    Silver catfish (Pangasius sutchi) skin gelatin was extracted to determine the effects of extraction time on the functional properties of the gelatin in terms of solubility, protein solubility as a function of pH and sodium chloride concentration, emulsifying capacity and stability, water holding capacity, fat binding capacities and foaming properties. Silver catfish skins were washed in sodium chloride (NaCl) solution prior to pre-treatment in sodium hydroxide (NaOH) and acetic acid solution. Gelatin was extracted at 50ºC for 6, 8, 10 and 12 hours extraction time followed by freeze drying. The extraction of silver catfish skin gelatin at 50 ºC for 12 hours was more effective than extraction at 6, 8 and 10 hours where the gelatin was characterized by higher emulsifying capacity (52.63%), emulsifying stability (47.83%), water holding capacity (31.78 mL/g), fat binding capacities (54.76%), foaming capacity (41.47 mL) and foaming stability (56.42%) than gelatins extracted at other extraction time. The longer the extraction time, the better the functional properties of the gelatin. Based on its good functional properties, silver catfish skin gelatin may be useful in various food applications such as soups, sauces and gravies.

    Tinospora crispa Miers (Menispermaceae) is a climbing vine with stems rich in warts. The plant is called Akar Seruntum or Patawali in Malaysia and is widely used for treating skin complaints, malaria, bacterial abscess, high blood pressure and diabetes. In the present study, the stems of T. crispa were collected from the locality and succesively extracted with petroleum ether, followed by chloroform and ethanol. The insecticidal active extract (ethanol extract) was subjected to column chromatography of silica gel eluted with a gradient mobile phase containing hexane, chloroform and ethanol. Among the chemical constituents isolated are n-tetracosyl trans-ferulate and n-octacosyl alcohol, along with three known aporphine alkaloids; N-formylnornuciferine, N-acetylnornuciferine and lysicamine. All compounds were identified by comparing their spectroscopic data (UV, IR, 1H NMR, MS) with data from corresponding values in the literature. Isolation of n-tetracosyl trans-ferulate and noctacosyl alcohol is reported the first time for T. crispa.

    Mercury emission into the atmosphere is a global concern due to its detrimental effects on human health in general. The two main sources of mercury emission are natural sources and anthropogenic sources. Mercury emission from natural sources include volcanic activity, weathering of rocks, water movement and biological processes which are obviously inevitable. The anthropogenic sources of mercury emission are from coal combustion, cement production and waste incineration. Thus, in order to reduce mercury emission it is appropriate to investigate how mercury is released from the anthropogenic sources and consequently the mercury removal technology that can be implemented in order to reduce mercury emission into the atmosphere. Many alternatives have been developed to reduce mercury emission and the recent application of activated carbon showed high potential in the adsorption of elemental mercury. This paper discusses the ability of activated carbon and variable parameters that influence mercury removal efficiency in flue gas.

    Increase in cross-sectional muscle area of major muscle groups associated by heavy resistance training has been well documented. However, there has been no published article of changes in the inferior orbicularis oculi muscle when heavy resistance training is applied. We present a case of inferior orbicularis hypertrophy detected on MRI in a gentleman who practised heavy resistance training using his lower eyelids.

    Introduction: None of the identified studies employed quantitative scales correlating the knowledge and attitudes toward epilepsy. Method: This study aimed to study the relationship between the knowledge and attitudes toward epilepsy, using the Epilepsy Knowledge Questionnaire (EKQ) and the Public Attitudes Toward Epilepsy (PATE) scale. Results: A total of 279 publics with a mean age of 36.6±14.7 years (ranged from 15-77 years), 50.8% female with majority (63.0%) Chinese, and 49.5% with tertiary education level, were recruited.     Background and Objective: Intravenous thrombolysis service for stroke was introduced at the Universiti Kebangsaan Malaysia Medical Centre (UKMMC) in 2009, based on the recommendations of a multidisciplinary team of clinicians. We report the experience at our center in establishing a stroke protocol incorporating computed tomography perfusion (CTP) of the brain, to assess the feasibility of incorporating CTP in the stroke protocol.
    Methods: A retrospective review of all patients who had a CTP between January 2010 and December 2011 was performed. Results: Of 272 patients who were admitted with acute ischemic stroke, 44 (16.2%) arrived within 4.5 hours from symptom onset and had a CTP performed with the intention to treat. The median time for symptom-to-door, symptom-to-scan and door-to-scan was 90.0 minutes (62.5 – 146.3), 211.0 minutes (165.5 – 273.5) and 85.0 minutes (48.0 – 144.8) respectively. Eight patients (2.9%) were thrombolysed of whom five received IV thrombolysis and three underwent mechanical thrombolysis. The median symptom-to-needle and door-to-needle times were 290.5 minutes (261.3 – 405.0) and 225.0 minutes (172.5 – 316.8) respectively. Four patients were thrombolysed despite being outside the window of treatment based on the CTP findings. Six of the thrombolysed patients had a Modified Rankin Score (MRS) of 1-2 at 5 months post procedure.
    Conclusions: CTP provides a benefit to management decisions and subsequent patient outcome. It is feasible to incorporate CTP as a standard imaging modality in a stroke protocol. The delays in the time-dependent pathways are due to our work flow and organisational process rather than performing the CTP per se.

