    Hyperdensity of basal ganglia in computed tomography (CT) of brain is always recognised as hemorrhagic stroke or calcification. Features of hyperglycemia include muscle weakness, hypotonia, pyramidal tract signs and hemichorea-hemiballismus (HC-HB), which mimic the symptoms of stroke. Hyperdensity of the basal ganglia was reported in patient with non-ketotic hyperglycemia (NKH). Inability to recognize the hyperdensity in CT brain as a feature of NKH may lead to failure of treatment. Early recognition and reversal of hyperglycemia will improve the outcome. This was a case of an atypical presentation of stroke-like symptoms with the neuroimaging finding showing asymmetric hyperdensity of basal ganglia. Neurological examination failed to elicit any sign of HC-HB. Laboratory test showed hyperglycemia with absence of acidosis and ketonaemia. The patient was diagnosed having basal ganglia hemorrhage and referred to the Neurosurgical team. However, the stroke-like symptoms completely resolved following the normalization of glucose level. This is the only other reported case of NKH with typical neuroimaging features not associated with HC-HB. One should be aware of the possibility of NKH in the absence of movement disorder with asymmetric basal ganglia hyperdensity in CT brain. Misinterpretation as acute intracranial bleeding may result in suboptimal management of the true underlying cause.

    Brachymetatarsia is a conditioned which has been described as abnormal short metatarsal bone due to premature closure of the epiphyseal growth plate. It usually involves a single metatarsal bone with the fourth metatarsal being the most common. It may be congenital or acquired. We here report a case in a 53-yearold lady who presented with foot pain and shoe wearing problem. Examination revealed shortening of the bilateral fourth toe and the toes were tilted dorsally. She was diagnosed with bilateral brachymetatarsia and operation was done mainly for cosmetic reasons. Gradual lengthening with distraction osteogenesis was performed using a MiniRail external fixator (Orthofix) and K-wiring of the right fourth metatarsophalangeal joint. Adequate length of fourth metatarsal and good bony union were achieved at the end of treatment.

    Acupuncture is a form of complementary medicine that has been practiced in China for thousands of years. Adverse effect of acupuncture is rarely reported in local literature. This is a case of a patient who developed bilateral pneumothorax following an acupuncture session. A 63-year-old lady with no significant medical illness presented with sudden onset of shortness of breath half an hour following acupuncture and massage session by traditional medicine practitioner. On examination, she was tachypnoiec and there was reduced air entry bilaterally on lung auscultation. Urgent portable chest X-ray was done and it showed bilateral pneumothorax. Bilateral chest tubes were inserted. Patient was discharged well following five days of hospitalization. As acupuncture is gaining popularity among Malaysian population, medical practitioners need to increase their awareness and knowledge regarding the adverse effect of such alternative practice.

    A 17-year-old male student of Indonesian parentage presented with two weeks history of progressive painless bilateral visual deterioration. There was no contact with tuberculosis (TB)-infected patients and parents claimed that all immunization including BCG was completed. However, BCG scar was not apparent. Visual acuity was 6/36 and 6/60 in the right and left eyes respectively. The anterior and vitreous chambers were quiet. Funduscopic examination revealed retinal vasculitis with perivascular exudates, branch vein occlusion, neovascularization and macular oedema. Fluorescein angiography confirmed large areas of capillary non-perfusion and leaking new vessels. Mantoux test was positive and full regime anti-TB therapy was instituted. HIV screening was negative. Three days later, an immunosuppressive dose of oral steroid was started. Both eyes received intensive laser photocoagulation.Interestingly, there was no development of vitritis throughout.

    Ascariasis is a common helminthic infestation in Malaysia, particularly in rural settings. Ascariasis lumbricoides normally lives in the upper small bowel without causing symptoms. Complications arise when these worms migrate into the bile duct (biliary ascariasis). A case of biliary ascariasis presenting as biliary colic is described. Patient presented with 2 days duration of right upper quandrant pain at the district hospital. Initial investigations were suggestive of acute cholecystitis and patient was treated with empirical antibiotics. However, due to worsening symptoms, she was transferred to the nearest tertiary setting. The diagnosis of helminthic biliary infestation was established using both ultrasound of the hepato-biliary system (HBS), CT-Scan abdomen and endoscopic retrograde cholangio-pancreatography (ERCP). Endoscopic removal of the worm led to rapid resolution of symptoms and patient was discharged home well.

