    Problems with assessing the effect of maternal nutrition and lactation performance are presented in relation to a conceptual mathematical ziggurat. Difficulties, variations and flows are to be found in stages of this sequence of "logic" [e.g., assessment of maternal nutrition, volume and composition of breast milk, nutritional satisfactions of the nursling). It is concluded that the various steps in this process of reasoning are subject to much more variation and adaptation than appreciated. Much so-called "faltering" in the second trimester is physiologically normal ("pseudo-faltering").

    405 cases of bronchial brushing cytology were evaluated for its effectiveness in detecting pulmonary carcinoma. Cytohistologic findings were correlated whenever endoscopic biopsies were performed. Sputum cytological investigations were also included in this paper to examine the total diagnostic sensitivity of all the three methods combined.

    Correlation analysis of mid-arm circumference (MAC), weight-for-age and weight-for-height obtained from 807 Malay preschool children aged 12-71 months show that MAC had a relatively low correlation with weight-for-age (r = 0.59, P <0.001) and weight-for-height (r = 0.63, P<0.001). MAC percentiles and sensitivity-specificity analysis indicate that when weight-for-age and weight-for-height were used separately to define 'true' malnutrition, MAC was not sensitive enough to detect Malaysian children with only mild to moderate protein-energy malnutrition (PEM). The value of MAC as a screening measure in malnutrition is only limited to identifying the more severe forms of PEM in young children and the present study indicates that a MAC of 13.0-13.7 cm may be used for identifying moderate to severe PEM and under 13.0 cm for severe PEM.

    This is a report of a case of vulvovaginitis due to Shigella flexneri in a three-year-old child. This is probably the first documented case of shigella vulvovaginitis In Malaysia. The patient was successfully treated with cotrimoxazole. Extraintestinal infections by Shigella are rare and are briefly reviewed in this article.

    Aqueous root extract of Selayak Hitam a plant commonly found in Malaysian jungles and reported to have abortifacient property was screened for oxytocic effect. Results obtained from in vitro experiments on isolated uterus preparation from both pregnant and non pregnant rats and in vivo experiments on uterus contraction in rats in situ, showed that the extract lacks oxytocic effect. It is concluded that the alleged abortifacient property if any, is not mediated through oxytocin or oxytocic-like effect.

    The prevalence of goitre was investigated in a sample from six longhouses and five primary schools located at varying degrees of remoteness along the Tinjar river, Sarawak. Together with this, a survey was made on the type of salt used by the households. The main ethnic groups in these communities were the Kenyah/Kayan and the Iban. The overall prevalence of palpable goitre detected from the age group 5-14 years were 71.7 percent and 77.0 percent for males and females respectively and 77.7 percent for females aged 15 years and above. The prevalence for both Iban and Kenyah/Kayan of either sex and for all age groups varied from 63.4 percent to 80.4 percent. The prevalence at each location did not differ significantly. On questioning 126 households, only 9 (7.1 percent) used iodized salt whilst 74 (58.7 percent) households used only uniodized coarse salt and 7 (5.6 percent) used only uniodized fine salt. The remaining 36 (28.6 percent) households used both fine and coarse uniodized salt. Undoubtedly at this point in time legislation on "table" salt iodization must be interpreted to mean not only the iodization of fine table salt, but in the case of the rural longhouse communities as are found in the Tinjar area, the iodization of coarse salt as well.

    Over the six-veer periodfrom. 1976 to 1981, there were 241 neonates referred to the U.K.M. Paediatric Surgical Unit, General Hospital, Kuala Lumpur for alimentary tract obstruction and 207 were operated on. The three commonest conditions were anorectal anomalies (91 cases), Hirschsprung's disease (31 cases) and oesophageal atresia (30 cases). Overall operatioe mortality was 28.0 percent. This was high when preoperative complications lihe gut perforation (88.9 percent) or pneumonia (61.9 percent) and associated severe anomalies (90.9 percent) or chromosomal abnormalities (66.7 percent) were present. Emphasis is placed on the establishment of early diagnosis and the significance of the green vomit and maternal hydramnios is highlighted, The need is felt for more specialised nurses and the creation of a separate neonatal ICU in this hospital.