    Introduction: None of the quantitative scale for public attitudes toward epilepsy was translated to Chinese language. This study aimed to translate and test the validity and reliability of a Chinese version of the Public Attitudes Toward Epilepsy (PATE) scale. Methods: The translation was performed according to standard principles and tested in 140 Chinese-speaking adults aged more than 18 years for psychometric validation. Results: The items in each domain had similar standard deviations (equal item variance), ranged from 0.85-0.95 in personal domain and 0.75-1.04 in general domain. The correlation between an item and its domain was 0.4 and above for all, and higher than the correlation with the other domain. Multitrait analysis showed the Chinese PATE had a similar variance, floor and ceiling effects, and relative relationship between the domains, as the original PATE. The Chinese PATE scale showed a similar correlation with almost all demographic variable except age. Item means were generally clustered in the factor analysis as hypothesized. The Cronbach’s α values was within acceptable range (0.773) in the personal domain and satisfactory range (0.693) in the general domain. Conclusion: The Chinese PATE scale is a validated and reliable translated version in measuring the public attitudes toward epilepsy.

    Myopathies, although presenting more commonly in the younger age group, can occur and contribute significantly to disability in the elderly. To describe the spectrum of elderly myopathies, we reviewed 52 elderly patients (> 65 years) from the University of Malaya Medical Centre muscle biopsy databank, constituting 6.8% of 759 adult patients (> 18 years) who underwent muscle biopsy between 1992 and 2012. Commonest were the inflammatory myopathies (41/52, 78.8%), of which 43.9% had dermatomyositis; 23.9% polymyositis; 14.6% sporadic inclusion body myositis; 9.8% undifferentiated myositis and 2.4% overlap myositis. Seven patients (13.4%) had genetic myopathy; 2 muscular dystrophy and 5 chronic progressive external ophthalmoplegia, while 4 patients (7.7%) had drug-associated myopathy, 3 with statins. Malignancies were seen in 9.8% of inflammatory myopathies at diagnosis. Both acquired and genetic myopathies are seen in elderly Malaysians of all ethnicities and should not be misdiagnosed as some are potentially treatable and/or associated with malignancy.

    Background and objectives: Radiation treatment in nasopharyngeal carcinoma (NPC) is known to be associated with increased prevalence of carotid stenosis. The objectives of the study was to determine the prevalence of radiation-induced extracranial carotid stenosis, plaque, carotid intima thickness (CIMT) in NPC patients; to explore whether the stenosis is due to direct effect of radiation rather than general tendency to atherosclerosis. Methods: This was a cross-sectional study conducted in the University Malaya Medical Centre from July 2011 to February 2012. The study subjects consisted of 47 NPC patients who were treated with radiation, and 47 healthy control. The patients and control had carotid duplex ultrasound and transcranial Doppler (TCD). Results: The mean age of the patients was 55.1 years, the time lapse from radiation was 76.7 ± 95.3 months. Internal carotid artery (ICA) and common carotid artery (CCA) stenosis of ≥50% was seen in 17.0% of patients vs 2.1 % of controls (p = 0.031), with 61.7% of patients and 19.1% of controls having plaque in ICA and CCA (p=0.004). CIMT was increased in 70.2% of patients and 44.7% of controls (p =0.022). Both the patient group and control had similar rate of intracranial stenosis of 12.8% in TCD.
    Conclusion: Extracranial internal carotid artery is the most common site of stenosis following radiotherapy in NPC. This suggests that local trauma from irradiation is the most important factor in predisposition to atherosclerosis following radiation therapy.

    Children with epilepsy are at increased risk of behavioral problems and psychiatric disorders compared with the general population and those with other chronic illnesses. Depression and anxiety disorders are the most common and require further attention because they carry the risk of reduced quality of life and have far reaching consequences in later life. The wide range of prevalence quoted in the literature reflects methodologic differences. Risk factors are often multifactorial and include illness severity, coexisting morbidity, and psychosocial factors. Diagnosis remains a challenge, as the presentation is heterogenous and age related. This is complicated by epilepsy issues and associated comorbidity that hamper the use of standardized assessment tools. Multimodal treatment involving psychotherapy and antidepressant therapy currently represents the best approach. The mental health needs of these children are largely unmet; increased awareness and better research are required to determine best practices.

