    N-methyl-D-aspartate (NMDA) receptors were found to be dysfunctional in hypertensive rats. Methyl palmitate (MP) has been shown to diminish the nicotine-induced increase in blood flow in the brainstem. The aim of this study was to determine how MP modulated NMDA-induced increased regional cerebral blood flow (rCBF) in normotensive (WKY), spontaneously hypertensive (SHR), and renovascular hypertensive (RHR) rats. The increase in rCBF after the topical application of experimental drugs was measured using laser Doppler flowmetry. Topical NMDA application induced an MK-801-sensitive increase in rCBF in anesthetized WKY rats, which was inhibited by MP pretreatments. This inhibition was prevented by pretreatment with chelerythrine (a PKC inhibitor). The NMDA-induced increase in rCBF was also inhibited by the PKC activator in a concentration-dependent manner. Neither MP nor MK-801 affected the increase in rCBF induced by the topical application of acetylcholine or sodium nitroprusside. Topical application of MP to the parietal cortex of SHRs, on the other hand, increased basal rCBF slightly but significantly. MP enhanced the NMDA-induced increase in rCBF in SHRs and RHRs. These results suggested that MP had a dual effect on the modulation of rCBF. MP appears to play a significant physiological role in CBF regulation.

    This study reports the physiological and performance profiles of a world-class tower runner during a 6-week period surrounding a successful Guinness World Record (WR) attempt, and discusses the efficacy of a tower running specific field test. The world-ranked number 2 tower runner completed four exercise tests [laboratory treadmill assessment (3 weeks before the WR attempt), familiarisation to a specific incremental tower running field test (1 week before), tower running field test (1 week after), and tower running time trial (TT) (3 weeks after)] and the WR attempt within 6-week period. Peak oxygen consumption (VO2peak) during the laboratory test, field test, and TT were 73.3, 75.5 and 78.3 mL·kg-1·min-1, respectively. The VO2 corresponding to the second ventilatory threshold was 67.3 mL·kg-1·min-1 (89.1% of VO2peak), identified at stage 4 (tempo; 100 b·min-1), during the field test. The duration of the TT was 10 min 50 s, with an average VO2 of 71.7 mL·kg-1·min-1 (91.6% of VO2peak), HR of 171 b·min-1 (92% of peak HR), vertical speed of 0.47 m·s-1, and cadence was 117 steps·min-1. A world-class tower runner possesses a well-developed aerobic capacity. A specific, field-based test revealed greater VO2peak than a laboratory test, indicating a need for sport-specific testing procedures.

    Australia is one of the largest nations in the globe in terms of land area and is home to numerous animals alongside unique and unusual climates and immense forests and oceans. Despite having a very tiny population, the nation is an extremely valuable ecological territory. Unfortunately, due to several changes in land use, habitat loss and deterioration-particularly in light of the recent severe bush fires exacerbated by climate change-the environmental issues in Australia have got the attention of many academics. Therefore, this paper seeks to assess the association between Australia's energy use, [Formula: see text] emission, trade liberalization, industrialization and economic growth from 1990 to 2018. An autoregressive distributed lag and a vector error correction model (VECM) are employed to take care of possible endogeneity and the long-run association. Our results demonstrated that economic growth and energy use have positive and statistically significant effects on emissions of [Formula: see text], but trade liberalization has a significantly adverse influence on emissions of [Formula: see text] both in the long and short term. Granger test in VECM uncovered single-direction Granger interrelationships among trade liberalization and industrialization, as well as among industrialization and carbon dioxide. When attempting to implement effective energy policies, Australian policymakers should first take into account the prominent role played by energy usage and trade liberalization in promoting economic development and impeding environmental health.

    INTRODUCTION: Professionalism is a crucial component of medical practice. It is a culturally sensitive notion that generally consists of behaviors, values, communication, and relationships. This study is a qualitative study exploring physician professionalism from the patients' perspective.

