    Seeking treatment is a vital when an individual falls sick. However, some may not do so perhaps they are ashamed to admitting having shameful diseases or unable to seek and reach help. Negative perception of the community on sensitive diseases like AIDS or sexual transmitted disease (STD) may drive the patients away from seeking treatment and suffer alone. In Islam we are thought to always have positive perceptions (husnu dzan) towards any issues in daily life especially when dealing with human interaction by avoiding bad perceptions (su’u dzan). Thus, this study aims to identify the factors affecting treatment seeking patterns among public in three selected areas in Kuantan, and to explore factors affecting people not able or avoid seeking treatment, also to determine the relationship between treatment seeking patterns and the influential factors ie distance, accessibility and behaviour; attitude to health, healthcare and self-perception. A cross sectional survey was carried out at Taman Gelora, Teluk Cempedak and Terminal Makmur, Kuantan, Pahang. Convenient sampling method was used in this study. 201 respondents participated and answered a semi-structured questionnaire modified from Adamson. Results showed that the data was normally distributed, 50.2% respondents were male and 58.7% the total respondents were between 16-26 years old.     Introduction: Scarcity of fresh water is currently blighting the population of the least developed countries around the world. According to WHO, waterborne diseases accounts for 4.1% of total disability adjusted life years (DALYs) with a mortality rate of 1.8 million per year. Numerous pathogens and chemicals may be the potential sources of water contamination that leads to waterborne diseases in humans. Some pathogens are only active and express disease whilst within the host. The aim of this article is to identify the cause of water-borne diseases and how to control it from the Islamic perspective. Material and Methods: In this review we had compiled the latest scientific findings related to waterborne diseases and integrated them with Islamic approach of cleanliness. There are many Hadithsof the Prophet (peace be upon him) (PBUH) regarding methods of cleanliness with scientific background to control waterborne diseases. Water pollution may be a natural process, men made, intentional or accidental. More often than not, man are infected after accidently consuming or coming in contact with contaminated water Results: Scientific literature search reveals that individual hygiene and sanitation is the most prominent way to prevent waterborne diseases. In Islam, Prophet Muhammad (PBUH) puts a great emphasize on purification in terms of personal cleanliness and made it mandatory to follow rule to remain pure both physically and spiritually. Conclusions: Sunnaticapproaches of cleanliness may hinder waterborne disease.

    For centuries, people have been practising phytomedicine in treating ailments or reducing risk of suffering certain diseases. It is considered as a part of an ancient medicine with interest in it becomes rapidly escalating in this modern era. Expansive and prescribed medicine is not the only true path to treat various illnesses. Without disclaiming the modern medicine, herbs can be used as an alternative or a combined therapy with conventional medication. Phytomedicine or ethnomedicine is not a new
    field in the world of research. It has been started long ago since the ancient times. As we realized the expensive modern medication is not the only mean to treat illness, interest in the discovery of remedy through natural or conventional products has become more escalating.     As age advances most of body system will follow a process of deterioration too and the same goes for reproductive health. There is a gap between the prevalence of reproductive health disorders and rates of treatment. This gap among middle aged and older men is still wide compared to other chronic disease. In Islam, seeking medical treatment for the cure of diseases is part of the Prophet’s tradition and the Prophet (PBUH.) said in the Hadith the following: “For every disease there is a medicine, and if that medicine is applied to the disease, he will recover by Allah’s Leave.” Therefore the aim of this study is to identify a health seeking behaviour among men in for men’s health (MH) disorders in Kuantan. A cross sectional study was conducted among 157 respondents from four governments agencies and the data was collected using self-administered questionnaire modified from a valid and reliable measure. The results showed that 68.2% of the respondents (n=107) have high attitude in seeking for health regarding men’s health while for the rest of the respondents (n=50, 31.8%) have low attitude in seeking for MH. Men from the Service factor are perceived the highest compared the other two factors which are cultural and economic factors. This study showed that MH that includes reproductive disorders has always been taken lightly by the community itself. A Lack of awareness regarding ME issue causes men to ignore the importance of health screening. This should be improved in order to produce a healthy community for better Ummahsince good heredity begins with good ME.

    Expression of beta-human chorionic gonadotropin (bHCG) mRNA in early cleavage cell stages is important in predicting embryo viability at blastocyst stage of development. Embryo viability is crucial for in vitro fertilization (IVF) treatment to be successful in terms of increment of implantation and pregnancy rate. In order to establish fertilization outside the human body, optimum condition mimicking the natural body environment like hormones and growth factors as well as impeccable timing must be established. ImamMuslim narrated from Hudhayfa ibn Asad that the Prophet Muhammad (SAW) said: “After the sperm-and-ovum drop (nut.fa) has been [in the uterus] forty-two days, Allah sends it an angel that gives it form and fashions its hearing, sight, skin, flesh, and skeleton”. Therefore the aim of this study is to find the significant association of bHCG expression with early cell stage cleavages and its relationship as predictive marker for potential embryo implantation. Our study focuses on leftover frozen embryos from eight patients consists of six pregnant patients and two non-pregnant patients. We assessed the human bHCG mRNA expression at different cell cleavage stages in these frozen embryos using reverse transcriptase-polymerase chain reaction (RT-PCR). Our results have shown three out of eight patients with five to ten cells of blastomeres were expressed with bHCG. This study indicated that bHCG was expressed on froze-thawed late cleavage stage of embryos in IVF patients.

