    Fraktur avulsi tulang belakang iliac anterior (ASIS) jarang terjadi, dan lebih lumrah berlaku di kalangan remaja muda semasa bersukan. Ini disebabkan oleh rawan apophyseal yang lemah semasa remaja, apabila kontraksi otot yang kuat mengatasi ASIS semasa aktiviti olahraga. Seorang lelaki berusia 15 tahun dibawa ke Jabatan Kecemasan (ED) mengadu sakit pinggul kanan selepas perlumbaan semasa Hari Sukan sekolahnya. Ketika dia melintasi garisan penamat dan menang, dia tiba-tiba rasa sensasi “pop” di pinggul kanannya dan jatuh ke tanah dengan kesakitan. Di ED, dia baring meniarap dengan skor sakit 6/10. Intravena Ketorolac diberikan untuk melegakan kesakitannya. Pemeriksaan fizikal menunjukkan kesakitan setempat di kawasan hadapan pelvis kanan, tanpa bengkak atau kecacatan pada sendi pinggul kanan. Tiada perbezaan antara kepanjangan kaki kanan berbanding kaki kiri. Pergerakan pinggul kanan terhad akibat kesakitan. Pemeriksaan neurovaskular adalah normal. Radiografi pelvis menunjukkan fraktur avulsi ASIS kanan. Beliau telah dirujuk kepada pasukan Ortopedik dan telah dirancang untuk rawatan konservatif rehat di katil, analgesik, dan ambulansi tanpa berat. Dia dibenarkan pulang dan dirancang untuk temujanji di Klinik Ortopedik. Pemeriksaan semula selepas 2 bulan kecederaan menunjukkan pesakit sihat dan bebas kesakitan. ASIS mudah terdedah kepada kecederaan avulsi semasa menjalankan aktiviti sukan kerana otot sartorius menarik anteroinferior ASIS semasa lekukan maksimum pinggul dan lanjutan lutut. Walaupun fraktur avulsi apophyses pelvis jarang berlaku, ia harus dipertimbangkan di kalangan atlet remaja jika mekanisma kecederaan adalah sangat sugestif.

    Fine needle aspiration cytology under radiologic guidance for diagnosis of renal cell carcinoma is well established and is increasingly utilized. This is because renal cell carcinoma displays fairly characteristic cellular features permitting correct cytologic identification. We present a case of a 66-year-old man who had advanced renal cell carcinoma with spread to aortic and cervical lymph nodes, lungs and liver. Fine needle aspiration cytology of the para-aortic mass showed tight clusters of malignant cells with abundant and vacuolated cytoplasm consistent with renal cell carcinoma. Histology of the left cervical lymph nodes together with immunohistochemistry findings were consistent with the cytologic diagnosis of metastatic renal cell carcinoma. The patient succumb to his illness three years after the diagnosis was made.

    Chronic obstructive pulmonary disease (COPD) patients are reported to have cardiovascular instability which leads to greater limitation for activities there by leads to poor quality of life. Physical training proved to be one of the moderators of these limitations. However, uncertainty prevails among the protocol and duration. The present study investigated the effect of physical training on blood pressure, heart rate and Rate pressure Product (RPP) among COPD patients. A total of thirty COPD patients aged between 40 to 55 years were recruited for the study based on the inclusion and exclusion criteria and were assigned in to experimental group (15 patients) and control group (15 patients). The mean difference of blood pressure, heart rate and RPP were analyzed using paired t-test. There was significant difference between the pre and post test values of all parameters between experimental and control group with p< 0.05. Diastolic blood pressure showed to be less significant compared to the systolic blood pressure. This showed that specifically designed physical improved the cardiovascular fitness among COPD patients.

    A 17-year-old male student of Indonesian parentage presented with two weeks history of progressive painless bilateral visual deterioration. There was no contact with tuberculosis (TB)-infected patients and parents claimed that all immunization including BCG was completed. However, BCG scar was not apparent. Visual acuity was 6/36 and 6/60 in the right and left eyes respectively. The anterior and vitreous chambers were quiet. Funduscopic examination revealed retinal vasculitis with perivascular exudates, branch vein occlusion, neovascularization and macular oedema. Fluorescein angiography confirmed large areas of capillary non-perfusion and leaking new vessels. Mantoux test was positive and full regime anti-TB therapy was instituted. HIV screening was negative. Three days later, an immunosuppressive dose of oral steroid was started. Both eyes received intensive laser photocoagulation.Interestingly, there was no development of vitritis throughout.

