Objective: Lysergic-acid-diethylamide (LSD) users are the hidden population of drug users as they do not present to medical personal for treatment. The method of transporting the drug is usually by-pass the law enforcers’ monitoring. We describe a rare case of LSD use in Malaysia Novel Psychoactive Substances (NPS) which is difficult to capture as LSD users remained as a hidden population of drug users in the country.
Methods: We reported a 23-year-old male with chronic use of Lysergic-acid-diethylamide (LSD) presented with retrospective history of acute intoxication. Later, he presented with depressive symptoms, occasional flashbacks and physical complications such as premature ejaculation (PE), urinary incontinence, reduced sweating and headache despite abstinence from LSD and other drugs. The reported physical complications remitted after 4 months of abstinence.
Result: Our case report suggested that LSD use not only causes acute intoxication but leads to several complications, which may persist even after cessation of LSD use.
Conclusion: This case-report had pointed out the importance of future studies to look into the complications of LSD and the case-report as well demonstrated the harmful psychological and physical effects of LSD use, which would warrant proper surveillance and interventions curbing the emergence of LSD use in Malaysia.
The Drug Dependency Treatment and Research Unit (DDTRU) in Myanmar established opioid substitution with methadone in 2006. Reducing HIV transmission could be affected by eliminating the unsafe needle sharing among injecting drug uses and treatment with opioid substitution. The quality of life (QOL) among the clients retained in the methadone program is important for their personal development and is an indication of the treatment efficacy. This study evaluated factors associated with the QOL of methadone patients to ensure efficient service delivery. It also identified how patients' characteristics had differed QOL scores of respondents. This cross-sectional study was conducted in five cities with stratified random sampling. The study assessed the QOL of methadone patients in Myanmar. The study recruited 210 respondents to answer structured questionnaires for their quality of life: WHOQOL-BREF questionnaires and urine sample collection for methadone and illicit drug use. Survey responses on the QOL were transformed into 100-scale ratings, and higher QOL scores reflect better QOL. The average score of total QOL was 60.82%; precisely 60.09% in the physical domain, 63.11% in the psychological domain, 59.87% in the social relation domain, 60.41% in the environmental domain respectively. Respondents who reported illicit drug use had lower QOL scores. Statistically significant association of the QOL category of the methadone patients was identified with frequent methadone treatment episodes, the infection status of HIV, current treatment on antiretroviral therapy (ART), tuberculosis (TB) treatment history, sexually transmitted infections (STI) history in their lifetime, current work status as peer, Addiction Severity Index (ASI) for drug use, satisfaction with current marital status, satisfaction with current leisure status, history of psychological abuse within 30 days, heroin injection within 30 days, frequency of injection, and reported use of barbiturates (p<0.05). Addressing these factors will improve the treatment service intervention and the quality of life among methadone patients.
Introduction: Kratom (Mitragyna speciosa Korth) dependence is a common psychiatric complication associated with regular kratom use in Malaysia. Hence, it is important to assess the severity of kratom dependence among regular kratom users. The Kratom Dependence Scale (KDS) was introduced to evaluate the degree of severity of kratom dependence among users. This study aimed to translate the original English version of the KDS into Malay and examine the psychometric properties of the KDS (Malay) among Malaysian kratom users.
Methods: Initially, parallel translation and back translation of the original English version of the KDS was performed. The KDS (Malay) then was administered to 20 active Malaysian kratom users to assess face and content validities in a pilot study. Finally, the final version was administered to 150 active kratom users to evaluate its internal consistency (Cronbach’s α), face and content validities, discriminative validity, and construct validity, i.e. via exploratory factor analysis (EFA).
Results: The KDS (Malay) Cronbach’s α of 0.94 exhibited excellent internal consistency. It also demonstrated construct validity, as EFA showed that all items of the KDS (Malay) fit into a single domain, similar to the original English version. Discriminative validity was also demonstrated by the KDS (Malay), as it could differentiate kratom dependent users from non-kratom dependent users.
