    To investigate the safety climate and knowledge, attitude and practice (KAP) on occupational safety and health (OSH)in the manufacturing industry, also to determine the association between safety climate factors and KAP of safety among manufacturing worker. A cross-sectional study was conducted among 59 respondents from two manufacturing plants located in Gebeng, Kuantan, Pahang. Most of the respondents were Malay (91.5%) and male (96.6%). Participants were administered a set of questionnaires (Cronbach alpha=0.674) that measured the safety climate as perceived by the workers towards their supervisor and KAP of the workers regarding safety-related matters at the workplace. Self-administered questionnaires consisted of 5 points Likert scale used to measure each of the items of safety climate and KAP. The scales for safety climate and KAP were probed using 16 items and 17 items in the questionnaires, respectively. The results were analysed using a non-parametric test, which is Spearman’s rho correlations and descriptive statistics. Bivariate analysis was performed.     The Department of Occupational Safety and Health Malaysia (DOSH) is the authority responsible to safeguard the occupational safety, health and welfare of workers in Malaysia. Occupational noise-related hearing disorders are the leading type of occupational diseases recorded by DOSH every year. Occupational Health Doctor (OHD) is a competency recognized by DOSH and their scope of duties in industries is currently confined to conduct medical surveillance for workers with hazardous chemical exposure and confined space medical fitness examination. The duties of OHD are good to be expanded by empowering more of their roles in industrial activities involving other legislations under DOSH. The noise exposure regulation under the Factories and Machinery Act (FMA) 1967 has been governing the worker’s exposure to hazardous industrial noise and preventing noise-induced hearing loss (NIHL) since 1989. However, the provisions of the regulation need to be strengthened in some essential medical elements of the Hearing Conservation Programme (HCP) for a comprehensive prevention of NIHL at the workplace. Recently enacted Occupational Safety and Health (Noise Exposure) Regulations 2019 offer a wider coverage of workers in ten sectors of industries applicable under the Occupational Safety and Health Act (OSHA) 1994. The current regulatory requirements for management of workplace noise have many improvements compared to the existing law. Enhancement in the medical requirements of industrial audiometry is made prominent by involving OHDs to interpret audiograms and conduct medical examination for workers. The reporting of occupational noise-related hearing disorders to DOSH is outlined better in the new regulation. The occurrence of NIHL and other related hearing disorders are expected to reduce eventually after the introduction of Occupational Safety and Health (Noise Exposure) Regulations 2019. The OHDs will play a pivotal role in industrial audiometry and prevention of hearing disorders among the working population.

    he practice of 5S has been adopted by many organizations as a method to organize the workplace towards reducing waste and improving productivity. Moreover, the practice also promotes a safe and efficient environment in organisations. As safety performance of the workplace is among the key issues in most industries including universities to become world class organizations, this study seeks to address empirically the impact of 5S practice on safety management performance. In this context, a survey questionnaire is employed and distributed to 60 employees of the selected university to examine the employees’ perception on the adoption of 5S practice as well as the impact of 5S practice towards safety management performance. The questionnaires were validated by a pilot test with an acceptable value of Cronbach's alpha. Overall findings highlight there was positive perceptions toward 5S practice among the respondents. The results also revealed that 5S Audit has a very strong positive correlation with safety management performance. Furthermore, 5S Training and Top Management Commitment were (2) factors that strongly correlated to safety management performance. These factors are highly important in contributing toward the success of safety management performance. Additionally, since most of the employees were not exposed with Occupational Safety and Health (OSH), this study reveals moderate negative correlation between compliance to OSH Requirements and safety management performance.

    Industries in Malaysia are entering a period of major disruption caused by new technologies such as Artificial Intelligent, Robotics, Blockchain, Nanotechnology as well as Building Information Modelling (BIM) and the Internet of Things (IoT). In this fourth industrial revolution where information is generated and exchanged at a rapid and huge scale, its reliability is of paramount importance. The success of Occupational Safety & Health Management System (OSHMS) is highly dependent on the reliability of the information gathered and used, where a large number of intermediaries authenticate the information to establish trust between the stakeholders. Blockchain technology is able to do verification by virtue of secured distributed storage brings about a paradigm shift in the way we establish trust. This paper gives an overview of the potential use of Blockchain technology for Occupational Safety & Health Management System. The discussions focused on the benefits and challenges of implementing the Blockchain technology in OSHMS. The conclusion is drawn based on the strength in the characteristics provided by the Blockchain technology itself.

