    Limb problem is a common complication for patients with diabetes. While the impact of diabetic limb problems
    on physical health is well known, the psychological impact of the condition is still largely unknown. People with
    diabetes with or without foot ulcers frequently suffer from major depressive disorder, and this is made worse
    by the presence of limb complications. Furthermore, depression itself can have negative consequences on the
    person’s diabetes. Hence, in caring for people with diabetic foot problem, the psychological sequelae must not
    be overlooked but managed accordingly

    Ewing sarcoma is a primary bone malignancy that rarely occurs in the hand. Resection and reconstruction will usually result in reduced or loss of thumb function. We describe a case of successful transplantation of non- vascularized osteoarticular second metatarsal autograft following wide resection of Ewing sarcoma of first metacarpal in an 11-year-old girl. The capsule of the graft’s metatarso-phalangeal joint was repaired to achieve mobility of the joint. This enabled preservation of thumb function by a relatively simple surgical technique. Detailed surgical procedures and excellence function after 2 years following surgery are described.

    Rapunzel syndrome is a syndrome whereby a gastric trichobezoar (hair ball) extends through the pylorus, in the form of a long tail, to cause gastric outlet obstruction. It was first described by Vaughan et al. in 1968. The syndrome is mostly seen in young females with psychiatric illness. Presentations can be non-specific especially in the early stages. If left untreated it may leads to severe complications, which may include gastric ulceration, intestinal obstruction, perforated viscus and obstructive jaundice. Treatment is essentially surgical and psychi- atric consultation is necessary to prevent relapses. We present the case of a 26-year-old mentally subnormal lady with gastric trichobezoar and provide a review of the literature.

    Solid papillary breast carcinoma is a rare type of breast carcinoma. We present four cases which highlight the radiological and histological findings of solid papillary carcinoma. Mammogram supplemented with ultrasound played an important role in detecting solid papillary carcinomas which usually presents as intraductal lesions or intracystic mass with Doppler signal. Excision biopsy is a better option than core biopsy as the latter modality may not be representative and the diagnosis of solid papillary breast carcinoma may be missed.

    We describe a case of median nerve compression by gouty tophi. This is a rare condition and presented with loss of function with paraesthesia of both hands in addition to multiple tophis. Surgery was chosen for immediate relief, where a debulking of left flexor digitorum superficialis with decompression of carpal tunnel was done and resulted in improvement of symptoms but became complicated due to inadequate physiotherapy.

    The Community Health Posting teaching module is incorporated in the fourth year medical curriculum at Universiti Sains Islam Malaysia (USIM). The integration of Islamic principles and values in the medical curriculum is emphasized during the Community Health Posting. The Community Health curriculum allow students to appreciate and understand the medical and fiqh aspects of health and disease, the social issues in medical practice and research and to inculcate the practice of Islamic professional etiquettes. The teaching module illustrates the relevance of humanities in understanding illness and medical care within the community. Teaching and learning activities include components that enable the students to explore a wide range of influencing factors and how these affect the patients and their families. Issues pertaining to psychosocial and ecological perspectives of the community are also discussed. This posting utilizes various teaching and learning techniques such as lectures, tutorials, seminars, group discussions, educational visits, practical sessions and patient bedside teaching. In addition, the students are equipped with Islamic knowledge through the integration of Naqli and Aqli components in the Community Health Posting curriculum.

    ntroduction: Currently, researchers are aiming to explore herbal plants to replace synthetic drugs because herbal plants contain high active compounds and fewer side effects. Our study was done to determine the antibacterial activity of Eurycoma longifolia Jack (E. longifolia) root using ethanol based extract. Methods: Five types of pathogenic bacterial strains were used; Gram-positive (Staphylococcus aureus and Bacillus ce- reus) and Gram-negative (Escherichia coli, Salmonella typhi and Pseudomonas aeruginosa). Disc diffusion assay and Minimum Inhibitory Concentration (MIC) tests were used to determine the inhibition zone and turbidity of suspension which reflects the antibacterial activity of the extract. Results: The ethanolic extract of E. longifolia Jack root extract showed positive results against Gram-positive bacteria (S. aureus and B. cereus) and Gram- negative (S. typhi). B.cereus and S.typhi showed inhibition zone values of 11.76mm and 14.33mm at the extract concentration of 150mg/ml that were higher than the positive control values (9.00, 12.67mm) respectively. However, E. coli and P. aeruginosa did not show any inhibition by the ethanol-based extract. Conclusion: From the results we can conclude that E.Longifolia root extract possesses antibacterial activity that can be further explored to produce new medicinal products.