    Dengue fever is a common pyrexial-viral infection in the Asian region and the incidence is increasing yearly. There are various ocular manifestations of dengue fever such as maculopathy, retinal hemorrhages, retinal lesions and vasculitis. Reduction in visual acuity and other visual disturbances such as metamorphopsia is the most common presentation of dengue-associated maculopathy. We report an interesting case of a young lady with bilateral maculopathy secondary to dengue fever who was managed conservatively with dramatic visual improvements. This case highlights the possibility of bilateral and simultaneous ocular involvements in patients infected with dengue fever. Both ophthalmologists and physicians need to be aware of ocular complications following dengue fever and advice patients regarding the symptoms.

    We report a case of a 37-year-old smoker who presented with painless and sudden onset of loss of upper field vision of the right eye, associated with one month history of dry cough which was unresponsive to antibiotic and subsequent anti- tuberculosis therapy. Visual acuity at presentation was counting finger. There was bullous retinal detachment on examination. Radiological imaging revealed multiple lung opacities, involving the brain and the right orbit. Bronchoscopy and biopsy confirmed the diagnosis of adenocarcinoma of the lung. This case illustrates masquerade syndrome of which a lung carcinoma harbours behind a seemingly innocent retinal detachment. It also highlights the importance of entertaining lung carcinoma as a differential diagnosis in suspected tuberculosis among heavy smokers.

    A proportion of patients with acute viral fever with thrombocytopaenia does not necessary have dengue infection. Managing them indiscriminately as dengue infection may not be appropriate. The prevalence of this problem is not exactly known. The objective of this study is to determine the prevalence of acute non-dengue febrile thrombocytopaenia among adult patients presenting with acute non-specific febrile illness in an outpatient setting. This was a clinic-based cross sectional study. Consecutive patients presenting with non-specific febrile illness of less than two weeks were selected from the Primary Care Centre of Hospital Universiti Kebangsaan Malaysia (HUKM) and the Batu 9 Cheras Health Clinic. Full blood count was done on the day of visit and dengue serology was done on day five of illness for all patients enrolled. Seventy three patients participated in this study from May to November 2003. Among the patients, 35 (47.9%) were noted to have thrombocytopaenia. Fourteen (40%) patients with thrombocytopaenia were serologically negative. The prevalence of non-dengue febrile thrombocytopaenia was 19.2%. A significant number of patients with acute non-specific febrile illness with thrombocytopaenia were negative for dengue serology. These patients should be differentiated from those with acute febrile thrombocytopaenia, as they might differ in their natural history from those with dengue infection, and hence require different management strategies.

    Tabiat langkau waktu makan adalah kaedah lazim untuk mengawal diet. Namun, amalan kaedah permakanan ini oleh pesakit diabetes melllitus (T2DM) masih belum diketahui berikutan kekangan kajian. Matlamat utama kajian ini adalah untuk mengenal pasti selazim mana pesakit T2DM melangkau waktu makan. Kaitannya dengan faktor sosiodemografi dan klinikal, HbA1c, makan di luar rumah dan kecelaruan gaya pemakanan turut dikaji. Kajian keratan lintang ini telah dijalankan pada tahun 2015 dalam kalangan 203 pesakit di klinik kesihatan awam di Kuala Lumpur. Borang soal selidik yang diisi sendiri oleh subjek termasuk borang Skala Kecelaruan Gaya Pemakanan dalam Bahasa Melayu. Seramai 41.4% subjek kerap melangkau waktu makan dan 61.6% subjek sering makan di luar. Hanya 2% sahaja yang mempunyai masalah kecelaruan gaya pemakanan. Regresi logistik berbilang menunjukkan subjek berbangsa Cina mempunyai kaitan yang kuat dengan tabiat melangkau waktu makan berbanding dengan subjek berbangsa Melayu (nisbah ganjil selaras: 0.36; 95% sela keyakinan: 0.16-0.77; nilai p: 0.009) setelah mengambil kira faktor usia, status pekerjaan, tahap pendidikan, HbA1c, komplikasi, jenis rawatan, makan di luar dan kecelaruan gaya pemakanan. Kesimpulannya, tabiat melangkau waktu makan merupakan satu amalan lazim begitu juga dengan makan di luar rumah. Namun, kecelaruan gaya pemakanan jarang dijumpai. Tabiat melangkau waktu makan tiada kaitan dengan amalan pemakanan ini dan juga kawalan gula dalam darah. Faktor budaya dan keagamaan berkemungkinan mempengaruhi amalan pemakanan individu. Kajian lanjut perlu dijalankan bagi mengenal pasti tahap keselamatan dan penerimaan berkaitan dengan amalan ini. Walau bagaimanapun, kesannya daripada sudut klinikal perlu diperiksa dengan teliti untuk mengelakkan komplikasi kesihatan.

