    Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV2) caused imminent acute infection of respiratory tract known as Coronavirus disease 2019 (COVID-19). Complications of hepatobiliary system especially liver often found in post-acute COVID-19 patients. However, there are only few studies specifically discussing about liver abscess in patients who had history of contracted COVID-19. We present a case of a 54-years-old gentleman with no previous medical illness and no history of vaccination, who was presented with ruptured liver abscess post COVID-19 infection Category 4 (symptomatic with lung infection and the need of oxygen supplementation). Percutaneous drainage was performed to drain the abscess and collections.

A systematic review. J Strength Cond Res XX(X): 000-000, 2024-This review aimed to assess the effects of exercise training on handball players' performance. A thorough search was conducted in 5 online databases (ProQuest, PubMed, Science Direct, Web of Science, and EBSCOhost (SPORTDiscus), as well as on Google Scholar and other gray literature references starting on April 11, 2022. The methodological quality of the included research was evaluated using the Physiotherapy Evidence Database scale. This systematic review includes 16 studies. Eight studies received "excellent" ratings (≥6 points), 5 received "good" ratings (5 points), and 3 received "moderate" ratings (4 points). The findings showed that the most frequently examined performance factors in exercise training intervention strategies with handball players were balance (n = 5), agility (n = 8), speed (n = 9), and jumping performance (n = 10). Muscular strength (n = 4), shooting and dribbling skill (n = 3), and muscular endurance (n = 4) were the second most frequently investigated performance factors. Regarding speed and agility, exercise training considerably improved the physical fitness of handball players. However, evidence related to muscular strength, shooting ability, and dribbling skills was limited. To achieve the optimum standard, handball players must have strong physical and physiological qualities. Meanwhile, no evidence was found related to the impacts of exercise training on power, endurance, flexibility, body composition, cardiovascular fitness, and cardiorespiratory fitness. Regarding limitations, there is a need for more research with solid evidence to determine the impacts of exercise training interventions on athletes' performance in handball.

    The present study intended to determine the nationality of the fastest 100-mile ultra-marathoners and the country/events where the fastest 100-mile races are held. A machine learning model based on the XG Boost algorithm was built to predict the running speed from the athlete's age (Age group), gender (Gender), country of origin (Athlete country) and where the race occurred (Event country). Model explainability tools were then used to investigate how each independent variable influenced the predicted running speed. A total of 172,110 race records from 65,392 unique runners from 68 different countries participating in races held in 44 different countries were used for analyses. The model rates Event country (0.53) as the most important predictor (based on data entropy reduction), followed by Athlete country (0.21), Age group (0.14), and Gender (0.13). In terms of participation, the United States leads by far, followed by Great Britain, Canada, South Africa, and Japan, in both athlete and event counts. The fastest 100-mile races are held in Romania, Israel, Switzerland, Finland, Russia, the Netherlands, France, Denmark, Czechia, and Taiwan. The fastest athletes come mostly from Eastern European countries (Lithuania, Latvia, Ukraine, Finland, Russia, Hungary, Slovakia) and also Israel. In contrast, the slowest athletes come from Asian countries like China, Thailand, Vietnam, Indonesia, Malaysia, and Brunei. The difference among male and female predictions is relatively small at about 0.25 km/h. The fastest age group is 25-29 years, but the average speeds of groups 20-24 and 30-34 years are close. Participation, however, peaks for the age group 40-44 years. The model predicts the event location (country of event) as the most important predictor for a fast 100-mile race time. The fastest race courses were occurred in Romania, Israel, Switzerland, Finland, Russia, the Netherlands, France, Denmark, Czechia, and Taiwan. Athletes and coaches can use these findings for their race preparation to find the most appropriate racecourse for a fast 100-mile race time.

    BACKGROUND: Helicobacter pylori (H. pylori) is frequently associated with non-cardia type gastric cancer, and it is designated as a group I carcinogen. This study aimed to systematically review and meta-analyze the evidence on the prevalence of CagA status in people with gastric disorders in the Indo-Pacific region, and to examine the association of CagA positive in the risk of gastric disorders. This study focused on the Indo-Pacific region owing to the high disability adjusted life-years related to these disorders, the accessibility of efficient treatments for this common bacterial infection, and the varying standard of care for these disorders, particularly among the elderly population in the region.

