    In Malaysia, the goods and services tax (GST) was implemented in 2015. Within this context, medicines are subjected to GST. However, essential medicines are exempted. In fact, exempting essential medicines from tax is in line with the Malaysian National Medicines Policy which aims to promote equitable access to affordable essential medicines of good quality to improve health outcomes of the people. The implementation of GST on medicines can generate a stable source of revenue for the nation. However, there are some ways/actions that could be considered to ensure the affordability and accessibility of all medicines post-implementation of GST in Malaysia.

    BACKGROUND: Because of the large size of Sarawak and the high proportion of people residing in rural areas in this Malaysian state, disseminating drug-related information there can be challenging. It is, therefore, important to recognize the type of mass media for drug-related information that are salient to the people of Sarawak. This study was aimed at identifying the use of and the preferences for mass media to obtain drug-related information among the public in Sarawak. We also aimed to recognize the media perceived as the most reliable for drug-related information.

    METHODS: This was a cross-sectional study using a self-administered questionnaire carried out from September to October 2013. Survey respondents were recruited from 4 divisions in Sarawak: Kuching, Sibu, Miri, and Bintulu.

    RESULTS: A total of 433 completed questionnaires were obtained at the end of the study period. All respondents had access to common mass media such as television (89.8%, 389/433), radio (68.6%, 297/433), and the Internet (66.1%, 286/433). Among all respondents, television (71.4%, 309/433) was noted as the most preferred media for drug-related information. Compared with rural respondents, urban respondents were significantly more likely to have access to and prefer the Internet to obtain drug-related information. On the other hand, rural respondents were more likely to have access to and prefer radio for such information compared to their urban counterparts.

    CONCLUSIONS: Television can be an important and attractive choice of mass media in a quality use of medicines (QUM) campaign. The Internet can be used to disseminate drug-related information in urban areas, whereas radio can be used in a QUM campaign targeting the rural public.

    This paper presents the development of a Graphical User Interface (GUI)
    for calculating the sum of nail-fold (NF) and distal interphalangeal joint
    (DIP) ratios for all ten fingers. The sum of NF:DIP ratios for all ten fingers
    leads to the Digital Index (DI) that was used as the measure for identifying
    and determining the presence of finger clubbing symptom. This GUI system
    was developed to serve as a simple and user-friendly interface for clinicians
    to calculate DI value of patients in a busy clinic practice. It is also equipped
    with the capability to keep the patient’s past diagnosis medical check-up
    data for future monitoring purposes. The result shows that the developed
    system helps the clinicians to perform calculation of DI value and identify
    the presence of finger clubbing in a very short time. The average time taken
    to measure both NF and DIP circumferences using Finger Clubbing Meter,
    and to compute DI values using Digital Index Evaluation System (DIES)
    interface is 6:36 ± 1:24 minutes (Mean ± SD) .This system is expected to
    contribute in detecting the finger clubbing problem at early stage of so the
    treatment can be performed immediately.

    Palm Oil Mill Effluent (POME) is the largest contributor of biomass from
    the palm oil milling industry. Conventional method of POME treatment
    using ponding system should be improved because of huge land resource
    requirement. In this study, microbubbles technology was applied to
    understand the recovery rate of residual oil from POME at different
    operating temperatures. Temperature for POME was set at 27 oC, 30 oC and
    50 oC to determine the microbubble size distributions and characteristics at
    different POME temperature. At each temperature, the size of microbubbles
    was measured based on six size range; 50 μm. The results showed that at different

    'Doa' is derived from Arabic word which means that one asks for the
    fulfillment or a need or the cure of sickness from him/her. Having to search
    and retrieve the relevant ‘doa’ for one needs at any particular time is
    beneficial. There are some search and retrieval applications that require
    using the exact match of the keyword search with the words stored in the
    database. This approach leads to the retrieval of insignificant results as
    users need to know the exact word to be searched. Therefore, this project
    allows for partial keyword search that utilises N-gram method for the
    search and retrieval process. Moreover, various words may have similar
    meaning thus to increase the accuracy of the retrieved result, this project
    compares the dice and overlap coefficient algorithms to find the synonyms
    of the searched word. The result produced indicates that overlap coefficient
    perform better than dice coefficient.

