    OBJECTIVES: Deprescribing has gained awareness recently, but the clinical benefits observed from randomized trials are limited. The aim of this study was to examine the effectiveness of a pharmacist-led 5-step team-care deprescribing intervention in nursing homes to reduce falls (fall risks and fall rates). Secondary aims include reducing mortality, number of hospitalized residents, pill burden, medication cost, and assessing the deprescribing acceptance rate.

    DESIGN: Pragmatic multicenter stepped-wedge cluster randomized controlled trial.

    SETTING AND PARTICIPANTS: Residents across 4 nursing homes in Singapore were included if they were aged 65 years and above, and taking 5 or more medications.

    METHODS: The intervention involved a 5-step deprescribing intervention, which involved a multidisciplinary team-care medication review with pharmacists, physicians, and nurses (in which pharmacists discussed with other team members the feasibility of deprescribing and implementation using the Beers and STOPP criteria) or to an active waitlist control for the first 3 months.

    RESULTS: Two hundred ninety-five residents from 4 nursing homes participated in the study from February 2017 to March 2018. At 6 months, the deprescribing intervention did not reduce falls. Subgroup analysis showed that intervention reduced fall risk scores within the deprescribing-naïve group by 0.18 (P = .04).     Horaeomorphus bicornis sp. n., a remarkable species with strongly modified head from Yunnan, Southwest China is described. A distinctly different female from the same locality is also recorded, its identity remains unconfirmed until associated males become available. The previously unknown female of H. hujiayaoi Zhou & Zhang, 2016, is discovered, with its spermatheca and female terminalia illustrated. Horaeomorphus punctatissimus Franz, 1992 is newly recorded from Mount Trus Madi, Malaysia. Two females of H. eumicroides Schaufuss, 1889 were discovered from Singapore, with female terminalia illustrated.

    Polypoid endometriosis is an uncommon variant of endometriosis which can mimic malignancy due to its presentation as masses. We present a case of polypoid endometriosis which simulated cervical malignancy both on clinical examination and on computed tomography (CT) scanning and discuss how magnetic resonance (MR) imaging, in particular Diffusion Weighted Imaging (DWI), can help to distinguish this condition from true malignancy and avoid invasive surgery.

    Licensed person is a third party who was awarded a license to perform statutory inspection for certificated
    machinery under the provision of section 40 (1A), Factories and Machinery Act (Amendment) (FMA) 2006. The
    objectives of the study is to identify issues related to statutory inspection in Malaysia and to make comparisons with
    respect to the statutory inspection by Licensed Person adopted by other countries. The enforcement of Licensed
    Person in Malaysia is still in not yet implemented although Section 40 (1A) in FMA (Revision) 2006 regulating on
    Licensed Person had been introduced on 2006 and compared with other countries especially our neighboring country
    Singapore. Based on the DOSH inspection data analysis, the main issues relating to statutory inspection in Malaysia
    is the increment in backlog cases due to lack of number officers compare to the number of registered certificated
    machineries in Malaysia. As a conclusion, the implementation of Licensed Person is crucial in Malaysia to reduce the
    work and financial burden of Department of Occupational Safety and Health (DOSH) officer in enforcing FMA 1967
    and also to improve the quality of statutory inspection for the safety in workplace.

    The Sundaic swamp clade of the genus Cyrtodactylus contains nine species that collectively range through Peninsular Malaysia and its associated land bridge islands, Singapore, Sumatra, Java, and Pulau Natuna Besar. Ancestral range reconstruction analyses using BioGeoBEARS based on an updated molecular phylogeny of the nine Sundaic swamp clade species of Cyrtodactylus demonstrated that this lineage evolved in Peninsular Malaysia, dispersed independently to Sumatra and Pulau Natuna Besar, Indonesia and most likely back into Peninsular Malaysia from Sumatra. This scenario is consistent with climate-driven, cyclical, ephemeral, geographic reconfigurations of Sundaic landmasses from at least the mid-Miocene to present.

    Accurate identification of mosquitoes is crucial for the management and control of vector species. Although Singapore is a tropical country with high mosquito diversity, there are no identification keys specific to its mosquito fauna. An illustrated identification key to the genera of the adult female mosquitoes of Singapore is presented. Twenty genera are characterized and two genera found in Malaysia, Culiseta and Topomyia, are also included. The key will aid non-taxonomically trained operational officers in adult mosquito identification. The key to the genera provides the first crucial step in the process to species identification.

    An inventory of orchid species diversity was carried out in cloud forest of Genting Highlands, which is one of the most developed highland areas in Peninsular Malaysia. Since 1967, construction of roads and hotel complexes has had a serious impact on its physical environment and vegetation. Habitat destructions together with climate change might have caused some orchid species to be extirpated locally or extinct. The cloud forest of Genting Highlands consists of four major peaks, Gunung Ulu Kali, Gunung Chin Chin, Gunung Lari Tembakau and Gunung Mengkuang. However, not much study on orchid diversity was conducted in these areas, therefore, this study is timely and essential to evaluate the current orchid diversity after a gap of three decades. Combined findings from the current study, previous published works and specimens deposited in local herbaria, including those at the Universiti Malaya, Universiti Kebangsaan Malaysia, Forest Researh Institute of Malaysia and Biology Department, Faculty of Science, Universiti Putra Malaysia and Singapore Botanic Gardens, an updated list of orchid species found in the cloud forests of Genting Highlands is presented. A total of 134 orchid taxa were recorded, comprised of 51 genera, 132 species, 1 subspecies and 2 varieties, of which 46 are new records to Genting Highlands. Hymenorchis javanica, a species previously reported as endemic to West Jawa was recently discovered in Gunung Ulu Kali. Thirty-three species discovered during this study are currently classified as endemic to Peninsular Malaysia, of which 20 are endemic to Pahang, Perak and Selangor and a very narrowly endemic species; Corybas villosus to Gunung Ulu Kali. Through our observations, most of the orchid species in Gunung Ulu Kali are very susceptible to disappearance due to loss of habitat and local climate change. An assessment of conservation status according to IUCN criterion revealed that 47 orchid taxa from Gunung Ulu Kali were threatened with extinction. Conservation actions were suggested to conserve the orchid diversity in the cloud forest areas of Genting Highlands.
