    This report presents a review of the Malaysian Journal of Medical Sciences' (MJMS) performance status throughout 2017, which covers the submission pattern of original manuscripts by month, the geographical distribution of submitting authors, the types of manuscripts and overall acceptance/rejection rates. As the years progress, MJMS continues to receive an escalating number of manuscript submissions. This contributes to an ever-increasing workload, which makes administrative tasks continually more challenging. Although the manuscript submission platform seeks to minimise the pre-publication workload of the journal administrator, it is still a time-consuming task, particularly when authors seek exclusive attention for their submitted manuscripts.

    This review highlights the clinical scoring tools used for the management of acute pharyngotonsillitis in primary care clinics. It will include the prevalence of group A pharyngotonsillitis among children and adults worldwide and the selective tests employed for diagnosing group A streptococcal pharyngotonsillitis. Pharyngotonsillitis is one of the common reasons for visits to primary care clinics worldwide, and physicians tend to prescribe antibiotics according to the clinical symptoms, which leads to overprescribing antibiotics. This in turn may lead to serious health impacts and severe reactions and may promote antibiotic resistance. These significantly add on to the health care costs. The available information from health organisations and previous studies has indicated the need to manage the diagnosis of pharyngotonsillitis to improve prescribing habits in primary care clinics.

    Glioblastoma multiforme (GBM) is one of the most common primary brain tumours in adults, accounting for almost 65% of all cases. Among solid tumours, GBM is characterised by strong angiogenesis, including the highest degree of vascular proliferation and endothelial cell hyperplasia. Despite numerous improvements in existing treatment approaches, the prognosis of GBM patients remains poor, with a mean survival of only 14.6 months. Growing evidence has shown significant overexpression of the ephrin type-A receptor 2 (EphA2) receptor in various malignancies, including GBM, as well as a correlation to poor prognoses. It is believed that EphA2 receptors play important roles in mediating GBM tumourigenesis, including invasion, metastasis, and angiogenesis. Despite the clinical and pathological importance of tumour-associated vasculature, the underlying mechanism involving EphA2 is poorly known. Here, we have summarised the current knowledge in the field regarding EphA2 receptors' roles in the angiogenesis of GBM.

    BACKGROUND: Following brain injury, development of hippocampal sclerosis often led to the temporal lobe epilepsy which is sometimes resistant to common anti-epileptic drugs. Cellular and molecular changes underlying epileptogenesis in animal models were studied, however, the underlying mechanisms of kainic acid (KA) mediated neuronal damage in rat hippocampal neuron cell culture alone has not been elucidated yet.

    METHODS: Embryonic day 18 (E-18) rat hippocampus neurons were cultured with poly-L-lysine coated glass coverslips. Following optimisation, KA (0.5 μM), a chemoconvulsant agent, was administered at three different time-points (30, 60 and 90 min) to induce seizure in rat hippocampal neuronal cell culture. We examined cell viability, neurite outgrowth density and immunoreactivity of the hippocampus neuron culture by measuring brain derived neurotrophic factor (BDNF), γ-amino butyric acid A (GABAA) subunit α-1 (GABRA1), tyrosine receptor kinase B (TrkB), and inositol trisphosphate receptor (IP3R/IP3) levels.

    RESULTS: The results revealed significantly decreased and increased immunoreactivity changes in TrkB (a BDNF receptor) and IP3R, respectively, at 60 min time point.

    CONCLUSION: The current findings suggest that TrkB and IP3 could have a neuroprotective role which could be a potential pharmacological target for anti-epilepsy drugs.

    Background: Strobilanthes crispus (S. crispus) leaves were traditionally consumed for its body weight lowering effect. In this study, we investigated the anti-obesity effect of S. crispus leaves extract (SCE).

    Methods: Mice (n = 48) were fed high-fat diet (HFD) for 25 weeks to induce obesity, after which half were maintained on HFD and half switched to low-fat diet (LFD)while they were given normal water (H2O) or 0.1% (w/v) SCE in water at week 0-4 which was increased to 1% (w/v) at week 5-9. Effects of treatment with SCE were compared between HFDH2O, HFDSCE, LFDH2O and LFDSCE groups. Respiratory exchange ratios (RER) were measured at weeks 0, 5 and 10. Food, water intake and body weight were measured weekly. Plasma lipid profile and organ weights were determined at week 10.

    Results: SCE had significantly reduced RER at week 9 (P = 0.011). Food intake, body weight, and abdominal adipose tissue weight were not altered by SCE at weeks 5 and 10. However, significant increase in plasma and liver cholesterol (P < 0.050) was observed.

