    Diabetic nephropathy (DN) has become the most common cause of end-stage renal failure. Early referral and specific nephrology treatment could delay the disease progression and should reduce the treatment cost, mortality and morbidity rate in these patients. This is a single-center, retrospective review of all DN patients referred to the nephrology clinic in Hospital Sultan Ahmad Shah, Temerloh, from 2000 to 2009, to study and define the clinical characteristics of DN patients at the time of the referral to the nephrology clinic. A total of 75 patient case records were reviewed. Forty-three (57.3%) of them were males, with a median age of 64.3 ± 8.5 years at the time of referral. Only 14.7% of them had blood pressure lower than 125/75 mmHg. Co-morbid and disease-related complications were also commonly diagnosed and 28.4% (n = 21) had ischemic heart disease, 23% (n = 17) had diabetic retinopathy and 20.3% (n = 15) had diabetic neuropathy. The mean serum creatinine at the time of referral was 339.8 ± 2.3 μmol/L, gylcated hemoglobin A 1c (HbA1C) was 8.1 ± 2.0 %, serum fasting glucose was 9.6 ± 4.7 mmol/L, serum cholesterol was 5.4 ± 1.2 mmol/L and hemoglobin level was 10.6 ± 2.9 g/dL. Although female patients were less frequently seen in the early stages of chronic kidney disease (CKD), they comprised at least 72.7% of CKD stage 5 (male:female; 6:16, P <0.05). Twenty-nine percent (n=22) of them were referred at CKD stage 5, 48% (n=36) were at CKD stage 4, 17.3% (n=13) were at CKD stage 3, 4% (n=3) were at CKD stage 2 and 1.3% (n=1) was at CKD stage 1. Advanced CKD patients were frequently prescribed with more antihypertensives. CKD stage 5 patients were prescribed with two-and-half types of antihypertensive as compared to two types of anti-hypertensive in CKD stage 2 and stage 3. Furthermore, ACE-inhibitors (ACE-I) were less frequently prescribed to them. Only 22.7% (n=5) of CKD stage 5 patients received ACE-I and 30% (n=11) in CKD stage 4 patients as compared to 53.4% (n=7) in CKD patients stage 3. This review shows that DN patients were referred late to the nephrologists and the overall disease management was suboptimal. Antihypertensive requirement was also increased and ACEIs were less frequently prescribed in the advanced diabetic nephropathy patients.

    The main aim of this study is to identify the quality of life (QoL) among communities residing near the Tembeling, Pahang and Muar Rivers in Malaysia. This quantitative study used a constructed questionnaire as main tool to collect data on the QoL of river communities. A total of 240 villagers were selected as respondents. The results indicated that the dimensions of settlement, safety, involvement and social relationships, as well as education scored highest, while dimensions of physical environment, financial and job security yielded moderate scores. Dimensions of infrastructure facilities yielded a low mean score. Recommendations are provided, in the hope that our results may be useful for strategies that could enhance QoL of these river communities.

    Acute kidney injury (AKI) is a common diagnosis among critically ill patients. Although the etiology of AKI will determine the appropriate initial management, the definitive management of established AKI is still debatable. This is a retrospective, observational, single-center analysis of a cohort of patients referred to the nephrology unit for AKI from 1st August 2010 to 31st January 2011. Those patients with indications for dialysis were treated with continuous renal replacement therapy, intermittent hemodialysis or stiff-catheter peritoneal dialysis as determined by their hemodynamic status and the technical availability of the method. The 30-day mortality rate, renal outcomes and independent prognostic factors were analyzed statistically. Seventy-five patients were reviewed. The mean age was 52.9 ± 14.5 years. Two-thirds were males and 75% were Malays. 53.3% were referred from intensive wards. Pre-renal AKI and intra-renal AKI were diagnosed in 21.3% and 73.3% patients, respectively. Sepsis was the most common cause (n = 59). The pathogens were successfully cultured in 42.3% of the cases. The median urea and creatinine were 30.4 [interquartile range (IQR) 20.3] mmol/L and 474 (IQR 398.0) μmol/L, respectively. Seventy-six percent had metabolic acidosis. Oliguria was only noted in 38.7% of the patients. Sixty percent (n = 45) of the referrals were treated with conventional, intermittent hemodialysis and 22.7% (n = 17) with continuous veno-venous hemofiltration. The 30-day mortality rate was 28%, and was higher in intensive wards (37.5% versus 17.1%). Thirty-one percent (n = 20) had complete recovery, 17 (26.6%) had partial recovery without dialysis and seven (10.9%) became dialysis dependent. The 30-day mortality rate was 28%. Referral from intensive wards was the only significant poor prognostic factor in our patients and not the dialysis modalities.