    Previous studies have shown that carbamazepine-induced Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) patients is associated with the HLA-B*1502 allele. Screening for HLA-B*1502 before using carbamazepine can prevent SJS/TEN particularly in populations with high frequency of the allele. The objective of this paper was to describe how the UKM Medical Centre, Malaysia was able to set up a cost effective screening of HLA-B*1502 for patients taking carbamazepine. The cost of in-house HLA-B⁄1502 screening was less than those commercially available, and was sensitive and specific.

    Congenital intracranial tumors are rare and account for 0.5 to 1.5% of all childhood tumours. We report a case of a 3 week old baby presenting with multi compartmental congenital intracranial immature teratoma, first of its kind in the literature. The child had gross total excision in two stages with aid of neuronavigation. The short term outcome was good. The four years of follow-up with serial imaging showed no tumour recurrence with a stable hydrocephalus after shunting. However, there is global developmental delay with full time dependence of care giver.

    Acute mountain sickness is an illness caused by climbing to a high altitude without prior acclimatization. Neurological consequences, like parkinsonism following acute mountain sickness without lesion of brain MRI have been reported rarely. A healthy 56-year-old man presented with dysarthria and gait disturbance. Neurological examination revealed tremor of hands, limb rigidity, and bradykinesia. The symptoms developed approximately 30 days following a 3,500 m climb of the Annapurna in the Himalayas. Brain MRI did not reveal any abnormalities including globus pallidus. The parkinsonism symptoms persisted for about 3 months before a complete recovered was made. We suggest that parkinsonism can develop after climbing to a high altitude but that the symptoms can be transient if a brain MRI detects no abnormalities.

    The purpose of this study is to explore and understand the exercise maintenance in women with breast cancer survivors adopting Guolin Qigong using the Social Cognitive Theory related constructs. A qualitative approach using in-depth interviews were employed in this study. 22 participants of the Malaysia Guolin Qigong Association from five regions in Peninsular Malaysia were purposely selected to participate in this study. Data was collected through a series of audiotapes during the interview sessions. They were asked on how do they maintain in their Guolin Qigong exercise. Three themes related to the Social Cognitive Theory constructs emerged from the data collected. (i) Personal Factors; (ii) Environmental Factors and (iii) Tailored Program. The belief in Guolin Qigong or in an exercise program is crucial in helping one to maintain with the exercise program and this findings is consistent with literature that those responded favourably to an exercise will have greater positive attitudes to exercise maintenance. To keep these women maintain the exercise it is important that these women have friends who understand them better to share their experiences related to their disease. It has been suggested that connecting to other survivors contributes to the survivors' positive well-being and to deal with their everyday living. Lastly, the type of exercise should be tailored to the physical condition of these women to keep them sustain the exercise. This theory can be used by the health practitioners to develop more effective interventions for increasing exercise maintenance in sedentary cancer survivors by taking into special attention personal factors particularly one's beliefs and inner motivation. Having to embark and engage into an exercise program of their preferred choice will help one to maintain an exercise program.

    Wearing kneepads is the best defence against occupational knee injury. It has been reported that the use of knee pads can reduce injuries and increase performance. Knee pads provide protection by disbursing pressure on the knees and preventing puncture wounds. This study focuses on the effect of knee pads on muscle activity and gait analysis. As a case study, measurement of muscle activities whilst kicking a ball was conducted. The normalized mean of the EMG data shows that the vastus lateralis and vastus medialis muscles are highly active when wearing a knee pad. Gait analysis was conducted on six subjects with and without wearing knee pad. Results shows that wearing knee pads increases the force (950 '— 1150N) acting on the ground, compared with not wearing knee pads (800 —900 N).