    Conclusion: Our findings suggest that SCE induced lipolysis and body fat oxidation and increased energy expenditure. Further studies in other animal models should be done to confirm the consistency of these results.

    BACKGROUND: A new direct microplate-based colorimetric drug susceptibility test that omits the initial isolation of Mycobacterium tuberculosis from sputum specimens was evaluated.

    METHODS: A total of 51 M. tuberculosis acid fast bacilli (AFB) smear-positive sputum specimens were inoculated directly into drug-free and serial dilutions of drug-containing Middlebrook 7H9 broth media. With this direct resazurin micro plate assay (REMA) method, resazurin dye was used as a growth indicator in microplate wells. The minimum inhibitory concentrations (MIC) of isoniazid (INH) and rifampicin (RIF) were compared with those of the 'gold standard' absolute concentration method (ACM). The turnaround time (TAT) of the direct REMA and the ACM were also determined.

    RESULTS: At the selected cut-off points (INH: 0.0625 μg/mL; RIF: 0.125 μg/mL), good drug susceptibility test results were obtained for INH and RIF with an average sensitivity, specificity and accuracy of 90%, 100% and 97%, respectively, with a TAT of 15 days. The REMA method also correctly classified the resistant isolates with positive predictive values of 95% and negative predictive values of 98% for the two drugs.

    CONCLUSIONS: The direct REMA was reliable in routine diagnostic laboratories for the drug susceptibility testing of M. tuberculosis and the rapid detection of multi-drug-resistant tuberculosis.

    Background: Chronic kidney disease of uncertain aetiology (CKDu) is one of the major health concerns among agricultural communities in Sri Lanka. Individuals involved in severe agricultural works for their livelihood are highly vulnerable for this disease and patients have been detected with persisting proteinuria at community-level screening. The current study was designed to evaluate the diagnosis of two functional markers of kidney damage using individuals with persisting proteinuria as the baseline.

    Methods: One hundred and fifty hard-working agricultural farmers from high-prevalence area for CKDu (Madawachchiya) were screened three times for proteinuria; 66 proteinuric and 21 non-proteinuric were identified as the baseline classification. Selected individuals were analysed further for creatinine, protein and cystatin C in urine and creatinine, cystatin C in serum. Urine protein-to-creatinine ratio (UP/UC) was calculated.

    Results: Based on creatinine and cystatin C cut-off levels in serum, individuals were classified as high or normal. Diagnosis of two functional markers (creatinine and cystatin C) were evaluated using receiver operating characteristic (ROC) curve and in terms of sensitivity and specificity using UP/UC as the baseline. Creatinine and cystatin C-based eGFR (estimated Glomerular filtration rate) levels were calculated, and Pearson's correlation coefficient was determined between different eGFR measurements using UP/UC. Mean (SD) UP/UC ratio, serum creatinine, and serum cystatin C levels of the proteinuric subjects were 129.0 (18.4) mg/mmol, 1.35 (0.39) mg/dL, 1.69 (0.58) mg/L. For non-proteniuric individuals, the results were found to be 14.4 (2.28), 1.22 (0.40) mg/dL, 0.82 (0.25) mg/L. The ROC analysis showed excellent accuracy in using cystatin C for identifying proteinuric patients than creatinine area under the curve (AUC): 0.9675, P < 0.001). Cut-off points were identified as 1.015 mg/dL for serum creatinine and 0.930mg/L for cystatin C. Furthermore, cystatin C based Hoek formula showed the better correlation (0.635, P < 0.001) with UP/UC compared with creatinine based modification of diet in renal disease (MDRD) formula.

    Conclusion: The study showed elevated serum cystatin C in patients with persisting proteinuria compared with non-responding serum creatinine. Moreover, cystatin C-based eGFR equations were more accurate to determine the kidney function than serum creatinine in proteinuric patients who are vulnerable for CKDu in high-prevalence areas.

    Background: Reliability studies are commonly used in questionnaire development studies and questionnaire validation studies. This study reviews the sample size guideline for Cronbach's alpha test.

    Methods: Manual sample size calculation using Microsoft Excel software and sample size tables were tabulated based on a single coefficient alpha and the comparison of two coefficients alpha.