    Occupational diseases are one of the major health problems related to workplace hazards.
    However, the epidemiological data for this problem is scarce especially among Small and
    Medium Industry (SMI) workers. These workers are vulnerable to occupational health problem
    due to lack of knowledge and implementation of health and safety in the workplace. In Malaysia,
    most of the SMI workers have limited coverage for basic occupational health services which
    may worsen their health. Thus, this article aims to provide a review on the burden of
    occupational health problems among them. The electronic and library searches were used to
    extract the information from both published and unpublished articles that were not limited to any
    year of publication until 2017. One hundred and ninety-six published articles and 198
    unpublished articles were retrieved from the database. Only 19 published articles and 25
    unpublished articles met the eligibility criteria. Prevalence data of occupational
    diseases/poisoning, including overall and body specific (musculoskeletal disorders) was
    extracted in raw data from the eligible studies. Prevalent statistics on occupational
    musculoskeletal diseases (1.3% - 97.6%), noise-induced hearing loss (29.4% - 73.3%),
    occupational skin diseases (10.5% - 84.3%), respiratory (1.9% - 92.2%) and occupational
    poisoning (14.9% - 17.7%) among the working population is different within published papers
    compared to unpublished ones. In Malaysia, there are no specific statistic that give a true picture
    of the burden of occupational diseases in the SMI. However, this review concludes that
    musculoskeletal diseases are significant occupational problems among SMI workers.

    Introduction: Malaysian traffic police are always working on the public roads to alleviate traffic congestion and therefore exposed them to the polluted air every day. In particular, polluted air containing PM2.5 is dangerous to their respiratory health as they can penetrate deep into the lungs, leading to bronchitis, lung cancer and many more. Hence, this research examined the relationship of personal exposure level to PM2.5 with respiratory symptoms among traffic policemen in Kuala Lumpur and Johor Bahru. Methods: 134 participants among traffic policemen were agreed to participate in this study. They were requested to complete a questionnaire regarding the sociodemographic back- ground and respiratory health information. The questionnaire was adopted from International Union Against Tuber- culosis Lung Diseases (1986). Personal exposure level of PM2.5 was measured using an air pump with 5.0µm pore size PVC filter. Results: The mean exposure level of PM2.5 among traffic policemen was 28.69 µg/m3. It was found that some of them possess respiratory symptoms (Coughing 33.6%, Phlegm 25.4%, Wheezing 14.9% and Shortness of breath 32.1%). There was significant association detected at p-value < 0.05 in coughing. Despite, there was no significant association in other symptoms such as phlegm, wheezing and shortness of breath. Conclusions: Traffic police were exposed to a relatively high level of PM2.5 (12.4 µg/m3 to 55.3 µg/m3) and showed symptoms of respi- ratory effects. Therefore, recognition of the risks connected with occupational lung disease and exposure monitoring must be a high priority. This baseline data can serve as a reference to the top management of traffic police officers in order to develop an occupational safety and health guideline for police officers as required by Occupational Safety and Health Act (OSHA, Act 514 1994).

    A guideline on Basic Occupational Health Services (BOHS) has been established jointly by ILO/WHO/ICOH in response to poor achievements of the Occupational Health Services (OHS), especially among workers in small and medium enterprises at the global level. Malaysia. The international guideline describes competent and skilled human resources as an essential strategy for BOHS implementation. This commentary will discuss the challenges faced by current occupational health personnel providing OHS in Malaysia and proposes improvements of human resource development for future BOHS in Malaysia to ensure fair and better OHS coverage for Malaysian workers.

    Background: Heat stress disorders may cause negative health outcome and subsequent productivity reduction especially in those who work under direct sunlight for an extended number of hours.

