    Much has been said in various anaesthetic journals about the need to teach medical undergraduates in areas such as basic life support skills and resuscitation. Anaesthesiology as a specialty can contribute significantly in this aspect of teaching. The question is how should it be incorporated into the existing curriculum, given the fact that anaesthesiology constitutes a minor role in the undergraduate medical curriculum? This article attempts to answer this question and proposes the possible integration of anaesthesiology with the other major clinical specialties. This curriculum forms the basis of anaesthesiology curriculum at the Kulliyah of Medicine, International Islamic University Malaysia (IIUM).

    This study investigates productivity growth and efficiency of Large Scale Enterprises (LSEs) in the
    Malaysian food processing industry. Malmquist productivity index of Data Envelopment Analysis (DEA) was employed to five-digit panel data for the period of 2000-2006. The findings suggest that average Technical Efficiency (TE) of the LSEs was 0.683 during the period of observation, which indicates that the industries are able to expand their output as much as 31.7 percent by using the same level of inputs. Total Factor Productivity (TFP) growth was positive at 7.3 percent, which is contributed by a Technical Efficiency Change (EFCH) of 4.3 percent and Technological Change (TECH) of 3.0 percent. Sub industries of manufacturing alcohol and wine as well as the processing and preserving of meat and meat products shows the highest productivity growth at 84.8 percent and 47.5 percent respectively. On the other hand, the sub industries of processing and preserving poultry and poultry products together with the manufacturing of chocolate are those which have the lowest TFP growth at -30.5percent and -14.8 percent respectively. The significant determinants of the productivity growth, with a positive relationship are public infrastructure, IT expenditure and foreign ownership, while energy price is the determinant with a negative relationship. The main contributor to the TFP growth of the LSEs in the Malaysian food processing industry is EFCH, however, the LSEs can also improve the TFP growth by moving forward the production frontier as well.

    Nitrocellulose (NC)-based propellants have played a pivotal role in the development of energetic materials for both military and civilian applications. This review offers a comprehensive exploration of NC-based propellants, tracing their evolution from their historical origins as smokeless gunpowder to modern advancements. It discusses the chemical composition and classifications of NC propellants, along with continuous efforts to refine smokeless powder formulations through studies on smoke formation, residues, and additives. Modern techniques such as PCR-based detection and dynamic light scattering have enabled precise analysis of NC properties, including variations in the degree of substitution and molar mass, which allow for tailoring the chemical structure to meet specific performance needs. Special attention is given to the combustion dynamics of NC-based propellants, with an emphasis on reaction zones, performance characteristics, and optimization strategies that enhance their overall efficacy. The review also highlights the significant impact of nitrogen content, additives, and processing methods on the performance, stability, and safety of NC-infused propellants. While higher nitrogen content improves energetic output, it also increases surface cracking and gas production, necessitating the use of stabilizers and additives like Bu-NENA, copper compounds, and MgH2 to enhance flexibility, thermal stability, and reduce sensitivity to mechanical and thermal stimuli. Aging and environmental factors further influence burn rate variation, underscoring the need for tailored formulations. In terms of environmental sustainability, this review addresses safety considerations in handling and disposing of NC-based materials, focusing on innovations such as alkaline hydrolysis and NC recovery to mitigate environmental risks. Stabilizers and eco-friendly additives effectively prevent chemical degradation, reduce shock sensitivity, and enhance thermal stability. The review concludes with future research directions aimed at further improving the stability and safety of NC-based propellants, ensuring their continued relevance in modern applications.

    INTRODUCTION: The aim of this study was to analyse the clinical characteristics of patients with rheumatoid arthritis receiving biologics therapy and investigate the association between types of biologics and tuberculosis (TB) infections in 13 tertiary hospitals in Malaysia.

    MATERIALS AND METHODS: This was a retrospective study that included all RA patients receiving biologics therapy in 13 tertiary hospitals in Malaysia from January 2008 to December 2018.

    RESULTS: We had 735 RA patients who received biologics therapy. Twenty-one of the 735 patients were diagnosed with TB infection after treatment with biologics. The calculated prevalence of TB infection in RA patients treated with biologics was 2.9% (29 per 1000 patients). Four groups of biologics were used in our patient cohort: monoclonal TNF inhibitors, etanercept, tocilizumab, and rituximab, with monoclonal TNF inhibitors being the most commonly used biologic. The median duration of biologics therapy before the diagnosis of TB was 8 months. 75% of patients had at least one co-morbidity and all patients had at least one ongoing cDMARD therapy at the time of TB diagnosis. More than half of the patients were on steroid therapy with an average prednisolone dose of 5 mg daily.

    CONCLUSION: Although the study population and data were limited, this study illustrates the spectrum of TB infections in RA patients receiving biologics and potential risk factors associated with biologics therapy in Malaysia.
