Presently there is a gross lack of information on cost and cost weights in many developing countries that implement
casemix system. Furthermore, studies that employed Activity Based Costing method (ABC) to estimate the costs of radiology
procedures were rarely done in developing countries, including Malaysia. The main objective of this study is to determine
the costs of radiology procedures for each group in casemix system, in order to develop cost weights to be used in the
implementation of the casemix system. An economic evaluation study was conducted in all units in the Department of
Radiology in the first teaching hospital using the casemix system in Malaysia. From the 25,754 cases, 16,173 (62.8%)
of them were from medical discipline. Low One Third and High One Third (L3H3) method was employed to trim the
outlier cases. Output from the trimming, 15,387 cases were included in the study. The results revealed that the total
inpatients’ charges of all the radiology procedures was RM1,820,533.00 while the cost imputed using ABC method was
RM2,970,505.54. The biggest cost component were human resources in Radiology Unit (Mobile) (57.5%), consumables
(78.5%) of Endovascular Interventional Radiology (EIR) Unit, equipment (81.4%) of Magnetic Resonance Imaging (MRI)
Unit, reagents (68.1%) of Medical Nuclear Unit. The one highest radiology cost weight, was for Malaysia Diagnosis
Related Group (MY-DRG®)B-4-11-II (Hepatobiliary and Pancreas Neoplasms with severity level II, 2.8301). The method
of calculation of the cost of procedures need to be revised by the hospital as findings from this study showed that the cost
imposed to patient is lower than the actual cost.

Giardia intestinalis is among the protozoa that is commonly found in the human fecal samples throughout the world. The prevalence of infection is higher among people living in developing countries than developed countries. One hundred and eleven Orang Asli children at Pos Titom, Pahang participated in this study. The prevalence study was conducted in accordance to gender, age groups and the diagnostic techniques employed. Fecal samples were collected and examined for the presence of G. intestinalis using three diagnostic techniques ie; the direct fecal smear, formalin-ether concentration and the trichrome staining technique. The overall prevalence of G. intestinalis infection among Orang Asli children at Pos Titom was 23.42%. The infection was higher among females (25%) as compared to males (21.57%) eventhough it was not statistically significant (p > 0.05). In terms of age groups, no positive case was observed among preschoolers whose age were below seven-years while the school aged children (7 to 12-years old) showed 24.53% infection. The trichrome staining technique was the most sensitive with a detection rate of 22.52% followed by the formalin-ether concentration technique with a detection of 19.82% whilst the direct smear technique only detected 12.61% of cases. The prevalence of giardiasis amongst the Orang Asli children at Pos Titom, Pahang was still high despite various efforts made and improvements in basic infrastructural facilities undertaken for their community. The health care status and personal hygiene of Orang Asli children remains poor and needs to be further improved.

Literature data on association between physical activity (PA) and health related quality of life (HRQoL) in non-clinical
adults population are still limited. Thereby, the objective of this study was to evaluate the association between PA and
HRQoL among the non-academic staff serving the Faculty of Health Sciences (FSK), Universiti Kebangsaan Malaysia (UKM),
Kuala Lumpur (KL). In this cross sectional study, 105 non-academic staffs aged between 18 to 60 years were recruited.
Self-administered questionnaires (Malay version of the International Physical Activity Questionnaire and World Health
Organisation Quality of Life-Brief Version) were administered among the participants. Mann Whitney test and Spearman’s
rho correlation were employed for the statistical analysis using SPSS version 22.0 Majority of participants (72.4%)
demonstrated high PA scores of ≥ 3000 MET-minutes/week. PA scores between genders was not statistically significant
(p = 0.137). However, total PA score of support staffs (median = 9039.0, IQR = 15811.00) was significantly higher compared
to staffs in management and professional group (median = 4329.0, IQR = 4189.00). The mean(SD) self-perceived HRQoL
score was 3.4 (0.76). There were no significant correlation between PA and all domains of HRQoL such as physical health,
    psychological, social relationship and environmental domains (Correlation coefficient, ρ = -0.108, 0.003, -0.023, 0.026
respectively) with p = 0.273, 0.978, 0.818, 0.794 respectively. Majority of non-academic staffs of FSK, UKM engaged in
high physical activity. The PA levels of support staffs were significantly higher than management and professional staffs.
However, there was no statistically significant correlation observed between PA and domains of HRQoL.