    There have been an increasing number of studies conducted on community preparedness, particularly on changing individual health behaviors in ways that minimizes individual risk to cope with the stress of a natural disaster. A variety of behavioral change theories and models used by disaster academics scrutinize the manner in which individual behavior is sought and transformed into disaster preparedness. This reflects the lack of knowledge about how these models identify certain behaviors regarding natural disaster preparation. This article seeks to address this lack of knowledge. It presents a set of health behavioral change models that can be used by scholars to comprehend variation in the nature and extent of individual disaster preparedness. The purpose of this study is to provide a review of the existing models on the subject, and also to present a comparative analysis of the models that may contribute to ways of understanding the investigation on natural disaster preparedness behaviors.

    The climate change phenomenon has been occurring in every part of the world, including Malaysia. In particular, changes such as rising temperature, sea level rise, and unstable rain pattern are proven to affect the socio-economic routine of the community. Hence, it is necessary to learn how to adapt to it, especially those who heavily rely on nature stability. The present study examined the adaptation towards climate change among islanders in Malaysia. In addition, the current research was performed quantitatively using a developed questionnaire as the main data collection tool. In this case, a total of 400 islanders were selected as the respondents through a multi-stage sampling technique. The results revealed that the respondents recorded a moderate to high mean score for adaptation aspects namely awareness, dependency and structure. Accordingly, a number of recommendations that were highlighted can be utilized as a basis to develop community adaptation policy that is in line with the islanders' need, ability, and interests.

    Problem-based learning (PBL) emphasizes learning behavior that leads to critical thinking, problem-solving, communication, and collaborative skills in preparing students for a professional medical career. However, learning behavior that develops these skills has not been systematically described. This review aimed to unearth the elements of effective learning behavior in a PBL context, using the protocol by Arksey and O'Malley. The protocol identified the research question, selected relevant studies, charted and collected data, and collated, summarized, and reported results. We discovered three categories of elements-intrinsic empowerment, entrustment, and functional skills-proven effective in the achievement of learning outcomes in PBL.

    BACKGROUND: Healthcare work is one of the most accident-prone occupations globally. Nurses, especially those who work in Intensive Care Units (ICU), are very likely to experience mishaps on the job due to the complicated duties they perform. Safety performance through compliance and participation in safety is a proactive approach and a critical tool to measure the protection of employees, like these, in the workplace. Although interest in this tool has increased among hospital administrators and managers, scientific research has been limited in this area.

    AIMS: The study's purposes were twofold: (1) to explore the effect of perceived respect safety on the safety performance of ICU nurses and (2) to explore the mediation effect of Management Commitment to Safety (MCS) between the relationship of perceived respect safety and safety performance.

    METHODS: Eight public hospitals from the Jordanian Ministry of Health (JMoH) were selected randomly using cluster sampling, and their ICU nurses were surveyed. A total of 285 nurses completed questionnaires. The SmartPLS3 bootstrapping technique was used to analyse data.

    Proposals have been made by several researchers to conduct the sequestration of carbon dioxide (CO2) through calcium and magnesium-rich materials. From these materials, ground granulated blast furnace slag (GGBS) containing 5% magnesium and 45% calcium is seen to be a good candidate and is available to sequester CO2. This study intends to ascertain the ability to absorb CO2, sequester it, and increase treated kaolin strength with different content of GGBS under various carbonation periods with varying CO2 pressure. The impacts of carbonated GGBS on the mechanical attributes of soil were examined by conducting the unconfined compressive strength (UCS) test, and microstructure analysis was conducted to identify the changes in the structure and Crestline phase. Stationary carbonation in a triaxial test with pure CO2 was conducted to accelerate the carbonation process. The outcome indicates that the strength rises as the carbonation period rises. Likewise, UCS rises as the CO2 pressure rises from 100 to 200 kPa. It could be concluded that augmentation of the strength is because of carbonated calcium and magnesium products which stuff the soil voids. Changes occur on the microstructure level due to carbonation as well.

    This study aims to gain more understanding on highland farmers' adaptation towards the impacts of climate change in Malaysia. Via a multi-stage cluster sampling, this quantitative study has surveyed a total of 400 highland farmers as respondents. The results indicated that the highest climate change-resilient farmers were from Kundasang, specifically among the females, Dusun ethnic group, and those who work side jobs to cover household expenses. Furthermore, recorded factors such as age and years of experience yielded significant negative relationship with adaptation whereas income yielded significant positive relationship with adaptation. The paper concludes with recommendations related to occupational diversification, consistent information disseminations, access to financial assistance, and the need to empower extension officers and local leaders in the hope that a comprehensive approach can help implement any community climate change-adaptation plan.

    Environmental global issues affecting global warming, such as carbon dioxide (CO2), have attracted the attention of researchers around the world. This paper reviews and discusses the ground improvement and its contribution to reducing CO2 in the atmosphere. The approach is divided into three parts: the Streamlined Energy and Emissions Assessment Model (SEEAM), the replacement of soil stabilisation materials that lead to the emission of a large amount of CO2 with alternatives and mineral carbonation. A brief discussion about the first two is reviewed in this paper and a detailed discussion about mineral carbonation and its role in enhancing soil strength while absorbing a large amount of CO2. It is emphasised that natural mineral carbonation requires a very long time for a material to reach its full capacity to form CO2; as a result, different acceleration processes can be done from increasing pressure, temperature, the concentration of CO2 and the addition of various additives. In conclusion, it was found that magnesium is more attractive than calcium, and calcium is complicated in terms of strength behaviour. Magnesium has a larger capacity for CO2 sequestration and it has a greater potential to increase soil strength than calcium.

