Introduction: This study is aimed to examine the predominant inflammatory cells in nasal polyps (NP) in the
local community and its correlation to the clinical presentations.
Materials and Methods: The study was
done retrospectively looking at patients who had undergone functional endoscopic sinus surgery (FESS) in
Hospital Universiti Sains Malaysia (HUSM), Kelantan, Malaysia with a histopathological diagnosis of nasal
polyposis (NP), between the years 2004 to 2008. Sixty two patients between the ages of 18 years to 60 years
old were selected and data relevant to the study were collected from the patient’s folders using a specially
created form prepared for the study. The NP histopathology report from each patient underwent FESS were
analysed and the patients were divided into eosinophilic and non eosinophilic dominant group. Clinical
presentations from each patient were also gathered and analysed according to the NP group.
Results: In
HUSM, there were a higher number of eosinophilic types NP as compared to the neutrophilic type NP which
is contrary to other study conducted on Asian populations. The clinical symptom correlations between either
eosinophilic or non-eosinophilic type of NP have not shown any significant associations.
Conclusion: The
study showed that the incidence of histological subtypes of nasal polyp in HUSM is almost the same as that
found in other parts of the world (Europe and North America) which will reduce the possibility of racial or
geographical influence on the pathogenesis of the nasal polyp. Clinical symptoms and presentation alone are
not enough to differentiate the type of the nasal polyp without the histological study.
Cellulitis in human following leech bite is rae, many of the case reports were associated with the use of medical leech. We report the first documented case of cellulitis following the bite of land leech (Haemodipsa species), known locally as 'pacat', in a 2 month-old-baby. The baby developed fever but subsided following an antibiotic cover. The baby had uneventful stay in the hospital and the cellulitis was successfully treated following a week cource of Cloxacillin.
Follicular adenoma defined as a well encapsulated benign neoplastic condition of thyroid follicles. We describe this interesting case of a boy presented with right sided neck swelling that was diagnosed as a branchial cyst but later after excision, the histopathology revealed follicular adenoma of the thyroid with hemorrhage and degenerative changes in the centre. To our knowledge, this is the first reported case of follicular thyroid adenoma presented as branchial cyst.
Obsessive-compulsive disorder (OCD) is a neuropsychiatric disorder characterized by recurrent intrusive, distressing thoughts and repetitive behaviours or mental rituals performed to reduce anxiety. The lifetime prevalence of OCD is 2.3% and it can happen to all people of various levels, including child and adolescent. The mean age of onset is 19.5 years, and a subset of patients, mostly males, have an early onset before 10 years of age. The lifetime risk of developing OCD is higher in females, who typically develop the disorder in adolescence [1]. Person with OCD usually presented with obsession involving various themes, namely contamination; repeated doubts; religious, need for symmetry and exactness, or taboo thoughts of a sexual, religious, or aggressive nature. Whereas, the most common compulsions are checking, washing, hoarding, and counting compulsions[1]. (Copied from article).
Objective: The objectives of this study were to determine the effect of a one and a half year educational intervention on the job dissatisfaction of teachers in 30 Community Based Rehabilitation (CBR) centres in Kelantan, Malaysia, and to identify the factors influencing changes in job dissatisfaction following the intervention. Method: Ten educational modules were administered to the teachers. A validated Malay version of Job Content Questionnaire (JCQ) was used pre intervention, mid intervention and post intervention. Result: Repeated Measure ANOVA revealed there was a statistically significant reduction in the mean of job dissatisfaction (p = 0.048). Multiple Linear Regression revealed that co- worker support (β= 0.034 (95% CI = 0.009, 0.059)), having less decision authority (β: -0.023; 95% CI: -0.036, -0.01) and being single (β: -0.107; 95% CI: -0.176,-0.038) were significantly associated with decreases in job dissatisfaction. Conclusion: The intervention program elicited improvement in job satisfaction. Efforts should be made to sustain the effect of the intervention in reducing job dissatisfaction by continuous support visits to CBR centres.