Health care services are not often accessible and available for all people in one country due to multiple reasons such as the geographical barrier, affordability, etc. The aim of this study was to analyse willingness to pay (WTP) for healthcare services user fees among Malaysian population and determine its’ influencing factors. Structured interviews were conducted involving 774 households in 4 states represents Peninsular Malaysia. Validated questionnaires with open ended, followed by bidding games were applied to elicit maximum amount of WTP. The study was analysed descriptively and with multivariate regression method to adjust for potential confounding factors. More than half of respondents WTP more than current fee for the government clinic outpatient registration fee with mean MYR3.76 (SD2.71). Majority of respondents not WTP more than usual for private clinic simple outpatient treatment charges with the mean MYR38.76 (SD5.45). Factors that were found to have significant associations with WTP for both government and private clinic were income and having health insurance. Community willing to pay for healthcare services user fees and charges but at certain amount. The healthcare services user fees and charges can be increased up to community WTP level to avoid from catastrophic expenditure.

Phorid flies play an important role in forensic cases and can cause myiasis in humans. Studies on phorid flies species diversity are still limited in Malaysia. This research was carried out to collect information about species and frequency distribution of phorid flies as to provide more information on their roles in forensic and medical entomology. Bait trap was used with 100 g beef liver as baits. The species of the flies were identified using identification keys from Disney as well as Brown and Oliver. There were 449 phorid flies found in Kepong, Kuala Lumpur including Megaselia scalaris, Megaselia spiracularis, Megaselia sp. and phorid flies of genus X. Female phorid flies (98.89%) were found more prone to be trapped compared to male phorid flies (1.11%). Most phorid flies trapped in Kepong, Kuala Lumpur were from genus Megaselia and consisted of female flies. A total of five species of phorid flies probably new to science were also discovered. This study showed that Megaselia flies were found indoors rather than in open spaces. This was corresponding to their discoveries among the decomposing corpse found inside premises.

The anatomical structures of the first, second and third instars of Chrysomya rufifacies (Macquart) were examined by light microscopy. Observations were documented on the three main characteristics; the cephalopharyngeal skeleton, anterior spiracle and posterior spiracle. The first instar larva bore cornuae of fairly pigmented delineation with slim hypostomal sclerite and distinct dental sclerite. First instar did not have obscured anterior spiracle but posterior spiracles were obscured with thin lining of opened peritreme. Intersegmental spines were evident. The second instar larva displayed a prominent anterodorsal process approaching closer to hypostomal sclerite while upper margin of the dorsal cornua was slightly pigmented. Each anterior spiracle consisted of nine to ten papillae, arranged in a single row. Peritreme of the posterior spiracle thick, opening at the end of peritreme was not wide and confined to two spiracular slits. The third instar larva showed a prominent arch of the ventral cornua with broad and bold appearance. It approached the dorsal cornua and became narrow at the incision median. The anterior spiracle consisted of a single row of nine to ten papillae while intersegmental spine could be identified with one to three dark pigmented tips. A dark pigmented and wide periterime was observed confining three short and thick spiracular slits while button was poorly pigmented. The most distinctive feature of this second and third instar larva was the slender, thorn-like tubercle with numerous spined tips on the middle line segment of the body. These findings provide identification features of C. rufifacies larvae instars.

This study was carried out in Agricultural Park, Teluk Cempedak and Bukit Pelindung at Kuantan, Pahang in October 2007. These three areas were different in ecological characteristic, Agricultural Park is a lowland region in Kuantan rural area, Teluk Cempedak is Kuantan’s most famous beach, and Bukit Pelindung is a reserved rainforest which is 200 meters from the sea level. Fly specimens were collected using four different kinds of baits: dry prawn, salted fish, pork and mango. Each of these baits was placed in a plastic container and exposed for one hour to attract flies. Within 5 minutes, flies started swarming around the baits. The flies were more attracted to the pork and salted fish compared to the other two baits. Fifty one flies, one moth (Lepidoptera) and one wasp (Hymenoptera) were collected. In Agricultural Park, two Lucilia cuprina, one Chrysomya megacephala and one Sarcophaga sp. were collected. For Teluk Cempedak beach, there were two Sarcophagids, 31 Chrysomya megacephala, five Musca domestica, one Lucilia cuprina and one moth were caught. Flies collected from Bukit Pelindung included five C.megacephala, two Sarcophagids, one Musca domestica and one wasp. Most C.megacephala were attracted to the pork and salted fish.