    METHODS: Relevant studies were identified in the health-related electronic databases including PubMed, Ovid, Medline, Ovid Embase, Index Medicus, and Google Scholar that were published in English between 1 January 2000, and 18 November 2023. For pooled prevalence, meta-analysis of proportional studies was done, after Freeman-Tukey double arcsine transformation of data. A random-effect model was used to compute the pooled odds ratio (OR) and 95% confidence interval (CI) to investigate the relationship between CagA positivity and gastric disorders.

    RESULTS: Twenty-four studies from eight Indo-Pacific countries (Bhutan, India, Indonesia, Malaysia, Myanmar, Singapore, Thailand, Vietnam) were included. Overall pooled prevalence of CagA positivity in H. pylori-infected gastric disorders was 83% (95%CI = 73-91%). Following stratification, the pooled prevalence of CagA positivity was 78% (95%CI = 67-90%) in H. pylori-infected gastritis, 86% (95%CI = 73-96%) in peptic ulcer disease, and 83% (95%CI = 51-100%) in gastric cancer. Geographic locations encountered variations in CagA prevalence. There was a greater risk of developing gastric cancer in those with CagA positivity compared with gastritis (OR = 2.53,95%CI = 1.15-5.55).

    CONCLUSION: Findings suggest that the distribution of CagA in H. pylori-infected gastric disorders varies among different type of gastric disorders in the study countries, and CagA may play a role in the development of gastric cancer. It is important to provide a high standard of care for the management of gastric diseases, particularly in a region where the prevalence of these disorders is high. Better strategies for effective treatment for high-risk groups are required for health programs to revisit this often-neglected infectious disease.

    The importance of incorporating an agile approach into creating sustainable products has been widely discussed. This approach can enhance innovation integration, improve adaptability to changing development circumstances, and increase the efficiency and quality of the product development process. While many agile methods have originated in the software development context and have been formulated based on successful software projects, they often fail due to incorrect procedures and a lack of acceptance, preventing deep integration into the process. Additionally, decision-making for market evaluation is often hindered by unclear and subjective information. Therefore, this study introduces an extended TOPSIS (Technique for Order Performance by Similarity to Ideal Solution) method for sustainable product development. This method leverages the benefits of cloud model theory to address randomness and uncertainty (intrapersonal uncertainty) and the advantages of rough set theory to flexibly handle market demand uncertainty without requiring extra information. The study proposes an integrated weighting method that considers both subjective and objective weights to determine comprehensive criteria weights. It also presents a new framework, named Sustainable Agility of Product Development (SAPD), which aims to evaluate criteria for assessing sustainable product development. To validate the effectiveness of this proposed method, a case study is conducted on small and medium enterprises in China. The obtained results show that the company needs to conduct product structure research and development to realize new product functions.

    Papua New Guinea (PNG) hosts distinct environments mainly represented by the ecoregions of the Highlands and Lowlands that display increased altitude and a predominance of pathogens, respectively. Since its initial peopling approximately 50,000 years ago, inhabitants of these ecoregions might have differentially adapted to the environmental pressures exerted by each of them. However, the genetic basis of adaptation in populations from these areas remains understudied. Here, we investigated signals of positive selection in 62 highlanders and 43 lowlanders across 14 locations in the main island of PNG using whole-genome genotype data from the Oceanian Genome Variation Project (OGVP) and searched for signals of positive selection through population differentiation and haplotype-based selection scans. Additionally, we performed archaic ancestry estimation to detect selection signals in highlanders within introgressed regions of the genome. Among highland populations we identified candidate genes representing known biomarkers for mountain sickness (SAA4, SAA1, PRDX1, LDHA) as well as candidate genes of the Notch signaling pathway (PSEN1, NUMB, RBPJ, MAML3), a novel proposed pathway for high altitude adaptation in multiple organisms. We also identified candidate genes involved in oxidative stress, inflammation, and angiogenesis, processes inducible by hypoxia, as well as in components of the eye lens and the immune response. In contrast, candidate genes in the lowlands are mainly related to the immune response (HLA-DQB1, HLA-DQA2, TAAR6, TAAR9, TAAR8, RNASE4, RNASE6, ANG). Moreover, we find two candidate regions to be also enriched with archaic introgressed segments, suggesting that archaic admixture has played a role in the local adaptation of PNG populations.