    Text localisation determines the location of the text in an image. This process
    is performed prior to text recognition. Localising text on shop signage is
    a challenging task since the images of the shop signage consist of complex
    background, and the text occurs in various font types, sizes, and colours.
    Two popular texture features that have been applied to localise text in
    scene images are a histogram of oriented gradient (HOG) and speeded up
    robust features (SURF). A comparative study is conducted in this paper
    to determine which is better with support vector machine (SVM) classifier.
    The performance of SVM is influenced by its kernel function and another
    comparative study is conducted to identify the best kernel function. The
    experiments have been conducted using primary data collected by the
    authors. Results indicate that HOG with quadratic kernel function localises
    text for shop signage better than SURF.

    Storytelling is considered as an interactive social arts that uses word and
    gestures to reveal the elements and images of a story while engaging the
    listener's imagination. Multimedia based digital storytelling learning
    approach provides interesting, interactive, engaging and multisensory
    learning experience to children. Children explore new experience and
    scenarios as new stories are being told. This study concentrates on
    determining the best combination of elements for designing effective digital
    storytelling applications specifically for the usage of dyslexic children.
    Dyslexic children are known to have a common learning difficulty that can
    cause problems with reading, writing, spelling and comprehension. These
    applications are design with the objective to help in improving dyslexic
    children ability in readings and comprehensions. Four elements were
    derived from extensive literature studies. The elements are multimedia
    components, multi-sensory instructional approach, emotional design and
    games design. The relationship among all the elements were determine
    and described in details as it will be used to contribute to the design and
    development of the application in further works. The strength of this study
    is it models the combinations of technology, psychology and instructional
    approach as a support components for developing an effective digital story
    telling learning application for dyslexic children.

    In recent years, the utility grid system is more essential for the power
    transmission and distribution system because it cannot produce harmful
    gases or no discharge waste in the environment. PWM based phase
    synchronous invert systems are generally utilised in the high efficiency
    energy supply, long distance and higher power quality. The inverter output
    voltage depends on the coupling transformer, input sources and invert
    controllers. An inverter using a three leg IGBT has been designed for utility
    grid and simulated by using MATLAB2014a. In this paper, both sides of
    the LCL filters are used for removing the DC ripple current, reducing the
    noise and synchronous the output phase between inverter and the utility
    grid. The PWM controller has created pulse signal to control the inverter,
    electronic switches and precisely synchronise with grid line frequency. In
    this system, the input DC voltage 500V, switching frequency 1.65 kHz, grid
    frequency 50Hz, 20 km feeder (resistance, inductance and capacitance per
    unit length, which are 0.1153, 1.05e-3 and 11.33e-09 ohms/km) with 30MW
    three phase load (active and inductive reactive power which are 30e6 W
    and 2e6 var) and also a balanced utility grid load of star configuration (00,
    1200, and 2400 degree) are considered in the design. On the other hand,
    three phase transformer consists of three signal phase transformers, normal
    power 100e3, magnetization resistance and inductance which are 500 pu and
    416.67pu are considered in this design. The system conversion efficiency
    is 99.94% and 99.96%, while the total THD are 0.06% on inverter side
    and 0.04% on grid side.

    A study on abundance and diversity of Hymenoptera was conducted in Gunung Datuk, Rembau. Samplings were conducted from November 2014 to February 2015 using six Malaise traps. Three traps were placed at Site 1 at 700m height for high elevation and the remaining traps were placed at Site 2 at 200m height for low elevation. A total number of 221 Hymenopteran were collected which consist of nine families namely Ichneumonidae, Formicidae, Braconidae, Bethylidae, Evaniidae, Tiphiidae, Vespidae, Pompilidae and Apidae. In this study, 93 individuals were obtained from Site 1, comprising nine families and 43 morphospecies while 127 individuals were obtained from Site 2 with nine families and 45 morphospecies. Formicidae was the most dominant family collected from both sites with a total of 104 individuals while the least family recorded was Apidae with only one individual. Shannon’s Weiner Diversity Index (H’) showed Site 1 had the higher diversity value with H’ = 3.17 compared to Site 2 with value H’ = 3.12. For Evenness Index, Site 1 had higher value compared to Site 2 with E’ = 0.84 and E’ =0.82 respectively. Moreover, for Margalef Richness Index, Site 1 recorded R’ = 9.24 while site two recorded R’ = 9.08 which concluded that Site 1 had higher species richness compared to Site 2. Paired t-test showed that both sites had no significant difference with p>0.05. Overall study showed that the diversity and abundance of Hymenoptera in Gunung Datuk were low since the value of H’ is less than 3.50.