    Objective: This study assessed the impact of heat on the health and productivity among maize farmers in a hot tropical country.

    Methods: A cross-sectional study was conducted among 396 maize farmers, randomly selected across Gombe province, Nigeria. The wet bulb globe temperature monitor (WBGT) Model QuesTemp036 was used in determining the heat index. Health was determined using a validated questionnaire, while productivity was determined by recording work output based on the number of ridges cultivated during the working hours.

    Results: The farms recorded mean heat index with standard deviation (SD) of 31.56 (2.19) and 34.08 (1.54) in the hours of 9 am to 12 pm and 12-3 pm respectively, which exceeded the threshold level set by the ACGIH. Heavy sweating (93.2%), tiredness (48.5%), dizziness (34.1%), and headache (40.4%) were experienced by the respondents almost on daily basis. The finding further showed a significant difference in the farmers' productivity during the three time duration of the work day (p < 0.001). The productivity was significantly higher between the hours of 6-9 am (p < 0.001) and 12-3 pm (p < 0.001), compared to the hours of 9 am to 12 pm (p < 0.001). The factors that significantly predict the productivity outcome include temperature (p < 0.001), gender (p < 0.001), age (p=0.033), and BMI (p=0.008).

    Conclusion: The farmers were frequently experiencing heat exhaustion which decreased their productivity.

    Introduction: Water based Metalworking fluids (MWF) are commonly used in machining industries and are excellent media for microorganism growth. The study aimed at determining the relationship between the airway inflammation as indicated by fractional exhaled nitric oxide (FeNO) with the microbial contaminants of MWF in aerosol and bulk sample as well as the workers’ reported respiratory health symptoms. Methods: This cross sectional study was carried out on 138 machining workers. Their FeNO were measured using NIOX-MINO instrumentation. The microbial as- sessments of bacteria and fungus were carried out on the MWF bulk samples and the aerosol using a sampler DUO SAS SUPER 360TM. Results: Findings showed significant difference in the FeNO levels in workers from various job sections (p=0.01).     Introduction: Responder’s action during mass environmental chemical incident involves collaboration of multiple agency. They clean the exposure site, control public safety and safe lives. The health of the responder is also as valuable as the public and always been neglected. Hence, this study is to highlights the method used and challenges during the incidents. Methods: A modified Occupational Health Surveillance Programme designed by Baker and Matte (Thirteen Steps in designing and implementing an Occupational Health Surveillance Programme) was imple-mented. These method full fill the criteria of impossible further reduce exposure to known hazards and uncertain health effect of the offending chemicals. Two core workplace namely the source of chemical contamination locale and victims’ evacuation centre were identified. The exposed responder is identified and categorised according to their expected exposure. Offending chemicals identified were known hazardous to health. An electronic survey form was developed and the surveillance were conducted during the event and post-exposures of one, third and sixth months. Laboratory tests were performed as a grab sample randomly and strategically to represent responder exposure. Results: During the event, 1338 responders were surveyed and twenty percent had symptoms of cough, headache, sore throat, nausea and skin irritation. After one-month post-exposure, three percent of responder noted to have continue symptoms. Laboratory tests were performed randomly due to the sheer numbers of responders in action. Only sixty-one samples were collected and sixteen percent had positive metabolites for urine cyanide. Re-sponders with abnormal laboratory result (FBC/LFT/RP/Urine) were interviewed and a repeat sample were performed. Conclusion: The health surveillance of the responder in mass environmental chemical incident is a great challenge when the exposure level is unsure and involve more than one chemicals. Likewise, reliability and validity adopting statistical test for health surveillance instruments is unattainable due to urgency. However, a proper method of occu-pational surveillance need to be devised and implemented to ensure the protection of worker’s health is guaranteed.

