    INTRODUCTION: The maxillomandibular relationship (MMR) record is a critical step to establish the new occlusion in implant supported complete mouth rehabilitation. Using patients existing denture for recording the MMR requires implant definitive cast to be modified extensively to completely seat the denture (with unaltered flanges) on it. This may influence the correct seating of the denture on the implant definitive cast causing faulty recording of the MMR.
    MATERIALS AND METHOD: Elastomeric record bases, reinforced with the resin framework, are fabricated and relined with the light body elastomeric material when all the healing abutments are in place. The MMR is recorded with these elastomeric record bases using vacuum formed facial surface index of the occluded existing dentures as a guideline.
    RESULTS: The elastomeric record bases with facial surface index of the existing dentures can allow clinicians to record MMR records without removing the healing abutments from the mouth with acceptable accuracy. This can save chair-side time of the procedure. The record of facial surfaces of existing complete denture in the form of vacuum formed sheet helps to set the occlusal vertical dimension.
    CONCLUSION: Use of facial surface index together with the elastomeric record bases can be the useful alternative technique to record the MMR in patients with implant supported full mouth rehabilitation. Further study is required to prove its routine clinical utility.

    The purpose of this report is to introduce Cu-sil like denture as a functional space maintainer. Here, we report two paediatric cases treated with Cu-sil like denture with multiple edentulous spaces and partially erupted/compromised permanent teeth. Cu-sil like denture not only serves as removable partial functional space maintainer, but also restores the vertical dimension of occlusion, mastication and aesthetics in children. Cu-sil like denture is used in elderly patients who are not willing for extraction of remaining few healthy teeth in the oral cavity. This concept utilizes the remaining natural teeth in the arch, accommodates them within the denture through perforations made in the denture base. The gap between the denture base and the tooth is sealed using a resilient liner. Such denture is used for the paediatric age group in this report for functional rehabilitation temporarily. The report describes the pros and cons of the Cu-sil like denture use in children with technique of preparation along with review of literature.

    BACKGROUND: This study aims to determine the hard and soft tissue shape variation and its changes in Class II division 1 malocclusion before and after orthodontic treatment using Geometric Morphometric Analysis.

    METHODS: This retrospective study included 141 pre-treatment and near-end treatment lateral cephalometric radiographs of Class II division 1 malocclusion patients aged 16-40 years with a skeletal II pattern (ANB > 4o). 32 landmarks in Cartesian coordinates were created and identified using MorphoJ software to establish a shape analysis.

    RESULTS: The vertical dimensions (hypodivergent to hyperdivergent facial profiles) showed the largest variation in the general shape of hard and soft tissue, followed by the anteroposterior dimensions (mild to severe skeletal II patterns). Variations of lip shape (long to short), lip protuberance (everted to inverted), and nasolabial angle (obtuse to acute) were present.     STATEMENT OF PROBLEM: Resin-bonded fixed partial dental prostheses (RBFPDPs) cemented at an increased occlusal vertical dimension (OVD) (the Dahl concept) to create space for a metal retainer remain controversial because of the lack of reported clinical studies. This study evaluated RBFPDPs by applying the Dahl concept.

    PURPOSE: The purpose of this retrospective study was to investigate the clinical factors affecting the occlusal re-establishment and short-term complications of RBFPDPs cemented at an increased OVD.

    MATERIAL AND METHODS: Occlusal re-establishment and clinical complications were reviewed in 109 participants (155 RBFPDPs) treated at the Faculty of Dentistry, Universiti Teknologi MARA from January 2013 to May 2018. Types of complications and risk factors were assessed from clinical data. Demographic factors, clinical factors, and prosthesis factors were distinguished and their association with occlusal re-establishment analyzed by using multiple regression analysis.

    RESULTS: A total of 155 RBFPDPs (42 anterior, 113 posterior) were placed in 109 participants. All achieved occlusal re-establishment, 89.9% complete occlusal re-establishment and 10.1% partial occlusal re-establishment, a minimum of 4 months after the RBFPDPs had been placed at an increased OVD.     STATEMENT OF PROBLEM: Space creation for a metal retainer of the resin-bonded fixed partial dental prostheses (RBFPDPs) with the Dahl concept remains controversial because of the lack of clinical studies.

    PURPOSE: The purpose of this clinical study was to investigate the occlusal force and occlusal contact reestablishment of RBFPDPs cemented at an increased occlusal vertical dimension (the Dahl concept) and to evaluate the factors affecting them.

    MATERIAL AND METHODS: A prospective clinical study was carried out on 28 participants receiving cantilevered RBFPDPs at an increased occlusal vertical dimension at the Faculty of Dentistry, University Teknologi MARA. Maximum occlusal forces were recorded at precementation, postcementation, and 12-week review visit by using pressure indicating film, while occlusal contact reestablishment was assessed at the precementation stage and 12-week review visit by using shim stock foils. The Wilcoxon signed-rank and chi-squared tests were used for statistical analysis (α=.05).

    RESULTS: Significant differences were found for maximum occlusal force between the precementation and the postcementation and between the postcementation and the 12-week review of RBFPDPs (P.05).

    CONCLUSIONS: Overall, occlusal force was reestablished after 12 weeks, and occlusal contact was completely reestablished in most participants after placement of RBFPDPs at an increased occlusal vertical dimension.