    Results: For a single coefficient alpha test, the approach by assuming the Cronbach's alpha coefficient equals to zero in the null hypothesis will yield a smaller sample size of less than 30 to achieve a minimum desired effect size of 0.7. However, setting the coefficient of Cronbach's alpha larger than zero in the null hypothesis could be necessary and this will yield larger sample size. For comparison of two coefficients of Cronbach's alpha, a larger sample size is needed when testing for smaller effect sizes.

    Conclusions: In the assessment of the internal consistency of an instrument, the present study proposed the Cronbach's alpha's coefficient to be set at 0.5 in the null hypothesis and hence larger sample size is needed. For comparison of two coefficients' of Cronbach's alpha, justification is needed whether testing for extremely low and extremely large effect sizes are scientifically necessary.

    Background: Nursing is a stressful occupation, and nurses face multiple stressors daily. Emotional intelligence causes an individual to overcome environmental stresses. The aim of the present study was to determine the relationship between emotional intelligence and job stress among nurses in Shiraz.

    Methods: A cross-sectional study was conducted with 500 nurses, selected through multistage cluster sampling, from the hospitals in Shiraz in 2017. The data collection tools were the Siberia Schering's Emotional Intelligence Standard Questionnaire and the Hospital Job Stress Standard Questionnaire, completed through the self-report method. The data were analysed using SPSS 22 software.

    Results: The mean scores of emotional intelligence and job stress were 113.59 ± 14.70 (total score = 165) and 97.10 ± 14.27 (total score = 175), respectively. The correlation test showed that there was an inverse relationship between emotional intelligence and job stress (r = -0.474, P < 0.001). Also, the multiple linear regression analysis showed that self-awareness, social consciousness, and income predicted 25% of the job stress in the subjects under study (r2 = 0.25).

    Conclusion: Regarding the relatively strong and inverse relationship between the nurses' emotional intelligence and job stress, it is suggested that emotional intelligence workshops be included in the in-service training programs of the nurses.

    Background: Colorectal cancer is a major public health problem. A way to decrease this cancer is through dietary behavioural changes. The aim of this study was to determine the effects of education on dietary behaviour based on the health belief model (HBM) using multimedia.

    Methods: In this clinical trial study, 98 participants were randomly allocated to an HBM group (n = 48) and a control group (n = 50). The HBM group received an audiovisual compact disc (CD) that contained information about nutritional behaviour of colorectal cancer (CRC) prevention based on HBM that lasted 45 min. Both groups completed questionnaires regarding demographic factors, knowledge and HBM constructs, and a three-day dietary recall at the beginning of the study, 1 week after, and 3 months after the education. The outcome of this study was measured by the amount of food servings consumed and dietary micronutrient intake.

    Results: At the baseline, there were no significant differences between groups regarding demographic factors. Findings showed that self-efficacy (P < 0.001), severity (P < 0.001), and benefits (P < 0.001) were perceived to be higher, and knowledge (P < 0.001) was increased in the HBM group compared to control group 3 months after education. There was a significant increase in fruit and vegetable (P < 0.001) and dairy (P = 0.001) intake and a significant decrease in red meat servings (P = 0.016) in the HBM group compared to the control group. Also, intake of vitamin D (P < 0.001), folate (P < 0.001), calcium (P = 0.008), and dietary fibre (P < 0.001) was increased in the HBM group compared to the control group 3 months after education.

    Conclusion: Education plans based on HBM and implemented through multimedia can change nutritional beliefs and behaviours for the prevention of colorectal cancer.

    Background: A poster presentation is an experiential learning activity that stimulates curiosity and interest among students. Moreover, it encourages exploration and integration of concepts and provides students with a novel way to demonstrate their understanding of scientific principles. This pilot projects aimed to analyse views of participants on the academic benefits and learning of medical sciences via poster presentations.

    Methods: This cross-sectional study used the sequential exploratory type of mixed methods design in which quantitative data analysis was performed via survey-based questionnaires and qualitative study. For this purpose, we performed a thematic analysis of semi-structured interview questions that were administered to all participants using the self-interview technique.

    Results: A majority of students were of the opinion that the process of making poster preparation acted as an opportunity to promote deep learning. Moreover, a majority expressed that making these presentations required teamwork, which gave them an insight into collaborative learning.

    Conclusion: Our study revealed that poster presentations, when used effectively as an assignment, can facilitate a learner's critical and reflective thinking and promoting active learning. Previous generic guidelines for making posters were found to be an important step that led to a systematic scientific approach amongst learners as well as for integrating basic science and medical knowledge.