    It is rare for anterior chamber migration of an Ozurdex® implant from vitreous cavity, but it is seen more frequently in aphakic eyes or in pseudophakic cases with zonular dehiscense. We describe a case of a middle-aged gentleman who had persistent diabetic macular oedema not responding to anti-VEGF (vascular endothelium growth factor), who was treated with intravitreal Ozurdex® in his post vitrectomized eye and developed anterior migration of the implant to the anterior chamber. Anterior dislocation of an intravitreal implant of dexamethasone can be managed by repositioning it to the vitreous cavity or removing it through a corneal limbal incision. Ozurdex® is a friable implant, especially after a few weeks of implantation. Therefore, removal of the implant by grasping or aspiration may lead to its fracture or dispersion of the implant material. This is a report of a simple,fast and effective technique to remove a migrated Ozurdex® from the anterior chamber using a modified silicone tip.

    Endogenous endophthalmitis is a devastating intraocular infection. Finding the primary infection and directed treatment is life-saving. We describe a 47-year-old man, with uncontrolled diabetes mellitus, who presented with two weeks history of progressive reduced vision and redness of the left eye (LE). He was generally unwell since a month, previously. Examination showed relative afferent pupillary defect (RAPD) in the affected eye and visual acuity was hand movement. There was moderate anterior chamber and vitreous reaction. Fundus examination showed a huge dome-shaped choroidal mass covering the entire macula. Diagnosis of severe endogenous endophthalmitis was made, with isolation of Enterobacter sp. from his blood culture. Meanwhile, he also had elevated inflammatory markers with presence of leucocytosis, neutrophilia and elevated erythrocyte sedimentation rate (ESR) as well as C-reactive protein (CRP). Vitreous tap and aqueous tap had no growth, He improved with combination of intravitreal and systemic antibiotic, as shown by the resolved fever and reduced inflammatory markers but progressive inflammation occluded the fundus details and the LE vision eventually became no projection of light despite treatment. Subsequently, he developed rubeotic glaucoma and was treated conservatively because the LE was painless with poor vision. The repeated blood culture and urine was normal. In conclusion, endogenous endophthalmitis with eventual panophthalmitis is difficult to treat and has very poor visual prognosis. Our case highlights the challenges faced in the management of vision-threatening endophthalmitis and panophthalmitis in this patient.

    Sexual dysfunction is highly prevalent in gynaecological cancer patients. Most of the time, sexual dysfunction in gynaecological cancer is underdiagnosed as there is overlapping of symptoms with other psychological morbidities, interplaying of multiple risks, patients’ reluctance to complain or incompetence of health care provider to assess. Determining the risk factors of sexual dysfunction in cancer patients enables us to pay more attention to those who are vulnerable and to device strategies for early detection, prevention and treatment of sexual dysfunction in them. The main aim of the study was to determine the prevalence of sexual dysfunction and its risk factors in gynaecological cancer patients in Hospital Sultanah Bahiyah, Alor Star, Malaysia. Sexual function of eighty-three gynaecological cancer patients who were married were assessed with self-rated MVFSFI (Malay version Female Sexual Function Index). Self-rated WHOQOL-BREF (World Health Organization- Quality of Life- 26) which assessed the domains of quality of life was used while MINI (Mini International Neuropsychiatry Interview) was used for diagnosis of major depressive disorder. The prevalence of sexual dysfunction among the married gynaecological cancer patients was 65% (54/83). Sexual dysfunction was significantly associated with low education level (OR 3.055, CI 1.009-9.250), shorter duration of cancer (OR 0.966, CI 0.966- 0.998), ongoing chemotherapy (OR 3.045, CI 1.149-8.067), pain perception (OR 3.230, CI 1.257-8.303), absence of sexual intercourse for more than one month (OR 1.862) and three domains of quality of life such as physical health, psychological health and social relationship (OR 0.942, CI 0.908-0.978; OR 0.955, CI 0.916-0.995; OR 0.933, CI 0.894-0.973, respectively). However, sexual dysfunction was not associated with major depressive disorder (χ2 ² = 1.224, p = 0.268). The prevalence of sexual dysfunction in gynaecological cancer patients was comparable to other studies of similar population. Since, the risk factors of sexual dysfunction in gynaecological cancer patients are multidimensional, the process of assessment and management need to be holistic and patient-oriented.