    Objective: The present study is designed to investigate the occurrence of Acanthamoeba in air ventilation and air-conditioning systems in selected buildings in Kuantan.

    Methods: Acanthamoeba was isolated from dust samples taken from filters of air ventilation systems in selected buildings in Kuantan. The dusts were collected by using sterile cotton swabs, cultured in a xenic culture medium on non-nutrient agar (NNA) plates and incubated at 37º C. The plates were examined daily for any presence of Acanthamoeba cysts up to 7 days of incubation.

    Results: Based on the results obtained, there were no cysts or trophozoites of Acanthamoeba successfully isolated from all 75 dust samples. After three days of incubation, all culture plates examined show negative findings.

    Conclusions: The negative findings of this study were probably due to the limitations of the sampling method. It is recommended that future studies use the method proposed by National Institute of Occupational Safety and Health (NIOSH) for indoor air quality monitoring.

    Introduction: This study was conducted to determine the association between knowledge, attitude and practice among Kuantan community towards anti-smoking campaign. An anti-smoking campaign can be referred to as a series of ads programs that are introduced through various objective approaches to give awareness to people on the negative impacts of smoking behavior. The objectives of this study are (i) to investigate the association between so- cio-demographic data (gender, income, education level) and level of knowledge towards anti-smoking campaign; (ii) to find out the association between socio-demographic data (gender, income, education level) and level of attitude against anti-smoking campaigns; (iii) to study the association between socio-demographic data (gender, income, education level) and level of practice towards anti-smoking campaigns; and (iv) to find out the correlation between level of knowledge with level of attitude, level of knowledge with level of practice, and level of attitude with level of practice towards anti-smoking campaigns. Methods: A descriptive cross-sectional study was conducted among adult Kuantan community and convenience sampling was used to collect the data. A validated questionnaire was used to collect data. Statistical analysis of the data was carried out using SPSS and association of variables was an- alysed using Chi-square test. Results: This study found that level of knowledge did not affect the attitude level of the community. However, level of knowledge did affect the level of practice towards the anti-smoking campaign. Conclusion: It revealed that respondents with high level of knowledge also had good level of practice towards an- ti-smoking campaign.

    Introduction: Abdominal obesity and inflammation are two independent risk factors for cardiovascular disease. However, the inter-relationship between these two factors among Malay population is fragmented. The purpose of this study is to determine the association between waist circumference as a measure of abdominal obesity and high-sensitivity C-reactive protein as an indicator of inflammation status among Malay obese adults in Kuantan, Pahang. Materials and Methods: Ninety-three apparently healthy obese adults (BMI between 27.5 and 39.9 kg/m2 ) aged between 20 to 62 years old were recruited in this study (68% men and 32% women). Circumference tape was used to measure the waist circumference and fasting venous blood was obtained to determine the high-sensitivity C-reactive protein level. Results: On average, the waist circumference of obese men and women was 103 cm (SD = 8.7) and 94 cm (SD = 7.0), respectively. The median (IQR) of the hs-CRP level of the study participant was 3.80 mg/L (1.5, 10.1). The Kendall’s Tau correlation reveals that there is a significant positive linear correlation between waist circumference and hs -CRP level in obese men (r (30) = 0.355, p = 0.006) and women (r (63) = 0.177, p = 0.043). Conclusion: There is a significant inter-relationship between waist circumference and hs-CRP level among apparently healthy obese adults in Kuantan, Pahang. This study suggests that assessment of hs-CRP level is beneficial to predict the risk for future cardiovascular disease event and facilitate the management of obesity in this population. A prospective clinical study is recommended to confirm the findings.

    OBJECTIVE: Hypertriglyceridemia is an independent risk factor for cardiovascular diseases. This study aimed to determine the prevalence and association of triglyceride level and lifestyle factors among Malay obese class I and II adults.

    METHOD: This is a cross-sectional study of 65 Malay obese class I and class II adults aged 20-62 years (21 male, 44 female) from sub-urban areas of Malaysia. Overnight fasting venous blood samples were obtained to determine the triglyceride level (mmol/L). Subjects were classified into either normal or elevated triglyceride level groups based on the triglyceride level (normal < 1.6 mmol/L, elevated > 1.7 mmol/L). Unhealthy lifestyle behaviors, defined as smoking status, hours per day spent on sitting passively and sitting with active motion, and the amount of saturated fat, mono-unsaturated and polyunsaturated fat from dietary intake, were measured from 24-h dietary intake and physical activity recall. We compare the variables of unhealthy lifestyle behaviors between subjects with normal and elevated triglyceride level using independent samples t-test.