    Electric vehicles (EVs), which are a great substitute for gasoline-powered vehicles, have the potential to achieve the goal of reducing energy consumption and emissions. However, the energy consumption of an EV is highly dependent on road contexts and driving behavior, especially at urban intersections. This paper proposes a novel ecological (eco) driving strategy (EDS) for EVs based on optimal energy consumption at an urban signalized intersection under moderate and dense traffic conditions. Firstly, we develop an energy consumption model for EVs considering several crucial factors such as road grade, curvature, rolling resistance, friction in bearing, aerodynamics resistance, motor ohmic loss, and regenerative braking. For better energy recovery at varying traffic speeds, we employ a sigmoid function to calculate the regenerative braking efficiency rather than a simple constant or linear function considered by many other studies. Secondly, we formulate an eco-driving optimal control problem subject to state constraints that minimize the energy consumption of EVs by finding a closed-form solution for acceleration/deceleration of vehicles over a time and distance horizon using Pontryagin's minimum principle (PMP). Finally, we evaluate the efficacy of the proposed EDS using microscopic traffic simulations considering real traffic flow behavior at an urban signalized intersection and compare its performance to the (human-based) traditional driving strategy (TDS). The results demonstrate significant performance improvement in energy efficiency and waiting time for various traffic demands while ensuring driving safety and riding comfort. Our proposed strategy has a low computing cost and can be used as an advanced driver-assistance system (ADAS) in real-time.

    Glycerine pitch is a highly alkaline residue from the oleochemical industry that contains glycerol and contaminants, such as water, soap, salt and ash. In this study, acidic heterogeneous glycerol-based carbon catalysts were synthesised for biodiesel production via single-step partial carbonisation and sulfonation using pure glycerol and glycerine pitch, producing products labelled as SGC and SGPC, respectively. Carbon materials were obtained by heating glycerol and concentrated sulfuric acid (1:3) at 200℃ for 1 h. The produced SGC and SGPC displayed high densities of sulfonic group (-SO3H), i.e. 1.49 and 1.00 mmol·g-1, respectively, alongside carboxylic (-COOH) and phenolic (-OH) acid. In the catalytic evaluation, excellent oleic acid conversions of 96.0 ± 0.4 % and 92.4 ± 0.5 % were achieved using SGC and SGPC, respectively, under optimised reaction conditions: 1:10 M ratio of oleic acid to methanol, 5 % (w/w) catalyst, 64℃ and 5 h. SGPC was found to be recyclable with 68.5 % conversion after the 6th cycle, which was attributed to the loss of -SO3H and catalyst deactivation by the deposition of oleic acid on its surface. Remarkably, despite the impurities present in the glycerine pitch, the obtained results demonstrated that the reactivity of SGPC is comparable to SGC and superior to that of commercial solid acid catalysts, which demonstrated that the presence of impurities appears to have minimal impact on the production of carbon materials and their properties.

    BACKGROUND: Despite the introduction of bedaquiline (Bdq) containing all-oral regimens for treating patients with rifampicin resistant/multidrug resistant tuberculosis (MDR/RR-TB) in 2019, data on its effectiveness in Pakistan, which has the fifth highest MDR-TB burden, is lacking. This study evaluates treatment outcomes and identifies factors associated with unsuccessful outcomes among MDR/RR-TB patients treated with an all-oral longer treatment regimen (LTR).