    METHODS: Focus group discussions with patients attending a family medicine center attached to a tertiary care hospital were carried out using the four gates model of Arabian medical professionalism that is appropriate to Arab culture. Discussions with patients were recorded and transcribed. Data were thematically analyzed using NVivo software.

    RESULTS: Three main themes emerged from the data. (1) In dealing with patients, participants expected respect but understood delays in seeing physicians due to their busy schedules. In communication, participants expected to be informed about their health conditions and to have their questions answered. (2) In dealing with tasks, participants expected proper examination and transparency of diagnosis, but some expected the physician to know everything and did not appreciate them seeking outside opinions. They expected to see the same physician at every visit. (3) In physician characteristics preferences, participants preferred friendly smiling physicians. Some cared about the outer appearance of the physician whereas others did not.

    DISCUSSION/CONCLUSIONS: The findings of the study explained only two themes of the four gates model namely dealing with patients and dealing with tasks. Cultural competence and how to benefit from patients' perceptions to be an ideal physician should be incorporated into the process of physicians' training.

    Transarterial chemoembolization (TACE) and transarterial radioembolization (TARE) are promising treatments for unresectable liver tumours. Some recent studies suggested that combining TACE and TARE in one treatment course might improve treatment efficacy through synergistic cytotoxicity effects. Nonetheless, current formulations do not facilitate a combination of chemo- and radio-embolic agents in one delivery system. Therefore, this study aimed to synthesise a hybrid biodegradable microsphere loaded with both radioactive agent, samarium-153 (153 Sm) and chemotherapeutic drug, doxorubicin (Dox) for potential radio-chemoembolization of advanced liver tumours. 152 Sm and Dox-loaded polyhydroxybutyrate-co-3-hydroxyvalerate (PHBV) microspheres were prepared using water-in-oil-in-water solvent evaporation method. The microspheres were then sent for neutron activation in a neutron flux of 2 × 1012  n/cm2 /s. The physicochemical properties, radioactivity, radionuclide purity, 153 Sm retention efficiency, and Dox release profile of the Dox-153 Sm-PHBV microspheres were analysed. In addition, in vitro cytotoxicity of the formulation was tested using MTT assay on HepG2 cell line at 24 and 72 h. The mean diameter of the Dox-153 Sm-PHBV microspheres was 30.08 ± 2.79 μm. The specific radioactivity was 8.68 ± 0.17 GBq/g, or 177.69 Bq per microsphere. The 153 Sm retention efficiency was more than 99%, tested in phosphate-buffered saline (PBS) and human blood plasma over 26 days. The cumulative release of Dox from the microspheres after 41 days was 65.21 ± 1.96% and 29.96 ± 0.03% in PBS solution of pH 7.4 and pH 5.5, respectively. The Dox-153 Sm-PHBV microspheres achieved a greater in vitro cytotoxicity effect on HepG2 cells (85.73 ± 3.63%) than 153 Sm-PHBV (70.03 ± 5.61%) and Dox-PHBV (74.06 ± 0.78%) microspheres at 300 μg/mL at 72 h. In conclusion, a novel biodegradable microspheres formulation loaded with chemotherapeutic drug (Dox) and radioactive agent (153 Sm) was successfully developed in this study. The formulation fulfilled all the desired physicochemical properties of a chemo-radioembolic agent and achieved better in vitro cytotoxicity on HepG2 cells. Further investigations are needed to evaluate the biosafety, radiation dosimetry, and synergetic anticancer properties of the formulation.