Conclusion: The KDS (Malay) has acceptable psychometric properties and is suitable to assess severity of kratom dependence in Malaysian kratom users.
Introduction: Kratom which is a tropical plant use as traditional remedy in rural areas of Malaysia and Thailand has recently been a research focus worldwide due to its potential as substitution therapy for opioid addiction. However, data on its effect on the quality of life of kratom users is scarce. This study aimed to describe the socio-demographic and history of kratom use as well as assessing the quality of life and its associated factors in Malaysian kratom users. Methods: This cross-sectional survey recruited 150 kratom users and they were administered with socio-demograph- ic and substance history questionnaires, the World Health Organization Quality of Life- BREF (WHOQOL-BREF) to assess quality of life and the Kratom Dependence Scale (KDS) to assess severity of kratom dependence. Results: Respondents were all males with mean age of 34.4 years old (SD= 11.2). Sixty-percent of respondents use kratom
> 6 years (mean duration = 8.5 years, SD= 5.3) while 55% (n=83/150) used >3 glasses of kratom daily. Duration of kratom use, quantity of kratom use and severity of kratom dependence were not associated with all the domains of quality of life except severe kratom dependence users had significant lower physical quality of life score when compared to that of users with mild to moderately severe kratom dependence. Conclusion: Based on our study, kra- tom consumption does not cause impairment in quality of life of kratom users except for severe kratom dependence which may cause deterioration in physical well-being of users.
The research was aimed to study the relationship between attachment styles and marital quality between spouses. A total number of 52 spouses representing teachers from Sekolah Menengah Agama Daeratul Ma'arifil Wataniah 1 (SMA DMW 1), in Peninsular Malaysia's northwest coast, participated in this study. There were two sets of questionnaires used in this study: Experience in Close Relationship (ECR) and Marital Adjustment Test (MAT). The results showed that there was a significant relationship between attachment styles and spouses' styles. However, there is no relationship between marital satisfaction among spouses, and there is also no relationship between attachment styles and marital satisfaction among spouses. Future research is suggested to enhance the research by moving beyond self-report instruments to develop alternative methods of assessment as some of the questions should be more specifically suitable in Malaysian culture.
This study aims to identify parental attachment's effects on trainee counsellors' personality traits in the Universiti of Malaysia Sarawak UNIMAS. This research adopted a correlational research design. The respondents were 209 trainee counsellors from the Counselling Program, Universiti Malaysia Sarawak UNIMAS. Two questionnaires were used in this study: The Inventory of Parents and Peer Attachment (IPPA) and the Big Five Personality. The findings indicated a significant relationship between Parents Communication, Attachment and Extraversion and the Agreeableness Trait of Personality. It also showed a significant relationship between Parents Communication Attachment and Extraversion, Agreeableness and Conscientiousness. We also found a significant relationship between Parents' Alienation Attachment and Conscientiousness, Neuroticism and Openness. The study informs society about the need to increase awareness about the importance of knowing attachment styles and personality traits as both impact individuals' lives.
Introduction: Kratom or (Mitragyna speciosa) leaves are consumed as a folk remedy and opioid substitute in the Southeast Asian region. There is still a lack of information about the long-term or toxic-causing effects of kratom use. Methods: A total of thirteen regular kratom users, with long-term (>20 twenty years) kratom use history were recruited for this cross-sectional pilot study. Respondents were required to undergo a blood-test and laboratory anaysis was conducted to determine the mitragynine content in an acquired street sample of kratom. Results: The regular, long- term consumption of brewed kratom decoction did not cause any significant alterations in haematological, kidney, liver, thyroid, inflammatory and gastrointestinal analytes in a cohort of kratom users who had no history of substance misuse. However, those who had a higher intake (>3 glasses per day) of kratom exhibited higher lipid values (except for HDL-cholesterol), and a moderate elevation of homocysteine level. Conclusion: Long-term (>20 years with a daily intake of 87.54mg of mitragynine) kratom consumption was not associated with altered biochemical levels, although prolonged and heavy use (>3 glasses daily) may result in cardiovascular risks. The latter finding, however, requires further investigation.
