    Primary non-Hodgkin's lymphoma of the brain is rare. Majority of the lesions are intraaxial, multicentric and involve the leptomeninges. We report a case of malignant primary non-Hodgkin's lymphoma arising from the cranial vault. Computed tomography of the brain showed an extraaxial lesion in the right parietal region mimicking a meningioma.

    Raised intraocular pressure (lOP) is generally held responsible for causing visual loss in chronic simple glaucoma. It is therefore desirable that a safe level of lOP be maintained all the time. Elevation of lOP with change of body position has been suggested as one of the factors which result in tissue damage in low tension as well as in primary wide open angle glaucoma. Postural behaviour of lOP was therefore studied in 58 normal and 30 glaucomatous Malaysian eyes. Clinical significance and possible pathogenesis of abnormal postural response of lOP has been discussed. More application of this simple procedure is advocated.

    Use of contraceptives by some selected characteristics was studied for married women in the reproductive age range (MWRA) in three south‐east Asian countries. The data were obtained from each country's national family planning/fertility survey conducted around the mid‐seventies. Only the gross relationship between each of the selected characteristics and contraceptive use was compared. The proportion of MWRA using contraceptives for Singapore was almost twice as high as the corresponding proportions for Peninsular Malaysia and Thailand. The pattern of contraceptive use was largely similar for Thailand and Malaysia but different from Singapore. The differences were largely attributed to the absence of a large rural population in Singapore, the apparently greater tendency of women in Singapore to use reversible contraceptive methods for birth spacing and the easier access by women to family planning information and services, particularly sterilisation, than in Peninsular Malaysia and Thailand.

    288 notifications of syphilis in military personnel for 1974 to 1978 are analysed. 56 cases are primary and 232 are latent syphilis in service personnel and 13 in women and children. Antenatal screening detected 56 cases (0.9%) of syphilis. Only 24 cases among soldiers had confirmatory tests done. Treatment varied and some were inadequately treated, others overtreated and many with incorrect schedules. Intramuscular penicillin was the treatment of choice in 85% of cases. Follow-up of cases was inadequate. In all, it indicated a poor control programme for STD. A good programme is possible within the armed forces set-up and a number of measures are suggested.

    One hundred patients with carcinoma of the cervix stages 1B to 4A were treated with intracavitary high dose rate radiation using a linear cobalt source. All cases have received external beam pelvic irradiation to 4500cGy mid plane in twenty fractions over four weeks. The results in terms of patient compliance and convenience were good while acute and late morbidities were comparable to standard Manchester technique of low dose rate intracavitary therapy as practised in the Institute of Radiotherapy and Oncology General Hospital Kuala Lumpur. The four year actuarial survival rate is 76% for stage II and 48% for stage III. All three stage IV patients died within 1 year. Four out of seven stage I patients are alive (minimum follow-up 18 months, longest 43 months). One died of systemic spread at 33 months while one is lost to follow up.

    The first two cases of human muscular sarcocystosis are reported from East Malaysia, in Sabalt and Sarawak respectively. Sarcocysts were seen iin biopsied specimen from the nasopharynx of both patients who had carcinoma of the nasopharynx. The measurements and appearances of the cysts and the zoites within the cysts were compared with the human cases of sarcocystosis previously reported in West Malaysia. Zoonotic and other aspects of these cases of East Malaysian sarcocystosis are discussed.

    An Indian family with four members having hereditary ataxia was presented. The inheritance was most likely autosomal dominant. The onset was at adult life. The main disability was cerebellar ataxia with pyramidal tract sign found at physical examination. Electroencephalography and nerve conduction study were abnormal in two cases where they were done. The clinical feature correspond to an intermediate form of hereditary ataxia.

    The ultrasound findings of 100 patients with chloedocholithiasis documented by cholangiography and/or surgery were reviewed retrospectively. Common duct stones were detected in 45% of patients. This detection rate which is comparable with most series confirmed the lack of reliability of ultrasound in the diagnosis of choledocholithiasis. All the stones detected were in dilated common ducts. The main limiting factor was overlying bowel gas which impair visualisation of the lower part of the common duct. Despite the apparent insensitivity of ultrasound to detect common duct stones, the modality is still a valuable non-invasive screening diagnostic tool, because in positive cases, patients may be spared from invasive cholangiographic procedures.

    Fifty-one consecutive arthroscopies of the knee were reviewed after an average of eight months follow-up. Thirty-eight patients underwent arthroscopic surgical procedures as well. A meniscus tear was the commonest (68%) primary pathology noted, followed by isolated anterior eructate tears (16%). Excellent/good results were obtained in 80% of arthroscopic partial meniscectomies. Synovial biopsies and loose body removal were all successfully achieved arthroscopically: Arthroscopic knee surgery is a useful and less traumatic technique of obtaining a diagnosis and treating amenable knee lesions compared to open arthrotomy.