    RESULTS: Among 65 obese class I and II adults, 16 subjects (24.6%) were found to have elevated triglyceride levels (mean ± standard deviation of body mass index 31.89 ± 3.29 kg/m2). There are significant differences between subjects having normal and elevated triglyceride level with gender, marital status, the number of children, smoking status, weight and monounsaturated fat intake (all P-values < .05).

    CONCLUSIONS: The findings of this study highlighted elevated triglyceride level in obese adults might be influenced by unhealthy lifestyle behaviors. We suggest that lifestyle modification intervention is appropriate to prevent cardiovascular disease among Malay obese class I and II adults.

    The expression product of VASA gene is widely conserved germ line marker and participates to regulate the development of reproductive system and spermatogenesis in healthy man. Azoospermic is a condition which the man is unable to produce any sperm cells for reproduction activity. This condition produced has bad impact to the man since the ability to produce their own progeny will be interrupted or blocked forever. Not only the family bloodline of the man would be ended, the psychological issue like shamefulness and low self-esteem occurs. In Islam, seeking knowledge is an obligatory to Muslims in order to solve problems and improve lives. Thus to unveil the problem of azoospermic man we aimed to determine the level of VASA gene expression in samples of testes cells of non-obstructive azoospermic (NOA) and compare it with sperms of healthy man. Samples were taken from three NOA patients by testicular sperm extraction (TESE) to obtain testicular biopsies. Testicular cells were isolated and cultured in supplemented knockout DMEM media. VASAgene expression was determined by reverse transcriptase polymerase chain reaction (RT-PCR). The VASA gene expression from sperm of healthy man was also determined for comparison purpose. It was demonstrated that the addition of growth factor significantly increased SSC-like cells colony formation in tissues obtained from NOA patients. No VASA expression was detected in spermatogonial-like stem cells culture on day 1, 7, 14 and 21 in each of the azoospermic samples. Our findings shown VASA gene was not expressed in spermatogenesis in vitroculture that might be associated with the abnormal differentiation of primordial germ cells that lead to male infertility. Islam also teaches us to not have feelings of despair and for problems that we are facing but to find the solution and consider the problem as a test from the Almighty. All the tests should be handled with strong believe since only Allah (SWT) knows what the best for us and each of the tests will have justified wisdom and benefits which we as human being are unable to realise thoroughly.

    Azoospermia is present in 15% of infertile cases and it is a major concern due to inability to produce sperm. Most of IVF (in-vitro fertilization) clinics abroad has been using sperm donation via sperm bank facilities as a solution for infertile couple to have their own offspring. In Islam, it is forbidden to use sample from male other than their spouse. It is according to maqasid syari’ah to ensure the heredity of the human being. Based on the latest technology, one approach of stem cell differentiation process had been established to produce mature cells from primitive or immature cells (stem cells). This technology is in line with the concept of maqasid syari’ah since we are using the cells from one person. We try to adopt this technology to study the potential of testicular cells from non-obstructive azoospermic (NOA) patient to undergo in vitrospermatogenesis.Samples were cultured in modified human embryonic stem cells (HESC) media with specific growth factors; basic fibroblast growth factor (bFGF) and leukemia inhibitory factor (LIF). Protein expressions were analyzed by immunofluorescent staining on day 49 and 90 of culture. Results show spermatogonial stem cell-like cells (SSC-like cells) colonies formed after 14 to 21 days, the cells were expanding successfully and were stable for 49 days duration. Then SSCs differentiated into later stage of spermatogenesis on day 90. Four specifics SSCs protein markers were identified on day 49; ITGA1, ITGB1, CD9 and GFRA1 whereas SCP3 and TP1 proteins were expressed on day 90. This in vitrospermatogenesis suggests a possible approach for future for Muslim NOA patients in order to have their own children.

    Oxidative stress induced by excessive and unopposed levels of reactive oxygen species in male reproductive system results in impaired sperm quality, fertilization capacity and poor embryo development. Our goal is to assess the potential effects of Aquilaria malaccensis(AM) leaves, a plant with strong antioxidant property on early embryo development in vitroand embryo quality following fertilization with cyclophosphamide (CP) exposed rat sperm. Materials and Methods:Twenty four male Sprague Dawley rats were allocated into eight groups of three rats (n = 3): control, CP only (200 mg/kg), AM only (100 mg/kg, 300 mg/kg and 500 mg/kg) and CP + AM (100 mg/kg, 300 mg/kg and 500 mg/kg). Animals were sacrificed after 63 days of treatment and sperm from caudal epididymis were taken for in vitrofertilization (IVF) with oocytes from untreated female. Fertilization, embryo division and embryo morphology were examined at 8 and 48 hours post insemination and compared between groups.
