Recent statistics from the Social Security Organization (SOCSO) of Malaysia shows Commuting Accident (CA)
increased consistently by 1500 cases per year from 2008 to 2012. This has resulted in the rising of fatality rate,
extensive claims of compensation payment and not to mention loss of valuable talents. However, little is known
about the contributing factors to the occurrences of CA in Malaysia because CA is never considered to be work
related before. This study aims to explore work related factors with occurrence of CA using 5-year statistical data
from SOCSO, Malaysia. Moreover, we also reviewed studies published between 1990s and 2014 to support the
statistical findings. Motorbike is found as the most common vehicle used which involved in CA. Individual factors
concerning workers behaviour is a main risk factor of CA. Family related factors (parenting responsibility), work
burden, workplace support as well as environmental factors such as bad weather and bad road conditions are also
significant contributions of CA. It is very important to develop behavioural intervention strategies and provide proper
training. Hence, more attention should be directed to young individual workers in balancing them with capabilities and
organization performance demand. This may lead to the elimination of the other causes of CA. In conclusion, solutions
to this problem involve not only a particular party to ensure the wellbeing of workers in Malaysia, but all authorities
should play roles in enhancing safety and health matter of workers especially on the occurrence of accident.
This study proposes a neural network (NN) model to predict and simulate the propagation of vehicular traffic noise in a dense residential area at the New Klang Valley Expressway (NKVE) in Shah Alam, Malaysia. The proposed model comprises of two main simulation steps: that is, the prediction of vehicular traffic noise using NN and the simulation of the propagation of traffic noise emission using a mathematical model. First, the NN model was developed with the following selected noise predictors: the number of motorbikes, the sum of vehicles, car ratio, heavy vehicle ratio (e.g. truck, lorry and bus), highway density and a light detection and ranging (LiDAR)-derived digital surface model (DSM). Subsequently, NN and its hyperparameters were optimised by a systematic optimisation procedure based on a grid search approach. The noise propagation model was then developed in a geographic information system (GIS) using five variables, namely road geometry, barriers, distance, interaction of air particles and weather parameters. The noise measurement was conducted continuously at 15-min intervals and the data were analysed by taking the minimum, maximum and average values recorded during the day. The measurement was performed four times a day (i.e. morning, afternoon, evening, and midnight) over two days of the week (i.e. Sunday and Monday). An optimal radial basis function NN was used with 17 hidden layers. The learning rate and momentum values were 0.05 and 0.9, respectively. Finally, the accuracy of the proposed method achieved 78.4% with less than 4.02 dB (A) error in noise prediction. Overall, the proposed models were found to be promising tools for traffic noise assessment in dense urban areas.
In Indonesia, arthrodesis becomes a choice of treatment in the absence of ankle arthroplasty implants for young adults. Arthrodesis on ankle osteoarthritis (OA) often leads to functional impairment. Low tibiofibular osteotomy is an alternative and it has been known to be the preferable option for those in the productive-ages. A 22-year-old male with a previous history of a motorbike accident, operated eight years ago, came with persistent pain on the left ankle that has worsened over the years. Plain radiography with a tibial-ankle surface angle (TAS) of 74o (normally 88o-93o) indicated varus deformity. Osteotomy was performed on distal tibia above the syndesmotic joint, as well as on the middle third of fibula. Open wedge osteotomy of the tibia was corrected until the normal TAS angle was reached by fluoroscopy. Cortical allograft was used to fill the osteotomy gap. Instrumentation was performed using a clover leaf® plate with 6 screws insertion for fixation stability. All results were satisfactory. Twelve weeks post-operatively, the patient was performing activities normally. Four-month post-operative radiological evaluation showed fusion of graft and the angle of TAS of 89°. Post-operative functional assessment using the American Academy of Orthopaedic Surgeon (AAOS) Foot and Ankle Measurement (FAM) questionnaires showed significant improvement (pre 89, post 38).
A retrospective cross-sectional study of injury patients who attended the Accident and Emergency Unit was conducted at the University of Malaya Medical Centre. Demographic data with regards to age, sex, race, occupation and the relevant data such as type of accident, type of injury sustained and also severity seen during the month of January 2005 were selected retrospectively from medical record. The objectives were to review road traffic accidents and their severity of injuries among the patients. There were two hundred and forty-three (197 males and 46 females) patients involved: 60% Malays, 16% Chinese, 21% Indians and 3% other ethnic groups. Peak accidents occurred among the 21 to 30 years age group. Among the type of accidents, 65.8% involved motorbike riders, 10.3% car drivers, 8.6% pillion riders, 7.8% car passengers, 5.8% pedestrians and 1.6% were bicycle rider injuries. About 57.6% sustained minor injuries and 42.4% major injuries. There was no significant association between outcome of road traffic accidents and sex, age group and race. However, there was a significant association between type of accidents and severity of injuries (p = 0.014). Males sustained more major injuries and motorbike accidents were the highest among road injuries. Therefore, further prevention and control with emphasis on behavioural changes, education and law enforcement may reduce the number of road traffic accidents in the future.