    METHODS: This retrospective record review included all microbiologically confirmed pulmonary MDR/RR-TB patients treated with an all-oral LTR between August 2019 and February 2021 across nine PMDT centres in Pakistan. Sociodemographic and clinical data were retrieved from the Electronic Nominal Recording and Reporting System. Treatment outcomes, defined by WHO criteria, were analysed using SPSS and multivariate binary logistic regression to identify factors associated with unsuccessful outcomes. A p-value  5 drugs (OR:3.12, 95 %CI:1.36-11.64, p = 0.013) were significantly associated with death and treatment failure. Whereas, lung cavitation had statistically significant association with LTFU (OR:2.66, 95 %CI:1.10-7.32, p = 0.045).

    CONCLUSION: Treatment success rate (70.3 %) in this study fell below the WHO recommended target success rate (>90 %). Enhanced clinical management, coupled with special attention to patients exhibiting identified risk factors could improve treatment outcomes.

    The aquaculture industry is hindered by various factors. One of the most noticeable factors is infection by parasites and pathogens. Argulus stands out as a prominent and economically significant ectoparasite in freshwater aquaculture. Argulus infestation causes severe immunomodulatory effects on its hosts by promoting argulosis, causing inflammation, extensive tissue damage, and death. Indian aquaculture sector faced a loss of 62.5 million USD due to Argulus infection. However, current control methods, such as pesticides, cause serious environmental damage. Herbal treatment methods are ineffective and have limitations. Hence, a more efficient and cost-effective control method is needed. In recent years, vaccine development has emerged as a promising avenue of research. Understanding the effect of the host-parasite relationship in the host immune system is essential to develop strategies for prevention, control, and management of argulosis. These interactions provide insights into the co-evolutionary dynamics between hosts and parasites. This review provides an overview of the current knowledge on the host-searching behaviour of Argulus, host-parasite interaction and control strategies. This review also highlights the need for further research and the development of sustainable control measures for Argulus infection.

    There is considerable evidence for mitochondrial-nuclear co-adaptation as a key evolutionary driver. Hypotheses regarding the roles of sex-linkage have emphasized Z-linked nuclear genes with mitochondrial function (N-mt genes), whereas it remains contentious whether the perfect co-inheritance of W genes with mitogenomes could hinder or facilitate co-adaptation. Young (neo-) sex chromosomes that possess relatively many N-mt genes compared to older chromosomes provide unprecedented hypothesis-testing opportunities. Eastern Yellow Robin (EYR) lineages in coastal and inland habitats with different climates are diverged in mitogenomes, and in a ~ 15.4 Mb nuclear region enriched with N-mt genes, in contrast with otherwise-similar nuclear genomes. This nuclear region maps to passerine chromosome 1A, previously found to be neo-sex in the inland EYR genome. To compare sex-linked Chr1A-derived genes between lineages, we assembled and annotated the coastal EYR genome. We found that: (i) the coastal lineage shares a similar neo-sex system with the inland lineage, (ii) neo-W and neo-Z N-mt genes are not more diverged between lineages than are comparable non-N-mt genes, and showed little evidence for broad positive selection, (iii) however, W-linked N-mt genes are more diverged between lineages than are their Z-linked gametologs. The latter effect was ~7 times stronger for N-mt than non-N-mt genes, suggesting that W-linked N-mt genes might have diverged between lineages under environmental selection through co-evolution with mitogenomes. Finally, we identify a candidate gene driver for divergent selection, NDUFA12. Our data represent a rare example suggesting a possible role for W-associated mitochondrial-nuclear interactions in climate-associated adaptation and lineage differentiation.

    BACKGROUND: In recent years, there have been many instances of negative sentiments expressed by and resignations observed from doctors working in the Ministry of Health (MOH), Malaysia. However, little is known about the perspectives of medical students and their career intentions. This study aims to determine the current Malaysian medical students' career intentions immediately after graduation and upon completing the 2 years of housemanship and to establish the factors influencing these intentions.

    METHODS: This was a cross-sectional study of 859 Malaysian medical students from 21 medical schools who voluntarily completed a self-administered online questionnaire that was disseminated by representatives from medical schools nationwide and social media platforms of a national medical student society.