    Introduction: In Malaysia, medical specialisation training is mainly carried out by the public universities. Methods: A survey was carried out to explore the views of medical specialists in the country on issues such as structure of medical specialty training, availability of human resource, public/private sector competition, competency and apprenticeship, and its impact on assurance and quality of medical specialty training. Results: Altogether 238 medical specialists from 30 hospitals and medical institutions in the country participated in the survey. Conclusion: Among the findings, competition for human resource between public and private sector and lack of uniformity on medical specialty training across universities in the country are among the issues found to be of concern. There is also a need to address governance issue which necessitates to clearly delineating what constitutes medical specialty and what constitutes a subspecialty so that an agreed uniformed nomenclature is exercised across all stakeholders. The respondents also strongly agreed on the need to ensure competence in medical specialist training.

    Introduction: Leptin is an adipokine that has strong correlation with the body mass index (BMI). Gestational diabetes mellitus (GDM) is a common medical complication associated with pregnancy. Leptin may lose its correlation with the body mass index (BMI) during diabetes due to hormonal rearrangement. Diet control is the first line management in GDM. Leptin reported to increase in pregnancy and further increases in diabetic patients during GDM screening. There is paucity in the reports concerning Leptin levels in GDM patients on diet control. The present study was aimed to evaluate the changes in maternal leptin in pregnancy complicated by GDM on diet control compared to the normal pregnancy in the 3rd trimesters by comparing the means and to find the correlation of Leptin with the body mass index in both groups.
    Methods: The study included 2 groups: normal pregnancy (n = 40) and pregnancy with GDM under diet control (n = 60) both groups are at 38-40 weeks of gestation. Leptin concentration in serum was measured in both groups and statistically tested using student t test. The BMI were measured and correlated with the Leptin level in test groups.
    Results: the results indicated that Leptin will nearly triple in the third trimester (38±30 ng/ml) of pregnancy compared to the standard normal non-pregnant. Leptin level was significantly lower in diabetic women on diet control (28±16 ng/ ml) when compared with the non-diabetics (38±30 ng/ml). The hormone has no correlation with the age of the patients but have a positive correlation with the body mass index before and during pregnancy in both groups.
    Conclusion: Leptin is increasing in pregnancy as part of the physiological changes. Dieting can decrease Leptin level in diabetics’ pregnant women. Diet can restore the hormonal dysregulation of Leptin. Assessment of Leptin level might be used as an indicator for good diet control during pregnancy.

    Introduction: Praying represents a fundamental activity of daily living in Muslim community. Muslims need to adopt several postures that require deep flexion of the knee and hip to perform this daily obligation. This is a preliminary report of the study on hip and knee range of motion conducted to obtain normative passive and functional range of flexion data during Muslim prayers in both weight-bearing joints of normal Muslim adults. Methods: A cohort of Malay men aged between 20 to 30 years was recruited in this cross-sectional study. Passive range of motion and flexion angles of the joints in various postures during prayer were measured using a standard goniometer. The difference against existing normative range of motion database available and the influence of various factors were analysed. Factors analysed include body mass index and other anthropometric measurements. Results: Sixty participants recruited for the preliminary phase of this study. The mean (SD) of passive hip and knee flexions were higher compared to other existing normative range of motion database. A unit increase in body mass index will have 0.782 units lower of knee range of motion. The hip and knee flexion arc were from 74.1° to 119.0° and from 3.3° to 119.7° respectively. Conclusion: The range of motion involved for prayers was more for the knee but less for the hip as compared to the measured passive range of motion. Body mass index has a significant linear negative relationship with the passive range of motion of the knee, but not the hip.

    Introduction: Laparoscopic fundoplication is performed for the management of symptomatic hiatus hernias and gastro-oesophageal reflux disease (GORD) refractory to medical therapy. We adopted the use of Gore Bio-A® for selected laparoscopic hiatus hernia repairs in 2011 and with this case series aimed to establish whether mesh augmentation affects symptomatic outcomes. Methods: A retrospective review of prospectively collected data from all laparoscopic fundoplications performed by a single surgeon between October 2011 and January 2013 was performed. Patient specific data were entered into a proforma and analysed using Microsoft ExcelTM. Patient reported outcomes were assessed with a system specific quality of life questionnaire (GORD-HRQL) both pre and post-operatively. Results: Twenty-three patients underwent laparoscopic fundoplication during the study period. Gore Bio-A® re-enforcement of the hiatal repair was used in 14 patients and was the preferred option for those with pre-operative evidence of a large hiatus hernia.     Dengue is an arthropod borne disease that has become important worldwide. There is still no specific drug available for treatment and also no protective vaccine that can be used. As such, specific diagnosis is essential to enable good management and prevention of large outbreaks. Diagnosis today in many countries is still based on serology though the detection of NS1 has slowly become incorporated. Diagnosis is critical for early intervention with specific preventive health measures to prevent fatalities and also to curtail spread and reduce economic losses. Serological assays mainly detect IgM which now as a single test is invalid unless a second sample is taken to confirm. As such to effectively diagnose dengue at all stages of infection, assays with two or more markers are required or two samples taken a few days apart. Other commonly used tests include NS1 detection, nucleic acid amplification and IgG detection. However the sensitivities of the current commercial kits vary quite considerably and have to be interpreted with caution. Hence knowledge of this disease is essential when conducting diagnostics for dengue.

