    Introduction: The aim of this study is to assess the outcomes of ala carte posteromedial release in children over two years of age who were not responding to the Ponseti method of treatment of idiopathic clubfoot.

    Material and Methods: A retrospective observational study from September 2013 to August 2015 was conducted at a tertiary level medical teaching institution. The clubfeet were classified according to the Harold and Walker classification. Radiographic parameters assessed were the talocalcaneal angle (AP, lateral), talus-first metatarsal angle (AP, lateral) and calcaneal-fifth metatarsal angle. The scar and the functional score, according to Laaveg and Ponseti, were evaluated as outcome measures at the final follow-up.

    Results: Twenty-four children with a mean age of 43.7 ± 24.7 months were enrolled in the study. There was a total of 36 clubfeet: 21 (65.6%) with a poor functional outcome; 12 (37.4%) with excellent to good scar in both horizontal and vertical components. There was a statistical significance between the pre-operative and post-operative radiological parameters (p<0.05). None of the patients presented with any limitation of activities of daily living despite the poor functional outcome in many of the children. There was no significant association between the qualities of scar (horizontal, vertical) and the functional outcome with age at presentation, pre-operative Harold and Walker classification and pre-operative radiographic angles.

    Conclusion: Surgical intervention in terms of ala carte posteromedial soft tissue release could not produce a good outcome over four years in CTEV. The threshold for surgery in CTEV should be high, given the poor results.

    MXene-based nanofluids are novel trends with improved dispersion stability and thermophysical characteristics over previously established nanofluids. In the present work, the thermohydraulic characteristics of a double pipe heat exchanger (DPHEX) with a Therminol55(TH55)/MXene + Al2O3 nanofluid and various geometrically shaped (triangular, rectangular) ribbed twisted tape (TT) inserts are numerically investigated using the ANSYS Fluent interface. A counter flow arrangement with three different types of inserts (RRTT, TRTT, TT) and TH55/MXene + Al2O3 nanofluid at 0.20 wt% are studied inside the heat exchanger. Adding ribbed inserts to the conventional TT insert enhances the turbulence intensity by creating extra vortices. The thermal boundary layer grows thinner due to increased axial and radial velocity. Due to the substantial flow obstruction, adding ribs increases the overall pressure drop between the inlet and outlet. The maximum increase in Nu is 11.04 % using nanofluid instead of water, whereas the combination of insert and nanofluid exhibited up to 105 % enhancement for rectangular-ribbed TT compared to the plain tube. Nevertheless, the pressure decrease is found to be maximum in rectangular-ribbed TT because of significant flow disruption. This was likewise true with triangular-ribbed TT and TT insert. According to the PEC assessment, the RRTT insert had a maximum PEC value of 1.67 greater than TRTT and traditional TT for both TH55 and nanofluid flowing inside the tube. To summarize, the combination of TH55/MXene + Al2O3 RRTT insert may be a promising choice for improving heat exchanger performance in a new generation efficient thermal system.

    In recent years, solar energy technologies have developed an emerging edge. The incessant research to develop a power source alternative to fossil fuel because of its scarcity and detrimental effects on the environment is the main driving force. In addition, nanofluids have gained immense interest as superior heat transfer fluid in solar technologies for the last decades. In this research, a binary solution of ionic liquid (IL) + water based ionanofluids is formulated successfully with two dimensional MXene (Ti3C2) nano additives at three distinct concentrations of 0.05, 0.10, and 0.20 wt % and the optimum concentration is used to check the performance of a hybrid solar PV/T system. The layered structure of MXene and high absorbance of prepared nanofluids have been perceived by SEM and UV-vis respectively. Rheometer and DSC are used to assess the viscosity and heat capacity respectively while transient hot wire technique is engaged for thermal conductivity measurement. A maximum improvement of 47% in thermal conductivity is observed for 0.20 wt % loading of MXene. Furthermore, the viscosity is found to rise insignificantly with addition of Ti3C2 by different concentrations. Conversely, viscosity decreases substantially as the temperature increases from 20 °C to 60 °C. However, based on their thermophysical properties, 0.20 wt % is found to be the optimum concentration. A comparative analysis in terms of heat transfer performance with three different nanofluids in PV/T system shows that, IL+ water/MXene ionanofluid exhibits highest thermal, electrical, and overall heat transfer efficiency compared to water/alumina, palm oil/MXene, and water alone. Maximum electrical efficiency and thermal efficiency are recorded as 13.95% and 81.15% respectively using IL + water/MXene, besides that, heat transfer coefficients are also noticed to increase by 12.6% and 2% when compared to water/alumina and palm oil/MXene respectively. In conclusion, it can be demonstrated that MXene dispersed ionanofluid might be great a prospect in the field of heat transfer applications since they can augment the heat transfer rate considerably which improves system efficiency.