    Smooth-coated otters (Lutrogale perspicillata) were transient in Singapore before one resident family group was observed in 1998, presumably having recolonized from Peninsular Malaysia. A population survey in 2017 revealed a minimum of 11 groups and 79 individuals. Since then, movements of otter groups within urban areas have led to increasing numbers of human-otter encounters, including conflicts. We determined the current abundance, population structure, and distribution of smooth-coated otters in Singapore. We assessed seven sampling zones nationwide through verified sighting records and social media. Mortality records from 2019 to 2021 were sourced from the Otter Working Group and Wildlife Reserves Singapore. In early 2021, there were a minimum of 17 groups and 170 individuals. Groups ranged from 2 to 24 individuals. Smooth-coated otters occupy coastal areas, waterways, reservoirs, and sites within the city center in urban gardens and ponds. Following territorial conflicts at waterways, smooth-coated otter groups moved into the urban matrix. Vehicle collisions are the main cause of mortality and are frequent at dams separating freshwater and coastal habitats. While there is a clear increase in smooth-coated otter numbers since 2017, there remain multiple natural and human-caused threats to otter persistence.

    Sonic pressure wave should be taken into consideration in all ballistic injury. We review a young gentleman with ballistic injury of his lateral chest wall. The bullet trajectory passed through the lateral chest wall. Chest radiograph shows a wedge-shaped consolidation adjacent to the wound with blunted right costophrenic angle. Subsequent CT scan confirms the consolidation adjacent to the bullet trajectory. This case report emphasizes on value of CT in ballistic chest trauma and indirect injury caused by the sonic pressure wave of the bullet.

    BACKGROUND: During the novel Coronavirus 2019 (COVID-19) outbreak, there was an overuse of antibiotics in hospitals. The improper use of antibiotics during COVID-19 has increased antibiotic resistance (AR), which has been reported in multiple studies.

    OBJECTIVES: To assess the healthcare workers' (HCWs) knowledge, attitude and practice (KAP) in relation to AR during the era of COVID-19, and identify the associated factors with good knowledge, positive attitude and good practice.

    METHODS: A cross-sectional design was used to assess the KAP of HCWs in Najran, Kingdom of Saudi Arabia (KSA). A validated questionnaire was used to collect participants' data, which consisted of the following information; socio-demographics, knowledge, attitude and items for practice. Data were presented as percentages and median (IQR). Mann-Whitney and Kruskal Wallis tests were used to compare them. Logistic regression was used to identify the associated factors linked to KAP.

    RESULTS: The study included 406 HCWs. Their median (IQR) knowledge score was 72.73% (27.27%-81.82%), attitude score was 71.43% (28.57%-71.43%) and practice score was 50% (0%-66.67%). About 58.1% of the HCWs stated that antibiotics could be used to treat COVID-19 infection; 19.2% of the participants strongly agreed and 20.7% agreed that during the COVID-19 pandemic, antibiotics were overused at their healthcare institutions. Only 18.5% strongly agreed and 15.5% agreed when asked whether antibiotics used properly for the right indication and duration can still result in AR. The significantly associated factors with good knowledge were nationality, cadre and qualification. A positive attitude was significantly associated with age, nationality and qualification. Good practice was significantly associated with age, cadre, qualification and working place.

    CONCLUSION: Although the HCWs had a positive attitude regarding AR during COVID-19, their knowledge and practice need significant improvement. Implementation of effective educational and training programmes are urgently needed. In addition, further prospective and clinical trial studies are needed to better inform these programmes.

    BACKGROUND: Cerebellar contusion, swelling and herniation is frequently encoutered upon durotomy in patients undergoing retrosigmoid craniotomy for cerebellopontine angle (CPA) tumors, despite using standard methods to obtain adequate cerebellar relaxation.

    OBJECTIVE: The aim of this study is to report an alternative cerebrospinal fluid (CSF)-diversion method using image-guided ipsilateral trigonal ventriculostomy.

    METHODS: Single-center retro- and prospective cohort study of n = 62 patients undergoing above-mentioned technique. Prior durotomy, CSF-diversion was performed to the point where the posterior fossa dura was visibly pulsatile. Outcome assessment consisted of the surgeon's intra- and postoperative clinical observations, and postoperative radiological imaging.