    Dengue is a serious mosquito borne disease common in tropical and sub-tropical countries including Malaysia. There is at present a lack of specific treatment and an effective tetravalent vaccine against dengue. The control of dengue depends solely on the suppression of the two most important vectors namely, Aedes aegypti and Ae albopictus. Despite intensive and extensive control efforts by health agencies, the disease continues to spread. This paper updates various innovations on control of dengue vectors. Gene-based sterile insect technique using the RIDL technology for both Aedes aegypti & Ae albopictus control has now been actively researched and field trials are pursued to evaluate the effectiveness of the technology. The release of Wolbachia-infected Ae aegypti is another dengue control innovation. The infected mosquito cannot support development of dengue virus and has shorter life span. Other innovations include: auto-dissemination of insect control agents using ovitrap, autocidal adult and larva trap, outdoor residual spraying, insecticidal paint and biocontrol agent. In other innovation, outbreak prediction capability is enhanced by developing model based on environmental data and analysis utilising neural network.

    The trend of dengue incidence in the regions and many countries has shown an increasing trend for the past few decades. The World Health Organization (WHO) reported that the incidence increased dramatically over the last 50 years and that dengue virus infections expanded to new countries, and from urban to rural settings. Malaysia is one of the countries that experience similar situation. The trend of dengue incidence in Malaysia has continued to increase since 2001 until 2014. In 2001, the dengue incidence rate (IR) was 72 cases in 100,000 population and progressively increased to 361 cases in 100,000 populations in 2014. There were temporary decrease of the incidence rate in 2011 and 2012 to 69.9 and 76 cases per 100,000 populations. Despite the close monitoring and continuous efforts from the Ministry of Health and Municipals to conduct the prevention and control activities, the number of dengue cases continues to increase due to multiple uncontrolled factors. There are at least five major factors that influence the transmission of dengue disease which include the dengue virus, the human as the host, the environmental condition such as cleanliness, the vectors and its behavior and the climate change. Due to these multiple factors that influence the disease pathophysiology and transmission of dengue virus, the control of mosquito-borne viral infection is very challenging and different from managing other infectious diseases. In addition, the rapid urbanization, population growth and human behavior together with international travel, making the control of dengue transmission even more difficult. Based on the circumstances, Ministry of Health Malaysia has implemented the intergrated strategy for dengue prevention and control program in the National Dengue Strategic Plan (NDSP) since 2011. There are seven strategies included in the NDSP which is strengthening of the dengue surveillance, practicing intergrated vector management, emphasizing on the dengue case management, social and community mobilization towards the prevention activity, ensuring rapid response in managing the dengue outbreak and developing new innovative method through dengue research. Most of the factors that contribute to the occurrence of dengue cases are difficult to be controlled and these leave with only manipulation and intervention with the environment, vector control and changing the human behavior for the prevention and control of dengue.

    The incidence of dengue in Malaysia has increased from 15.2 per 100,000 in 1973 to 361.0 per 100,000 population by the year 2014. The same trend was seen in case fatality rates that went up from 0.16% to 0.62% between 2000 to 2013.1,2 More than 60% of the cases were from the Klang Valley region. In Pahang the dengue incidence rates from 2004 to 2014 showed a similar increase from 40.9 to 193.8 per 100,000 population and an increase in case fatality rates from 0.07% to 0.3% too was observed between 2000-2014. Fortunately these figures were below the national average.3

    Simulium (G.) bakalalanense, is described based on male, pupa and larvae collected in Mount Murud, Sarawak, Malaysia. S. (G.) bakalalanense sp. nov. is placed in the S. batoense species-group and is distinguished from most of known species of the group by the pupal gill with an inflated basal fenestra, shoe-shaped cocoon, and lack of grapnel-shaped hooklets on the pupal abdominal segment 9. Three known species, S. (G.) terengganuense Takaoka, Sofian-Azirun & Ya’cob from Peninsular Malaysia, S. (Simulium) alberti Takaoka from Sabah, and S. (S.) beludense Takaoka from Sabah, are newly recorded from Sarawak.

