    The purpose of this study is to compare the road conditions (straight road, winding road and hill road) with the hand grip pressure force and muscle fatigue for male and female drivers. Ten subjects were participated in this study. The force measurement and electromyography (EMG) responses were taken and evaluated by using the tactile grip and pressure measurement (Grip System) and Electromyography (EMG) device. The result indicated that the winding road produced more muscle fatigue and high hand grip pressure force compared than downhill road, hill up road, and straight road for both male and female subjects. The result compared the muscle fatigue and hand grip pressure force between the first 15 minutes and last 15 minutes of driving activity. The muscle fatigue increasingly high for the last 15 minutes compared to first 15 minutes. However, the hand grip pressure forces become high during the winding road for first 15 minutes of driving session. The muscle fatigue become high as the hand grip pressure force value is high. Furthermore, the male drivers exert higher hand grip pressure force and higher muscle fatigue compared to female drivers. This study can be used as a guideline for the future studies, primarily in solving the driving fatigue problem among the Malaysian’s drivers. The method of this study could also be used for early detection of driver fatigue issues. Indirectly, the findings could reduce the number of car accidents in Malaysia.

    A patient with bipolar I disorder has been treated with lithium and haloperidol for the last 20 years and received an ACE inhibitor for his hypertension since 9 years ago. Despite regular clinic follow-ups and blood monitoring, he recently developed tremors and delirium. On hospital admission, serum level of lithium was far above toxic level. Mental state examination revealed an anxious and disorientated man with irrelevant speech. Immediate discontinuation of lithium resulted in slow reduction of serum lithium levels and gradual resolution of tremor but his delirium persisted for 2 weeks. His condition took a turn for the worse when he developed acute renal failure and arm abscess. We discussed about lithium toxicity and the vulnerability factors which have induced delirium and renal failure in this patient.

    As with many other countries, Malaysia is also developing and promoting biomedical research to increase the understanding of human diseases and possible interventions. To facilitate this development, there is a significant growth of biobanks in the country to ensure continuous collection of biological samples for future research, which contain extremely important personal information and health data of the participants involved. Given the vast amount of samples and data accumulated by biobanks, they can be considered as reservoirs of precious biomedical big data. It is therefore imperative for biobanks to have in place regulatory measures to ensure ethical use of the biomedical big data. Malaysia has yet to introduce specific legislation for the field of biobanking. However, it can be argued that its existing Personal Data Protection Act 2010 (PDPA) has laid down legal principles that can be enforced to protect biomedical big data generated by the biobanks. Consent is a mechanism to enable data subjects to exercise their autonomy by determining how their data can be used and ensure compliance with legal principles. However, there are two main concerns surrounding the current practice of consent in biomedical big data in Malaysia. First, it is uncertain that the current practice would be able to respect the underlying notion of autonomy, and second, it is not in accordance with the legal principles of the PDPA. Scholars have deliberated on different strategies of informed consent, and a more interactive approach has recently been introduced: dynamic consent. It is argued that a dynamic consent approach would be able to address these concerns.

    Driving activity has become more important as this medium being practical, it is also cheaper and faster in
    connecting human from one to another place. However, in some occurrence, it can cause accidents as they become
    fatigued while driving. Driver fatigue is one of the top contributors to the road accidents and can be dangerous as
    other road safety issues such as drink driving. Worst is, there are no laws regulating driver fatigue. Therefore, the
    main purpose of this study is to develop the regression model of apsychophysical factor for drivers’ fatigue which can
    predict the relationship between the process input parameters and output responses. The study was participated by
    ten subjects. The heart rate was taken and recorded using heart rate monitor. Design Expert 8.0.6 software was used
    for the regression analysis. The modeling validation runs werewithin the 90% prediction intervals of the developed
    model and the residual errors were less than 10%. The R

    value is 0.9400 whichmeans that the linear regression line
    passed exactly through all points. The significant parameters that influenced the heart rate were also identified.The
    parameters are time exposure, type of road, and gender.

    Malaysia aspires to develop regenerative medicine through stem-cell technology. It needs a regulatory system that could facilitate development and prevent unethical practices. A comparative legal analysis on the regulation of stem-cell technology, with a focus on stem-cell research in Malaysia and selected Commonwealth countries that are experienced in regulating this complex technology, demonstrates that the selected Commonwealth countries have adopted a hybrid of different regulatory mechanisms. This paper argues that Malaysia should consider adopting a similar approach to equip relevant authorities with different regulatory mechanisms that are able to promote innovation in stem-cell research activities and cultivate a successful and profitable regenerative medicine industry in the future. Such a strategic action can produce an optimal regulatory outcome and help Malaysia to realize its aspiration.

    Even though there is a significant amount of scholarly work examining the ethical issues surrounding human genomics research, little is known about its footing in Malaysia. This study aims to explore the experience of local researchers and research ethics committee (REC) members in developing it in Malaysia. In-depth interviews were conducted from April to May 2021, and the data were thematically analysed. In advancing this technology, both genomics researchers and REC members have concerns over how this research is being developed in the country especially the absence of a clear ethical and regulatory framework at the national level as a guidance. However, this study argues that it is not a salient issue as there are international guidelines in existence and both researchers and RECs will benefit from a training on the guidelines to ensure genomics research can be developed in an ethical manner.