    Hemichorea-hemiballismus (HC-HB) is a movement disorder characterized by involuntary movements involving limbs on one side of the body. Many etiological factors were identified and these included stroke, infection, and neoplasm. However, acute hyperglycemia-induced HC-HB is less well known. We present two cases of non-ketotic hyperglycemia-induced HC-HB. The cases depicted here Illustrate that HC-HB can be the sole presentation from among a variety of neurological manifestations of poorly controlled diabetes which can be easily reversed when hyperglycemia is corrected.

    Cardiac concussion is a sudden, direct blunt trauma cause to the chest which led to death. However, there are patients who did not completely fulfil this definition. We report two cases which did not fit into the definition domain. Two male patients presented to Emergency Department with moderate anterior chest pain after motor vehicle crash showed transient anterior ST segment elevation at chest lead V2 and V3 with raised creatine kinase and normal troponin T. The electrocardiogram changes fully resolved after 24 hours. Both patients were discharged uneventfully after 24 hours monitoring in Emergency Department short-stay ward. Conventional definition of cardiac concussion (commotion cordis) and cardiac contusion may be unsuitable to describe these cases. Therefore, we propose the diagnosis of focal
    cardiac concussion. We also highlighted the ability of Emergency Department to manage these patients in short-stay ward.

    Placenta praevia  is a known obstetric condition  that causes complications  to mother and fetus. This study was done to evaluate the knowledge of placenta praevia amongst the obstetric patients. A cross sectional study was carried out in Hospital Ipoh, Perak among 323 antenatal and postnatal patients. Socio-demographic parameters (ie age, race, parity, occupation, educational  level) and history of placenta praevia were studied  in relation to level of knowledge and attitude towards placenta praevia. Twenty (6.2%) from 323 women had current or past history of placenta praevia. Three had history of placenta praevia while 17 had current placenta praevia with prevalence of 5.3%. The mean score of knowledge achieved by patients was 11.8 which indicated overall poor knowledge. Occupation, level of education and history of placenta praevia were found to have a relationship with level of knowledge regarding placenta praevia in all obstetric patients. There was a significant relationship between attitude of patients with current and history of placenta praevia to level of knowledge regarding placenta praevia. (p=0.037,

    One of the most important cervical cancer risk factors is human papillomavirus (HPV) infection. The p53 gene is one of the most important targets of the HPV E6 gene. E6 protein has the ability to stimulate p53 degradation, inhibits several functions of wild-type p53 and it competes with its function including suppression of malignant growth. The aim of this study is to determine the differences in p53 expressions in pre-malignant and malignant cervical neoplasms. This is a retrospective study on 100 cases of cervical neoplasms. There were 21 cases of CIN 1, 8 cases of CIN 2, 25 cases of CIN 3, 36 cases of squamous cell carcinoma, 7 cases of adenocarcinoma and 3 cases of adenosquamouscarcinoma. All cases were evaluated by immunohistochemistry using p53 monoclonalantibody. Thirty six of the 54 pre-malignant cases (66.7%) were positive for p53 protein, n contrast to the malignant cases in which, 40 of the 46 cases (87.0%) were positive. he majority of CIN showed absent to focal staining (29/54, 53.7%). In contrast, 84.8% (39/46( of the invasive carcinoma showed regional to diffuse staining. The expression of p53 is greater in the malignant cervical neoplasms than the pre-malignant cervical lesions, suggesting that p53 overexpression is not an early phenomenon in the pathogenesis of cervical cancer. It is also shown to be slightly higher in percentage in CIN 2 and 3 when compared with CIN 1. However, a number of cases were p53 negative, suggesting that other factors may be involved and further HPV studies are indicated.