    Monostotic fibrous dysplasia of the vertebra is a rare entity. A case of a 53-year-old lady who presented with an 8 months history of pain in the thoracic spine region with paraparesis is discussed. She had a history of papillary thyroid carcinoma and had undergone total thyroidectomy one year prior to her current problem. Magnetic resonance imaging revealed isolated osteolytic lesion over the posterior element of the T12 vertebra with narrowing of the spinal canal causing compression of the cord. The diagnosis of fibrous dysplasia was made histologically. Fibrous dysplasia rarely occurs in axial bones compared with peripheral bones. This case illustrates that osteolytic lesion of the vertebrae should be evaluated with detailed radiological and histopathological examination before an empirical diagnosis of spinal metastasis is made in an adult with a background history of primary malignancy well-known to spread to the bone.

    A total of 225 samples from poultry farms and the surrounding environment were screened for vancomycin-resistant enterococci (VRE) and bifunctional aminoglycoside-resistant enterococci using conventional microbiological tests and a nanoplex polymerase chain reaction (PCR) assay. Three (1.3%) of the samples were found to contain vancomycin-resistant isolates (MIC>256 microg/mL) that had a vanA genotype. The three vanA positive VRE isolates were identified as different species. Only one isolate (Enterococcus faecium F 4/13_54) was sensitive to teicoplanin (MIC<0. 12-0.35 microg/mL); the other two VRE (E. faecalis A 21_35 and E. gallinarum F 5/10_1) were resistant to teicoplanin (MIC 3.6-->16 microg/mL). The vanC genotype was observed in nine (4%) of the samples collected. High-level gentamicin-resistant (HLGR) enterococci (with MIC ranging between 100 and 500 microg/mL) were detected in 44 samples. However, only 40 of these were found to possess the aac(6')-aph(2'') gene. The overall prevalence of VRE among the samples from the poultry farms and environment was 5.3%, but the prevalence of the clinically significant vanA VRE was 1.3%, and the prevalence of bifunctional aminoglycoside-resistant enterococci was slightly higher, at 19.5%.

    The spread of infectious diseases was further promoted due to busy cities, increased travel, and climate change, which led to outbreaks, epidemics, and even pandemics. The world experienced the severity of the 125 nm virus called the coronavirus disease 2019 (COVID-19), a pandemic declared by the World Health Organization (WHO) in 2019. Many investigations revealed a strong correlation between humidity and temperature relative to the kinetics of the virus's spread into the hosts. This study aimed to solve the riddle of the correlation between environmental factors and COVID-19 by applying RepOrting standards for Systematic Evidence Syntheses (ROSES) with the designed research question. Five temperature and humidity-related themes were deduced via the review processes, namely 1) The link between solar activity and pandemic outbreaks, 2) Regional area, 3) Climate and weather, 4) Relationship between temperature and humidity, and 5) the Governmental disinfection actions and guidelines. A significant relationship between solar activities and pandemic outbreaks was reported throughout the review of past studies. The grand solar minima (1450-1830) and solar minima (1975-2020) coincided with the global pandemic. Meanwhile, the cooler, lower humidity, and low wind movement environment reported higher severity of cases. Moreover, COVID-19 confirmed cases and death cases were higher in countries located within the Northern Hemisphere. The Blackbox of COVID-19 was revealed through the work conducted in this paper that the virus thrives in cooler and low-humidity environments, with emphasis on potential treatments and government measures relative to temperature and humidity. HIGHLIGHTS: • The coronavirus disease 2019 (COIVD-19) is spreading faster in low temperatures and humid area. • Weather and climate serve as environmental drivers in propagating COVID-19. • Solar radiation influences the spreading of COVID-19. • The correlation between weather and population as the factor in spreading of COVID-19.

    Developing a simple and environmentally friendly method to vary the physical, mechanical, and thermal properties of cellulose films is of great importance. This study aimed to characterize 2,2,6,6-tetramethylpiperidinyl-1-oxyl (TEMPO)-oxidized bacterial cellulose (BC) films prepared using non-pressurized hot water vapor (NPHWV) method. A wet BC-pellicle that had been oxidized with TEMPO was treated with NPHWV for 60, 120, and 240 minutes, respectively. As a control, a TEMPO-oxidized BC (TOBC) film without NPHWV was prepared. The results show that the longer NPHWV duration of the TOBC film increased the tensile and thermal properties. This film became more hydrophobic and showed lower moisture absorption, thermal conductivity and organic solvent uptake, more crystalline structure, and higher fiber density after NPHWV treatment. The acquired results provide a simple, inexpensive, and ecologically friendly method for varying TOBC film properties.