    The aim of the review was to critically appraise the various pros and cons of the synthetic and herbal agents used in mouthwashes against halitosis and facilitate users to choose appropriate mouthwashes according to their need. Oral Malodour (OMO) or halitosis is a global epidemic with social and psychological impact. Use of mouthwash has been adopted worldwide to control halitosis within a past few decades. Alcohol and Chlorhexidine are common agents in synthetic mouthwashes, while Tannins and Eugenol are derived traditional herbal extracts. Each agent signifies some unique properties distinguishing them from others. Herbal ingredients are gaining the attention of the profession due to its mild side effects and competitive results. Herbal mouthwashes can be a safer choice in combating OMO, as an alternate to synthetic mouthwashes.

    Re-expansion pulmonary edema (RPE) after chest drain insertion is rare. The objective of this clinical case report is to highlight the importance of this chest drain insertion complication. A 35-year-old man presented to the emergency department with a chief complaint of shortness of breath and pleuritic chest pain. Further physical examination and radiographic investigations showed a left-sided hemipneumothorax. A chest drain was inserted, but subsequently the patient developed worsening shortness of breath, desaturation, and coughed out pink frothy sputum. Repeated chest radiographic and computed tomographic thorax findings suggested RPE. A nonrebreathable mask with high-flow oxygen was given to the patient to maintain his oxygen saturation. The patient was referred to the cardiothoracic team and was admitted to the hospital. Despite conservative management in the ward, the patient underwent lung decortication. Postdecortication, the left-sided lung re-expanded well, and the patient was discharged home. This case highlighted this rare, potentially fatal complication of chest drain insertion for spontaneous pneumothorax.

    Geopolymer concrete has the potential to replace ordinary Portland cement which can reduce carbon dioxide emission to the environment. The addition of different amounts of steel fibers, as well as different types of end-shape fibers, could alter the performance of geopolymer concrete. The source of aluminosilicate (fly ash) used in the production of geopolymer concrete may lead to a different result. This study focuses on the comparison between Malaysian fly ash geopolymer concrete with the addition of hooked steel fibers and geopolymer concrete with the addition of straight-end steel fibers to the physical and mechanical properties. Malaysian fly ash was first characterized by X-ray fluorescence (XRF) to identify the chemical composition. The sample of steel fiber reinforced geopolymer concrete was produced by mixing fly ash, alkali activators, aggregates, and specific amounts of hook or straight steel fibers. The steel fibers addition for both types of fibers are 0%, 0.5%, 1.0%, 1.5%, and 2.0% by volume percentage. The samples were cured at room temperature. The physical properties (slump, density, and water absorption) of reinforced geopolymer concrete were studied. Meanwhile, a mechanical performance which is compressive, as well as the flexural strength was studied. The results show that the pattern in physical properties of geopolymer concrete for both types of fibers addition is almost similar where the slump is decreased with density and water absorption is increased with the increasing amount of fibers addition. However, the addition of hook steel fiber to the geopolymer concrete produced a lower slump than the addition of straight steel fibers. Meanwhile, the addition of hook steel fiber to the geopolymer concrete shows a higher density and water absorption compared to the sample with the addition of straight steel fibers. However, the difference is not significant. Besides, samples with the addition of hook steel fibers give better performance for compressive and flexural strength compared to the samples with the addition of straight steel fibers where the highest is at 1.0% of fibers addition.

    Intrahepatic cholestasis of pregnancy (ICP) is the commonest among the specific dermatoses of pregnancy. The disease is characterised by intense pruritus and specifically by elevated bile acid levels and owing to the rarity of data published in this context, the disease carries a great challenge in both diagnosis and management. The disease is associated with significant maternal as well as perinatal adverse effects, hence, this article aims at improving the knowledge of the women's health carers with the up-to-date and evidence-based, whenever possible, recommendations while managing patients with ICP.

    This paper aims to find out the effect of different weight percentages of geopolymer filler in glass-reinforced epoxy pipe, and which can achieve the best mechanical properties and adhesion between high calcium pozzolanic-based geopolymer matrices. Different weight percentages and molarities of epoxy hardener resin and high calcium pozzolanic-based geopolymer were injected into the glass fiber. By manually winding filaments, composite samples were produced, and they were then allowed to cure at room temperature. To determine how well the geopolymer matrices adhere to the fiber reinforcement, the microstructure of the composites' surfaces and perpendicular sections were examined. Maximum values of compressive strength and compressive modulus were 94.64 MPa and 2373.58 MPa, respectively, for the sample with a weight percentage of filler loading of 30 wt% for an alkali concentration of 12 M. This is a relatively wide range of geopolymer weight percentage of filler loading from 10 wt% to 40 wt%, at which we can obtain high compressive properties. By referring to microstructural analysis, adhesion, and interaction of the geopolymer matrix to glass fiber, it shows that the filler is well-dispersed and embedded at the fiber glass, and it was difficult to determine the differences within the range of optimal geopolymer filler content. By determining the optimum weight percent of 30 wt% of geopolymer filler and microstructural analysis, the maximum parameter has been achieved via analysis of high calcium pozzolanic-based geopolymer filler. Fire or elevated temperature represents one of the extreme ambient conditions that any structure may be exposed to during its service life. The heat resistance or thermal analysis between glass-reinforced epoxy (GRE) pipe and glass-reinforced epoxy pipe filled with high calcium pozzolanic-based geopolymer filler was studied by investigating burning tests on the samples, which shows that the addition of high calcium pozzolanic-based geopolymer filler results in a significant reduction of the melted epoxy.