    Participation in contact sports has been shown to carry a considerable risk of sustaining dental injuries. It is important for athletes to use available preventive gear to protect themselves from these injuries. Aim of the study: To assess the knowledge, habit and preferences of using mouthguards among university athletes. Materials and Method: Self-administered questionnaires, consisting of Part A - Age, gender, type of sporting activities, total hours dedicated to sports training, level of sports representation; Part B - Awareness of risk of dental injuries, knowledge on mouthguards such as definition of a mouthguard, role of mouthguard, use of mouthguards and reasons for not wearing mouthguards; and Part C — Experience of sustaining dental injuries during sports activities, were distributed. Athletes who were university students over 18 years and under 30 years of age and athletes who claimed to know about mouthguards were included in the study. Results: Data of a total of 225 respondents were analysed, consisting of 68% (n=154) male and 32% female (n=71) with the mean age of 21 years old. Only 46% (n=104) knew exactly about mouthguards and its role in the prevention of dental injuries during sports. Thirty seven percent of them (n=84) have used mouthguards and they were mostly in the martial arts group. The mouthguards were usually provided by the management team (62%). Forty percent of the respondents (n=91) claimed to have sustained dental injuries while playing sports and the injuries occurred more frequently in hockey (65.3%), basketball (60%) and soccer (45.2%). Conclusions: The incidence of dental trauma in contact sports shows that the awareness and use of mouthguards must be intensified. Awareness campaigns focusing on dental trauma should be organized to improve the knowledge of athletes.

    This study examined the significance of movement pattern recognition ability in the judging performance of 30 rhythmic gymnastics judges who were classified correspondingly as expert, non-expert and novice. Results indicated that the expert and non-expert participants were superior to the novice participants in their movement pattern recognition ability and their judging performance but there were no differences between the expert and non-expert participants. When the participants were regrouped according to their movement pattern recognition ability; low, average and high, the results showed that there were very significant differences in the judging performances of the three groups classified according to their movement pattern recognition ability. This suggests that movement pattern recognition ability influences the judging performance of a rhythmic gymnastics judge.

    Sports engineering can be considered as a new engineering discipline. It bridges the gap between two distinctive fields: sport science and engineering. Sports engineers are responsible in designing and building new equipment based on athlete's demands, besides measuring the performance of the athlete, the equipment itself, as well as their interaction. It is without doubt that engineering and technology have played an important role not only in improving the performance of an athlete, but also in making sports more entertaining and safe. This article provides an insight into how engineering and technology have affected sports in many ways transforming it from just a past time to more exciting and competitive world events. Apart from the impact of engineering in sports, the history of the application of engineering and technology in sports is also elaborated. Furthermore, research conducted in related fields worldwide is highlighted. A brief overview of sports engineering research in Malaysia is also presented.

    The objective of this study is to identify the type of occupational related musculoskeletal disorder among Malaysian Shipping Industry workers and to determine the relationship between workers sosio demographic factors with occupational related musculoskeletal disorder and injuries. This is a cross sectional, retrospective study using secondary data that is available at the physiotherapy centre of Malaysia Shipping in Selangor. The study population is the shipping port workers received physiotherapy treatment from 2011 and 2012. A total of 90 samples comprise of 85 male workers and the remaining is female. The mean age is 34.1 (±7.36). Crane operator is the largest number of workers seeks for physiotherapy treatment (68), office (15) and 7 from maintenance. The mean of employment duration is 8.02 (±4.47) years with the maintenance group of workers have longest working duration of 9 years. Muscle and ligament sprain strain known to be the commonest condition (80%) refer for physiotherapy rehabilitation, tendinitis (14%) and fracture (6%). Young age group of workers were significantly 9 times higher (95% CI 1.83 – 40.35) of getting back injuries. The prevalence of musculoskeletal disorder based on work categories vary with office type workers has 4.5 times higher (95% CI 1.06 – 19.7) on hand injuries. This study has revealed that workers age, different type of work categories, working experience, and body mass composition were associates with the occupational related injuries. The training programme emphasise on preventive measures should be tailored to empower the employee on safety measures at work.

    Malaysia’s palm oil industry is growing in complexity and successively to succeed on the global level by accounts for about 36% of the word production of palm oil [1]. But, Occupational Health and Safety (OHS) issues are still problematic areas that need to be addressed by all parties concerned in this industry. In the olden days, unlike construction or manufacturing industry, palm oil industry was green in OHS management system. However, due to stringent in the legislative enforcement in the past few years, it has lead some of the plantation companies to develop OHS management system, which are based on Occupational Health and Safety Assessment Series (OHSAS), towards corporate sustainability. Sustainability is not about paying lip-service to the latest corporate buzzword; neither is it about superficially meeting minimum requirements for the sake of compliance. Rather, sustainability is a core value that lies at the heart of the companies’ business conduct. In practical terms, this means strive to operate with due consideration for the interest of all stakeholders by making the health and safety of all workers a priority. This paper describes the certification of OHSAS 18001 and MS 1722 in Genting Plantations Berhad (GENP) prove the commitment to sustainability by forming guiding principle on safety management. Further, this paper also demonstrates that the implementation of safety management can help to reduce the accident rate, especially fatal accident.
