    Electrical injuries make up a relatively small portion of burn injuries. Safety measures in place on domestic electricity supply have reduced the occurrence of high voltage electrical injuries. We present the case of a young man who sustained a high voltage electrical injury on all four limbs. Early fasciotomy was performed on both his hands and forearms. Despite early compartment release, the left upper limb deteriorated and required amputation. In this article we discuss the indications, outcomes and complications of early fasciotomy.

    The coronavirus (COVID-19) has arisen as one of the most severe problems due to its ongoing mutations as well as the absence of a suitable cure for this virus. The virus primarily spreads and replicates itself throughout huge groups of individuals through daily touch, which regretfully can happen in several unanticipated way. As a result, the sole viable attempts to constrain the spread of this new virus are to preserve social distance, perform contact tracing, utilize suitable safety gear, and enforce quarantine measures. In order to control the virus's proliferation, scientists and officials are considering using several social distancing models to detect possible diseased individuals as well as extremely risky areas to sustain separation and lockdown procedures. However, models and systems in the existing studies heavily depend on the human factor only and reveal serious privacy vulnerabilities. In addition, no social distancing model/technique was found for monitoring, tracking, and scheduling vehicles for smart buildings as a social distancing approach so far. In this study, a new system design that performs real-time monitoring, tracking, and scheduling of vehicles for smart buildings is proposed for the first time named the social distancing approach for limiting the number of vehicles (SDA-LNV). The proposed model employs LiFi technology as a wireless transmission medium for the first time in the social distance (SD) approach. The proposed work is considered as Vehicle-to-infrastructure (V2I) communication. It might aid authorities in counting the volume of likely affected people. In addition, the proposed system design is expected to help reduce the infection rate inside buildings in areas where traditional social distancing techniques are not used or applicable.

    One of the main difficulties in designing online signature verification (OSV) system is to find the most distinctive features with high discriminating capabilities for the verification, particularly, with regard to the high variability which is inherent in genuine handwritten signatures, coupled with the possibility of skilled forgeries having close resemblance to the original counterparts. In this paper, we proposed a systematic approach to online signature verification through the use of multilayer perceptron (MLP) on a subset of principal component analysis (PCA) features. The proposed approach illustrates a feature selection technique on the usually discarded information from PCA computation, which can be significant in attaining reduced error rates. The experiment is performed using 4000 signature samples from SIGMA database, which yielded a false acceptance rate (FAR) of 7.4% and a false rejection rate (FRR) of 6.4%.

    Soft biometrics can be used as a prescreening filter, either by using single trait or by combining several traits to aid the performance of recognition systems in an unobtrusive way. In many practical visual surveillance scenarios, facial information becomes difficult to be effectively constructed due to several varying challenges. However, from distance the visual appearance of an object can be efficiently inferred, thereby providing the possibility of estimating body related information. This paper presents an approach for estimating body related soft biometrics; specifically we propose a new approach based on body measurement and artificial neural network for predicting body weight of subjects and incorporate the existing technique on single view metrology for height estimation in videos with low frame rate. Our evaluation on 1120 frame sets of 80 subjects from a newly compiled dataset shows that the mentioned soft biometric information of human subjects can be adequately predicted from set of frames.