    RESULTS: 37.8% of the respondents were optimistic about a career with the Ministry of Health (MOH), Malaysia in the future. Most of the respondents (91.2%) plan to join and complete the MOH Housemanship programme as soon as possible after graduation, with the majority of them (66.2%) planning to complete it in their state of origin. After 2 years of Housemanship programme, only more than half of the respondents (63.1%) plan to continue their careers in MOH. Slightly more than a quarter (27.1%) of the total respondents plan to emigrate to practise medicine, with 80.7% of them planning to return to Malaysia to practise medicine after some years or after completing specialisation training. Combining the career intentions of Malaysian medical students immediately after graduation and upon completion of the 2 years housemanship programme, only a slight majority (57.5%) of the respondents plan to continue their career in MOH eventually. Most of the respondents (85.0%) intend to specialise.

    CONCLUSION: A concerning number of Malaysian medical students plan to leave the Ministry of Health workforce, the main healthcare provider in Malaysia, in the future. Urgent government interventions are needed to address the underlying factors contributing to the potential exodus of future doctors to prevent further straining of the already overburdened healthcare system, posing a significant threat to public well-being. An annual national study to track medical students' career intentions is recommended to gather crucial data for the human resources for health planning in Malaysia.

    Humour processing is complex and may be influenced by an individual's emotional state. This study explores the differences in incongruity-resolution and nonsense joke humour processing between three groups with different emotional states (positive, neutral and negative). Each group consisted of 32 participants. The positive and negative emotion groups were asked to elicit positive and negative emotions, respectively, through autobiographical recall. In contrast, the neutral emotion group was asked to answer questions unrelated to emotions. Participants were also asked to rate their emotional valence, arousal and conformity with the provided emotion words. Subsequently, they were directed to rate their humour appreciation, humour comprehension and level of surprise at the jokes. The results indicate that these variables interacted with the level of amusement derived from the two humour types. The positive emotion group perceived nonsense jokes as funnier than the negative emotion group, while the negative emotion group perceived nonsense jokes as less amusing than incongruity-resolution jokes. No significant difference was found between the three emotion groups in terms of humour comprehension and surprise at the jokes. The results deepen the understanding of the correlation between emotional states and processing of different humour types.

    OBJECTIVES: This study aimed to devise a modified oximetry scoring system and calculate its diagnostic accuracy in detecting paediatric obstructive sleep apnoea syndrome (OSAS).

    STUDY DESIGN: This prospective diagnostic accuracy study was divided into two phases.

    SETTING: The study was conducted at a quaternary teaching hospital.

    METHODS: Polysomnograms performed from 1 April 2014 to 31 December 2021 were included. In Phase 1, the parameters of 95 oximetry trend graphs were evaluated, and a modified scoring system was constructed. In Phase 2, the modified scoring system was employed in 272 oximetry trend graphs, and its diagnostic accuracy was determined. A logistic regression model was used to assess the ability of each scoring system to predict paediatric OSAS.

    RESULTS: A total of 367 patients were recruited. In Phase 1, a four-tier severity classification system was constructed. In Phase 2, its diagnostic accuracy was found to be 53.3% sensitive, 97% specific, with positive predictive value of 98.5% and negative predictive value of 34.6%. The lowest detectable apnoea-hypopnoea index (AHI) was 4.5. The inter-rater reliability calculated was 80%. Logistic regression was applied to assess associations of the modified McGill score (MMS) or McGill oximetry score (MOS) with OSAS. The area under the receiver operating characteristic curve was higher for the MMS than for MOS (0.78 [95% CI 0.73-0.84] vs. 0.59 [95% CI 0.51-0.66]).

    CONCLUSION: This study demonstrated that our modified scoring system had increased sensitivity at detecting OSAS at a much lower AHI and showed a much greater ability to predict paediatric OSAS.