    Background: Caregivers of patients with mental illness are exposed to stigma. The internalisation of this stigma among caregivers is known as affiliate stigma and can be measured by the Affiliate Stigma Scale (ASS). The aim of this study was to validate the Malay version of the ASS.
    Methods: A cross-sectional study was performed from May to December 2017 with 372 caregivers of patients with mental illness. The ASS was first translated into Malay using standard forward and backward translation procedures. The final version of the ASS-Malay (ASS-M) was completed by participants. The data analyses involved assessment of construct validity by exploratory factor analysis, confirmatory factor analysis and construct reliability.
    Results: The final model of the ASS-M consists of four factors with 21 items, as compared to the original version, which has three factors with 22 items. The results showed that the final model has good model fit based on RMSEA (0.065) and SRMR (0.055) and a satisfactory composite reliability (affective = 0.827, cognitive = 0.857, behaviour = 0.764, self-esteem = 0.861).
    Conclusion: The study showed that the four-factor, 21-item ASS-M model has good psychometric properties. The scale is valid and reliable for measuring affiliate stigma among caregivers of patients with mental illness in Malaysia.

    The aim of this cross-sectional study was to evaluate the knowledge and attitudes concerning antibiotic use and resistance among members of the local community in Pulau Pinang, Malaysia. The study was conducted among 326 residents of the Jelutong district, Pulau Pinang state, from August to October 2013. A self-administered five-part questionnaire was used for the data collection. The respondents exhibited inadequate knowledge of antibiotics in general. Approximately 80% of them did not know the indications for antibiotic use, while 76% believed that antibiotics were useful in terms of resolving viral fever. Additionally, 52.6% believed that antibiotics could be used to treat all types of infections. Inadequate knowledge of antibiotic resistance was also evident among the respondents, since 72.9% of them did not agree that resistant bacteria can spread from human or animal to human, while 32% were unaware that bacteria can develop resistance to antibiotics. More than 60% of respondents admitted that they took antibiotics in order to accelerate their recovery from illness, while 34.8% claimed that they only stopped taking antibiotics when they felt better. The findings hence indicate that most respondents had poor knowledge and attitudes concerning antibiotic use and resistance, which suggests the need for more community-based educational campaigns designed to improve the public's knowledge and attitudes regarding antibiotics.

    Tajweed is one of most important elements in learning of al-Quran recitation accordingly. Long and short or even buzzing (dengung) in quran recitation would give a different meaning. Traditional learning method is still considered as the main option in learning the tajweed but a new requirement that necessarily suits to recent technology and game-based learning method is most preferred among youngsters nowadays. There are a lot of significant relationships between game-based learning with positive impact on learning capabilities. The development of game-based learning or edutainment shows a good potential but perception on tajweed game-based learning or edutainment must be discovered. The methodology of this study is divided into two main components; the development of application of edutainment called as ‘Teroka Tajwid’; and purview study on the perception towardsthe application which has become the main focus of this study. Model of Rapid Application Development (RAD) was chosen as the model of the application development and a quantitative survey was conducted to evaluate the users’ perception on design; audio, video and visual; and navigation and userfriendly. As a result, overall, the survey has shown that the performance of the application is suitable and attractive in learning basic tajweed.

    Planning guidelines are important tools in controlling the development of housing projects in urban and rural areas to ensure sustainable development (a development that meets the needs of the present, without compromising the ability of future generations in meet their own needs). Planning guidelines are used by local planning authorities (PBPT) to ensure uniformity, comfort and safety in land use activities to achieve effective housing development projects which meet the needs of the communities and create a comfortable living environment. The local planning authorities have developed a set of planning guidelines to be adhered to in all development projects in Malaysia. The main problem of the existing planning guidelines is that they are prepared in the form of reports which are not user friendly. In order to address this problem, a digital version of planning guidelines called the Public Facilities Guidelines Calculator for Sustainable Housing Development is invented. The main objective of this invention is to develop a digital version of planning guidelines which are more user friendly to help the users in accessing the ‘exact’ information ‘easily’ and in an ‘express’ manner (3E’s). The calculator is hoped to be able to provide assistance to town planners, consultants, land developers, and students in preparing the development layout which fulfils the development requirements for planning permission applications. Additionally, the calculator is expected to ease the job of local authorities in determining whether the proposed development layout is aligned with the planning guidelines so that timely planning permission can be issued.