    Case of co-existence of twin pregnancy of complete hydatidiform molar with viable intrauterine pregnancy is extremely rare with low incidence of 1 case for 20,000 – 100,000. It is associated with high risk of spontaneous abortion, preterm delivery, intrauterine death, bleeding, pre-eclampsia, and persistence trophoblastic disease (PTD). It may associate with biochemical derangement that may induce symptomatic manifestation to the mother. There are few cases reported in Asia population with significant clinical dilemma and management to the maternal and foetus. Here, we report a case of a young woman with previous bad obstetric history who presented with antepartum per-vaginal bleeding and was noted to have a twin pregnancy with complete hydatidiform molar and viable foetus. It was complicated with markedly elevated human chorionic gonadotropin (hCG) and hyperthyroidism. Postpartumly, her hCG level was persistently high and her condition progressed into gestational trophoblastic neoplasm.

    Transgender adalah satu keadaan biopsikososial yang kompleks dan merupakan sebahagian daripada dimensi seksualiti manusia. Ia merangkumi komponen kognitif-emosi dan tingkah-laku yang membuatkan seseorang insan itu unik di dalam ekspresi seksualitinya. Kaum transgender cenderung untuk menggunakan hormon-silang-seks untuk mengubah keadaan dirinya menyerupai gender bertentangan yang diingini dengan cara menutupi ciri sekunder seksnya. Seks hormon yang biasa digunakan ialah: ““Female to Male Treatment Options (FMTO)” iaitu testosterone and hormon wanita pada “Male to Female Treatment Options (MFTO)”, di mana kombinasi estrogen dan anti-androgen digunakan. Hormonsilang-seks iaitu FMTO, atau MFTO mempengaruhi sifat biologi dan psikologi individu transgender. Namun begitu, hormon-silang-seks mempunyai profil kesan sampingan tertentu yang juga memberi impak biologi dan psikologi. Impak psikologi menjurus kepada masalah kesihatan mental dan bunuh-diri. Pelbagai tahap pengaruh biopsikososial kesan hormon-silang-seks diterangkan dan ulasan ini memberi ruang membincangkan kepelbagaian pilihan rawatan. Di dalam psikiatri, perubahan biologi memberi pengaruh yang besar ke atas individu transgender, terutamnaya dari aspek psikososial dan budaya.

     We report two cases of biphenotypic acute leukaemia diagnosed in Hospital Universiti Kebangsaan Malaysia (HUKM), their clinical, haematological characteristics and response to chemotherapy. Both patients are middle-aged ladies who presented with hepatosplenomegaly and high white cell count, mainly composed of blast cells. Their bone marrow aspirations were hypercellular comprising of more than 90% heterogenous blast cells. Cytochemical analyses show more than 3% positivity towards peroxidase, with smaller blasts showing block positivity towards PAS. Immunophenotypically, the blasts showed dual expression of CD 33 and CD 19, CD 19 and CD34, intra CD22, intra TdT and intraMPO. One of the patients showed presence of the Philadelphia chromosome on cytogenetic analysis which was confirmed by Fluorecsence In Situ Hybridisation (FISH). Molecular analysis also showed presence of the BCR-ABL fusion protein. Both patients were given combination chemotherapy consisting of daunorubicin and cytosine arabinoside.However, the patient with positive Philadelphia chromosome BCR-ABL did not achieve morphological remission after induction chemotherapy. In view of the poor prognosis of this disease, both the patients were planned for upfront peripheral blood stem cell transplantation.