    RESULTS: Fifty-two out of n = 62 (84%) cases were eligible for analysis. The surgeons consistently reported successful ventricular puncture and a pulsatile dura prior durotomy without cerebellar contusion, swelling or herniation through the dural incision in n = 51/52 (98%) cases. Forty-nine out of n = 52 (94%) catheters were placed correctly within the first attempt, with the majority of catheter tips (n = 50, 96%) located intraventricularly (grade 1 or 2). In n = 4/52 (8%) patients, postoperative imaging revealed evidence of a ventriculostomy-related hemorrhage (VRH) associated with an intracerebral hemorrhage [n = 2/52 (4%)] or an isolated intraventricular hemorrhage [n = 2/52 (4%)]. However, these hemorrhagic complications were not associated with neurological symptoms, surgical interventions or postoperative hydrocephalus. None of the evaluated patients demonstrated radiological signs of upward transtentorial herniation.

    CONCLUSION: The method described above efficiently allows CSF-diversion prior durotomy to reduce cerebellar pressure during retrosigmoid approach for CPA tumors. However, there is an inherent risk of subclinical supratentorial hemorrhagic complications.

    The acute locked knee is a common presentation of meniscal tears or other intra-articular injuries. However, a popliteus tendon tear, an uncommon cause of acute locked knee, is often overlooked as a possible diagnosis. Here, we present the case of a 29-year-old male who experienced an acute locked knee following a sports injury. An arthroscopic examination revealed an intrasubstance tear in the popliteus tendon and a complete anterior cruciate ligament tear, while the menisci remained intact. Due to the extension lag caused by the popliteus tendon tear, the anterior cruciate ligament reconstruction was postponed. The patient then underwent a course of physiotherapy before the anterior cruciate ligament reconstruction and eventually achieved full knee extension after six weeks. Further surgical intervention was then performed to address the ligament injury. Our case highlights the importance of considering a popliteus tendon tear as a possible cause of an acute locked knee. Proper diagnosis and management are crucial for achieving optimal outcomes for patients with an acute locked knee and concomitant ligamentous injury.

    BACKGROUND: Covid-19 lockdown had caused lifestyle changes especially in sleep, physical activity, and body weight. Thus, this study aimed to determine the weight changes before and after the lockdown period and further assessed the association between sleep quality, physical activity, and body mass index (BMI).

    MATERIAL AND METHODS: This was a retrospective cross-sectional study involving 107 undergraduate students in Universiti Sains Malaysia. Subjects recalled information during the first lockdown implemented in Malaysia from early March 2020 to July 2020. The questionnaire consisted of socio-demography, anthropometry, and physical activity using International Physical Activity Questionnaire and sleep quality using Pittsburgh Sleep Quality Index. Chi-square analysis was used to determine the association between the variables using Statistical Package for Social Sciences software version 26.0.

    RESULTS: There was a significant increase of 1.8 kg in weight before and after the lockdown period. The majority of respondents had poor sleep quality (80.4%) and low physical activity (60.2%), respectively. Almost 29% of the subjects had sleep latency of more than 30 min while 69.1% of them had sleep duration of <7 h. There was no significant association between sleep quality and BMI as well as physical activity and BMI.

    CONCLUSION: Our study demonstrated that the prevalence of poor sleep quality and low physical activity among university students was high during Covid-19 confinement. Moreover, youths have a significant increase in body weight during the lockdown period. Thus, university students may adopt exciting leisure activities to keep themselves active such as doing meditation or joining online exercise classes.

    PURPOSE OF REVIEW: Primary urethral carcinoma (PUC) is a rare urologic tumor. There is limited evidence on this entity. This review summarizes the existing evidence on lymph node dissection (LND) in patients with PUC.

    RECENT FINDINGS: We performed a systematic search of the PubMed, EMBASE, and Web of Science databases to evaluate the impact of inguinal and pelvic LND on the oncological outcomes of PUC and to identify indications for this procedure.