    Protective action by annatto-derived delta-tocotrienol (δ-TCT) and soy-derived alpha-tocopherol (α-TOC) through the regulation of PI3K/Akt-Cyclin D1 pathway against the nicotine-induced DNA damages is the focus of the present study. Nicotine, which has been widely reported to have numerous adverse effects on the reproductive system, was used as reproductive toxicant. 48 female balb/c mice (6-8 weeks) (23-25 g) were randomly divided into 8 groups (G1-G8; n = 6) and treated with either nicotine or/and annatto δ-TCT/soy α-TOC for 7 consecutive days. On Day 8, the females were superovulated and mated before euthanized for embryo collection (46 hours post-coitum). Fifty 2-cell embryos from each group were used in gene expression analysis using Affymetrix QuantiGene Plex2.0 assay. Findings indicated that nicotine (G2) significantly decreased (p < 0.05) the number of produced 2-cell embryos compared to control (G1). Intervention with mixed annatto δ-TCT (G3) and pure annatto δ-TCT (G4) significantly increased the number of produced 2-cell embryos by 127 % and 79 % respectively compared to G2, but these were lower than G1. Concurrent treatment with soy α-TOC (G5) decreased embryo production by 7 %. Supplementations with δ-TCT and α-TOC alone (G6-G8) significantly increased (p < 0.05) the number of produced 2-cell embryos by 50 %, 36 % and 41 % respectively, compared to control (G1). These results were found to be associated with the alterations in the PI3K/Akt-Cyclin D1 gene expressions, indicating the inhibitory effects of annatto δ-TCT and soy α-TOC against the nicotinic embryonic damages. To our knowledge, this is the first attempt on studying the benefits of annatto δ-TCT on murine preimplantation 2-cell embryos.

    One of the issues frequently discussed in Malaysia is the low level of English language proficiency among language learners. Although English is the country’s second language, students at all levels including those in schools and higher institutions are found to have difficulties in mastering the language. Considering this problem, as well as the rapid growth and strong interest among students in the use of mobile devices in language learning, a mobile application called MApEC was developed. The objective of developing MApEC is to help students at higher institutions to learn and understand common terminologies used in the learning of the English language in a more interactive approach rather than the conventional one. MApEC can be accessed offline and has 8 main features - course info, about us, contacts, download, reading, writing, dictionary and podcast. Even though MApEC is still at the initial stage of development, findings from the survey showed positive feedback from the respondents. They are satisfied with MApEC as the application can be easily accessed and is user-friendly. The respondents also believe that MApEC can help them in understanding common terminologies used in the learning of the English language.

    The industrial training is a mandatory course for Diploma Civil Engineering, UiTM Pahang. It is to expose students to real engineering practices to meet the Engineering Technician Accreditation Council (ETAC) requirements. However, the current method for monitoring and assesssing students’ progress during their industrial training is inadequate and not systematic due to incomplete student log books, unspecified scoring rubrics, inaccurate assessments by lecturers who are not experts in the field, infrequent visits and monitoring by faculty supervisors, inability to monitor student daily activities by faculty supervisors and unstandardized format for student reports. Thus, ‘Indus-Train’ system was developed by the Civil Engineering Innovative and Creative group (CivTEX), UiTM Pahang to assist the faculty (Civil Engineering) in solving problems related to student monitoring and assessment during industrial training. This system is more systematic, competitive, reduces management costs and able to achieve the adequacy of the industrial training assessments. The main positive impact of applying this system in the recent semester was an increment in student achievements. Student achievements were assessed from the course outcomes (CO) which showed an increase of 13.2 % (CO1). The system also saves time and reduces cost. Industrial training certificates are given to students at the end of the semester where each certificate has its unique barcode that can be used to track all students’ activities and reports during industrial training. Furthermore, this certificate is useful for job applications and can be one of a student’s supporting documents.

    Tourism industry is constantly changing. A strong and positive destination image is always desired especially from potential market segments. The disability market is the next big consumer market that is often overlooked. In sports tourism, most of the facilities developed for the disabled are limited, preventing them from experiencing the industry as a normal person would. People with disability have the rights to travel and live an active and healthy lifestyle. Promoting sports tourism and making it accessible to the disabled is seen as a problem as there is currently no easy access to information that is available for them. Verve App is an innovation to assist the disabled to navigate their ways to sport facilities such as the stadiums and many other disabled-friendly sporting facilities near them. This innovation is also designed to offer a wide-range of information to people with disabilities, to provide exposure to the disabled on disabled-friendly facilities, to provide a convenient and easy way to search for sport facilities within close proximity, to navigate the disabled, as well as to provide on facility accessibility.