    The global demand for edible bird nests (EBNs) is high, especially from Hong Kong and Peoples Republic of China. Recently, this industry was greatly affected when China banned the import of all the EBNs from Malaysia (except for canned version) due to detection of high levels of nitrites. Several cases of anaphylaxis following ingestion of EBNs were reported. The source(s) of these allergens remain unknown. Mites have been reported to trigger allergic responses. Hence, this study was designed to quantify, isolate and identify the mites that are associated with EBNs. The raw EBNs were purchased from swiftlet farms in five locations in Peninsular Malaysia while the commercial nests were purchased from five different Chinese traditional medicinal shops. The average mite density of all the raw nests was 285 ± 603 mites per gram of EBN while the commercial nests had a much lower mean value of 21 ± 32 mites per gram of EBN (p = 0.082). Among the raw EBNs, the nests from Kajang had the highest average mite density (946 ± 1443 mites/g of EBN) whereas the nests from Kuala Sanglang had the lowest (54 ± 34 mites/g of EBN). Among the commercial EBNs, the nests from Company D had the highest average mite density (76 ± 18 mites/g of EBN) whereas the nests from Company A were free of mites. Overall, the average densities of mites in the raw nests obtained from southern regions of Malaysia (Selangor and Johor) were higher than those nests obtained from the northern regions (Kedah and Kelantan). Thirty types of mites were isolated from both the raw and commercial nests. Among these, some are
    probably feather mites (Eustathia cultrifer, Pteroherpus garrulacis, Pterodectes amaurochalinus, Laminalloptes sp., Berlesella alata and Neochauliacia sp.), house dust and storage mites (Suidasia sp., Austroglycyphagus sp., and Aleuroglyphus ovatus), mesostigmatid mites (Dermanyssus sp.), prostigmatid mites (Cheyletus sp., tarsonemid and cunaxid mites), astigmatid mites (Collocalidectes sp., Streetacarus sp. and Hemisarcoptes sp.) and oribatid mites. This study provides baseline information on the density and type of mites that are probably associated with EBNs. This study also heightens the importance of mites as a possible source of EBN-associated anaphylaxis.

    Malaria remains one of the most important communicable diseases. A rapid, simple and accurate method is a crucial part of malaria diagnosis. The aim of this study was to reevaluate the microwave irradiation method to extract DNA from Plasmodium falciparum and compare with six other existing DNA extraction methods such as QIAamp DNA mini kit (Qiagen), FTA elute card, phenol-chloroform, Chelex, Chelex without proteinase-K and Rapid boiling. Two different P. falciparum isolates were used: (i) Laboratory strains with 0.3% parasitemia and (ii) clinical isolate with 0.6% parasitemia. Each DNA extraction method was validated for the presence of P. falciparum by a routine nested and real time PCR. In order to evaluate the sensitivity of the DNA extraction by microwave, double serial dilution of P. falciparum from in vitro culture at parasitemia that ranged from 0.0001 to 0.17% were used to extract the DNA by microwave and the P. falciparum DNA was then detected by nested and real-time PCR. The nested and real-time PCR were able to detect. P. falciparum DNA at
    the parasitemia level as low as 0.0003% and 0.0001%, respectively. Our results can reproduce
    the results from earlier studies and reveal microwave as a rapid and simple tool to extract P.
    falciparum DNA and subsequent molecular diagnosis of malaria.

    Malaria is still endemic in Sarawak and Sabah. Numerous studies have indicated that patients with malaria are commonly co-infected with helminthes particularly in endemic regions. The aim of this study was to investigate the incidence of soil-transmitted helminth (STH) infection among malaria patients using microscopy and multiplex real-time PCR at two district hospitals in Sarawak. A total of 94 patients who were clinically-suspected to have malaria were confirmed to be infected by both microscopy and multiplex real-time PCR. By the molecular method, 23.4%, 74.5% and 2.1% of the samples were positive for Plasmodium falciparum, P. vivax and mixed P. falciparum and P. vivax, respectively. Among the malaria patients, 48.9% were found to be co-infected with STHs. In comparison, microscopic examinations showed that 6.4% of the malaria patients were infected with STHs. From the real-time PCR positive samples, 31.9% had single helminth infections while 17% had mixed infections. In conclusion, this study showed that almost half of the malaria patients at the
    two Sarawak hospitals were co-infected with helminth. Future studies should be specifically designed to determine if there is any correlation between the two infections in terms of incidence and intensity.

    Amoebic liver abscess in pregnancy is genuinely rare in its presentation. Yet, the main issue surrounding this agenda is the diagnostic challenge that it poses especially when symptomatology is vague and clues are subtle which altogether evades the diagnosis proper. We would like to dwell mainly on these issues in hopes of enlightening clinicians towards these diagnostic dilemmas. We report an extremely rare case of amoebic liver abscess occurring in the third trimester of pregnancy in a 29-year-old lady living in an interior village in Sabah. It was a combination of biochemical, radiographic and molecular investigations that ultimately led to the final diagnosis. In lieu of the high risk of mortality amongst pregnant mothers afflicted with amoebic liver abscess, the inherent need for early diagnosis requiring a high index of suspicion is vital. Elevated alkaline phosphatase alongside neutrophilia appears to be the most consistent liver parameters in guiding clinicians towards the presence of liver abscess.