    RESULTS: Three studies met the inclusion criteria. The cancer detection rate in clinically nonpalpable inguinal lymph node (cN0) was 9% in men and 25% in women. In clinically palpable lymph node (cN+), the malignancy rate was 84% and 50% in men and women, respectively. Overall cancer detection rate in pelvic lymph nodes in patients with cN0 was 29%. Based on tumor stage, the detection rate was 11% in cT1-2 N0 and 37% in cT3-4 N0. Nodal disease was associated with higher recurrence and worse survival. Pelvic LND seems to improve overall survival for patients with LND regardless of the location or stage of lymph nodes. Inguinal LND improved overall survival only in patients with palpable lymph nodes. Inguinal LND had no survival benefit in patients with nonpalpable lymph nodes.

    SUMMARY: The available, albeit scarce, data suggest that inguinal LND derives the highest benefit in women and in patients with palpable inguinal nodes, whereas the benefit of pelvic LND seems to be more pronounced across all stages of invasive PUC. Prospective studies are urgently needed to further address the prognostic benefit of locoregional LND in PUC.

    Dihydrofolate reductase (DHFR) is a key enzyme involved in the folate pathway that has been heavily targeted for the development of therapeutics against cancer and bacterial and protozoa infections amongst others. Despite being an essential enzyme for Mycobacterium tuberculosis (Mtb) viability, DHFR remains an underexploited target for tuberculosis (TB) treatment. Herein, we report the preparation and evaluation of a series of compounds against Mtb DHFR (MtbDHFR). The compounds have been designed using a merging strategy of traditional pyrimidine-based antifolates with a previously discovered unique fragment hit against MtbDHFR. In this series, four compounds displayed a high affinity against MtbDHFR, with sub-micromolar affinities. Additionally, we determined the binding mode of six of the best compounds using protein crystallography, which revealed occupation of an underutilised region of the active site.

    Neurokinin B (NKB), a recently discovered neuropeptide, plays a crucial role in regulating the kiss-GnRH neurons in vertebrate's brain. NKB is also characterized in gonadal tissues; however, its role in gonads is poorly understood. Therefore, in the present study, the effects of NKB on gonadal steroidogenesis and gametogenesis through in vivo and in vitro approaches using NKB antagonist MRK-08 were evaluated. The results suggest that the NKB antagonist decreases the development of advanced ovarian follicles and germ cells in the testis. In addition, MRK-08 further reduces the production of 17β-estradiol in the ovary and testosterone in the testis under both in vivo and in vitro conditions in a dose-dependent manner. Furthermore, the in vitro MRK-08 treatment of gonadal explants attenuated the expression of steroidogenic marker proteins, i.e., StAR, 3β-HSD, and 17β-HSD dose-dependently. Moreover, the MAP kinase proteins, pERK1/2 & ERK1/2 and pAkt & Akt were also downregulated by MRK-08. Thus, the study suggests that NKB downregulates steroidogenesis by modulating the expressions of steroidogenic marker proteins involving ERK1/2 & pERK1/2 and Akt/pAkt signalling pathways. NKB also appears to regulate gametogenesis by regulating gonadal steroidogenesis in the catfish.

    Aim: Survival of patients with Hodgkin's lymphoma is lower in in low- and middle-income countries, but factors leading to these outcomes are poorly understood. The objective of this study was to identify predictive factors associated with overall survival among cancer patients undergoing therapy in seven low- and middle-income countries. Materials & methods: A multicenter cohort was conducted in Egypt, Malaysia, Mexico, Peru, Philippines, Thailand and Ukraine. Results. A total of 460 patients were included. Phone-based support during patient follow-up and number of patients seen by the physician provided a positive impact, while the number of adverse events remains a predictor of death and physician decision to stop treatment. Conclusion: Furthers research on the potential benefit of phone-based programs to support patients with chronic diseases treatments should be explored in less developed countries.