    BACKGROUND: Compliance of community pharmacists (CPs) and private general medical practitioners (GPs) with Malaysian Laws on Poisons and Sale of Drugs is crucial in encouraging rational supply of medicine to patients that will subsequently lead to rational use of medicine, especially controlled medicine and psychotropic substances. This study aims to identify the trend of yearly compliance rate of both CPs and GPs with the Malaysian Laws on Poisons and Sale of Drugs, and to quantify the effectiveness of disciplinary actions in improving their compliance level.

    METHODS: This is a retrospective observation study from the Sarawak state Pharmaceutical Enforcement Division (PED) inspection reports on CPs and GPs from January 1, 2012, to December 31, 2014. Descriptive statistics in numbers and percentages are used to present the results.

    RESULTS: From years 2012 to 2014, the compliance rate of GPs increased from 34% to 51%, while the compliance rate of CPs remained almost constant, with a slight drop from 53% (2012) to 50% (2014). The most common noncompliance found among CPs is with the Poison Acts 1952 Section 26 Condition 2: "Records for the supply of preparations containing Pseudoephedrine, Ephedrine and Dextromethorphan," and among GPs, it is the Regulation 12 of Poisons Regulation 1952: "labeling of dispensed medicines." Warning letter is the most effective disciplinary action for both CPs (75% improvement) and GPs (67.8% improvement).

    CONCLUSION: This study serves as a baseline that provides valuable insights to policy makers, researchers, and other stakeholders in developing better enforcement strategies.

    The Internet and rapid development of technology has led to the application of pharmacoinformatic technologies in improving the efficiency of the medication use process. By integrating pharmacoinformatics in pharmacy education, a qualified workforce of pharmacists well trained in the perspective of both pharmacy practice and informatics for safe and positive health-related outcomes can be produced. This survey assessed the level of importance of the suggested topics for pharmacoinformatics courses for pharmacy bachelor's and master's degrees by giving a questionnaire to pharmacy lecturers in both public and private universities, and to pharmacists registered with the Ministry of Health, Malaysia. Briefly the study of pharmacoinformatics was classified into 5 major areas: Drug Information Services, Information Technology & Internet, Drug Formulary Management, Supply Chain Management, and Health Policy. The relative importance of all relevant topics were evaluated and reported. It was found that compared with the undergraduate level, master's degree had higher expectations, and thus a more in-depth pharmacoinformatics curriculum content. In addition, the experiential method of learning instead of formal lectures alone was used on pharmacy master's courses. The findings from this survey could serve as a guide to improve pharmacoinformatics curricula in order to produce pharmacists who can safely and effectively utilize pharmacy informatics to disseminate information in drug use.

    BACKGROUND: There is a need for a collation and comparison of the content of the mobile medical applications (apps) to allow health care professionals to know precisely which app they can rely on to gain access to appropriate drugs references. This study aims to evaluate the features of mobile medical apps based on 3 major functions: Dosage recommendation, drug adverse Reaction, And Drug Interaction (DoReADI).

    METHOD: A review and comparison of mobile apps available in Google's Play Store (Android system) and Apple's App Store (iOS system) were performed. The comparison was based on the availability of options, especially DoReADI functionalities. The assessment criteria were as follows: requirement for an Internet connection, subscription fee charged, size of app, dose recommendation, drug indication, dose calculator, drug picture, dose adjustment, pregnancy safety, interaction checker, interaction classification, clinical teaching advice, contraindicated drug, black box warning, adverse effect, contraindication or precaution, as well as toxicology and pharmacology information.

