Background/Purpose: Visual impairment is a disability more commonly caused by diseases that lead to several disadvantages to the daily activities amongst those blind. For almost a century since the white cane was first introduced, the cane has remained as the most reliable tool for those affected by blindness.Methods: By using a combination of the capabilities of Internet of Things (IoT) and existing devices, such as mobile phones, an InWalker system is proposed to expand the functionality of the typical white cane, so as to introduce several new features that enhance the safety and confidence amongst people who are blind. As such, this paper explores the existing works and projects to comprehend the motivation and the standard practices for each proposed feature. Each of the strength and drawback has been assessed thoroughly to refine the scope of this project.Results: The proposed project, InWalker, is an intelligent system that has an embedded board system with various sensors to enhance the usability of white cane. The inputs from the sensor are processed on a microcontroller, which then pass the data to a smartphone via Bluetooth for additional features, such as global positioning system (GPS) tracking and SMS services.Conclusions: Based on the initial user testing, the proposed system has successfully fulfilled most of the users' need.Implication for RehabilitationVisual impairment is a disability more commonly caused by diseases that lead to several disadvantages amongst those blind.The white cane has been seen as the most reliable tool for the visual impaired.This tool could be further improved with the integration of additional sensors that works with today's mobile devices.The proposed system, InWalker, is able to improve the overall quality of life among people who are blind through several features: obstacle detection, GPS tracking and a light illumination in dark environments for increased safety.
Lightning strike is a natural phenomenon with potentially devastating effects and represents one of the important causes of deaths from environmental phenomena. Almost every organ system may be affected as lightning current passes through the human body taking the shortest pathways between the contact points. A 10 years retrospective study (1996-2005) was conducted at University Hospital Kuala Lumpur (20 cases) also including cases during last 3 years from Hospital Tengku Ampuan Rahimah, Klang (7 cases) from the autopsy reports at Forensic Pathology Units of these 2 hospitals. Both these hospitals are attached to University of Malaya. There were 27 fatal cases of lightning strike with male preponderance(92.59%) and male to female ratio of 12.5:1. Majority of victims of lightning strike were from the age group between 30 and 39 years old. Most of the victims were foreign workers. Indonesians workers contributed to 59.26% of overall cases. Majority of them were construction workers who attributed i.e.11 of 27 cases (40.74%). Most of the victims were brought in dead (37.04%). In majority of the cases the lightning incidence occurred in the evenings, with the frequency of 15 of 27 cases (62.5%). The month of December represented with the highest number of cases (5 cases of 23 cases); 2004 had the highest incidence of lightning strike which was 5 (19.23%). Lightning strike incidence occurred when victims had taken shelter (25.9%) under trees or shades. Lightning strike in open areas occurred in 10 of 27 cases (37.0%). Head and neck were the most commonly affected sites with the incidence of 77.78% and 74% respectively in all the victims. Only 29.63% of the cases presented with ear bleeding.
Mullerian duct anomalies are congenital abnormalities involving the female genital systems. A double uterus or uterine didelphys is one of the uterine duplication anomalies that result from impaired fusion with regard to the Mullerian ducts. The uterine didelphys can be diagnosed earlier in symptomatic patients. However, the diagnosis can be delayed or the patients may remain undiagnosed throughout their lifetime if asymptomatic. Pregnant women with uterine didelphys are at a greater risk for spontaneous miscarriage, malposition, premature labor, and cervical incompetence. Uterine didelphys are also associated with renal anomalies such as renal agenesis or duplex kidneys. We present a case of a 24-year-old primigravida who had a spontaneous pregnancy and underwent a lower segment cesarean section (LSCS) after a failed external cephalic version for the breech fetal position. The uterine didelphys was diagnosed postnatally. Her condition was associated with right renal agenesis and concomitant left renal calculus.
Sonic pressure wave should be taken into consideration in all ballistic injury. We review a young gentleman with ballistic injury of his lateral chest wall. The bullet trajectory passed through the lateral chest wall. Chest radiograph shows a wedge-shaped consolidation adjacent to the wound with blunted right costophrenic angle. Subsequent CT scan confirms the consolidation adjacent to the bullet trajectory. This case report emphasizes on value of CT in ballistic chest trauma and indirect injury caused by the sonic pressure wave of the bullet.