    In a 3 year period 73 cases of perforated peptic ulcers were treated by simple closure. The overall mortality of 3.77 percent is acceptable when compared with other reports. There was a marked male preponderance (11 males: female 1). Biopsy results confirmed the operatiue findings that perforations were seven times more common in gastric peptic ulcers than duodenal ones. The disease affected young adults, the middle aged and elderly, the peak incidence occurring between the fifth and seventh decades oflife. Simple closure was an effective surgical procedure for our group of patients.

    Left atrial myxoma almost always arises in the inter-atrial septum. A case is described where it arose from the posterior wall of the left atrium. Clinical presentation was suggestive of mitral stenosis and sub-acute bacterial endocarditis and diagnosis was arrived at necropsy.

    Fifty-Jour patients with oral lichen planus were studied over a three-year period. The important sites were the cheek mucosa, tongue and gingiva. The reticular pattern was the most common type. The professionals and the white collar workers formed the majority of the cases. Emotional disturbances was a predominant factor in this lesion. The rate of malignant change ranges from one to ten percent. Treatment included reassurance, good oral hygiene, avoidance of hot and spicy foods and topical corticosteroids.

    The results of the food consumption study suggested that the calorie and protein intakes of ID and 20 malnourished preschool children in the study area were much lower than the RDA and thus these children should benefit from a supplementary feeding project. However, after 42 days of supplementation with Nutri-Pak, the project's objective of increasing by 5%, the percent standard weight for age of at least 60% of the subjects was not achieved since only one child's weight reached this target. From the records of monitoring, it was noticed that the low consumption of Nutri -Pak. a partial replacement by the the food supplement of the children's diet in the home. uncooperative mothers, and illness or disease factors had contributed to the much lower weight gains than expected in most of the children.

    Thirty girls who had been detained in a remand home on suspicion of prostitution were examined in the University Hospital, Kuala Lumpur. All except two were below 21 years old. Endocervical and high vaginal swabs were taken for cultures. A sample of blood was taken for serology. Eleven of the girls [37%] had no evidence of a sexually transmitted disease. The others [63%] had one or more of the following: 5 with positive serology for syphilis, 3 had gonorrhoea, 6 with trichomonas, 2 with candidiasis.rl with Chlamydia trachomatls, 2 with Ureaplasma urealyticum and 1 with C. vaginale. With such a high incidence of STD in these young girls with a history of prostitution, it is recommended that a more regular and comprehensive screening be carried out on such high risk women.

    PIP: While Malaysia's National Family Planning Board is primarily responsible for family planning activities, several organizations and ministries, both governmental and voluntary, participate in various service programs. Current population policy attempts to go beyond family planning. Population education, treatment of infertility, and cancer screening are offered as well as family planning, to make the program more useful to greater numbers of people. The government also wishes to improve the status of women thereby giving them alternative choices of lifestyle. Rural women are reached through the Ministry of Agriculture's community development program. While the National Family Planning Board receives only 0.12% of the national budget, this figure is not expected to increase. Tables giving vital rates show that population fell below the 30 per 1000 mark for the first time in 1977. A higher rate of fertility decline has taken place between 1967 - 1977 than occurred from 1957 - 1967. Current demographic objectives are to reduce crude birthrate to 28.2 per 1000 by 1980. This goal would require 817,963 new acceptors. While the pill accounts for 80% of acceptor's choice, the proportion using condoms has increased from 1.4% during 1969 - 1970 to 11.4% in 1979. Despite reported side effects with the pill and the illegality of induced abortions, virtually all acceptors are well satisfied with the program in its current form.