An entomological study was conducted in Tanjung Sepat, Selangor, Malaysia in May until September 2007 revealing five species of butterflies (all from family Nymphalidae) were attracted to pig carcasses placed in an oil palm plantation. Euploea mulciber (Cramer 1777), Hypolimnas bolina (Linnaeus 1758), Elymnias hypermnestra (Linnaeus 1763), Mycalesis mineus (Linnaeus 1758) and Ypthima baldus (Fabricius 1775) came to the carcasses at different stages of decomposition. From this study, we know that nymphalid butterflies are attracted to carcasses but their roles are most probably unimportant in post-mortem estimation.

Hypertension and estrogen deficiency in women have been identified as significant risk factors for cerebrovascular diseases. Hypertension causes excessive vascular stretch and contributes to the initiation of cellular injury in blood vessels while estrogen has been demonstrated to exert beneficial protective effects on the vascular system. Although the specific biological outcomes exerted by either excessive stretch or estrogen exposure are well established, the combined biochemical effects of both stimuli remain unclear. Therefore, this study was conducted for quantitative proteomics study on human cerebral microvascular endothelial cells (HCMECs) subjected to 20% “pathological” cyclic stretch for a period of 18 hour in the presence or absence of 17β-estradiol by isobaric Taqs for Relative and Absolute Quantification. The results showed that only some proteins responded to 17β-estradiol (e.g., thioredoxin reductase-1), stretch (e.g., 14-3-3 protein epsilon or acidic leucine-rich nuclear phosphoprotein 32 family member B) and interestingly, some proteins returned to control pre-treatment levels when exposed to both (e.g., d-dopachrome decarboxylase, thrombospondin-1). In addition, HCMECs that exposed only to estrogen had a very similar proteomic profile (i.e., up-regulation of structural, cellular adhesion and proliferation proteins) as to those exposed to estrogen with 20% stretching for 18 hour, suggesting that estrogen ablated the detrimental effects by the stretch alone. These findings sheds light on the molecular mechanisms by which the cerebrovascular protective actions of estrogen on HCMEC exposed to pathological levels of cyclic stretch which could provide a platform for future research in therapeutic approach.

Reduced muscle strength and quality of life (QoL) has an impact on adults at risk of osteoporotic fractures. Muscle strength and QoL among Malaysian adults at risk of osteoporotic fractures is not known. The aim of our study was to examine muscle strength and QoL status and its relationship among adults at low and moderate to high risk of osteoporotic fractures. In this cross-sectional study, we recruited 27 men and 78 women (mean age 69.3 ± 8.5 years) from Orthopaedic Clinic, Hospital Canselor Tuanku Muhriz Universiti Kebangsaan Malaysia. A 10-year probability of major osteoporotic fractures were calculated according to WHO fracture risk assessment tool (FRAX) with femoral neck BMD (cut-point 10%). Participants were categorized as low and moderate to high risk of osteoporotic fractures based on FRAX results. Back extensor muscle strength and dominant hand grip strength were measured using a load cell system and a hand dynamometer respectively. European QoL questionnaire (EQ5Dvas) was used to measure QoL. Multiple linear regression was performed with QoL as a dependent variable and back extensor muscle strength and hand grip strength as the independent variables. Approximate 40% of adults were categorized into moderate to high risk of osteoporotic fractures. The mean values among the low and moderate to high risk of osteoporotic fractures for QoL was 80.5 ± 13.2 and 76.9 ± 16.9 (p = 0.77); hand grip strength was 25.7 ± 17.3 and 20.6 ± 5.5 kg (p < 0.001) and back extensor muscle strength was 25.0 ± 7.0 and 18.9 ± 9.5 Newton (p < 0.01) respectively. Hand grip muscle strength appeared to be a determinant of QoL and accounted for 5.8% of the total variance. Improved muscle strength may lead to better QoL among adults at risk of osteoporotic fractures.