    The Hypoglycaemia Symptom Rating Questionnaire (HypoSRQ) is potentially
    useful for local research on hypoglycaemia. However, it requires adaptation and
    validation in local settings. This study reports the process and results of cross-cultural
    adaptation and linguistic validation of HypoSRQ for Malay and English versions in
    our local setting. The HypoSRQ underwent forward and backward translation and
    adaptation with support from professional translators and a clinical psychologist.
    Cognitive debriefing was done among patients with Type 1 and Type 2 diabetes
    mellitus from varying sociodemographic backgrounds. Discussion was done
    together with the original developers of the HypoSRQ to decide on the best version
    for local use. The finalised versions were proofread and formatted with the help
    of Health Psychology Research. Cognitive debriefing for Malay version involved 7
    patients and for the English version5 patients. Direct literal translation into Malay
    language was unsuitable due to technical terms which were difficult for laypersons
    to understand. Amendments were made based on findings from the cognitive
    debriefing process. Participants found the questionnaire fairly easy to understand.
    The HypoSRQ-My (Malay) and HypoSRQ-EMy (English) is easily understood by
    local participants. These tools may undergo psychometric evaluation for future
    use in local settings.

    A cross-sectional study was undertaken to evaluate if outpatient administration of in-travenous iron sucrose complex (Venofer) was a sensible option in treating iron defi-ciency anaemia during pregnancy and puerperium. A total of 120 patients with iron deficiency anaemia were recruited from the Obstetric Day Care Clinic at the Universiti Kebangsaan Malaysia Medical Centre (UKMMC) over 18 months from March 2003 to August 2004. The main outcome measures were haemoglobin increment, patients’ compliance, adverse effects and saving from hospitalization fees. The pre-treatment haemoglobin (Hb) level was 8.5+0.85g/dl for the antenatal patient and 7.6+0.80 g/dl in the post-partum group. The mean post-treatment haemoglobin increment at day four-teenth was 3.52+0.75g/dl. One patient developed skin rash while another had low-grade pyrexia. Seven patients experienced mild metallic taste. There were no serious side effects or anaphylactic reactions. Ten patients (8.3%) did not complete their ther-apy - eight delivered before completion of treatment; another two defaulted following delivery. The average number of Venofer used was seven ampoules i.e. 700mg per person, most of them required three sessions to complete the course. Outpatient treatment allows each patient to save hospitalization fees of RM45 per day, which to-talled up to RM135 for a 3-days ward stay. An estimation of RM16,200 hospitalization fees for the 120 patients was avoided during the study period. In conclusion, outpatient treatment of anaemia in pregnancy and post-partum period using Venofer was safe and feasible, with high patient compliant and cost-savings from hospitalization fees.

    The present study aimed to determine the factors associated with CKD stage 3b among type 2 diabetics attending primary care follow-up, specifically the role of angiotensin blockade dosage. This was a pilot unmatched case-control study conducted in a teaching primary care centre. Clinical data of 25 cases of diabetic patients with CKD stage 3b (GFR 30-45ml/min/1.73m2) in 2012 were selected for this study, as well as 103 controls who were diabetic patients with GFR more than 45ml/min/1.73m2 in 2012. Systematic random sampling was employed. Data was obtained from patients’ diabetic records, computerised clinical medical information system and medical case notes. Univariate analysis was done using Chi-square, t-test, Fisher’s exact test and Mann-Whitney U-test. Multiple logistic regression was used to determine the associated factors for development of CKD stage 3b. Cases and controls were different in terms of age, duration of diabetes, use and dosage of angiotensin blockade medications, systolic blood pressure and baseline GFR.     Primary fallopian tube carcinomas are rare and are difficult to diagnose preoperatively.
    We describe a fallopian tube carcinoma in a 50-year-old lady who presented with symptoms of pelvic inflammatory disease. Ultrasound examination showed bilateral hydrosalphinx. However, CT scan showed features suggestive of a malignant ovarian tumour. At surgery, a markedly dilated left fallopian tube was found and histopathological examination confirmed the presence of a primary serous adenocarcinoma. The uterus and both ovaries were free of tumour. This report highlights that carcinoma of the fallopian tube should be considered as a differential diagnosis in females who present with lower abdominal pain.
