transplantation. Decreasing amount of donor chimerism to 24% was shown after three months and this concurred with clinical relapse. Following a second transplant, full donor chimerism was reestablished where donor chimerism rose to 100%. High resolution microchip electrophoresis could be useful in predicting the occurrence of increasing recipient chimerism which may herald impending relapse in patients while the disease burden is still low. This investigational approach may provide useful information for clinicians to select appropriate intervention strategies to ensure successful transplantation.

    A group of 265 urban private sector workers in Kuala Lumpur and adjacent Petaling Jaya responded to a self-administered questionnaires. Their knowledge of cardio-pulmonary resuscitation, risk factors for and preventive measures against cardiovascular diasease a n d w h e n these should be started are relatively weak, though knowledge on symptoms of heart a attack are satisfactory. Relatively more males than females are current smokers and consumers of alcohol. Males tended to start smoking and drinking in their teen years while females tended to do so later in their non-teen years.

    Malaysia has undergone rapid pace of industrialization and urbanization in recent decades and this has brought about imminent changes in the lifestyle of Malaysians. This is a cross-sectional study which attempts to examine the lifestyle practices and the prevalence of obesity of a group of security guards and their spouses of the University of Malaya, Kuala Lumpur. Data collection was conducted by both the methods of face-to-face interview and self-administered questionnaire. The respondents were surveyed on lifestyle practices such as smoking habits, exercise and eating pattern. Anthropometric measurement such as weight and height were also taken to establish the extend of obesity by using Body Mass Index (BMI). This study reveals that the community did have some unhealthy lifestyle practices such as smoking (27.7%; 95%CI 20.2%, 36.2%), low prevalence of adequate exercise (13.8%; 95%CI 8.4%, 21.0%); high prevalence of overweight and obesity (64%; 95%CI 55.1%, 72.3%); and high prevalence of co-morbidities such as diabetes mellitus and cardiovascular diseases. In conclusion, the community is considered to be a vulnerable and high-risk group for morbidity and mortality with the above predisposed risk factors.     This paper examines the results of a section on recent illness of the health survey among 799 foreign workers from three selected study locations. The main objective was to study illness/injury patterns and the utilisation of available health care services. It also attempts to examine the use of health supplements as an indicator of self-care. Findings indicate that the illness/injury rate was 46.6%. The illness/injury rate increased with age and was highest in the 45-54 age group (65.0%) and among the Thai workers (69.6%). The main illnesses reported were injuries and accidents (19.6%), musculoskeletal problems (18.0%) and gastrointestinal complaints (16.7%), and it varied with gender, age and nationality. Almost 90 percent of the foreign workers sought treatment at modern health care facilities, with a third utilising government health care services. The employers contributed towards 60% of all the treatment costs. Nearly a third of the foreign workers took health supplements, and the rates were higher among the younger age group (40.0%) and among the Indonesian workers (52.0%). Majority had obtained the health supplements from the pharmacies or retail shops (43.4%) and private health care facilities (35.4%), and about 70 percent paid out of their own pocket. Some of the implications and limitations of these findings are discussed.

    In 1999, the Department of Social and Preventive Medicine set up its website on the Internet (http://ummc.med.uni.edu.my/sps/. Setting up the website was a whole new experience for the author and many new things were learnt along the way. This paper describes how the website was constructed, the problems, and solutions to all these problems.     The age-old wisdom is that “women live longer than men”. Gender difference in life expectancy is becoming a
    worldwide phenomenon both in developed and developing countries. The process of ageing may be viewed
    from the perspectives of physical, psychological, and social-economic wellbeing. We investigated gender
    difference in understanding ageing in relation to life expectancy, fears relating to diseases and deteriorating
    economic status, and perceived old age comfort and their preparedness. Data were obtained from an online
    survey and in-person interview of 518 respondents aged 40 years and older residing in Malaysia, which was
    based on a convenience sample collected from May 2015 to January 2016. Data were analysed using chisquared
    tests and multinomial logistic regression. There were varying views between men and women when
    it came to understanding ageing in relation to life expectancy, fears of ageing, deteriorating economic status
    and their perception of old age comfort. Women were more optimistic about living longer compared to men
    but feared more the consequences of old age diseases. In spite of displaying less concern about financial
    preparedness, women were, however, willing to cut down expenses, while men would prefer longer working
    hours to ensure a comfortable retirement.

    Rhabdomyosarcoma of the genitourinary tract is rare and predominantly affects paediatric patients. We present an unusual case of such a lesion in an adult with extensive occupation of the bladder cavity by the lesion, resulting in bilateral ureteric obstruction, without evidence of ureteric outlet invasion. We outline the unusual CT and macroscopic appearance of this lesion. We also discuss the literature data and management strategies of rhabdomyosarcoma of the genitourinary tract.

    In early 2001, Nadi IT (the IT department of the UMMC) migrated the UMMC leased line from its old 64 kbps line 10 the 34 Mbps University of Malaya leased line. The migration was a new leuning experience for Nadi IT and many lessons were learnt along the way. This paper describes the ideOll behind the migration process, the reason (or the migration, problems and solutions 10 aU these problems as well as the benefits derived from the migration.