    The construction industry plays a significant role in contributing to the economy and development globally. During the process of construction, various hazards coupled with the unique nature of the industry contribute to high fatality rates. This review refers to previous published studies and related Malaysian legislation documents. Four main elements consisting of human, worksite, management and external elements which cause occupational accidents and illnesses were identified. External and management elements are the underlying causes contributing to occupational safety and health (OSH), while human and worksite elements are more apparent causes of occupational accidents and illnesses. An effective OSH management approach is required to contain all hazards at construction sites. An approach to OSH management constructed by elements of policy, process, personnel and incentive developed in previous work is explored. Changes to the sub-elements according to previous studies and the related Malaysian legislation are also covered in this review.

    In 2012, there were about 2.3 million deaths worldwide attributed to work. The highest workplace fatality rate (WFR) was reported on construction sites due to high risk activities. Globally, fall from height is the leading cause of fatal injuries for construction workers. The objectives are to determine Brunei Darussalam's demographic distribution of occupational fatality; identify causal agents and industry where occupational fatalities commonly occur; and determine WFR by year. This cross-sectional study retrospectively reviewed records of occupational fatality which were notified to the Occupational Health Division, Ministry of Health, from January 2012 until December 2016. Notified occupational fatalities in Brunei over a five-year period was 50. Most of the cases were in 31-40 age group. 38% of fatality cases occurred in Indonesian workers. 60% were from the Construction industry. 38% were due to fall from height. WFR averaged 5.28 and the highest industry-specific fatality rate was seen in the Construction industry, ranging from 27.94 to 56.45 per 100,000 workers. WFR for Brunei Darussalam from 2012 to 2016 was similar to that of Malaysia, but higher than Singapore and the UK. Industry-specific fatality rate for the Construction and Manufacturing industries were higher than those of Singapore and the UK.

    Prolonged standing can cause discomfort on the body of the workers and can lead to injury and occupational disease. One of the ergonomic intervention is through improving the work-rest scheduling. The purpose of this study are to identify the fatigue level from the perception of the worker and to investigate the impact of the work-rest scheduling to the standing workers for 12 h working time with a different gender. This study involved two methods which are self-assessment of the worker and direct measurement by using electromyography (EMG). For self-assessment, 80 workers have been interviewed using questionnaire in order to identify the fatigue level. For direct measurement, EMG was attached to the 15 selected workers at their respective leg and lower back to analyse the muscle efforts. In terms of perception, the results show the discomfort and fatigue level at the lower body region in the following order as foot ankle, lower back and leg. There is a significant difference between gender on discomfort pain for foot ankle and leg. The results show short frequent break by 10 min can reduce the fatigue at the leg and infrequent long break is preferable in order to reduce the fatigue at the lower back. In conclusion, it was found that prolonged standing affect the muscle fatigue and discomfort especially lower extremities such as foot ankle, lower back and leg. Besides that, different type of work rest scheduling and gender have significant result towards the muscle fatigue development.

    Psychosocial safety climate (PSC; climate for psychological health) is an organizational antecedent to work conditions articulated in the job demands-resources model. We responded to calls for broader consideration of organizational climate in terms of both climate level and strength. We tested PSC level and strength as main and interactive predictors of work conditions, psychological health, and engagement. Using multilevel analysis and cross-sectional data, the effects of unit-level PSC constructs were investigated in 21 hospital work units (n = 249 employees) in Australia. The correlation between PSC levels (measured at the unit mean) and PSC strength (measured as unit -1 × SD) was moderate and positive, suggesting that ceiling effects of PSC scores were not problematic. PSC level was a better predictor than PSC strength or their interactions for job demands (psychological and emotional demands), job resources (e.g., skill discretion and organizational support), and health (emotional exhaustion). For engagement, the interaction was significant-improving engagement, therefore, benefits from high levels of PSC and PSC strength within the work units. So, in answer to the research question regarding PSC theory extension, "it depends on the outcome." Research limitations are acknowledged, and the potential of the PSC model to guide the reduction of workplace psychosocial risk factors and the negative consequences is discussed.     BACKGROUND: The investigation of risk factors of cardiovascular disease (e.g., major endocrine, nutritional and metabolic diseases) across job sectors is useful for targeted public health intervention. This study examined the occurrence of type 2 diabetes mellitus (T2DM), hypercholesterolemia and obesity in 21 job sectors in the general population.