    RESULTS: Eight mobile medical apps were included and used to compare their features and functionalities. The 4 apps that scored the highest (14/17 points) are: Lexicomp®, Epocrates®, Micromedex®, and Drugs.com ®. Lexicomp and Micromedex do not provide the image of the drug and have an access subscription fee. Epocrates does not provide interaction classification and clinical teaching advice, and occupies a large space in the memory to be installed. Meanwhile, My Blue Book® scored the lowest (9/17 points) because certain features such as toxicology information, drug interaction, clinical teaching advice, contraindicated drug, and black box warning were not included.

    CONCLUSION: Based on the features assessment criteria of each mobile medical application, Lexicomp, Epocrates, Micromedex, and Drugs.com are the apps that scored the highest. Epocrates and Micromedex are useful for checking drug interactions. In addition, some of the apps have additional features for the DoReADI criteria, for example, dose calculator and interaction classification.

    This article aims to discuss the main consequences of the implementation of the Trans-Pacific Partnership Agreement (TTPA) in the pharmaceutical sector in regard to public health, focusing on the accessibility and affordability of medicines. This paper also looks at the likely impact of the TPP agreement on access to affordable medicines. The potential effects of provisions in the final text are explored based on the context of developed and developing countries. A meta-synthesis study design was used. The thematic analysis technique was used to generate themes and a decision tree of the TTPA meta-synthesis. PubMed, EBSCOhost, Ovid, and Scopus databases from inception until the first week of January 2016 were used. Only peer-reviewed journals that discussed TPPA's impact on the pharmaceutical sector were included. Data were extracted by 2 reviewers and then verified by 3 senior researchers. The extracted data were imported into Excel spreadsheets and coded line by line. Codes were organized into descriptive themes. The identified themes were cross-checked against original articles to ensure consistency. A total of 85 full articles and reports were reviewed and, finally, 32 of them were used in the meta-synthesis. Two central themes to the TTPA emerged: intellectual property rights and transparency. Five subthemes were identified under intellectual property rights: patent subject matter (representing scope of patentability), patent term adjustment for patent office delays (representing patent term extension), protection of undisclosed test or other data (representing data exclusivity), protection of undisclosed test or other data (representing patent linkage), and compulsory licensing. Meanwhile, transparency and anti-corruption-procedural fairness, which presents restriction of coverage program and reimbursement, were identified as the subthemes of transparency. Findings indicate that the TPPA could potentially hinder the affordability and accessibility of medicine, which could increase risks to public health.

    Medical students in Malaysia and globally were considered among the most exposure groups in the community to stress due to several sources. The most significant and severe sources of stress in medical students is an academic sources, includes a lot of exams/tests, lack of time and facilities for entertainment, stay in a hostel, high parental expectations and vastness of syllabus, in addition to emotional problems and others personal sources. Stress is a very important issue that leads to a worsening of health problems. Stress develops in the body and leads to oxidative stress which in turn leads to a disorder in the whole body. Oxidative stress may lead to abnormal haematological indices elevated white blood cells (WBCs) count. Oxidative stress can lead to massive destruction of red blood cells (RBCs). The brain and the gastrointestinal system (GI) are intimately connected as one system. The brain has a direct impact on the GI tract. A stressful brain can send signals to the gut, just as a troubled intestine can send signals to the brain. Therefore, stress can be the cause of block the breakdown and assimilation of food for energy and nutriment. This malabsorption can then lead to a reciprocal negative effect to the stress and can be another cause of anaemia through malabsorption minerals and vitamins that are to erythropoiesis. So, stress can be one of the leading causes of anaemia among medical students. Stress is a chronic epidemic in the most medical students and can directly affect how well body works. This review article discovers the effect of stress in medical students that can be effect on their studies and further create researcher's interest to generate database that help to reduce stress response and bring about the empowerment of balanced life among Malaysian medical students besides the increasing level of health and academic performance.