    This exploratory qualitative study investigates older adults' unmet needs in the age-friendly city of Ipoh, Malaysia. Seventeen participants were interviewed, including ten older adults residing in Ipoh City for at least six months, four carers, and three professional key informants. Interviews were conducted using semi-structured questions based on the WHO Age-Friendly Cities Framework. A 5P framework for active ageing based on the ecological ageing model was adapted for data analysis. The 5P framework consists of domains of person (micro), process (meso), place (macro), policymaking (macro), and prime, which allows for the dissection of older adults' unmet needs in planning for multilevel approaches, which were employed for analysis. Person: the personal needs requiring improvement included digital divide disparity, inadequate family support, and restricted sports activities attributed to physical limitations. Process: There were fewer social activities and a lack of low-cost and easily accessible venues for seniors. Economic challenges include expensive private healthcare services, variation in the quality of care in older residential care facilities, and limited savings for retirement. Place issues include unequal distribution of exercise equipment, public open spaces, the need for more conducive parking for seniors, and a place for social activities. Difficulties assessing public transportation, digitalized services, and unaffordable e-hailing services are common among seniors. Housing issues for seniors include a lack of barrier-free housing design and unaffordable housing. Policymaking: Insufficient private sector commitment to improving services to older adults, lack of policy governance on the quality of nursing homes, and insufficient multidisciplinary governance collaboration. Prime: Health promotion for preventing age-related illness is required to preserve health in old age, and full-time family caregivers' psychological well-being is often overlooked.

    INTRODUCTION: Early and effective treatment is fundamental in status epilepticus (SE) management. At the initiative of the Epilepsy Council of Malaysia, this study aimed to determine the treatment gap in SE across different healthcare settings in Malaysia.

    METHODS: A web-based survey was sent to clinicians involved in the management of SE, across all states and at all levels of healthcare services.

    RESULTS: A total of 158 responses were received from 104 health facilities, including 23 tertiary government hospitals (95.8% of all government tertiary hospitals in Malaysia), 4 (80.0%) universities, 14 (6.7%) private, 15 (11.5%) district hospitals and 21 clinics. Intravenous (IV) diazepam was available in 14 (93.3%) district and 33 (80.5%) tertiary hospitals for prehospital management. Non-IV benzodiazepine (rectal diazepam and intramuscular midazolam) was not widely available in prehospital services (75.8% and 51.5%). Intramuscular midazolam was underutilised (60.0% in district and 65.9% in tertiary hospitals). IV sodium valproate and levetiracetam were only available in 66.7% and 53.3% of the district hospitals, respectively. Electroencephalogram (EEG) services were available in only 26.7% of the district hospitals. Non-pharmacological therapies such as ketogenic diet, electroconvulsive therapy, and therapeutic hypothermia were not available in most district and tertiary hospitals for refractory and super-refractory SE.

    CONCLUSIONS: We identified several gaps in the current practice of SE management, including limited availability and underutilization of non-IV midazolam in prehospital services, underutilization of non-IV midazolam and other second-line ASMs, and lack of EEG monitoring in district hospitals and limited treatment options for refractory and super-refractory SE in tertiary hospitals.

    AIMS: In order to be competitive on the market, the production of biopreparations needs to be optimized, modelled, and assessed in the early stages of its development. The aim of this paper was to optimize medium for the production of Trichoderma harzianum K179 biocontrol agent, to analyze its kinetics at enlarged laboratory scale and finally economic analysis of the production of this high-value product through simulation modelling.

    METHODS AND RESULTS: The results showed that the bioprocess of T. harzianum K179 bioagent production in a laboratory bioreactor on the medium with optimal composition (dextrose 10 g l-1, soy flour 6.87 g l-1, K2HPO4 1.51 g l-1, KCl 0.5 g l-1, and MgSO4 × 7H2O 0.5 g l-1), at stirring speed of 1.75 × g and aeration intensity of 1.5 vvm, can be shortened from 96 to 36 h. The results of bioprocess economic analysis showed that with a 25-year project lifetime and an investment payback time of 7.58 years, this project represents an economically viable system.

    CONCLUSIONS: Complete analysis of the bioprocess of T. harzianum K179 biocontrol agent production showed that the biologically produced preparation can be competitive on the market with synthetic preparations.