    This study aims to develop an alternative aquafeed derived from insect meal for Lates calcarifer juveniles, with the objectives of exploring the physiological performance, biological parameters, and economic analysis of substituting fishmeal (FM) with defatted black soldier fly (Hermetia illucens) larvae (BSFL) as part of the diet of L. calcarifer juveniles. Five practical diets were formulated to include 0% (BSFL0, serves as control group), 5% (BSFL5), 10% (BSFL10), 15% (BSFL15), and 20% (BSFL20) of BSFL meal, partially or fully replacing FM, respectively. Each diet was randomly assigned to triplicate groups of 30 fish (10.70 ± 0.07 g) per tank (300 L). The fish were fed twice daily to apparent satiation. A 56-day feeding trial was conducted to evaluate the impacts of defatted BSFL meal replacing FM on the growth performance, feed efficiency, composition analysis of fish muscle, cumulative mortality rate challenged with Vibrio parahaemolyticus, and economic returns of L. calcarifer. These results show that differences in weight gain and specific growth rate among the different treatments were statistically significant (p  BSFL0 > BSFL5 > BSFL15 > BSFL20. However, the feed conversion ratio and protein efficiency ratio showed the opposite trend as above. Although the diets experienced a decline in crude protein content and an increase in crude fat content with the increasing proportion of BSFL substituting FM, the crude protein and fat content of fish muscle were only slightly influenced. It is worth mentioning that levels of nonessential amino acids, delicious amino acids, saturated fatty acids, omega-6, omega-9 in BSFL10 group all showed an increase compared with the control group. After a 7-day challenge test with V. parahaemolyticus, the cumulative mortality rates of the BSFL5 and BSFL10 groups, respectively, dropped to 5.20%, 5.28% compared to the control group's 16.88%; however, the mortality rates of BSFL15 (34.67%) and BSFL20 (41.77%) groups were found to be significantly (p 15%) leads to a negative effect. From an economic point of view, 10% inclusion of BSFL in practical diets is recommended for L. calcarifer juveniles.

    Cytochrome P40 (CYP) enzymes dominate the metabolism of numerous endogenous and xenobiotic substances. While it is commonly believed that CYP-catalysed reactions result in the detoxication of foreign substances, these reactions can also yield reactive intermediates that can bind to cellular macromolecules to cause cytotoxicity or irreversibly inactivate CYPs that create them.Mechanism-based inactivation (MBI) produces either irreversible or quasi-irreversible inactivation and is commonly caused by CYP metabolic bioactivation to an electrophilic reactive intermediate. Many drugs that have been known to cause MBI in CYPs have been discovered as perpetrators in drug-drug interactions throughout the last 20-30 years.This review will highlight the key findings from the recent literature about the mechanisms of CYP enzyme inhibition, with a focus on the broad mechanistic elements of MBI for widely used drugs linked to the phenomenon. There will also be a brief discussion of the clinical or pharmacokinetic consequences of CYP inactivation with regard to drug interaction and toxicity risk.Gaining knowledge about the selective inactivation of CYPs by common therapeutic drugs helps with the assessment of factors that affect the systemic clearance of co-administered drugs and improves comprehension of anticipated interactions with other drugs or xenobiotics.

    BACKGROUND: Research on microplastics has largely focused on the environment and marine organisms until recently. A growing body of evidence has detected microplastics in human organs and tissues, with their exact entry routes being unclear and their potential health effects remain unknown. This scoping review aimed to characterise microplastics in human tissues and organs, examine their entry routes and addressing gaps in research analytical techniques.

    METHODS: Eligibility criteria included English language full text articles, in-vivo human studies only, and searching the databases using pre-defined terms. We based our analysis and reporting on the PRISMA guideline and examined the quality of evidence using the risk of bias assessment tool.

    RESULTS: Of 3616 articles screened, 223 evaluated and 26 were eventually included in this review. Nine were high risk for bias, three were unclear risk and the rest low risk for bias. Microplastics were detected in 8/12 human organ systems including cardiovascular, digestive, endocrine, integumentary, lymphatic, respiratory, reproductive and urinary. Microplastics were also observed in other human biological samples such as breastmilk, meconium, semen, stool, sputum and urine. Microplastics can be characterised based on shape, colours, and polymer type. Potential entry routes into human included atmospheric inhalation and ingestion through food and water. The extraction techniques for analysis of microplastics in human tissues vary significantly, each offering distinct advantages and limitations.

    CONCLUSIONS: Microplastics are commonly detected in human tissues and organs, with distinct characteristics and entry routes, and variable analytical techniques exist.