    METHODS: A baseline cross-sectional analysis of the Malaysian Cohort was conducted, which included 105 391 adults. Multiple logistic regression analyses were conducted for these three diseases across 20 job sectors compared with the unemployed/homemaker sector.

    RESULTS: The prevalence of T2DM, hypercholesterolemia and obesity was 16.7%, 38.8% and 33.3%, respectively. The Accommodation & Food Service Activities and Transportation & Storage sectors had significantly higher odds for T2DM (adjusted [adj.] prevalence odds ratio [POR] 1.18, p=0.007 and adj. POR 1.15, p=0.008, respectively). No job sector had significantly higher odds for hypercholesterolemia compared with the unemployed/homemaker sector. Only the Accommodation & Food Service Activities sector had significantly higher odds for obesity (adj. POR 1.17, p≤0.001).

    CONCLUSIONS: Many job sectors were significantly associated with lower odds of having these three diseases when compared with the unemployed/homemaker sector. These differing associations between diverse job sectors and these diseases are important for public health intervention initiatives and prioritization.

    BACKGROUND AND PURPOSE: Healthcare workers are prone to occupational hazards. The study aims to identify the occupational health hazards among healthcare workers in the Philippines and its essential relevant developmental framework. This article evolved on the responses of participants on how they can improve strategies and barriers for healthcare workers to comply with Occupational Health and Safety (OSH).

    METHODS: A qualitative study design in which 15 healthcare workers from nurses (4), pharmacists (3), medical technologies (4) and medical doctors (4) participated: two focus group of three to four participants each and eight in-depth interviews. The thematic sessions were identified, including occupational health and safety policy implementations, hazards experiences, barriers, and strategies for quality improvement for OSH. Focus groups and interviews using transcript-based analysis were identified relating to emerging themes on the challenges they had experienced while accessing provisions of OSH in their workplace.

    RESULTS: Majority of the participants revealed the existence of policy on Occupational Health and Safety (provisions, guidelines and regulations on OHS from the government) and mentioned that there were limited OHS officers to supervise the healthcare workers in their workplace. Some have limited accessibility to the requirements of the implementation of OHS (free facemasks, gloves, disinfectants, machines, OSH staff, etc.) among healthcare workers, while the workload of the staff in the implementation of OHS in the workplace gradually increased. The results indicated that the respondents were knowledgeable in the implementation of OHS in the workplace, and that there was no existing ASEAN framework on the protection and promotion of the rights of healthcare workers in their workplace. Facilities need to improve health assessment, and to ensure constant evaluation of the existing laws for healthcare workers (quality assurance of existing policies) in their working areas. Direct access to OSH officers, occupational hazards education, emergency contact etc. must be improved. Adherence must be strengthened to fully comply with the OHS standards.

    CONCLUSION: The researchers inferred that issues and concerns regarding compliance on provisions of occupational health and safety among health care workers must be properly addressed through immediate monitoring and reevaluation of personnel in terms of their knowledge and practices in OHS. Barriers and challenges have been identified in the study that can lead to improved compliance among healthcare workers in regards to OHS.

    BACKGROUND: Dietary pattern analysis is a complementary method to nutrient analysis in evaluating overall diet-disease hypotheses. Although studies have been conducted to derive dietary patterns among Malaysians, their consistency across subgroups has not been examined.

    OBJECTIVE: The study aimed to derive dietary patterns empirically and to examine the consistency and generalizability of patterns across sex, ethnicity, and urban status in a working population.

    DESIGN: This was a cross-sectional study using data from the Clustering of Lifestyle Risk Factors and Understanding its Association with Stress on Health and Well-Being among School Teachers in Malaysia study collected between August 2014 and November 2015. Dietary intake was assessed using a food frequency questionnaire, and dietary patterns were derived using factor analysis.

    PARTICIPANTS/SETTING: Participants were teachers from selected public schools from three states in Peninsular Malaysia (n=4,618).