    The advantages of breastfeeding have been well established. The objective of this study was to determine the practice and knowledge of breastfeeding in an urban com-munity and identify the influencing factors.  This is a cross-sectional study. All mothers attending the Maternal and Child Health Clinic during the study period, who fulfilled the selection criteria, were included in the study. The mothers were interviewed using a self administered questionnaire. A total of 162 mothers participated in the study.     This is a retrospective descriptive study done to look at common presentation and method of detection of breast cancer. A total of 366 case records of patients attending the Breast and Endocrine Clinic at Hospital Kuala Lumpur were reviewed. The peak age of breast cancer presentation was 40 to 49 years (39.6%). Most (81.4%) patients presented with a lump in the breast and the lump was mainly self-detected (97.3%). The mean tumour diameter on presentation was 4.7± 3 cm. Medical staff detected the disease in 1.6% cases and 1.1% of cases were detected by mammogram. Most women detected the lump themselves, suggesting that Breast Self Examination (BSE) can be used for detection of the disease in places where there is cost and availability constrains for mammogram. Early detection with BSE can possibly offer better treatment options and quality of life despite the evidence that it does not reduce the mortality due to breast cancer.

    Pengukuran ketumpatan mineral tulang oleh 'Dual-energy X-ray Absorptiometry' (DXA) adalah penting untuk mengenalpasti osteoporosis. Ralat ketepatan DXA adalah ukuran yang penting untuk menentukan perubahan sebenar dalam nilai ketumpatan mineral tulang. Kajian ini bertujuan untuk mengkaji pekali variasi jangka pendek mesin QDR Wi DXA Discovery Hologic. Ketumpatan mineral tulang pinggul dan tulang belakang untuk lima belas sukarelawan (purata umur: 30.67 + 10.41 tahun) dan ketumpatan tulang keseluruhan badan untuk lima belas ekor tikus Sprague-Dawley betina (berusia tiga bulan) diimbas menggunakan mesin HDR Discover QDR Wi DXA. Setiap sukarelawan dan tikus menjalani imbasan sebanyak tiga kali untuk menilai kebolehulangan nilai ketumpatan tulang. Imbasan untuk subjek manusia dilakukan dalam tempoh 1 hingga 12 minggu. Untuk sampel haiwan, imbasan diulang pada hari yang sama selepas posisi semula. Ralat kepersisan dinyatakan sebagai peratusan pekali variasi (%CV). %CV diperolehi untuk tulang belakang lumbar adalah 1.8% dan 1.2% untuk tulang pinggul. %CV untuk keseluruhan BMD tikus adalah 1.4%. %CV jangka pendek yang ditunjukkan untuk kedua-dua manusia dan haiwan dalam kajian ini adalah setanding. Ralat kepersisan DXA mesti dipantau untuk memastikan prestasi yang optimum.

    Sindrom distensi beg kapsul ialah salah satu komplikasi yang jarang berlaku selepas fakoemulsifikasi dengan implan kanta intraokular dimasukkan ke dalam bag kanta dalam mata. Kami menerangkan satu kes sindrom distensi beg kapsul yang berlaku pada seorang lelaki berusia pertengahan yang menjalani fakoemulsifikasi dengan ‘plat haptic’ implan kanta intraokular (Zeiss CT ASPHINA 509MP) ke dalam mata kanan. Penglihatan mata kanan semasa pemeriksaan susulan 1 minggu and susulan 5 minggu selepas pembedahan tidak menunjukan penambahbaikan dengan refraksi kuasa mata bertukar menjadi minus 2.5 diopter. Tekanan intraokular hanya meningkat pada 2 jam selepas pembedahan dan normal semasa pemeriksaan susulan berikutnya. Beg kapsul distensi telah disahkan dengan menggunakan IOL Master 700. IOL Master 700 menunjukkan pengasingan abnormal beg kapsul dari implan kanta intraokular. Diagnosis sindrom distensi beg kapsul yang disebabkan oleh pengekalan viskoelastik dibuat. Pengeluaran viskoelastik yang tertinggal di dalam mata kanan telah dilakukan dan sindrom distensi beg kapsul berjaya diselesaikan dengan penglihatan mata kanan kembali ke 6/6. Oleh itu, pengeluaran viskoelastik dengan sepenuhnya semasa fakoemulsifikasi dengan ‘plat haptic’ implan kanta intrakular adalah penting untuk mengelakkan sindrom distensi beg kapsul.