    Artificial intelligence (AI) has become a crucial element of modern technology, especially in the healthcare sector, which is apparent given the continuous development of large language models (LLMs), which are utilized in various domains, including medical beings. However, when it comes to using these LLMs for the medical domain, there's a need for an evaluation platform to determine their suitability and drive future development efforts. Towards that end, this study aims to address this concern by developing a comprehensive Multi-Criteria Decision Making (MCDM) approach that is specifically designed to evaluate medical LLMs. The success of AI, particularly LLMs, in the healthcare domain, depends on their efficacy, safety, and ethical compliance. Therefore, it is essential to have a robust evaluation framework for their integration into medical contexts. This study proposes using the Fuzzy-Weighted Zero-InConsistency (FWZIC) method extended to p, q-quasirung orthopair fuzzy set (p, q-QROFS) for weighing evaluation criteria. This extension enables the handling of uncertainties inherent in medical decision-making processes. The approach accommodates the imprecise and multifaceted nature of real-world medical data and criteria by incorporating fuzzy logic principles. The MultiAtributive Ideal-Real Comparative Analysis (MAIRCA) method is employed for the assessment of medical LLMs utilized in the case study of this research. The results of this research revealed that "Medical Relation Extraction" criteria with its sub-levels had more importance with (0.504) than "Clinical Concept Extraction" with (0.495). For the LLMs evaluated, out of 6 alternatives, ( A 4 ) "GatorTron S 10B" had the 1st rank as compared to ( A 1 ) "GatorTron 90B" had the 6th rank. The implications of this study extend beyond academic discourse, directly impacting healthcare practices and patient outcomes. The proposed framework can help healthcare professionals make more informed decisions regarding the adoption and utilization of LLMs in medical settings.

    INTRODUCTION: Methanol poisoning usually occurs in a cluster and initial diagnosis can be challenging. Mortality is high without immediate interventions. This paper describes a methanol poisoning outbreak and difficulties in managing a large number of patients with limited resources.

    METHODOLOGY: A retrospective analysis of a methanol poisoning outbreak in September 2018 was performed, describing patients who presented to a major tertiary referral centre.

    RESULT: A total of 31 patients were received over the period of 9 days. Thirty of them were males with a mean age of 32 years old. They were mostly foreigners. From the 31 patients, 19.3% were dead on arrival, 3.2% died in the emergency department and 38.7% survived and discharged. The overall mortality rate was 61.3%. Out of the 12 patients who survived, two patients had toxic optic neuropathy, and one patient had uveitis. The rest of the survivors did not have any long-term complications. Osmolar gap and lactate had strong correlations with patient's mortality. Serum pH, bicarbonate, lactate, potassium, anion gap, osmolar gap and measured serum osmolarity between the alive and dead patients were significant. Post-mortem findings of the brain were unremarkable.

    CONCLUSION: The mortality rate was higher, and the morbidity includes permanent visual impairment and severe neurological sequelae. Language barrier, severity of illness, late presentation, unavailability of intravenous ethanol and fomipezole and delayed dialysis may have been the contributing factors. Patient was managed based on clinical presentation. Laboratory parameters showed difference in median between group that survived and succumbed for pH, serum bicarbonate, lactate, potassium and osmolar and anion gap. Management of methanol toxicity outbreak in resource-limited area will benefit from a well-designed guideline that is adaptable to the locality.

    INTRODUCTION: The burden of cardiovascular disease and early morbidity and mortality is exacerbated by hypertension. According to the 2019 National Health and Morbidity Survey, 30% of adults in Malaysia aged over 18 years had hypertension. Our study aimed to investigate the association between medication adherence and blood pressure control and the factors associated with antihypertensive medication adherence.

    METHODS: This cross-sectional study was conducted among adult patients with hypertension who had been under treatment at public primary health clinics in the Melaka Tengah District. A self-administered questionnaire was used, and blood pressure was measured. Descriptive and multivariate logistic regression analyses were conducted using the Statistical Package for the Social Sciences (version 28).

    RESULTS: A total of 1531 patients were recruited in this study. Among them, 74.1% had good antihypertensive medication adherence, and 51.4% had their blood pressure controlled. Medication adherence was significantly associated with blood pressure control (P<0.005). The multivariate analysis showed that the determinants for antihypertensive medication non-adherence were Malay ethnicity, secondary education, farther distance from the clinic, experience of side effects of antihypertensive medications, concern about long-term side effects and usage of alternative medicine (P<0.05). Taking multiple antihypertensive medications was the only factor associated with uncontrolled blood pressure (P<0.05).

    CONCLUSION: Addressing issues on medication adherence is important to ensure blood pressure control. The factors associated with non-adherence should be closely monitored to improve blood pressure control and prevent adverse health outcomes. Single-pill combination antihypertensive medications are encouraged to reduce pill burden and improve blood pressure control.