    Objective/Background: Occupational therapy that focuses on servicing clients demands motivated workers for quality service delivery. The objectives of this study were: (a) to determine the level of work motivation among occupational therapy graduates in Malaysia and (b) to determine if there is a difference in work motivation among these graduates based on work sector, job position, length of work experience, and gender.

    Methods: This cross-sectional study recruited occupational therapy bachelor's degree graduates using an online survey. The Work Extrinsic and Intrinsic Motivation Scale (WEIMS) was used to measure level of work motivation.

    Results: Responses from 82 (60.3%) graduates (male: 26.8%; female: 73.2%) were analysed. Sixty-two (75.6%) graduates worked locally and 20 (24.4%) worked in foreign countries. The average Work Self-Determination Index (W-SDI) score for WEIMS is +11.38 with 78 (95.1%) of graduates demonstrated a self-determined motivational profile and 4 (4.9%) demonstrated a nonself-determined profile. Graduates in the private sector (13.10 ± 6.47) show significantly higher W-SDI score compared to those in the public sector (9.40 ± 6.06), p = 0.01. W-SDI scores appeared higher among clinician (11.67 ± 6.40), case manager (13.33), and others (14.90 ± 8.23); and those with work experience of 5-6 years (13.11 ± 6.90) and less than one year (12.65 ± 7.12). Male (10.29 ± 6.86) and female (11.79 ± 6.39) graduates shared equally high score. There is no significant difference in W-SDI score based on job position, length of work experience, and gender.

    Conclusion: Occupational therapy graduates have high work motivation as evident by their self-determined profile. Only work sector imposes difference in work motivation among these graduates.     Holttumochloa has previously only been recorded from Malaysia. Here we describe and illustrate a new species, Holttumochloa hainanensis sp. nov., from the lowland montane forests of Diaoluo Mountain on the Island of Hainan, South China. Morphologically, H. hainanensis is similar to Holttumochloa korbuensis, but can be clearly distinguished from it in having larger culms covered by white wax, longer leaf blades, larger pseudospikelets and anthers. Furthermore, molecular phylogeny based on the nuclear gene GBSSI corroborates the identification of the new species and its affinity. The biogeographical significance of the new record of Holttumochloa in South China is also highlighted in this study.

    The Novel target compounds (CP-1-7) were synthesized and tested at doses up to 1000 mg/kg for their entitled activities. They exerted promising results without any behavioral changes and mortality in mice. Therefore, according to the results obtained in our study, it could be categorized as highly safe agents for treating UC since substances possessing LD50 higher than 50 mg/kg are considered nontoxic. They also possessed a potent anti-ulcerogenic activity with different potentials. The most effective compound was CP-4, it produced 97.7% ulcer protection of control followed by CP-3, which produced 90.3% protection, while the standard drug ranitidine (100 mg/kg) produced 49.2% protection. Compound CP-1 showed lowest activity among the current series, it produced 55.5% protection. The target compounds were significantly more effective than the standard in reducing ulcer index. The anti-ulcerative colitis activity was tested using acetic acid induced colitis model. The curative effect of the tested compounds at a dose of 50 mg/kg oral administration on rats showed a potent anti-ulcerative colitis activity with different potentials. They induced a significant decrease in ulcer score, ulcer area, ulcer index and weight/length of the colon specimens. The percent protection of control colitis ranged from 66.8% for CP-7 to 22.3% for CP-5; however the percent protection for dexamesathone (0.1 mg/kg) was 59.3%. The effect of the tested compounds CP-7 and CP-3 at dose 50 mg/kg were significantly more effective than dexamesathone (0.1 mg/kg) in reducing all parameters. Liver functions were not affected as there is no effect on the activity of both AST and ALT in animals that received the compounds, so the compounds didn't reveal hepatotoxic manifestation. Although, the results on kidney functions showed that, CP-1 slightly elevated blood urea concentration and CP-3 & CP-4 slightly elevated serum creatinine; no apparent nephrotoxic manifestations were recorded.

    This case illustrates the difficulty in diagnosing TAA from an uncommon symptom of haemoptysis, especially in a patient with recent cardiac eventandthe possibility of having a lung malignancy as he is anex-smoker. It appears that CT Angiography is the modality of choice here.