    Child birth is associated with severely painful experience for the parturient, and often exceeds one’s expectations. Even though, severe pain is non life-threatening condition in healthy parturient women, it may lead to undesired neuropsychological consequences. When no analgesia was used, postnatal depression may be more common, and this labour pain leads to the development of post-traumatic stress disorder. Epidural analgesia is now considered gold standard for effective pain relief during labour. We here report a case of a 37-year-old G1 P0 patient at term gestation who successfully used epidural analgesia for labour pain management.

    Miscarriage is one of the most common complications in pregnancy. There is emerging evidence that psychological impact following miscarriage is not unusual. Understanding the magnitude of psychological morbidity is important in the management of miscarriage. The main objective of this study was to compare the mean Hospital Anxiety and Depression Scale (HADS) score between women with miscarriage and women with successful pregnancy and to determine the sociodemographic factor and clinical characteristic that are associated with anxiety and depression. A descriptive case control study was conducted in a teaching hospital, over a period of 12-months (from October 2014 till September 2015). A total of 65 women were recruited with 32 women as the study group (miscarriage) and another 33 women as the control group (women with successful pregnancy). Mean HADS-anxiety score was higher in the study group compared to control group although it was not statistically significant (6.53 ± 3.427 vs 5.73 ± 2.875, p=0.309). Mean HADS-depression score was higher in the control group (4.34 ± 2.695 vs 4.45 ± 3.073, p=0.878). Women with maternal age more than 35 years and history of previous miscarriage had a higher tendency of anxiety and depression with higher mean HADS score. There was no association between other sociodemographic data and clinical characteristic with risk of anxiety and depression. As conclusion, there was no significant difference in women with miscarriage as compared to those with successful pregnancies, although older women with history of miscarriage had a preponderance to both disorders.

    Cavernous lymphangioma is a congenital malformation of lymphatic system causing dilated lymphatic sinuses that involve the skin and subcutaneous tissues. This was an interesting case of dystrophic macrodactyly of the left ring and little finger in a 18-month-old girl who presented with swollen and sausage like fingers deformity which turned out to be an isolated cavernous lymphangioma. This tumor, although rare to occur in the extremeties, must be differentiated from other congenital vascular lesions of the hand that include arteriovenous malformations and hemangiomas. Diagnosis should be solely based on histopathological analysis of the excised tissue mass. Surgical excision is usually necessary for satisfactory functional and cosmetic outcome

    Kajian ini menemui faktor-faktor cabaran kempen anti-merokok dari perspektif petugas kesihatan di Kuantan. Objektif kajian ini adalah untuk menerokai cabaran kempen anti-merokok daripada pandangan petugas kesihatan, kesedaran tentang kewujudan Klinik Berhenti Merokok dan cara untuk menambah baik kempen antimerokok. Kajian ini adalah penting untuk membantu pihak berkuasa Malaysia untuk membangunkan strategi yang lebih baik dalam mempromosikan kempen anti-merokok pada masa akan datang. Persampelan “purposive” digunakan dalam pengumpulan data dan wawancara separa berstruktur telah dilakukan di kalangan lima petugas kesihatan dari tiga Klinik Berhenti Merokok yang terpilih. Kajian ini mendapati petugas kesihatan berpendapat bahawa perokok yang menerima rawatan dari Pusat Kesihatan mempunya pemahaman yang rendah mengenai tatacara piawai (SOP) dalam pemberhentian merokok, komitmen yang rendah dan senang dipengaruhi oleh masyarakat. Petugas kesihatan juga berpendapat tugas pelbagai disiplin menyebabkan tumpuan yang rendah terhadap pesakit yang merokok. Di samping itu, responden merasakan bahawa kewujudan Klinik Berhenti Merokok perlu diperluaskan penghebahannya kepada komuniti. Melalui penemuan ini, beberapa strategi dicadangkan untuk menambah baik pendekatan semasa dalam pengurangan bilangan perokok di Malaysia seperti penambahbaikan penghebahan kepada orang ramai, meningkatkan penguatkuasaan undang-undang serta menyediakan insentif.