Interprofesion learning (IPL) occurs when students from two or more different professionals study with and learn about different professions. IPL is believed to enhance the collaboration level within the different profession after graduated. The objective of this research is to compare perception levels among the respondents coming from eleven different professions including Medical, Nursing, Dentistry, Pharmacy, Occupational Therapy, Dietetics, Audiology, Speech Science, Physiotherapy, Optometry and Diagnostic Imaging & Radiotherapy. Each respondent were selected randomly representing each year of study for each profession. They were required to fill up the survey form provided. 330 survey forms were returned instead of 367 distributed copies or 90% from overall surveys. Using one-way ANOVA analysis, all professions indicate same level of perception in knowledge subscale (p = 0.11), the practice of teamwork and cooperation (p = 0.23) and various methods to start the IPL (p = 0.61). However, for professional identity scales and interprofession educational preparation the value was (p < 0.05). Post-hoc Turkey test, indicated Physiotherapy students had a higher average score (M = 4.35 ± 0.54) compared to Speech Science (M = 3.75 ± 0.59) in professional identity subscale whereas in students preparation subscale for interprofession education, only Physiotherapy students had higher score (M = 4.15 ± 0.82) in comparison to Diagnostic Imaging & Radiotherapy (M = 3.25 ± 0.83). Besides, independent T test showed the students were in favour of having IPL in early year of education with the average score (M = 3.53 ± 1.029) for year one and year two compared to year three to five (M = 3.34 ± 1.089). The result form this research shows that the students have a positive perception towards IPL based on the average values not more than 3.0.

This study aimed to identify the foot sensory status of Malay women with type 2 diabetes mellitus and to determine its relation with: demographic factors, glycated haemoglobin level in plasma, and body mass index. A cross-sectional study was conducted on 71 Malay women with type 2 diabetes mellitus who attend three health clinics in Hulu Langat District, Selangor. Foot sensation was assessed using the Semmes-Weinstein Monofilament 5.07 in nine locations on the plantar and dorsal of the feet. Loss of protective sensation was defined as inability to sense the monofilament in one or more sites of either foot. This study found that 56.3% of women had loss of protective sensation, with common sites being the heel, fifth metatarsal head, lateral mid-foot and little toe. Duration of diabetes mellitus were significantly related to foot sensation. The age, HbA1C level and body mass index did not show any significant correlation. Loss of protective sensation may lead to serious foot complication and therefore early screening involving multidisciplinary team is essential for prevention.

Snacks are required for athletes to fuel their higher demand of energy. Meanwhile, social influences and multimedia can
affect the athletes’ choices of snacks. Social influences are divided into three categories: Compliance (sports dietitian/
nutritionist & medical officer), Conformity (friend) and Obedience (parent & coach), while multimedia refers to internet,
magazines and newspaper. The objective of this study was to investigate the different sources of social influence and
multimedia on athletes’ preferred snacks. Out of 26 snack choices, 3 most preferred snacks were chosen by 69 endurance
athletes from the National Sports Institute (NSI) through a questionnaire. They are Popiah & Mixed Fruit Jelly, Chicken
Salad and Egg & Cheese Sandwich. Out of the 6 choices of beverages, the top 3 preferred beverages were sports drinks,
orange smoothie and apple smoothie. Social influence from parent (p = 0.041) has significant effect on the athlete’s most
preferred snack choice, followed by influence from dietitian (p = 0.02) on their second preferred snack choice and lastly
internet (p = 0.024), has its effect on the third preferred snack. Other social influences such as friends, medical officer
and coach show no significance effect on the preferred snack choices. The parent factor also has significant effect on
the most preferred beverage choices with p = 0.043. To conclude, parent influence is strongly related to both snack and
beverage choices of the athletes, followed by dietitian and internet on the athlete’s preferred snack choices.