    MAIN OUTCOME MEASURES: Dietary patterns derived using factor analysis.

    STATISTICAL ANALYSES PERFORMED: Separate factor analysis was conducted by sex, ethnicity, and urban status to identify dietary patterns. Eigenvalue >2, scree plot, Velicer's minimum average partial analysis, and Horn's parallel analysis were used to determine the number of factors to retain. The interpretability of each dietary pattern was evaluated. The consistency and generalizability of dietary patterns across subgroups were assessed using the Tucker congruence coefficient.

    RESULTS: There was no subgroup-specific dietary pattern found. Thus, dietary patterns were derived using the pooled sample in the final model. Two dietary patterns (Western and Prudent) were derived. The Western dietary pattern explained 15.4% of total variance, characterized by high intakes of refined grains, animal-based foods, added fat, and sugar-sweetened beverages as well as fast food. The Prudent dietary pattern explained 11.1% of total variance and was loaded with pulses, legumes, vegetables, and fruits.

    CONCLUSIONS: The derived Western and Prudent dietary patterns were consistent and generalizable across subgroups of sex, ethnicity, and urban status. Further research is needed to explore associations between these dietary patterns and chronic diseases.

    The study aimed to compare nurses' quality of life and investigate key determinants among Asian countries with different economic status. A cross-sectional survey was conducted across five Asian countries (Japan, Singapore, Malaysia, Thailand, and Bhutan). Quality of life (WHOQOL-BREF), job stress (National Institute of Occupational Safety and Health questionnaire), and demographic data were assessed. Stepwise multivariate linear regression analysis was performed to identify the key determinants of quality of life. Participants were 3,829 nurses (response rate: 82%) with a mean age of 33 ± 10 yr and majority were women (92%). Regarding quality of life, Bhutan yielded the highest scores, followed by Malaysia, Thailand, Singapore, and Japan, and these results were statistically significant. The key determinants that were significantly related to quality of life were "stress coping ability," "life satisfaction," "Japan," "social support," "job stress," and "Singapore" (adjusted R2=0.46). In conclusion, nurses' quality of life differs across Asian countries and is not linked to the country's economic development. To maintain a good quality of life for nurses, an international exchange program like international nursing conferences for work environment and staff coping strategies is recommended to broaden institution' minds and share experiences and exchange views to be able to realize their own problems and discover global solutions to them.

    This research covers the current status of occupational health and safety (OHS)-related practices in the informal construction segment of Pakistan. Data were collected, through interviews, from 316 construction sites employing 3577 workers. The results of the study reveal that both employers and workers lack knowledge of OHS laws/standards and no practices of this nature are enacted at these construction sites. Alarmingly, work-related accidents, whenever they happen, are not given due attention and there is no formal injury-report system. The informal construction industry employs a huge portion of the informal workforce, and lack of OHS happens at tremendous human cost. These research findings may thus play their role in strengthening the case for reforms in the sector. This study, if properly utilized, may also enable employers of the sector by increasing their knowledge about OHS practices and, as a result, trying to offer safer environments for their workers.

    PURPOSE: To assess Malaysian dentists' perceptions of their mental well-being.

    METHODS: A self-administered questionnaire was developed based on a conceptual framework of mental health and well-being model. Two aspects were assessed, namely the physiological (two domains) and the psychological (six domains). Participants were asked to rate their experiences of the aforementioned aspects using a 5-point Likert scale ranging from all the time to never.

    RESULTS: The response rate was 81%. Most of the dentists (61.7%) perceived having positive mental well-being. Under the physiological aspect, most respondents reported that they were 'generally happy' (93.3%), but about 30% stated they were 'stressed physically and emotionally'. Of the six domains under the psychological aspect, positive well-being was observed in the 'sense of coherence' and 'behavioural stress' domains. Participants who were above 40 years old, married and had children reported having a more positive mental well-being when compared with their counterparts.

    CONCLUSION: Overall, most Malaysian dentists perceived having a positive mental well-being. It is crucial, however, to closely monitor and initiate early interventions for those with negative symptoms to ensure the safe practice of dentistry.