    Traditional and Complimentary Medicines (TCMs) can potentially cause Adverse Cutaneous Drug Reactions (ACDRs). The aim of this study was to describe the characteristics of ACDRs due to TCMs and compare with those due to conventional medicine. This was a retrospective study with 134 cases being diagnosed with ACDR at the Dermatology Clinic of a tertiary hospital in Kuala Lumpur. Most (82.1%) ACDRs were caused by conventional drugs, while 17.9% were due to TCMs. Majority (70-75%) of the ACDRs were of mild to moderate severity. The most common ACDR to TCM was Exfoliative Dermatitis (ED) and Acute Generalized Exanthematous Pustulosis (AGEP) (both 16.7%) while maculopapular rash was the most common reaction for conventional medications (25.5%). The onset of adverse reaction to TCM was about 4 weeks (60.2%) while reactions due to conventional medication was earlier (1 to 6 days, 65.4%; p

    This case report highlights Charles Bonnet Syndrome as a precursor to the development of major neurocognitive disorder due to Alzheimer’s disease in the elderly with visual impairment and the possible differential diagnoses that could be considered. We report a case of an elderly lady with no known previous medical illness, who presented for the first time to the psychiatric clinic with complex visual hallucinations consisting of well-formed images of people and inanimate objects of long standing duration of 18 months. About a year later, there was emergence of cognitive impairment which progressed gradually accompanied by other psychotic symptoms such as auditory hallucinations and persecutory delusions. There were no affective or obsessional symptoms. She had poor insight towards her illness. Ophthalmological examination revealed significant reduction in bilateral visual acuity. Otherwise, physical examination was unremarkable. She received inpatient treatment with Rivastigmine patch 4.6 mg/24 hours and Zydis 10 mg noctre. Her cognitive impairment and psychotic symptoms gradually improved over a period of 2 weeks upon commencing treatment and on subsequent follow-ups at outpatient clinic. She had also gained some insight into her illness. Charles Bonnet syndrome could be a possible precipitating factor and precursor to the development of major neurocognitive disorder due to Alzheimer’s disease in the elderly with visual impairment which is often overlooked or misdiagnosed and hence under reported.

    The present study aimed to determine the factors associated with CKD stage 3b among type 2 diabetics attending primary care follow-up, specifically the role of angiotensin blockade dosage. This was a pilot unmatched case-control study conducted in a teaching primary care centre. Clinical data of 25 cases of diabetic patients with CKD stage 3b (GFR 30-45ml/min/1.73m2) in 2012 were selected for this study, as well as 103 controls who were diabetic patients with GFR more than 45ml/min/1.73m2 in 2012. Systematic random sampling was employed. Data was obtained from patients’ diabetic records, computerised clinical medical information system and medical case notes. Univariate analysis was done using Chi-square, t-test, Fisher’s exact test and Mann-Whitney U-test. Multiple logistic regression was used to determine the associated factors for development of CKD stage 3b. Cases and controls were different in terms of age, duration of diabetes, use and dosage of angiotensin blockade medications, systolic blood pressure and baseline GFR.     The increasing health care cost and public awareness on the delivery of high quality services has forced healthcare service providers to look into the healthcare delivery system. Clinical Pathway (CP) has been introduced in many hospitals and has been accepted as a beneficial tool in assisting healthcare organizations worldwide. Further, the CP also provides consistently high quality and coordinates services with minimum resources. It is proven to be a beneficial tool in other countries. CP is recently being introduced into the Malaysian healthcare system. The aim of this article was to high-light the benefits of CP in improving healthcare quality and controlling the medical cost. The relevant articles have been reviewed.  The majority of literature reviewed con-cluded that there were positive effects in  implementing CP. The CP was found to be significant in reducing length of stay and medical cost. The introduction of evidence based medicine, clinical outcomes, clinical audit, multidisciplinary communication, teamwork and care planning were also improved by CP. The challenges for healthcare providers and healthcare managers are to participate and be fully committed in path-way development and implementation in order to improve healthcare quality and cost control.
