    To create a national registry of contact-lens related corneal ulcers (CLRCU) patients in Malaysia with the aim of detecting outbreaks, identifying pattern of causative organisms, determining patient demography, risk factors, wearing patterns and monitoring outcome of treatment. The CLRCU registry is an ongoing patient registry established in 2007 as a surveillance tool used by Malaysian Ministry of Health ophthalmology departments. Notification of patients clinically suspected of CLRCU was performed online through the National Eye Database (NED). Data collected included patient demography, contact lens type, causative organism and treatment outcome. During 2007-2008, a total of 202 patients were notified to the CLRCU registry with a mean age of 26.7 years (71.8% female). All registered patients wore soft contact lens and monthly disposable lenses were the most popular (83.5%). The majority of patients had bacterial CLRCU and the most common causative organism was Pseudomonas (79.7% of bacterial cases). No epidemics were identified during the period of data examination. Use of contact lenses, which is increasing during modern times, may lead to CLRCU as a severe complication. The CLRCU registry is an effective tool which uses a web-based notification system that allows quick and up to date reports of CLRCU cases. This provides the ability to monitor outbreaks of disease and identify important causative and associated factors of the disease which may be used to reduce future incidence.

    PURPOSE: To report an unusual case of bilateral simultaneous hypopyon corneal ulcer in a contact lens wearer caused by polymicrobial infection with rare organisms.
    METHODS: A case report of a 21-year-old soft contact lens wearer, who visited the emergency department with a 3-day history of pain, redness, decreased vision, photophobia, and tearing in both eyes. Examination showed a central corneal ulcer with hypopyon in both eyes. The cultures from corneal scrapings of both eyes, the contact lenses, and the contact lens solution showed heavy growth of Pseudomonas aeruginosa, Alkaligenes species, and Flavobacterium meningosepticum.
    RESULTS: The corneal ulcers healed completely with aggressive antibiotic treatment for 4 weeks. The best-corrected visual acuity after 6 months of follow-up was 20/400 in the right eye and 20/60 in the left eye.
    CONCLUSIONS: The possibility of infectious keratitis should be kept in mind for an acute red eye in contact lens wearers, and appropriate initial management is essential for a successful outcome. It is believed that this is the first report of Flavobacterium meningosepticum as a causative organism in contact lens-related keratitis.

    PURPOSE: To investigate factors associated with the presence of microdot deposits or white dots (WDs) on confocal microscopy in regular soft contact lens (SCL) users.
    METHODS: This cross-sectional observational study investigated changes in the cornea in regular SCL users by using an in vivo slit-scanning microscope (ConfoScan 3). Images were analyzed by noting the presence of highly reflective WDs. Factors associated with WDs were analyzed by using an unpaired t test with Welch correction.
    RESULTS: There were 56 SCL wearers. Of these, a group of 10 had WDs (GWD) in various parts of the cornea. They had worn SCLs for 7 to 20 years and had a mean total duration of SCL wear of 13.6 +/- 4.4 years. Their mean age was 35.8 +/- 10.4 years. They were compared with a group of SCL wearers with no evidence of WDs (GNWD). The mean age of GNWD was 29.1 +/- 7.2 years, with a mean duration of SCL use of 8.17 +/- 5.1 years. The two groups were compared in terms of age, total duration of SCL wear (years), duration in hours per week, SCL water content (%), mean cell density in the endothelium and stroma, endothelial cell coefficient of cell size variation, and percentage of hexagonal cells. Only the duration of SCL wear was significantly associated with the presence of WDs (p=0.0042). WDs were most commonly found in the posterior stroma (n = 9). Two patients had WDs in the epithelium, with one of these having WDs in the endothelium. All patients except one with a hazy left eye scan had WDs bilaterally and symmetrically.
    CONCLUSIONS: Confocal microscopy allows visualization of WDs in the corneas of Asian regular SCL users. Patients with WDs have a longer history of SCL wear. WDs may represent an early stage of corneal disease or degeneration associated with alterations in cell behavior.

    PURPOSE: To analyse and compare the alterations in corneal endothelium morphology induced by different materials and durations of wearing soft contact lenses (CL) among young adults living in Kuala Lumpur.

    METHODS: Healthy soft CL wearers were invited to participate in this study. Visual acuity (VA) was measured using the Snellen chart, and subjective refraction was performed using cross-cylinder technique. Standard ocular assessments were conducted using a slit lamp biomicroscope and morphology of corneal endothelial cells (endothelial cell density, ECD, coefficient variation, COV, hexagonality, HEX and central corneal thickness, CCT) were evaluated using a non-contact specular microscope. Statistical analysis was conducted using ANOVA and data from the right eye only is included.

    RESULTS: A total of 72 subjects (32 SiHy and 40 HCL wearers) and 24 non-CL wearers (control) participated in this study. The gender distribution for study subjects was 13 males and 59 females, with a mean age 22.15 ± 1.84 years old. The mean refraction was -1.86 ± 1.25DS. The duration of wearing CL ranged from 1 to 9 years. Subjects were later divided into 2 groups following duration of CL wear: Group 1 (<5 years) and Group 2 (≥5 years) for analysis purposes. Optometrists are recommended to regularly evaluate the morphology of corneal endothelial cells in CL wearers and provide necessary intervention when required.