    Till today, there is no effective treatment protocol for the complete clearance of Wuchereria bancrofti (W.b) infection that causes secondary lymphoedema. In a double blind randomized control trial (RCT), 146 asymptomatic W. b infected individuals were randomly assigned to one of the four regimens for 12 days, DEC 300 mg + Doxycycline 100 mg coadministration or DEC 300 mg + Albendazole 400 mg co-administration or DEC 300 mg + Albendazole 400 mg sequential administration or control regimen DEC 300 mg and were followed up at 13, 26 and 52 weeks post-treatment for the clearance of infection. At intake, there was no significant variation in mf counts (F(3,137)=0.044; P=0.988) and antigen levels (F(3,137)=1.433; P=0.236) between the regimens. Primary outcome analysis showed that DEC + Albendazole sequential administration has an enhanced efficacy over DEC + Albendazole co-administration (80.6 Vs 64.7%), and this regimen is significantly different when compared to DEC + doxycycline co-administration and control (P<0.05), in clearing microfilaria in 13 weeks. Secondary outcome analysis showed that, all the trial regimens were comparable to control regimen in clearing antigen (F(3, 109)=0.405; P=0.750). Therefore, DEC + Albendazole sequential administration appears to be a better option for rapid clearance of W. b microfilariae in 13 weeks time. (Clinical trials.gov identifier - NCT02005653).

    This study examines the impact of varying the porosity density of twisted tape inserts (TTI) on the temperature distribution, fluid velocities, heat transfer coefficients (HTC), Nusselt numbers (Nu), turbulent kinetic energy (TKE), and performance from 5000 to 12500 Reynolds numbers (Re). The entire process involved the design of TTIs and double pipe heat exchangers using SolidWorks. Subsequently, a three-dimensional fluid flow model was employed to solve equations related to energy mass, energy, and momentum within the ANSYS Fluent interfaces. The findings highlight the noteworthy impact of high porosity TTIs, which consistently reduce temperature spans, increase fluid velocities, and greatly HTC and Nu when compared to low porosity TTI, typical TTI, and plain tubes. Furthermore, high porosity TTI significantly increases TKE, indicating increased fluid turbulence and higher heat transfer efficiency, especially at Re = 12500. The assessment of PEC emphasizes the superiority of high porosity TTI, demonstrating their significant performance increase potential of over 6.44 % over low porosity TTI and a staggering 62.5 % above typical TTI. In conclusion, high porosity TTI emerges as a potential solution for improving heat transfer efficiency and overall system performance in a variety of industrial applications, promising enhanced energy efficiency and superior performance.

    Objectives: To examine the descriptive epidemiology of the patient population referred to paediatric rheumatology centres (PRCs) in Southeast Asia (SEA) and to compare the frequency of conditions encountered with other PRC populations.

    Methods: A web-based Registry for Childhood Onset Paediatric Rheumatic Diseases was established in 2009 and seven PRCs in four SEA countries, where paediatric rheumatologists are available, participated in a prospective 24 month data collection (43 months for Singapore).

    Results: The number of patients analysed was 4038 (788 from Malaysia, 711 from the Philippines, 1943 from Singapore and 596 from Thailand). Over 70% of patients evaluated in PRCs in Malaysia, the Philippines and Thailand had rheumatic diseases (RDs), as compared with one-half of the proportion seen in Singaporean PRCs, which was similar to the Western PRC experience. Among RDs diagnosed (n = 2602), JIA was the most common disease encountered in Malaysia (41%) and Thailand (61%) as compared with systemic vasculitides in the Philippines (37%) and Singapore (35%) among which Henoch-Schönlein purpura was the most prevalent. SLE and related diseases were more common, but idiopathic pain syndrome and abnormal immunological laboratory tests were rarer than those seen in the West. JIA subtype distributions were different among countries. Among non-RDs (n = 1436), orthopaedic and related conditions predominated (21.7-59.4%).

    Conclusion: The frequencies of RDs seen by SEA PRCs were different from those in the West. Systemic vasculitides and SLE were common in addition to JIA. Paediatric rheumatologist availability and healthcare accessibility partially explain these observed discrepancies.