    The management of patients with severely worn dentition is challenging due to the loss of occlusal vertical dimension and tooth structure creating an uneven plane of occlusion. This case report describes the importance of every step of the conventional and improvised methods in treating tooth wear patients. Stages from the initial work-up of tooth wear assessment, substantial surgical crown lengthening, the controlled method of increasing vertical dimension, the precise method of crown preparations, advanced impression techniques till the cementation procedure of final restorations.

    The whole treatment was in a reorganized approach such that the new inter-cuspal position (ICP) coincided with the retruded axial position (RAP). When restoring worn dentition, cl inician should always have a proper planning, decent implementation for each stages thus guarantee excellent performances. However maintenance and recall visits are the main keys to long term success.

    AIM: To assess the knowledge and perceptions of Malaysian government dentists regarding the shortened dental arch (SDA) concept and its application in clinical practice.

    BACKGROUND: The SDA concept refers to a specific type of dentition with intact anterior teeth and a reduction in posterior occlusal pairs. Dentists' knowledge and perceptions of the SDA concept can influence its application in clinical practice.

    METHODS: A self-administered questionnaire on the SDA concept was distributed to 326 government dentists in the states of Selangor and Kuala Lumpur, Malaysia. The data were analysed using SPSS version 22 software.

    RESULTS: The response rate was 84.0%. The majority of respondents had good knowledge on five of six knowledge items and good attitudes towards 10 of 17 perception items. However, only one-fifth (20.4%) reported having applied the SDA concept in the clinic. A larger number of participants who graduated locally than who graduated abroad perceived that patients <60 years of age, without molar support, can attain acceptable chewing function and that SDA treatment does not lead to loss of occlusal vertical dimension (P < 0.05). A larger number of participants with ≤5 years of work experience than with >5 years of work experience perceived that the SDA concept enables simpler treatment planning (P < 0.05). Finally, a larger number of participants who graduated abroad than who graduated locally observed that patients without molar support had temporomandibular joint problems (P < 0.05).

    CONCLUSION: Although Malaysian government dentists have good knowledge and perceptions of the SDA concept, it is not widely applied in the clinic. Concerted efforts in SDA training of dentists are needed to help to shorten denture waiting lists and reduce costs.

    This article describes a technique of constructing a set of maxillary and mandibular complete dentures in three visits instead of the usual five clinical appointments. This system of complete-denture construction is made possible because of the combined use of visible light-cured material as an impression tray and record base material, as well as the use of new biometric wax occlusion rims. Unlike some earlier techniques that use light-cured resin composites as the denture base materials, this method retains the use of heat-cured polymethylmethacrylate as the denture base material.

    The aims of this study were to assess the quantitative values of measurements using proportion indices in the craniofacial region in patients with repaired, non-syndromic, complete unilateral cleft lip and palate (UCLP), and compare them with a control group who did not have clefts using the non-invasive systems of 3-dimensional technology. Three-dimensional measurements of the facial surfaces of 15 Malay patients who had UCLP repaired and 100 Malay control patients aged 18-25 years were analysed. The 3-dimensional images of the respondents' faces were captured using the VECTRA-3D Stereophotogrammetry System. Eleven craniofacial proportions were assessed using a combination of 18 linear measurements obtained from 21 anthropometric soft tissue landmarks. These measurements were used to produce proportion indices to find the differences in the morphological features between the groups, and assessed using the independent sample t test and z scores. There were significant differences between the groups in 7 out of 11 craniofacial proportion indices (p=0.001-0.044). Z scores of 2 indices were disproportionate. They were nasal index (which was severely supernormal) and upper lip index (which was moderately supernormal). Patients with UCLP had higher mean z scores, indicating that patients with UCLP tended to have larger faces than the control group. There were clinically important differences mainly in the nasolabial area, where the nose and the upper lip were wider, larger, or flatter in patients with UCLP.

    Thalassaemia is a public health problem in Malaysia. It is known to cause skeletal deformity. The purpose of this study was to compare the skeletal, dentoalveolar, and soft tissue features of Malay transfusion-dependent thalassaemia (TDT) patients with a Malay control group. Lateral cephalometric radiographs of 30 Malay (14 males and 16 females aged 6.4-21.8 years) TDT patients and 60 normal Malays matched for chronological age and gender were analysed and compared using an independent t-test. The TDT group showed a similar sagittal relationship to the control group but with a significantly increased (P < 0.01) mandibular plane inclination. They also showed a significantly shorter (P ≤ 0.001) mandibular body, ramus length, and posterior face height and consequently a smaller ratio of posterior to anterior face height (P < 0.01). The upper and lower lips were significantly procumbent (P < 0.001) in the TDT group together with a significantly smaller nasolabial angle (P < 0.05). Dentoalveolar measurements showed less proclined maxillary teeth in the TDT group compared with the controls (P < 0.05). The cephalometric features of Malay TDT patients were characterized by a mild Class II skeletal pattern, prominent vertical growth direction of the mandible, and protruded upper and lower lips.

    This article describes the occlusal rehabilitation of a partially edentulous patient who did not want a removable partial denture. Implants and extensive fixed restorations were used to restore posterior support and treat severely worn dentition, respectively. The treatment offered the patient a functional and esthetic result.
